1.Effects of Reactive Oxygen Metabolite on the Calcium Transport of Cardiac Mitochondria.

BACKGROUND: Intracellular calcium overload is a common final feature of the ischemic-reperfused heart and mediates the genesis of irreversible cell damage. Reactive oxygen medabolites have been known to play and important role as toxic mediators in myocardial injuries resulting from ischemia and reperfusion. In order to investigate the mechanism of intracellular calcium accumulation in the ischemic-reperfused myocardium, the present study observed the possible contribution of the reactive oxygen metabolite to the calcium transport of cardiac mitochondria. METHODS: Mitochondrial were isolated from rabbit hearts. The effects of a reactive oxygen metabolite, H2O2 on calcium uptake and release, redox states of endogenous pyridine nucleotides and glutathiones of mitochondria respiring with succinate were observed. Calcium uptake and release were monitored by dual-wave length spectrophotometer using a calcium indicator, arsenaze III. Contents and redox states of pyridine nucleotides and glutathiones were measured by enzymatic methods using spectrofluorometer and HPLC. RESULTS: Hydrogen peroxide(10-500microM) promoted calcium release dose-dependently from CA++-preloaded mitochondria, but did not affect the mitochondrial calcium uptake. The H2O2-induced calcium release was accompanied by simultaneous oxidation of the pyridine nucleotides and decrease in the content of the reduced form of glutathione(GSH). When mitochondria were treated with BCNU(N,N=bis(2-chloroethyl)-N-nitrosourea) to inhibit glutathione reductase and so as to reduce the GSH content, there were no increase in calcium release from the mitochondria. These results may indicate that H2O2 increases the permeability of cardiac mitochondrial membrane to calcium in association with the changes in redox state of endogenous pyridine nucleotides, but not with that of glutathiones. CONCLUSION: It is suggested that the reactive oxygen metabolites induce the release of calcium from mitochondria by altering the redox state of pyridine nucleotides, and it may partly be involved in the elevation of cytosolic calcium concentration in the ischemic-reperfused myocardial cells.

4.Ender Nailing with Distal Wiring Technique in the Treatment of Intertrochanteric Femoral Fracture

In the treatment of elderly hip fracture, early mobilization as a means of reducing early postoperative morbiyt and morbidity is mandatory. Even though Ender nailing have many advantages, most serious problems with use of Ender nailing method are distal migration of nails, subsequent loss of fixation and limited knee joint motion. To prevent these difficult problems, we have used distal wiring technique with Ender nailing since 1988. In operation, we performed Ender nailing as usual manner under image intensifier and for the prevention of the nail migration, we performed circumferential wiring through the each Ender nail eye and above the adducter tubercle of the distal femur. Forty six cases were available with minimum follow-up of six months which were healed radiologically and clinically (Mean follow up: 21 months). The mean age was 74 years (Range: 52 years-88 years)and showed 26 cases of famale and 20 cases of male. The radiological morphometry using Singh index showed poor bone quality: 36 cases were below Grade III and the fracture type was classified by Tronzo criteria. In results, patients who had Ender nailing with distal wiring showed no distal migration, however one case of proximal migration was observed at the beginning stage of this technique. So, in elderly trochanteric fracture, to reduce the complication of the Ender nailing, especially nail migration, the authors recommend the Ender Nailing with distal wiring technique.

5.Clinical analysis of Tennis Elbow: 148 Cases Analysis

One hundred and forty eight cases of tennis elbow were treated by conservative managements firstly such as rest, medication, immobilization, physical therapy and/or local steroid injection, from Jan. 1985 to Jun. 1994 at Department of Orthopaedic Surgery, National Medical Center. Among the 148 cases, 16 cases who failed conservative managements, were treated with Nirschl & Pettrone operation. The results were summarized as follows, 1. Among the 148 cases, 110 cases(74%) were female, and 61 cases(41.2%) were in the age group 41 to 50 years old, and mean age was 42.3 years old. 2. Ninty nine cases were housewives and only 15 cases were related to sports. 3. The conservative results of one hundred and sixteen patients were graded excellent and good; and of thirty two patients, fair and failure. 4. The operative results of seven patients were graded excellent; of three, good; of four, fair; and of two, failed. 5. Recurence was developed in 18 cases(12.2%), and we obtained excellent and good result in 17 cases with both conservative and operative treatment. Above results suggest that the term, tennis elbow is a misnomer because it occurs more commonly in non-athletes such as housewives than in tennis players. So it seems to be an occupational disease rather that sports injury. Now, we propose to eliminate of the term of tennis elbow and to substitude with new terminology that describes the true understanding and therapeutic orientation of it.

6.Surgical Treatment in Recurrent Tennis Elbow

Tennis elbow is common, and offen disabling problem even in daily living;conservative treatments commonly make satisfactory results, but recurrence is frequent and discouraging. So, surgical treatments are considered in certain cases. The authors report the retrospective analysis of 15 patients treated by nirschl and pettrone procedure. Patients were evaluated with Nirschl and Pettrone assessment method at an average 10.3 months(range, 5 to 32 months0 after surgery. thirteen of 15(87%) patients had more than fair postoperative rating. Two patients had postoperative complications;one with infection and the other with transient mild limitation of elbow motion. No postoperative deterioration of the clinical state was seen. And biopsy specimens of all case showed degenerative changes. Resection of the degenerated site of tendon origin of the humeral epcondyle(The Nirschl and Pettrone procedure) yield satisfactory results in properly selected patients.

7.Production of Superoxide Anion and Damage of Mitochondria Isolated from Ischemic Repertused Heart.

BACKGROUND: Oxygen free radicals have been implicated as a cause of deleterious effects in the setting of coronary reperfusion, and they are believed to be generated by the xanthine oxidase system, from activated neutrophiles and from mitochondria. We evaluate the contribution of mitochondria to the production of oxygen free radicals and clarify the mechanism of cellular damage in ischemic reperfused hearts. METHODS: Mitochondria isolated from the ischemic rabbit hearts were incubated in the reaction conditions with different oxygen tensions. Generation of superoxide anion and activities of defensive enzymes aginst oxidative stress were mesured. RESULTS: Superoxide anion genertion by mitochondria incubated in 21% oxygen condition were 0.54+/-0.09 and 0.27+/-0.04(O2./min/mg protein) in ischemic mitochondria and in control respectively(P<0.05). Activites of defensive enzymes against oxidative stress, superoxide dismutase and glutathione peroxidase, were significantly reduced in mitochondria isolated from either ischemic or reperfused hearts. With the lapse of respiration in 21% oxygen condition, ADP-stimulated state 3 oxygen consumption(306.4+/-31.5 vs 214.4+/-11.4n atoms O/min/mg protein) at 30 minutes, P : O ratio and phosphorylation rate were significantly decreased in ischemic mitochondria. CONCLUSION: Elevation of oxygen free radical generation as well as the reduction of defensive enzyme activities in ischemic reperfused mitochondria are injurious to mitochondrial respiratory function. It may contribute to the mechanism of cellular damage in ischemic reperfused hearts.

8.MR Cholangiography: Usefulness in Obstructive Jaundice.

PURPOSE: Three-dimensional(3D) magnetic resonance(MR) projection imaging was evaluated as a noninvasive alternative to direct cholangiography for the assessment of its reliability in patients with obsructive jaundice. METHODS AND MATERIALS: A heavily T2-weighted gradient-echo sequence(PSIF) was used for 3D MR projecion imaging of the biliary system in five healthy volunteers and 25 patients with obstructive jaundice. The 3D images of the bile ducts were formed by stacking consecutive coronal MR images obtained with a fast imaging method to a maximum-intensity projection algorithm. RESULTS: In the volunteers, MR cholangiography could demonstrate the anatomy of the biliary tract in only two subjects. The extrahepatic and intraheparic bile ducts were well visualized in 23 patients with obstructive jaundice. The leve of obstruction and the grade of dilatation were dipicted with MR cholangiography in all cases. And the cause of obstruction could be determined with MR cholangiography in 18 cases, MR cholangiography was successful in two patients in whom ERCP failed. The gallbladder or dilated pancreatic duct not demonstrated by means of direct cholangiography was demonstrated with MR cholangiography in six cases.

9.The significance of fundus photographs during health mass screening.

BACKGROUND: Fundus photographs performed during health check-up were reviewed to determine the usefulness in mass screening. METHODS: Subjects consistine of 3017 people who had undergone physical health check up at the Health Clinic, Dong-Eui Medical Center in 1997 and whose fundus photographs of both eyes were taken showing clearly visible posterior pole and optic disc were chosen as subjects. Medical records which included cases of reported close exam and causes, actual cases of close exam and final diagnoses. RESULTS: Of the 3017 people, reported close exam was performed in 665 (22.0%). Among them glaucoma was suspected in 460 retinal hemorrhagic lesion in 63, retinal degenerative lesion in 67 and others in 75. There were 187 actual cases(28.1%) of close exam including glaucoma suspect in 79, retinal hemorrhagic lesion 47, retinal degenerative lesion 34 and others 27. Among 187 actual cases of close exam, 140 people (1.3%) were eventually diagnosed as glaucoma suspect (60), retinal hemorrhagic lesion (38), retinal degenerative lesion (21) and others (21). CONCLUSIONS: Fundus photographs are useful in mass screening, but performing close exam to enhance the usefulness of the fundus photographs is necessary.
