1.Assessment of Left Ventricular Regional Diastolic Function in Different Degrees of Coronary Stenosis by Three-dimensional Speckle Tracking Imaging

Purpose To evaluate the left ventricular regional diastolic function in coronary heart disease (CHD) patients without regional wall motion abnormality by real-time three-dimensional speckle tracking imaging (RT3D-STI) to investigate the value of RT3D-STI in the diagnosis of early CHD. Materials and Methods 140 CHD patients with 420 coronary artery main branches were enrolled and divided into 4 groups according to the results of coronary angiography (CAG): normal group (101 coronary artery branches without stenosis), mild stenosis group (112 coronary artery branches with stenosis<50%), moderate stenosis group (95 coronary artery branches with stenosis 50%-75%) and sever stenosis group (112 coronary artery branches with stenosis>75%). Three dimensional full volume images were collected through the chest, and the corresponding strain imaging diastolic index (SI-DI) of each direction was calculated, their value for the diagnosis of coronary artery disease was analyzed. Results The longitudinal, area and radial SI-DI of mild coronary artery stenosis group were lower than those of normal group (P<0.05);the longitudinal, area, radial and circumferential SI-DI of moderate coronary artery stenosis group were lower than normal group (P<0.05) and the area, longitudinal and circumferential SI-DI were lower than those of the mild stenosis group (P<0.05);the longitudinal, area, radial and circumferential SI-DI were all lower than those of normal and mild stenosis group (P<0.05), and the area, longitudinal and radial SI-DI were all lower than the moderate stenosis group (P<0.05). ROC curve showed that the sensitivity of longitudinal SI-DI (84.0%) was highest, while the specificity of area SI-DI (78.9%) was the highest. Conclusion RT3D-STI has important significance for early detection and diagnosis of CHD, regional diastolic function decrease of the coronary artery supply area occurs earlier than systolic function reduction, and diastolic function is more sensitive for myocardial ischemia, as a result, SI-DI is able to reflect the stenosis degree of coronary artery in a certain extent.
2.Hemodynamic changes following cerebrovascular stenting

Intravascular stent as a foreign body induces platelet-monocyte aggregation following implantation, mediates various actions of monocyte, induces monocyte to express tissue factors, promotes fibrin sedimentation at injury site, and accelerates inflammatory reaction of vessel wall injury. In addition, it stimulates platelet aggregation on the stent, activates platelet system and coagulation system, resulting thrombosis. Smooth muscle cells migration to injury site, neointima hyperplasia, and vessel wall reconstruction cause in-stent restenosis. The influential factors that induce restenosis include type of stent, original diseases of the host, stenosis severity before stenting and residual stenosis length after stenting. Inhibition of tunica intima hyperplasia or improvement of neointima as well as vessel wall reconstruction could effectively prevent restenosis.
4.May HOXD13-zinc finger protein 548 complex develop in foot tissue during human embryonic development?

BACKGROUND:The HOX gene family is a highly conserved transcription factor family,which affects the formation of basic axis and secondary axis during embryonic development,at the same time,it plays a pivotal role in the development of the central nervous system,axial skeleton,stomach intestine,urogenital system and external genitalia.OBJECTIVE:To investigate the interactional protein of HOXD13 in human embryo foot development at 14 weeks pregnancy.METHODS:Fetal foot tissues were harvested and protein extraction was performed using protein extraction kit,6 μg HOXD13 antibody was added for immunOprecipitatjOn experiment,which aims to obtain HOXD13 interactional proteins,while the non-specific IgG served as a negative control Subsequent to Bradford staining,the sediment protein bands were cut for mass spectrometry.RESULTS AND CONCLUSION:We obtained an interactional protein of HOXD13 in the human embryo foot at 14 weeks pregnancy,at the relative molecular mass of approximately 60 000.The protein was identified as zinc finger protein 548 using mass spectrum.HOXD13 may play a very important role in transcriptional control by HOXD13-zinc finger protein 548 protein complex in the human embryo foot at 14 weeks pregnancy.
5.FHL1 gene expression and idiopathic congenital clubfoot: Comparative analysis of 15 patients and 3 normal controls

BACKGROUND: Preliminary study shows that the FHL1 gene expression is down-regulated in muscle tissue of patients with clubfoot, and the gel retardation experiments have verified HOXD13 and FHL1 gene promoter region transcription factor predicted binding sites in vitro, but the experimental results are not enough to truly reflect in vivo transcriptional regulatory protein and DNA binding conditions.OBJECTIVE: Western-blot technique is utilized to further validate FHL1 and HOXD13 gene expression in protein levels in muscle tissue of patients with congenital clubfoot, the in vivo HOXD13 and predicted binding sites in embryonic foot development was verified using chromatin immunoprecipitation technology.METHODS: Muscle tissues were samples from 15 children with congenital clubfoot, in the Department of Pediatric Surgery, at the Second Affiliated Clinical Hospital of China Medical University, 3 copies of normal children foot muscle tissues at the same age were provided by the Forensic Medicine College of China Medical University, 1 case of aborted embryo at pregnancy 13 weeks were offered by Department of Obstetrics and Gynecology, at the Second Affiliated Clinical Hospital of China Medical University. All specimens used are given informed consents by the patients and their families. Western-blot method was applied to detect the expressions of HOXD13 and FHL1 in foot muscle tissue of 15 patients of congenital clubfoot and 3 normal children at the same age; The FHL1 gene upper stream HOXD13 binding sites was predicted using software, the interaction of HOXD13and FHL1 during embryonic development was verified with chromatin immunoprecipitation experiment, brain tissue without HOXD13 protein expression served as controls.RESULTS AND CONCLUSION: Compared with normal children foot muscle tissues, both HOXD13 gene (5/15) and FHL1 (7/15)were down-regulated in 15 patients of congenital clubfoot. An enrichment of the predicted HOXD13 binding site was observed in the precipitated human embryo foot tissues chromatin. No enrichment of the predicted site containing sequence was observed in the control brain chromatin; additionally, there was no enrichment of the control sequence. This study further verifies that, the FHL1 and HOXD13 gene expression are down-regulated in the foot muscle tissue of congenital clubfoot children patients; during development of human embryos, HOXD13 protein can bind with FHL1 promoter region binding sites to play its role in transcriptional regulation. It is indicated that in the human foot embryonic development, the down-regulation of HOXD13expression may result in reduced expression levels of FHL1, thereby affecting the foot muscle growth and differentiation, leading to clubfoot deformity occurring.
7.Clinical Effects of Cisplatin Combined with Temozolomide and Radiotherapy in Treatment of Glioblastomas after Surgery

Objective To investigate the clinical effect of cisplatin combined with cisplatin and radiotherapy in the treatment of glioblastomas patients receiving surgery.Methods 68 cases of glioblastomas patients with surgery were divided into control group (n=36) and treatment group (n=32).The control group was treated with temozolomide and radiotherapy;the treatment group was treated with cisplatin combined with temozolomide and radiotherapy.Short-term efficacy,adverse reactions,and survival time were documented during following up in both groups.Results The efficiency in the treatment group (87.5%) was significantly higher than that in the control group (66.67%),P<0.05.There were no significant difference of adverse reaction including bone marrow inhibition,digestive tract symptoms,and psychiatric symptoms among the two groups,P>0.05.The medium survival time of the treatment group (26.0 month) was significantly longer than that of the control group (17.5 month),P <0.05.There was no significant difference of 1-year survival rate between the treatment group (81.25%) with control group (63.89%),P>0.05;however,2-and 3-year survival rate in treatment group (52.13%,31.25%) were significantly higher than that in the control group (27.78%,11.11%),P<0.05.1-year progression free survival (PFS) rate of treatment group (68.75%) was significantly higher that of control group (41.67%),P<0.05.Conclusion Cisplatin combined with temozolomide and radiotherapy can significantly improve the treatment efficiency without leading to higher rate of adverse reactions and significantly improve the 2-,3-year survival rate as well as 1-year progression free survival (PFS) rate in patients with glioblastomas,it is worth being promoted in clinical application.
8.A clinical evaluation of nuclear matrix protein 22 in urine as an adjuvant diagnostic marker for transitional cell carcinoma of the upper urinary tract

Objective To evaluate urinary nuclear matrix protein(NMP 22) as an adjuvant diagnostic marker for transitional cell carcinoma of the upper urinary tract. Methods 24 patients with transitional cell carcinoma of the upper urinary tract and 20 patients with benign urinary diseass were evaluated with urinary NMP 22(cutoff level 10U/ml) and voided urinary cytology.NMP 22 was determined using a commercial test kit. Results The sensitivity and specificity of NMP 22 were 87.5% and 85.0% respectively whereas these of cytology were 58.3% and 95.0%. Conclusions Urinary NMP 22 might be an useful adjuvant diagnostic marker for transitional carcinoma of the upper urinary tract.
10.Hyperplasia of prostatic outer gland studied with transcrectal ultrasound guided biopsies

ObjectiveThe possibility of the hyperplasi a of prostatic outer gland was studied with transrectal ultrasound (TRUS) guided biopsies. MethodsTwenty-seven prostatic outer glands in patients suspicious for prostatic cancer (PCA) were biopsied by TRUS guided, the total sites were 47. The same site was biopsied in sagittal and longitudinal section respectively. The hypoechoic lesion was biopsied 2 times if it was discovered in the outer gland. The histological results were diagnosed in double blind. Results Twenty prostatic inner glands looked symmetrical hyperplasia; 9 outer glands became thin because of the compression, there was no compressive sign in 18 patients; there was a hypoechoic lesion in outer gland of 3 patients. Benign prostatic hyperplasia was diagnosed in the specimens of 47 sites. ConclusionsThe hyperplasia of prostatic outer gland can occur like inner gland because it is glandular tissue.
