1.Predictors of Depression and Quality of Life among Older Adults with Osteoarthritis.

PURPOSE: The purpose of this study was to identify predictors of depression and quality of life among older adults with osteoarthritis. The predictors included in the model were the client's characteristics(age, pain, disease duration, ADLs), personal resources(hardiness, self-care agency and family support), and depression. METHOD: 150 subjects who were older than 65 years and had diagnosis of osteoarthritis participated in the study. To answer the research questions, descriptive analysis, Pearson correlation, and hierarchical multiple regression were utilized using SPSS WIN program. RESULT: Older adults who were younger and had lower levels of pain and dependency on ADLs, and higher levels of self care agency and hardiness reported lower levels of depression(R2=0.517). Older adults who had lower levels of depression, pain, and dependency on ADLs, higher levels of family support and hardiness, and who are younger reported higher levels of quality of life(R2=0.804). CONCLUSION: Based on the findings of this study, development of nursing intervention program including pain reduction, enhancing ADL abilities and personal resources (hardiness, family support) can be suggested. Further study is needed to increase the ability of generalization of the study findings to the broader population.

5.Ferritin Level Cereberospinal Fluid in Patients with Tuberculdus Meningitis.

By means of a sensitive radioirnrnunoassay method,ferritin was assessed in the cerebrospinal fluid (CSF) of 50 hospitalized subjects to evaluate its potential value for the diagnosis of tuberculous rneningititis. The rnean ferritin value obtained in the controls was 4.29ng /rnl with ranging between 1.7 and7.4ng/rmL The differences of CSF ferritin content between controls and patients with aseptic rneningitis as well as other non-infectious neurologic disorder were not significant. But rnarked elevation ranging 14 and 88ng/rnl (rnean, 37.4ng/rnl) were observed in patients with tuberculous rneningitis. And the ratio of CSF ferritin to serum ferritin in paients with tuberculous rneningitis was elevated significantly rnore than those in other groups inculuding controls, patients with aseptic rneningitis and other non-infectious neurologic disorder. The level in the serurn and CSF were independent, but that in CSF correlated vrith its total protein content. Results of this study suggest rneasuring CSF ferritin level are a valuable adjunct in differentiating aseptic meningitis from uncomplicated tuberculous meningitis.

6.A Study on Serum Lipid in Hypertension and Ischemic Heart Diseases.

The present study has been undertaken to investigate the values of serum lipids with hypertension and ischemic heart diseases. Serum lipids and lipoproteins were measured from 231 cases of hypertension, 23 cases of angina pectoris, 18 cases of acute myocardial infarction and 67 healthy adults together with normal value as controls. The results obtained were as follows: 1. The mean values of serum lipids in healthy adults were 111.0+/-41.3mg% for triglyceride, 171.2+/-33.5mg% for cholesterol, 175.1+/-36.3mg% for phospholipid, 479.9+/-82.6mg% for total lipid. The values of serum lipoproteins were 219.8+/-46.8mg% for beta-lipoprotein, 93.4+/-53.8mg% for pre beta-lipoprotein and 164.1+/-40.8mg% for alpha-lipoprotein. The values of cholesterol, triglyceride and beta-lipoprotein were gradually increased with aging. 2. The mean values of serum lipids in patients with hypertension were 176.3+/-94.5mg% for triglyceride, 199.7+/-36.9mg% for cholesterol, 207.8+/-38.0mg% for phospholipid, 601.9+/-139.9mg% for total lipid. The values of serum lipoproteins were 266.0+/-73.5mg% for beta-lipoprotein, 147.7+/-89.5mg% for pre-beta-lipoprotein and 187.7+/-56.7mg% for alpha-lipoprotein. The serum lipid values in patients with hypertension were significantly higher than those in healthy controls. 3. The mean values of serum lipids and lipoproteins in patients with angina pectoris were significantly higher than in healthy controls. 4. The serum lipid values in general were higher rather in patients with acute myocardial infarction than healthy controls, but the values of serum phospholipid, pre-beta-lipoprotein and alpha-lipoprotein in them were not significantly higher than those in healthy controls. 5. The values of serum HDL-cholesterol in patients with hypertension, anginal pectoris and acute myocardial infarction were lower than those in healthy controls. The values of serum HDL-cholesterol in all cases were in general higher rather in femal than male.

8.Abdominal Masses in Infants and Children.

Palpation of an abdominal mass in an infant or child presents a challenging problem in diagnosis and treatment. We reviewed the data on 166 patients under age 15 years who admitted to Ped. Dept. of PMC due to palpable abdominal mass in Jan. 1972-July 1977. The results are as follows 1. Of the 57 surgical cases, pathologically confirmed abdominal tumors were 39 cases. Of the 20 retroperitoneal tumors, Wilms tumors were 13 cases, neuroblastomas were 3 cases, polycystic kidney was 1 case, and retroperitoneal teratoma was 1 case. Of the 19 intraperitonel tumors, hepatomas were 3 cases, hepatoblastoma was 1 case, choledocal cysts were 3 cases, mesenteric and omental cysts were 4 cases, malignant lymphomas were 4 cases and ovarian cysts were 3 cases. Other surgical diseases were 2 cases of ascariasis and 2 cases of bezoar. 2. Medical cases were as follows : 35 cases of leukemia, 15 cases of infectious hepatitis, 14 cases of congenital syphilis 7 cases of liver cirrhosis 7 cases of lirerabscesses, 4 cases of miliary Tbc., 3 cases of congenital spherocytosis, 1 case of cryptococosis, I case of osteopetrosis and 1 case of erythroblastosis fetalis. 3. In age distribution, almost all cases(94%) of Wilms tumor and neuroblastoma were under age of 4 and half of medical cases in infancy were congenital syphilis. 4. Of the pathologically confirmed 39 abdominal tumors, 20 cases were retroperitoneal tumor and 19 cases were intraperitoneal tumor. Of the 20 retroperitonel tumor cases, 16 cases were renal origin, 3 cases were adrenal origin, and 1 case was teratoma. Of the 19 intraperitoneal tum orcases, 8 cases were hepatobiliary origin, 3 cases were ovary origin, 4 cases were omental and mesenteric origin and 4 cases were lymphatics origin. 5. Of the 39 abdominal tumor cases, 34 cases (87%) visited the hospital with the chief complaint of palpable abdominal mass. But, of the 94 medical cases, only 16 cases(17%) visited the hospital with the chief complaint of palpable abdominal mass. 6. In some cases presumptive diagnosis on the base of history taking, physical examination, chest X-ray, simple abdominal X-ray. Peripheral blood findings on admission were uncorrect. In 1 case of Wilms tumor, we suspected liver abscess on admission. In 1/3 case of intraperitoneal tumors, we suspected retroperitoneal tumor on admission. We suspected lymphoma on admission in 1 case of ascariasis and 1 case of bezoar. We misdiagnoed 1 case of miliary Tbc. As hepatoma, 1 case of liver cirrhosis as retroperitoneal tumor and 1 case of congenital syphilis as retroperitoneal tumor on admission. 7. In the treatment of malignant abdominal tumor, we tried all possible measures such as surgery, chemotherapy, and radiation therapy. But prognosis of all malignant abdominal tumors were very poor. Only 1 case of Wilms tumor and 1 case of hepatoblastoma were survived at the time of review.

9.Clinical Analysis of Anterior Interbody Fusion Using Kaneda Device in Unstable Thoracolumbar Spinal Lesions(Long Term Follow-up Results).

Recently anterior spinal device(Kaneda device) for the treatment of the unstable spine has been used. We have experienced 33 patients of the unstable thoracolumbar spine, which were composed of 24 cases with burst fracture, 1 case with kyphotic angulation, and 8 patients with tuberculous spondylitis. These 33 patients were treated with and one stage anterior operation consisting of anterior decompression by vertebrectomy and diskectomies, and interbody fusion using autogenous ribs or iliac bone realigment and stabilization with Kaneda device. No patient showed neurologic deterioration after surgery. Follow-up periodes was 6 months to 2.5 years. The anterior spinal instrumentation with Kaneda deviced affored enough stability to enable early ambulation with good aligment and solid fusion.

10.Mammographic and US Findings of Steatocytoma Multiplex: A Case Report.

Steatocystoma multiplex, a cutaneous disorder involving the appearance cysts on the trunk and upper arms, is often inherited as an autosomal dominant trait. The authors present mammographic and US findings of steatocystoma multiplex in a 35-year-old woman with breast masses. On mammograms, multiple uniform thin walled round or oval shaped oil cysts were seen in the peripheral portion of the breasts and axilla, and on US, well-defined or smooth ill-defined round or oval shaped nodules were seen in the subcutaneous layer of the breasts and axilla ; compared with subcutaneous fat, there was iso- or slightly increased echogenicity.
