1.Observation of Clinical Efficacy of Puerarin Combined with β-aescine Sodium in the Treatment of Patients with Postoperative Edema of Hand Trauma

OBJECTIVE:To observe the clinical efficacy of puerarin combined with β-aescine sodium in the treatment of pa-tients with postoperative edema of hand trauma. METHODS:118 patients with postoperative edema of hand trauma were random-ly divided into observation group (n=59) and control group (n=59) by number. Both groups were taken infrared physiotherapy and appropriate physical treatment such as hand functional exercise;patients in control group was added β-aescine sodium 10 mg based on the physical treatment,iv,once a day;observation group was added puerarin 200 mg based on the treatment of control group,iv,once a day. Both groups were taken 10 days for one course of treatment. Compared the swelling scores,perimeter of suf-fered limb and edema scores,clinical efficacy and adverse reactions of patients between the two groups before and after treatment. RESULTS:The symptoms of hand limb swelling in 2 groups were obviously improved(P<0.01 or P<0.05),however,observa-tion group was better than that of control group,with significant difference(P<0.05);after treatment,the swelling,perimeter of suffered limb and swelling score in observation group were lower than control group,with significant differences(P<0.05);the to-tal effective rate in observation group was 94.91% and control group was 83.05%,with significant difference(P<0.05). No ADR was found in 2 groups. CONCLUSIONS:Puerarin combination with β-aescine sodium has obvious efficacy and safety in the treat-ment of postoperative edema of hand trauma.
2.Effects of volume resuscitation with different fluids on expression of aquaporin 1 in pulmonary capillary endothelial cells of endotoxemic rats

Objective To evaluate the effects of volume resuscitation with different fluids on expression of aquaporin 1 (AQP-1) in pulmonary capillary endothelial cells of endotoxemic rats.Methods Fifty pathogen-free male Sprague-Dawley,rats,weighing 250-300 g,aged 6-8 weeks,were randomly divided into 4 groups (n =10 each):control group (group C),lipopolysaccharide group (group LPS),NaCl group (group NS),6% hydroxyethyl starch 130/0.4 group (group HES) and hypertonic hydroxyethyl starch 40 group (group HSH).Sepsis was induced with LPS 5 mg/kg injected via the femoral vein.At l0 min after the model was successfully established,the corresponding fluids were infused into the femoral vein at a constant speed (15 ml/kg over 6 h) via a pump.At 0,3 and 6 h after LPS administration,blood samples were collected from the femoral artery for measurement of serum tumor necrosis factor-α (TNF-α) concentrations and for blood gas analysis.PaO2 and lactate (Lac) concentrations were recorded and oxygenation index (PaO2/FiO2) was calculated.The rats were sacrificed at 6 h after LPS administration,and lungs were removed for determination of AQP-1 expression in pulmonary capillary endothelial cells and wet-to-dry lung weight ratio (W/D ratio),and for microscopic examination of the pathological changes which were scored.Results Compared with group C,W/D ratio,serum TNF-α and Lac concentrations,and pathological scores were significantly increased,AQP-1 expression was down-regulated,and PaO2 and oxygenation index were decreased in LPS,NS,HES and HSH groups.Compared with group LPS,W/D ratio,serum TNF-a and Lac concentrations,and pathological scores were significantly decreased,AQP-1 expression was up-regulated,and PaO2 and oxygenation index were increased in NS,HES and HSH groups.Compared with group NS,W/D ratio,serum TNF-α and Lac concentrations,and pathological scores were significantly decreased,AQP-1 expression was up-regulated,and PaO2 and oxygenation index were increased in HES and HSH groups.Compared with group HES,the blood Lac concentrations were significantly decreased,and no significant changes were found in the other parameters in group HSH.Conclusion Volume resuscitation with NaC1,hydroxyethyl starch 130/0.4 and hypertonic hydroxyethyl starch 40 can reduce the lung injury effectively in endotoxemic rats,and hypertonic hydroxyethyl starch 40 provides more significant efficacy,and up-regulated AQP-1 expression in pulmonary capillary endothelial cells is involved in the mechanism.
4.HUMAN TfR GENE EXPRESSION BEFORE AND AFTER IRON SUPPLEMENTATION IN WHITE BLOOD CELL DETECTED BY FLUORESCENCE QUANTITATIVE POLYMERASE CHAIN REACTION

Objective:To quantitively detect TfR gene expression of human white blood cell before and after iron supplementation. Methods:Before and after iron supplementation,10 healthy women were phlebotomized separately,and the real-time fluorescence quantitative PCR method was used to analyze the expression difference of TfR gene in human white blood cell. Results:TfR gene expression decreased after iron supplementation in 10 women,more evidently in 7 cases,where the expression level was half less than before iron supplementation. Conclusion:TfR gene expression of human white blood cell could be quantitatively measured by FQ-PCR,and was decreased after iron supplementation in 10 women. It explained that the regulative mechanism of TfR mRNA expression by iron is also suitable to white blood cell.

China donation after Citizen's death (CDCD) has been organized since 2010,and now has been synchronized with the international organ transplantation.At present,liver transplantation has become the only safe and curative treatment for the end-stage liver diseases.Nevertheless,there is much restriction over further exploration of this technique.This article will mainly focus on donation after brain death (DBD),and summarize the four dominating injuries of donor liver,including the donor's primary injury,cut and perfusion injury,graft preservation injury,and ischemia-reperfusion injury.
6.Application of continuing quality improvement on reducing incidence of PICC-related phlebitis in breast cancer patients undergoing chemotherapy

Objective To investigate the effect of the continuing quality improvement on PICC-related phlebitis in breast cancer patients undergoing chemotherapy. Methods Eighty breast cancer patients undergoing PICC for chemotherapy in our hospital from September to December 2014 were selected as control group, receiving routine nursing care. Another eighty patients undergoing PICC for chemotherapy from January to March in 2015 were selected as experiment group and the quality control cycle was used, the measures including investigating the present situation aiming to theme, goal setting and analyzing, working out and using countermeasures and effect comfirming. Result After applying continuing quality improvement in the experiment group the incidence rate of phlebitis in the continuing quality improvement group was significantly lower than those before continuing quality improvement and in the control group (P<0.05). Conclusion The implementation of continuing quality improvement activities can reduce the incidence of pzce-related phlebitis in patients with breast cancer during chemotherapy. It can improve the safety of PICC catheter and therefore, it is worthy of popularization.

10.Efficacy of lidocaine solid lipid nanoparticles for sciatic nerve blockade in rats

Objective To evaluate the efficacy of lidocaine solid lipid nanoparticles (L-SLNs) for sciatic nerve blockade in rats.Methods Lidocaine-loaded SLNs were prepared using high pressure homogenization.Ninety SPF male Wistar rats,weighing 220-280 g,were randomized into 6 groups (n =15 each) using a random number table:control group (group C),1% L-SLN group (group L1-SLN),1% lidocaine group (group L1),2% L-SLN group (group L2-SLN),2% lidocaine group (group L2),and blank SLN group (group SLN).In C,L1-SLN,L1,L2-SLN,L2 and SLN groups,normal saline,1% lidocaine SLN,1% lidocaine,2% lidocaine SLN,2% lidocaine and blank SLN (200 μl) were injected,respectively,around the sciatic nerve.Before sciatic nerve blockade (baseline) and at 10,20,30,60,120,180,240,300,360,420,480,540 and 600 min after blockade,the paw withdraw latency to a thermal stimulus was measured,and maximum possible effect (MPE) was calculated to reflect the degree of sensory block.Before sciatic nerve blockade and at 10,20,30,60,120 and 150 min after blockade,extensor postural thrust (EPT) of the hind limbs was detected to reflect the degree of motor block.The sciatic nerve at the injection site and the tissues around the site were obtained for observation of the pathological changes at 2 days and 1 and 4 weeks after blockade.Results Compared with the baseline value before blockade and group C,the MPE was significantly increased in at 10-30 min after blockade group L1,at 10-60 min after blockade in group L2,at 10-360 min after blockade in group L1-SLN,and at 10-540 min after blockade in group L2-SLN,and the EPT was decreased at 10-30 min after blockade in group L1,at 10-60 min after blockade in group L2 and group L1-SLN,and at 10-90 min after blockade in group L2-SLN.Compared with group L1,the MPE was significantly decreased at 10 min after blockade,no significant change was found at 20-30 min after blockade,and the MPE was increased at 60-360 min after blockade,and the EPT was increased at 10-30 min after blockade,and no significant change was found at the other time points in group L1-SLN.Compared with group L2,no significant change was found in the MPE at 10-30 min after blockade,the MPE was significantly increased at 60-540 minafter blockade,and the EPT was increased at 10-60 min after blockade,and no significant change was found at the other time points in L2-SLN group.In SLN,L1-SLN and L2-SLN groups,no pathological changes were found in the sciatic nerve at the injection site and the tissues around the site,and only mild inflammatory responses were observed.Conclusion L-SLNs can prolong the duration of block when applied for sciatic nerve blockade in rats and biocompatibility is good.
