PURPOSE: This study was done to examine the effects of oral care with essential oil in improving the oral health status of hospice patients with terminal cancer. METHODS: The participants were 43 patients with terminal cancer admitted to K hospital in G city, Korea. Twenty-two patients were assigned to the experimental group and 21 to the control group. Participants in the experimental group received special mouth care with essential oil (application of essential oil mixture consisting of geranium, lavender, tea tree, and peppermint). The control group received special mouth care with 0.9% saline. The special mouth care was performed twice daily for one week in both groups. The scores for subjective oral comfortness, objective oral state, and numbers of colonizing Candida albicans were measured before and after the treatment. RESULTS: The score for subjective oral comfortness and objective oral state were significantly higher in the experimental group compared to the control group. The numbers of colonizing Candida albicans significantly decreased in the experimental group compared to the control group. CONCLUSION: Oral care with essential oil could be an effective oral health nursing intervention for hospice patients with terminal cancer.

PURPOSE: Thyroid disease is the most common endocrine disease in childhood. Thyroid hormone has critical effects on growth and development, especially in childhood. We survey the prevalence, sex and age distribution, symptoms and thyroid function states of thyroid diseases in childhood. METHODS: Three hundred ninety one children who were diagnosed as having thyroid disease at department of pediatrics, St. Mary's hospital from Jan. 1987 to Dec. 1998 enrolled in this study. RESULTS: 1) The ratio between male and female patients was 1: 5.3. Age distribution was puberty, school age and infancy in their order of frequency. 2) The results of thyroid function tests showed normal function in 210 cases (53.7%), increased function in 95 cases(24.3%), and decreased function in 86 cases (22%). 3) Simple goiter, Graves disease, and chronic lymphocytic thyroiditis were the most common diseases in euthyroid, hyperthyroid, and acquired hypothyroid state, respectively. 4) The most common sign and symptom was goiter in euthyroid(100%), hyperthyroid(98.8%) and acquired hypothyroid state(96.2%). Forty of 68 cases(58.8%) with congenital hypothyroidism were detected by neonatal screening. CONCLUSION: Simple goiter, chronic lymphocytic thyroiditis and Graves disease were common acquired thyroid diseases in childhood, and goiter is the most common clinical manifestation in acquired thyroid disease. Recently, increasing number of congenital hypothyroidism was detected by neonatal screening test.

Cystitis glandularis is a benign metaplastic proliferative lesion of the urinary bladder which usually occurs in the setting of chronic irritation and infection or in some cases as a congenital process. Sometimes it presents as a tumor mass-like florid lesion, grossly mimicking malignancy. We report a case of 59-year-old man with multiple mass lesions around the trigone and the neck portion, which suggested the possibility of malignancy in clinical and radiological evaluations. Final diagnosis was confirmed by transurethral resection. The surface urothelial lining was intact. The submucosa showed von Brunn's nests, cystitis glandularis and cystitis cystica in the edematous lamina propria. There were numerous glands lined by tall columnar, mucin producing epithelium without atypia, conforming to the appearance of the intestinal variant of cystitis glandularis. The cystitis glandularis may mimic a neoplasm on gross evaluation. The intestinal variant of cystitis glandularis is particularly likely to be problematic when florid.

In the Republic of Korea, relevant documents are submitted to forensic doctors or agencies when courts grant confiscation warrants for autopsy. If the essential data on unusual death are not submitted at the time of the autopsy, it may be difficult to properly understand the situation relating to an unusual death prior to the autopsy, thus reducing the accuracy of the autopsy. As many as 6,133 out of 6,610 autopsy data (92.8%) in the Republic of Korea in 2015 were analyzed. Most autopsy appraisal requests (99.8%) were submitted. Unusual death occurrence reports (86.0%) and command recommendations of unusual death (70.3%) were submitted in many cases. However, prosecutor commands on unusual death were submitted only in 27.8% cases, and confiscation warrants were not submitted in 7.4% cases. As for postmortem inspection and death scene investigation reports, 29.3% and 34.1% cases were submitted, respectively. In addition to the above two documents, death certificates and records of statement of a relative had significant regional variations (0.3%–80.1%, 3.1%–64.7%, 27.8%–81.3%, and 40.8%–96.8%, respectively). For postmortem inspection and death scene photos, 2.7% and 3.2% were submitted in black-and-white photographs, respectively. The authors propose a list of forensic autopsy requests including autopsy appraisal requests, unusual death occurrence reports, command recommendations of unusual deaths, prosecutor commands on unusual death, and confiscation warrants unconditionally, as an essential document reflecting the progress of investigations. We suggest that postmortem inspection reports and photos, death scene investigation reports and photos, and death certificates should be included as part of postmortem investigation data.

The gastric adenoma, which is also called as borderline epithelial lesion, dysplasia, atypical epithelial lesion, is occasionally encoutered during gastroscopy, but the natural history of a gastric adenoma has not been clarified in view of the pre-melignant potential. In Korea, there has been few report for its gastroscopic feature, gastroscopic diagnostic accuracy, or loag-term clinical follow-up yet. (continue...)
