6.A Statistical Review of 3738 Cases of Skin Tumors Referred from Primary Physicians.

BACKGROUND: Many stu iies focused on the distribution of skin tumors, but none of them included the data refered from primat y physicians in Korea. OBJECTIVE: This study was to investigate the incidence and distribution of skin tumors among Korean people. METHODS: A Total of 3738 cases of skin tumor biopsy specimens referred from primary physicians nationwide to Ewon Reference Laboratory(EWON) from July 1989 to June 1992 were reviewed with a microscope, and classified. The result was compared with the data of skin tumor cases biopsied during the same period at the department of dermatology of Asan Medical Center (AMC). RESULTS: 1. Of all skin turnors reviewed, malignant tumors were 3.9% at EWON and 11.0% at Asan Medical Center. 2. The most common primary malignant tumors of the skin were in the order of basal cell carcinoma, squamous cell carcinoma, and malignant melanoma in both centers. 3. The most common benign tumors were in the order of epidermal cyst, lipoma, pilomatricoma, hemangioma, and dermatofibroma at EWON, and epidermal cyst, seborrheic keratosis, syringoma, pyogenic granuloma, derr iatofibroma and lipoma at AMC. 4. While the prevalence of benign skin tumors were highest in the fourth & fifth decades, that of malignant skin tumorware highest in the sixth & seventh decades. CONCLUSION: Our study shows that the data obtained from tertiary hospitals can be quite different from those of primaiy physicians and the incidence of malignant skin tumors can be overestirnated when only data from large university hospitals were included.

7.A Case of Cognitive Behavioral Therapy for Superior Semicircular Canal Dehiscence.

Semicircular canal dehiscence is a rare but well-described condition to be induced noise or straining. Patients with Superior semicircular canal dehiscence (SSCD) present with a wide variety of symptoms. Reported case is a 51-year-old woman who presented with intermittent vertigo and left-side aural fullness for one month after traffic accident. To establish the diagnosis, high resolution temporal bone CT scanning has been performed to demonstrate bone defect overlying superior semicircular canal, and vestibular auditory symptoms and signs are evaluated through vestibular function test and auditory examination. Cognitive behavior therapy, vestibular rehabilitation and vestibular suppressants are prescribed. Auditory symptoms and vertigo have been subsided. At the same time, Presented depression and anxiety have been improved after 3 month treatment and the quality of life of patient has been improved.

8.Multiplex polymerase chain reaction test for the diagnosis of acute viral hepatitis A.

BACKGROUND/AIMS: The early diagnosis of acute hepatitis A (AHA) is hindered because serum IgM against hepatitis A virus (HAV) can yield false-negative results during the window period. This study evaluated the diagnostic accuracy of a polymerase chain reaction (PCR) kit for HAV RNA for the diagnosis of AHA. METHODS: Samples were collected from 136 patients with acute severe hepatitis at their admission to Asan Medical Center between June 2010 and July 2010. Samples were analyzed for serum IgM anti-HAV using an immunoassay test and for qualitative HAV RNA using the Magicplex HepaTrio PCR test kit. The diagnostic accuracies of these methods were tested on the basis of clinical and laboratory diagnoses of AHA. RESULTS: The concordance rate and kappa value between IgM anti-HAV and HAV RNA PCR were 88.2% and 0.707, respectively. For the diagnosis of AHA, the sensitivity and specificity of IgM anti-HAV were 90.7% and 100%, respectively, when an "equivocal" result was regarded as positive; and 79.1% and 100%, respectively, when an "equivocal" result was regarded as negative. The sensitivity and specificity of HAV RNA PCR were 81.4% and 100%, respectively. All four patients with negative IgM anti-HAV and positive HAV RNA PCR results and all four patients with equivocal IgM anti-HAV RNA and positive HAV RNA PCR results were eventually diagnosed with AHA. CONCLUSIONS: The qualitative HAV RNA PCR test has an equivalent diagnostic accuracy for AHA compared to IgM anti-HAV and may be more sensitive during the window period.
