1.Differential diagnosis between traction and compression of trachea.

The trachea is a cartilagenous and membranous tubular midline structure with parallel walls. Tracheal deviation may be caused either by traction toward the diseased hemithorax or by compression toward the normal side. Unless an obvious mass is observed radiographically, occasionally it can be difficult to decide whether the trachea has been pushed or pulled from its normal position in the mediastinum. We studied the differences between tracheal deviation patterns in 23 patients with fibroatelectatic pulmonary tuberculosis and 35 patients with elongated and dilated aortas. In cases of retraction of the trachea by fibroatelectatic pulmonary tuberculosis, the diameter of the deviated segment was greater than that of the normal segment and deviation of the wall adjacent to the fibroatelectasis from its normal position was greater than that of the opposite wall. In cases of compression of the trachea by the elongated and dilated aorta, the diameter of the diviated segment was smaller than that of the normal segment and deviation of the wall adjacent to the aortic arch from its normal position was greater than that of the opposite wall. We conclude that these differences between tracheal deviation patterns are useful signs for discriminating retraction from compression. Thus when the trachea is retracted, the deviation of the juxtalesional wall is greater than that of the lesion-free wall, and vice versa.

Deep infection of Achilles tendon is one of the serious complications that occur after open repair of the tendon. It sometimes leads to a very large tendon defect during the course of treatment. We report on a case of massive defect in Achilles tendon, which was successfully treated with Achilles tendon allograft and flexor hallucis longus tendon transfer.

3.Reduced allergenicities of irradiated egg white ovalbumin determined by skin prick test and ELISA inhibition test.

BACKGROUND: Hen's egg is the most frequent cause of food hypersensitivity in infants and young children. The major egg white(EW) proteins are ovalbumin(OVA), ovomucoid(OM), ovotransferrine, and lysozyme. OVA and OM have been generally considered to be the most allergic EW proteins. Food irradiation has been permitted in the fields of agriculture and food handling. Currently, it was suggested that the binding ability of specific IgE to gamma-irradiated OVA was rapidly decreased depending upon the irradiation dose. OBJECTIVE: The purpose of this study is to evaluate the application of food irradiation technology as a method for reducing the allergenicities of EW proteins. METHODS: A total of 10 egg-allergic patients less than 24 months of age were enrolled in this study. OVA was gamma-irradiated at 10 kGy in an aqueous state(2.0 mg/mL). Skin prick test and ELISA inhibition test were done to compare the IgE binding capacities before and after irradiation of OVA. RESULTS: One hundred percent of patients showed positive skin reactions with EW and native-OVA. Negative skin reactions to irradiated-OVA were observed in 7 of 10 patients and remainder 3 showed reduced skin reactions(p=0.001). In IgE-ELISA inhibition test, the IgE-binding capacities of irradiated-OVA reduced 1/80 (50% inhibition concentration: native-OVA-0.1 microgram/mL, irradiated-OVA-8 microgram/mL), respectively. CONCLUSION: We suggest that the allergenicity of OVA can be reduced by the treatment with gamma irradiation.

4.Eaton-Lambert Syndrome with Small Cell Lung Cancer: A case report.

Eaton Lambert Syndrome (ELS) is a disorder of neuromuscular transmission. The defect of neuromuscular transmission is due to decrease in the release of acetylcholine quanta from nerve terminal. This syndrome is frequently associated with bronchogenic carcinoma. The diagnosis is established by electromyography, which characteristically shows 1) low amplitude of evoked compound muscle potential to a single supramaximal stimulus on nerve, 2) significant decremental response at low rates of stimulation 3) marked incremental response at high rates of stimulation. Our patient is 52year old man with dyspnea, coughing and muscle weakness of proximal lower limbs. He has small cell lung cancer and associated with ELS, Superior vena cava syndrome and has metastatic lesion on right supraclavicular lymph node confirmed by pathology. Metastatic mass and SVC syndrome are marked improved following chemotherapy and radiotherapy, however follow up EMG finding does not improved. We are here reporting one case which considered compatible for ELS, with a few elementary reviewed literatures.

5.Treatment of the Distal Femur Fracture with Retrograde Intramedullary Nailing.

PURPOSE: To evaluate the results and complications of the retrograde intramedullary nailing for the treatment of distal femur fracture. MATERIALS AND METHODS: Thirty three patients who received retrograde IM nailing for fractures of the distal femur between October 1998 to December 2003. Average age was 53.8+/-17 (17~86) years. The average follow up period was 19.4 (12~36) months. Clinical information included age, sex distribution, associated fracture and fracture was classified by AO classification. Functional result was evaluated by Schatzker's criteria. RESULTS: The most common cause of injury was traffic accident (60%). The type of fracture were 6 A1 cases, 5 A2 cases, 11 A3 cases, 5 C2 cases, 6 C3 cases by AO classification. Among the 33 cases, 15 cases were excellent, 9 good, 6 fair and 1 failure according to Schatzker's criteria. Average union time was 9.7+/-3.5 months. CONCLUSION: Treatment of distal femur fracture with retrograde intramedullary nailing was useful due to its minimal invasiveness and early range of motion, more rigid fixation.

6.Performance of Wisconsin Card Sorting Test in the Offspring of Schizophrenic Patients.

In order to evaluate whether Wisconsin Card Sorting Test(WCST) could be used to detect the vulnerability markers of schizophrenia, three groups such as offsprings of schizophrenic patients(n=28), offsprings of alcoholic patients(n=18), and offsprings of psychiatrically normal persons(n=41) were examined for their concept formation and abstract thinking by means of WCST.The results were as follows; 1) No significant differences were noted in all variables of the WCST such as number of totol administered trials, total correct response %,total error response $,perseverative response %,perseverative error response %,nonperseverative error response %,conceptual level response %,number of completed category,number of trials to complete 1st category,number of failure to maintain a set,and learning to learn among three groups. 2) There was no difference in the number of cases with extreme low WCST total correct % of lower 10% of the normal controls among three groups. These results suggest that Wcst could be an ineffective instrument for using to detect the vulnerability markers of schizophrenia.

7.Fexofenadine-Induced Urticaria.

Fexofenadine (Allegra(R) 180) is a second-generation antihistamine. It is widely used as anti-allergic drug, which suppresses various allergic reactions mediated by histamines. A few cases of H1-antihistamine-induced urticaria have been reported. Herein, we report a rare case of fexofenadine-induced urticaria which was confirmed by a prick test, oral provocation test, and flow cytometry assisted-basophil activation test.

8.Periumbilical Perforating Pseudoxanthoma Elasticum in a Young Male Patient.

Periumbilical perforating pseudoxanthoma elasticum is a rare acquired disorder of connective tissue. It has been suggested that repeated traumas of pregnancy, obesity, or abdominal surgery promote localized degeneration of elastic fibers. Most cases reported in the English literature occurred in obese, middle aged, multiparous women. We describe a 29-year-old male who had two hyperpigmented plaques on the periumbilical area. He had angioid streaks, but was not obese and did not have hypertension. A skin biopsy revealed transepidermal elimination of degenerated and calcified elastic fibers, which was consistent with periumbilical perforating pseudoxanthoma elasticum. We report an interesting case of periumbilical perforating pseudoxanthoma elasticum that occurred in a young man with no known predisposing risk factors.

9.Treatment of Tibial Plateau Fractures using Ilizarov Fixation (Schatzker Type IV, V, VI).

PURPOSE: To evaluate the effectiveness of Ilizarov fixation in tibial plateua fractures (Schatzker type IV, V, VI), the clinical and radiological results were analysed retrospectively. MATERIALS AND METHODS: Of the tibial plateau fractures (Schatzker type IV, V, VI) which had been treated by using Ilizarov fixatrion method at Dankook university from June 1995 to June 2004, we clinically, radiologically analysed the 47 cases with follow-up study of a mean 38 months. Overall results which were evaluated according to Blokker's evaluation system. RESULTS: The average start time of the range of motion excercise was 4.2 weeks, and the average start time of partial weight bearing was 4.6 weeks. Results which were evaluated according to Blokker's evaluation systems were "satisfactory" in 8 cases (80%) of the type IV fractures, in 9 cases of the type V fracures, and in 18 cases (69%) of the type VI. Overall results were "satisfactory" in 35 cases (74.4%), "unsatisfactory" in 12 cases (25.5%). CONCLUSION: When use Ilizarov fixation in tibial plateau fracture (Schatzker type IV, V, VI), we have many advantages that the early start time of the range of motion, the early start time of weight bearing, the acceptable results of Blokker's evaluation system. Therefore, we conclude that Ilizarov fixation in tibial plateua fracure (Schatzker type IV, V, VI) is effective.

10.Impact of Maximal Sterile Barrier during the Insertion of Central Venous Catheters in Adults Intensive Care Units.

BACKGROUND: Catheter-related bloodstream infections (CR-BSIs) are common, costly, and potentially lethal. The purpose of this study was to ascertain the effect of maximal sterile barrier on CR-BSIs in intensive care units. METHODS: We monitored CR-BSIs in intensive care units of Sunlin Hospital in Pohang, before (September 2005 to May 2006) and after (June to December 2006) implementation of maximal sterile barrier. CR-BSIs were identified by using the definition of Centers for Disease Control and Prevention. RESULTS: During the intervention period, the proportion of conducting maximal sterile barrier was 84%. In the pre-intervention period, 10 episodes of CR-BSIs were recorded out of a total of 1,749 catheter-days, compared to 1 episode of CR-BSI out of a total of 1,277 catheter-days in the post-intervention period. The rate of CR-BSIs was significantly reduced from 5.72 to 0.57 per 1,000 catheter-days (P=0.03). CONCLUSION: Implementation of maximal sterile barrier resulted in a significant reduction in CR-BSIs.
