Among the primary carcinoma of the extrahepatic bile duct, papillary carcinoma is the least common form and usually located in the distal common bile duct. The cholangiographic findings of the papillary carcinoma is similar to translucency produced by stone. We report a case of multiple papillary adenocarcinoma of the extrahepatic bile duct of which the cholangiographic findings mimicking stones.

3.Gastric Heterotopia in the Gallbladder.

Heterotopia (of Ectopia) is defined as the occurrence of normal tissue in an abnormal location. Heterotopic gastric mucosa has been found throughout the length of the gastrointestinal tract from oral cavity to the rectum. Curiously, it is extremely rare in the gailbladder, but when it occurs, it tends to cause symptoms of acute cholecystitis in patients under 20 years of age, and chronic cholecystitis and gallstones in older patients. The heterotopic mucosa results in an intramural mass, a polyp or multiloculated gallbladder. A firm diagnosis of gastric heterotopia is based on the presence of fundic or pyrolic mucosa replete with parietal and chief cells. A clear distinction from intestinal rnetaplasia should be made, but at times may be difficult. Potential complications include mucosal ulceration, obstruction, and hemorrhage. Treatment is cholecystectomy. We report a case of gastric heterotopia in the gallbladder of a 35-year-old-man. Ultrasonography showed fatty change of liver with a 1.5 cm-sized polypoid lesion in the gallbladder. Endoscopic retrograde cholangiography showed a small filling defect, revealed by pooling of the dye in the center, in the body of gallbladder. Laparoscopic cholecystectomy was performed. A sessile polypoid leision with central umbilication was seen in the upper body of gallbladder, without gallstones. The microscopic finding of polypoid lesion consisted of gastric pyloric glands with parietal and chief cells. The surrounding mucosa revealed ordinary gallbladder epithelium without any metaplastic change. We report a case of this condition in which there was a separate loculus lined by gastric epithelium.

4.Overview of the Unites States Air Force Suicide Prevention Program and Implications for Korean Armed Forces.

Suicide is a major public health problem among Korean military personnel. As multiple factors and their interactions are related to suicide, multicomponent intervention might be useful for reducing suicide and suicide-related outcomes. The United States Air Force Suicide Prevention Program (AFSPP) launched in 1996 is a good example of an evidence-based multifaceted program focusing on leadership involvement and whole community-level efforts. For early identification and management of an at-risk population, AFSPP aims to encourage help-seeking and reduce stigma by changing the community's social norms regarding mental health and suicide. The present study reviewed the key elements of this successful intervention and made a proposal for improving the suicide prevention strategy in the Korean armed forces. This review might be useful for establishing a comprehensive intervention in local communities and organizations outside of the military.

6.Effects of Psychotropic Drugs on Quantitative EEG among Patients with Schizophrenia-spectrum Disorders.

OBJECTIVE: We examined how psychotropic medications affected quantitative EEG (qEEG) results among patients with a schizophrenia-spectrum disorder. METHODS: The drugs were clustered into nine groups depending on their mechanism. We hypothesized that drugs would affect the relative power shown in qEEG results independently and investigated the effect of each drug group on relative power using multiple linear regression analysis and independent samples t-tests. RESULTS: We found that antipsychotics other than clozapine induced an increase in the relative power of alpha activity. Clozapine markedly increased slow waves and decreased alpha activity in the occipital area. The main findings for antidepressants and antiepileptic drugs were the beta increment and lithium increased the power of delta and theta activity. However, we found no evident changes in power due to benzodiazepine. CONCLUSION: Our results are generally consistent with previous pharmaco-EEG studies, despite some differences. Therefore, the EEG effect in each drug group could be singled out even under the polypharmacy condition, with the possible exception of benzodiazepines. Our results support using a new methodological approach to identify the qEEG effects of various psychotropic drugs in clinical settings.

Intestinal lymphangiectasia is characterized by protein- losing enteropathy, and is diagnosed by a small bowel biopsy demonstrating dilated lymphatics in the mucosa, submucosa and serosa in the absence of coexistent inflammation. We report a case of primary intestinal lymphangiectasia that occurred in a 2-year-6-month-old girl who was treated successfully with antiplasmin and octreotide. Initially, the patient was treated with a lipid restriction diet with medium chain triglyceride oil, but her symptoms were not relieved. This case shows that antiplasmin and octreotide therapy might be useful for treating refractory primary intestinal lymphangiectasia.

9.Clinical and Biomechanical Analysis of Transarticular Screw Fixation for Atlantoaxial Instability.

Transarticular screw fixation augmented with interspinous wiring technique for atlantoaxial instability was analyzed to provide immediate multidirectional rigid fixation and increase likelihood of fusion of C1-2 for atlantoaxial instability in several biomechanical studies. Transarticular screw fixation eliminates anterior, posterior translation at C1-2 and reduce flexion and extension movement. 13 patients with atlantoaxial instability were operated with posterior atlantoaxial facet screws fixation augmented with an interspinous C1-2 strut graft and posterior wire fixation technique. One patient died postoperatively from cardiac problem. All surviving patients restored C1-2 alignment and stability without complication due to instrumentation and osseous unions have developed even 2 cases of screw breakage developed. This technique was analyzed to be superior to wiring or clamp fixation biomechanically and leads to success without external orthosis in several series. But precaution is needed to avoid the vertebral artery injury.
