OBJECTIVE:To observe the clinical efficacy of puerarin combined with β-aescine sodium in the treatment of pa-tients with postoperative edema of hand trauma. METHODS:118 patients with postoperative edema of hand trauma were random-ly divided into observation group (n=59) and control group (n=59) by number. Both groups were taken infrared physiotherapy and appropriate physical treatment such as hand functional exercise;patients in control group was added β-aescine sodium 10 mg based on the physical treatment,iv,once a day;observation group was added puerarin 200 mg based on the treatment of control group,iv,once a day. Both groups were taken 10 days for one course of treatment. Compared the swelling scores,perimeter of suf-fered limb and edema scores,clinical efficacy and adverse reactions of patients between the two groups before and after treatment. RESULTS:The symptoms of hand limb swelling in 2 groups were obviously improved(P<0.01 or P<0.05),however,observa-tion group was better than that of control group,with significant difference(P<0.05);after treatment,the swelling,perimeter of suffered limb and swelling score in observation group were lower than control group,with significant differences(P<0.05);the to-tal effective rate in observation group was 94.91% and control group was 83.05%,with significant difference(P<0.05). No ADR was found in 2 groups. CONCLUSIONS:Puerarin combination with β-aescine sodium has obvious efficacy and safety in the treat-ment of postoperative edema of hand trauma.

Objective:To quantitively detect TfR gene expression of human white blood cell before and after iron supplementation. Methods:Before and after iron supplementation,10 healthy women were phlebotomized separately,and the real-time fluorescence quantitative PCR method was used to analyze the expression difference of TfR gene in human white blood cell. Results:TfR gene expression decreased after iron supplementation in 10 women,more evidently in 7 cases,where the expression level was half less than before iron supplementation. Conclusion:TfR gene expression of human white blood cell could be quantitatively measured by FQ-PCR,and was decreased after iron supplementation in 10 women. It explained that the regulative mechanism of TfR mRNA expression by iron is also suitable to white blood cell.

Objective To evaluate the effects of volume resuscitation with different fluids on expression of aquaporin 1 (AQP-1) in pulmonary capillary endothelial cells of endotoxemic rats.Methods Fifty pathogen-free male Sprague-Dawley,rats,weighing 250-300 g,aged 6-8 weeks,were randomly divided into 4 groups (n =10 each):control group (group C),lipopolysaccharide group (group LPS),NaCl group (group NS),6％ hydroxyethyl starch 130/0.4 group (group HES) and hypertonic hydroxyethyl starch 40 group (group HSH).Sepsis was induced with LPS 5 mg/kg injected via the femoral vein.At l0 min after the model was successfully established,the corresponding fluids were infused into the femoral vein at a constant speed (15 ml/kg over 6 h) via a pump.At 0,3 and 6 h after LPS administration,blood samples were collected from the femoral artery for measurement of serum tumor necrosis factor-α (TNF-α) concentrations and for blood gas analysis.PaO2 and lactate (Lac) concentrations were recorded and oxygenation index (PaO2/FiO2) was calculated.The rats were sacrificed at 6 h after LPS administration,and lungs were removed for determination of AQP-1 expression in pulmonary capillary endothelial cells and wet-to-dry lung weight ratio (W/D ratio),and for microscopic examination of the pathological changes which were scored.Results Compared with group C,W/D ratio,serum TNF-α and Lac concentrations,and pathological scores were significantly increased,AQP-1 expression was down-regulated,and PaO2 and oxygenation index were decreased in LPS,NS,HES and HSH groups.Compared with group LPS,W/D ratio,serum TNF-a and Lac concentrations,and pathological scores were significantly decreased,AQP-1 expression was up-regulated,and PaO2 and oxygenation index were increased in NS,HES and HSH groups.Compared with group NS,W/D ratio,serum TNF-α and Lac concentrations,and pathological scores were significantly decreased,AQP-1 expression was up-regulated,and PaO2 and oxygenation index were increased in HES and HSH groups.Compared with group HES,the blood Lac concentrations were significantly decreased,and no significant changes were found in the other parameters in group HSH.Conclusion Volume resuscitation with NaC1,hydroxyethyl starch 130/0.4 and hypertonic hydroxyethyl starch 40 can reduce the lung injury effectively in endotoxemic rats,and hypertonic hydroxyethyl starch 40 provides more significant efficacy,and up-regulated AQP-1 expression in pulmonary capillary endothelial cells is involved in the mechanism.

Objective:To compare the clinical effects of restrictive blood transfusion combined with hyperbaric oxygen preconditioning (HBOPC) and restrictive blood transfusion in the treatment of hip,knee arthroplasty (THA,THA).Methods:40 patients in the period of epidural anesthesia,femoral nerve hysteresis hip and knee arthroplasty were selected and randomly divided into two groups:restrictive transfusion group (maintain 80 g/L≤ Hb ＜100 g/L,n=20) and restrictive blood transfusion combined with HBOPC (HBOPC+maintain 80 g/L =Hb ＜100 g/L,n=20).The red blood cell transfusion,red blood cell transfusion rate,perioperative Hb,blood oxygen saturation (SO2),the incidence of hypotension during operation,hospitalization time and postoperative cerebral infarction,acute pulmonary embolism,pneumonia,myocardial infarction,wound infection rate and 90 days mortality rate were compared between two groups.Results:Compared with the restrictive transfusion group,the postoperative Hb,blood oxygen saturation (SO2) of restrictive blood transfusion combined with HBOPC group were significantly increased(P ＜ 0.05);the red blood cell transfusion,red blood cell transfusion rate,incidence of pneumonia,wound infection rate were significantly decreased (P＜0.05).Conclusion:Restrictive blood transfusion combined with hyperbaric oxygen preconditioning could improve the anoxic state of the hip,knee arthroplasty patients,which could effectively reduce red blood cell transfusion,reduce postoperative complications,has good clinical curative effect.

Objective To investigate the clinical and electrophysiology features of infantile spinal muscular atrophy,and explore the clinical significance of genetic diagnosis. Methods The clinical data of 13 infants suffering from infantile spinal muscular atrophy were analysed.The serum creatine phosphokinase was examined,and nerve conduction velocity was tested in median nerve,ulnar nerve,tibial nerve and peroneal nerve.The parameters such as distal motor latency,motor nerve conduction velocity and amplitude of compound motor active potential were analysed.Electromyography was performed in no less than four muscles,and the insertion potential,spontaneous potential and motor unit action potential were observed.Deletion of exon 7 in SMN1 gene was detected by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP). Results All these infants were characterized by progressive flaccid paralysis in limbs.In all cases,amplitude of muscle response was significantly decreased,with prolonged distal latency and slowed conduction velocity.Electromyography demonstrated motoneuron degeneration.Deletion of exon 7 in SMN1 gene was detected in all 13 infants. Conclusion There are unique clinical and electrophysiology features for infantile spinal muscular atrophy,and electromyography may play an important role in the diagnosis.Prenatal genetic diagnosis may help to avoid the birth of this kind of infants.

The specific primers and Taqman probe were designed and synthesized according to the UL31 gene sequence available in GenBank(EF417996).The fragment is 76 bp.A Taq Real-time fluorescent quantitative PCR assay was established for detection of Duck Plague Virus DNA based on DNA copy obtained from attenuated vaccine strain as a positive template.The sensitivity of the test for DEV is 2.1 × 100-2.1 × 109 copies.The minimum detection limit is 2.1 copies(2.1 fg).Tissues,excrement and blood of 4 ducks challenged with DEV were detected repeatly 3 times by this method.All results were positive (3/3).The specificity of the assay was proven based on no amplification by detecting DNA from normal duck cells,duck viral hepatitis virus,Pasteurella,E.coli,S.gall inarum,Newcastle disease virus,goosling plague virus et ai.The whole process of the test could be completed within 4 hours.It brings about quantification detection of DEV DNA.
