Objective: A preliminary study on the Mental Health Test Modified. Methods: According to regionalism of Zhejiang Province, 2084 middle school students were sampled from 36 classes of 6 junior middle schools and 6 senior middle schools from 8 cities and towns. Results: The norm of the Mental Health Test Modified toward Zhejiang Province was made. The scores of students in junior middle school of Learning Anxiety, Sensitivity Tendency, Body Symptom, Hor- ror Tendency and Impulsion Tendency are higher than those of students in senior middle school. Among students of junior middle schools, the state of Self- accusation Tendency of Grade Two is more serious. Among students in senior middle school, the state of Learning Anxiety and Self- accusation Tendency, Sensitivity Tendency and Horror Tendency of Grade One are also more serious. Conclusion: There should be different emphases in the process of Mental Health Education for students of different grades.
2.Analysis of iatrogenic retinal breaks compficating pars plana vitrectomy in eyes with idiopathic macular pucker

Objective To determine the incidence and risk factors of iatrogenic retinal breaks in eyes undergoing pars plana vit- rectomy for idiopathic macular pucker.Design Retrospective case series.Participant 88 consecutive vitrectomies performed on eyes with idiopathic macular pucker.Method Consecutive vitrectomies performed on eyes with idiopathic macular pucker at Beijing Tongren Eye Center between 2002 and 2006 were retrospectively reviewed.Cases with iatrogenic retinal breaks were recorded and analyzed. Main Outcome Measure Number and location of retinal breaks,and anatomic outcome after surgical managements.Result A total of 88 consecutive vitrectomies were included in the study.Of the 88 eyes,8 eyes had 14 iatrogenic retinal breaks detected,with an aver- age incidence of 9.1%.Peripheral retinal breaks(8.0%)were more common than posterior retinal breaks(1.1%).All peripheral retinal breaks occurred around the selerotomy sites(100%)and the quadrant of predominant hand was involved most commonly(62%).Most of the breaks(88%)were detected during the surgery.All eyes with iatrogenic retinal breaks obtained anatomic retinal reattachment (100%).Conclusion Despite improvements in instrumentation and surgical techniques,iatrogenic retinal break continues to be an im- portant complication of pars plana vitrectomy in eyes with idiopathic macular pucker.This complication tends to occur more commonly at peripheral retina and is mainly selerotomy-related.
3.Reassessment subacute MPTP-treated mice model of Parkinson disease

OBJECTIVE ① To estimate the value of the subacute MPTP mouse model in aspects of behavioral performance, biochemical changes and pathological abnormalities. ② To find effective positive drugs. METHODS Male C57BL/6 mice were injected with MPTP (30 mg·kg- 1 ·d- 1, ip) for 5 consecutive days. Three days before MPTP injection, the mice were orally administered selegiline (3 mg·kg-1·d-1), pramipexole (3 mg·kg-1·d-1), or medopar (100 mg·kg-1·d-1) for 18 d. Behavioral perfor?mance was assessed in the open field test, pole test and rotarod test. Neurotransmitters in the striatum were detected using HPLC. Protein levels were measured by Western blot. Pathological characteristics were examined by immunohistochemistry. Ultrastructure changes were observed by electron microscopy. RESULTS The subacute MPTP treatment did not induce evident motor defects despite severe injuries in the dopaminergic system. Additionally, MPTP significantly increased the α-synuclein levels and the number of astrocytes in the striatum, and destroyed the blood-brain barrier (BBB) in the substantianigra pars compacta. Both selegiline and pramipexole were able to protect the mice against MPTP injuries. CONCLUSION The subacute MPTP mouse model does not show visible motor defects; it is not enough to evaluate the validity of a candidate just based on behavioral examination, much attention should also be paid to the alterations in neurotransmitters, astrocytes, α- synuclein and the BBB. In addition, selegiline or pramipexole is a better choice than medopar as an effective positive control for the subacute MPTP model.
4.The screening model for dopamine receptor agonists by a dopamine sensor

Parkinson's disease (PD) is a degenerative disease of the central nervous system due to the loss or death of dopaminergic neurons in the substantia nigra. Clinically, levodopa is the most effective and commonly used drug for PD treatment. However, long-term levodopa therapy is prone to motor complications and other side effects caused by excessive peripheral dopamine production, which has become an urgent problem to be solved in PD treatment. Dopamine receptor (DR) agonists are similar to dopamine. They can directly stimulate postsynaptic dopamine receptors, produce the same effect as dopamine, delay the application of levodopa as much as possible, and reduce complications caused by long-term use of levodopa. Therefore, screening effective dopamine receptor agonists has become a key issue in the study and treatment of PD. In order to establish a rapid, stable and reliable method for dopamine receptor agonist screening, this study used the human dopamine receptor 2 (DRD2) gene fused with a circular permuted EGFP (cpEGFP) to construct a recombinant gene, packaged with lentiviral vector, and the vector replaced the parted inner transmembrane domain of the third intracellular loop (ICL3) of genetically-encoded GPCR-activation based (GRAB) sensors. The fluorescence of GPCR-fused cpEGFP is regulated by conformational changes mediated by the interaction of dopamine receptor agonists with GPCRs without altering GPCR activity. The HEK293T cells were infected with viral vector, screened by puromycin to select highly expressed cells. Dopamine receptor agonists (including dopamine, bromocriptine mesylate, cabergoline, pramipexole) were used as positive drugs to explore the best screening and detection conditions, establishing a stable model to evaluate the dopamine receptor agonist. The results showed that the optimal filter for the dopamine receptor agonist in this study was the cell seeding count of 7×104, and the effective concentration of the positive drug was 1-100 µmol·L-1. In addition, pretreated with 10 µmol·L-1 dopamine receptor antagonists (including chlorprothixol hydrochloride, domperidone, and sulpiride), the positive fluorescence signal of overexpressed DRD2-cpEGFP HEK293T cells could not be detected when exposed to 10 µmol·L-1 dopamine receptor agonists, which proved that dopamine receptor antagonists could block the activity of dopamine receptor agonists, so they cannot activate dopamine receptor allosteric, indicating that the model has good specificity and can also be used for the screening and detection of new dopamine receptor antagonists. In summary, the study constructs a stable dopamine sensor detection system, which can effectively screen potential dopamine receptor agonists. The operation procedures are simple and rapid. And it can be used for a large-scale screening providing a fundamental methodology for drug development and PD treatment targeted on DRD2.

6.Salvage central neck dissection for papillary thyroid cancer.

OBJECTIVETo evaluate the salvage central neck dissection (CND) for papillary thyroid cancer in the patients with suspicious residual positive lymph nodes in the central compartment.
METHODSA total of 85 cases undergoing salvage CND between January 2006 to January 2009 was reviewed.
RESULTSFifty-nine (69.4%) of the 85 patients showed residual positive lymph nodes in the central compartments. The number of dissected lymph nodes for each dissection ranged 2 - 13 nodes, with an average of 5.7 nodes. A total of 159 positive lymph nodes were found in the 59 cases, with an average of 2.7 positive nodes in each case. Four of 6 patients who underwent initial CND in other hospitals were found with residual positive nodes in the central apartments. The incidence of postoperative complications for the salvage CND was 10.6% (9/85), of them 5 cases with hoarseness, 2 cases with transient hypoparathyroidism, 1 case with postoperative bleeding, and 1 case with subplatysmal effusion. The median of follow-up after salvage CND was 44 months (3 - 5 years), showing contralateral central recurrence in 1 case, lateral neck recurrence in 4 cases, lung metastasis in 1 case, and no death case.
CONCLUSIONSSalvage CND should be done for those patients without undergoing initial CND but with positive nodes found after thyroidectomy or the high risks (T3, T4, extra thyroid extension, residual in primary site, and vascular or lymphatic vessel invasion); and also for those patients with initial CND but insufficient extent.

7.Relationship between immune imbalance and sthenia-asthenia syndromes in patients suffering from severe intra-abdominal infection.

OBJECTIVETo explore the characteristics of immune imbalance in patients with multiple organ dysfunction syndrome (MODS) induced by severe intra-abdominal infection and its relationship with changing of TCM sthenia-asthenia syndrome.
METHODSForty-six patients with MODS induced by severe intra-abdominal infection and treated with etiological and syndrome differentiation of integrative medicine were observed in succession. Patients' peripheral blood levels of interleukin-6/interleukin-10 ratio (IL-6/IL-10), human leukocyte antigen DR site (HLA-DR), helper T lymphocyte1/2 ratio (Th1/Th2), and the regulatory T lymphocyte (Treg) were measured on the 1st, 3rd and 7th day of the research respectively. And the distribution laws of TCM syndrome types, sthenia (S), asthenia (A), and mingled sthenia/asthenia (M), in patients were observed as well.
RESULTSIL-6/IL-10 ratio at all the testing time points showed insignificant difference in patients of types S and M, while in those of type A, it was more lowered on the 7th day than that on the 1st day. HLA-DR lowered to <30% on the 7th day in all patients of type A and showed significant difference to that on the 1st day (P <0.05), while HLA-DR <30% was not found in all patients of types S and M. Th1/Th2 ratio in patients of types S and A was insignificant different at the foremost 3 days, but lowered significantly on the 7th day, while in patients of type M, it was unchanged in all the 7 days of observation. Treg level was unchanged in the foremost 3 days in patients of types S and M, while in those of type A, it raised on the 3rd day, but no raising was found in the subsequent 4 days. Comparisons of various indexes detected at corresponding time points respectively among patients with various syndrome types showed that, for levels of IL-6/IL-8, HLA-DR, and Th1/Th2, the sequence was S>M>A; and for Treg, it was A>M>S.
CONCLUSIONIn the pathological process of MODS induced by severe intra-abdominal infection, the index IL-6/IL-10, reflecting the balance of the pro-/anti-inflammatory cytokines and the indexes HLA-DR, Th1/Th2 and Treg reflecting the immune function, all can exactly reflect the TCM asthenia-sthenia syndrome types. The sequence in patients of various syndrome types for levels of IL-6/IL-10, HLA-DR and Th1/Th2, is S> M>A, but for Treg it is the inverse, as A>M>S.

8.Perioperative glycemic control by insulin glargine in type 2 diabetic patients.

OBJECTIVETo investigate the feasibility of perioperative glycemic control with insulin glargine in type 2 diabetic patients.
METHODSWe retrospectively analyzed the clinical data of 16 type 2 diabetic inpatients treated with insulin glargine (research group) and 16 type 2 diabetic inpatients treated with the traditional intensified insulin therapy (control group) for perioperative glycemic control.
RESULTSThe fasting blood glucose values of the diabetic patients in the research group on the day of surgery and the first 3 postoperative days were (7.5 +/- 1.8), (8.2 +/- 1.8), (7.6 +/- 1.6), and (7.2 +/- 1.1) mmol/L, respectively, and were (9.0 +/- 2.8), (10.4 +/- 2.4), (8.8 +/- 2.7), (9.0 +/- 2.0) mmol/L in the control group, respectively. The fasting blood glucose values in the research group were significantly lower than the control group on the first and third postoperative day (P = 0.02 and 0.01, respectively). No hypoglycemic events were observed and all wounds were healed well in both groups.
CONCLUSIONWith satisfied fasting blood glucose level and fewer episode of hypoglycemia, perioperative glycemic control by insulin glargine in type 2 diabetic patients is safe, effective, and convenient.

10.Survey on prevalence of atopic dermatitis in 0 - 6 year-old children in Tianjin

Objective To investigate the prevalence of and predisposing factors of atopic dermatitis (AD ) in 0 - 6 year-old children in rural and urban area of Tianjin. Methods The subjects were chosen by cluster sampling from kindergartens in Tianjin's urban and rural districts. Questionnaires were designed based on the International Study of Asthma and Allergies in Children (ISAAC ) and distributed to the children's parents via the teachers. Results In this survey, 3 749 questionaires were returned, of which 3 708 were valid. The response rate was 82.7% in total, 79.3% in the rural area, and 84.9% in the urban area. The age and gender distributions were similar in the urban and rural area. The prevalence rate of AD in these children was 2.9% in total, and it was higher in the urban area than in the rural area (3.5% vs 2.4%, x2 = 3.98, P
