3.A Study on Perception and Performance of Health Assessment among Clinical Nurses.

PURPOSE: The purpose of this study was to describe perception and performance of health assessment among clinical nurses. METHODS: The sample of this study consisted of 177 staff nurses working at university affiliated hospitals and a self-report questionnaire was used to collect data. Data were analyzed with SPSS WIN 12.0 program. RESULTS: The overall mean scores of perception and performance of health assessment were 3.67 and 1.71 respectively. There were no significant differences according to educational level, working department, and type of health assessment courses taken for perception of health assessment. The variable significantly affecting health assessment performance was the working department. There was a statistically significant correlation between perception and performance of health assessment. CONCLUSION: These findings suggest the need to develop a nursing education strategy to increase the performance of health assessment.

5.Microsatellite Instability and hMSH2 Gene Mutations in Sporadic Colorectal Cancers.

Microsatellites are short nucleotide repeat sequences present throughout the human genome. Alterations of microsatellites, comprising extra or missing copies of these se quences, have been termed microsatellite instability(MSI, genetic instability, replication errors, RER(+) phenotype). To date, at least four genes involved in DNA mismatch repair, hMSH2, hMLH1, hPMS1 and hPMS2, are thought to account for the observation of microsatellite instability in tumor from Hereditary nonpolyposis colorectal cancer (HNPCC) patients. The genetic defect responsible for the MIN+ phenotype in sporadic colorectal cancer, however, has yet to be clearly delineated. The purpose of this study was to determine the presence of MSI in sporadic cancer and to correlate its occurrence with clinicopathological parameters, we have studied six microsatellite loci by use of polymerase chain reaction amplification and denaturing polyacrylamide gel electrophoresis. We found that 20%(9 of 46 cases) sporadic colorectal cancers showed RER at two or several loci(RER+). Microsatellite instability was associated with location of the tumor in the proximal colon 66%(6 of 9 cases) and with poorly differentiated tumor phenotype 56%(5 of 9 cases). In order to better understand the role of somatic alterations within hMSH2 in the process of colorectal tumorigenesis, we examined the most conserved regions(codon 598~789) of this gene in nine patients with MIN spotadic colorectal cancer. 6 patient of RER(+) colorectal ca. patients had a polymorphism which was a T to C base change in the intron sequence at -6 position of the splice acceptor site at the 5'end of exon 13. This particular sequence variation is a polymorphism rather than a mutation which increase cancer susceptability. These data suggest that the genetic instability is detect ed in some colorectal cancers and play an important role in the pathogenesis of sporadic colorectal cancer.

8.Comparisons of the Left Ventricular Ejection Fraction Between M-Mode, Two-Dimension Echocardiographic Studies and Digital Cardiac Imaging(DCI) System in Ventricular Septal Defects*.

BACKGROUND: Left ventricular performance is one of the most important factors for the prognosis of congenital and acquired heart diseases. Measure of left ventricular performance such as chamber volume and ejection fraction are usually deemed most reiable when derived from cardiac catheterization data. Measurement of ventricular volume and ejection fraction by M-mode echocardiography may be compromised by segmental dysfunction or paradoxical septal motion. We compared ejection fraction derived from M-mode and two-dimensional echocardiographic methods to those derived from newly developed DCI (Digital Cardiac Imaging) system to determine the most accurate noninvasive technique for prediction these variables in children. METHODS: The ejection fraction measured by M-mode, Two-dimensional echocardiography was compared with the ejection fraction by DCI system in 22 patients who were admitted to the department of Pediatrics, Dong San Hospital during the period of 8 months from November 1992 to June 1993 for the evaluation of ventricular septal defect. RESULTS: M-mode measured ejection fraction correlated with Two-dimensional echocardiography measured ejection fraction(gamma=0.54). M-mode measuredd ejection fraction correlated with the DCI system measured ejection fraction(gamma=0.52). Two-dimensional echocardiography measured ejection fraction correlated well with the DCI system measured ejection fraction(gamma=0.88). CONCLUSION: 1) For ejection fraction, the best correlation with DCI was achieved by two-dimensional Simpson's method. 2) Two-dimensional Simpson's method appeared a safe and useful noninvasive method of evaluation left ventricular ejection fraction.

9.User Satisfaction of Nursing Information System.

OBJECTIVE: User satisfaction of nursing information system and its user interface were measured in the S general hospital which new electronic medical record system had been introduced to. The new system had been developed based on nursing job's analysis. METHODS: 487 nurses who were working in the S hospital joined to survey through QUIS (Questionnaire for User Interface Satisfaction) about satisfaction of system they used. There were 4 categories-convenience, rapid response, content/configuration, accuracy of information. Those 4 categories were used analytical factors to know how the users were satisfied with its interface and function through the selected 13 system interfaces. RESULTS: With 9 scales of satisfaction score, we measured the satisfaction that the hardware provided. Terminology and system information got the highest score, and user friendly interface, easiness of learning, system performance in high score order. As like as above, we defined 5 scales of user interface satisfaction score. Content, configuration, information accuracy, convenience, rapid responses were identified as analytic categories. With interface satisfaction aspect, the pages which were reconfigured from physician order page to nurse interface got the highest score. And content and page configuration were acknowledged as the highest place. CONCLUSION: This study showed the nurses were satisfied with nursing information system. Also the trend of system development can be revealed.

10.A Comparison between Home Care Nursing Interventions for Hospice and General Patients.

PURPOSE: The purpose of the study was to compare home care nursing intervention activities analyzed by the Nursing Intervention Classification (NIC) system for hospice and general patients. METHOD: For the descriptive survey study, data was collected by reviewing charts of 151 hospice patients and 421 general patients who registered in the department of home health care nursing at K Hospital. RESULTS: According to the NIC system application, there were 2380 total nursing interventions used for the hospice patients and 8725 for the general home care patients. For both sets of patients (hospice vs. general), the most frequently used nursing intervention in level 1 was the Physiological: Complex domain (40.13 vs. 31.06 percent), followed by the Safety domain; in level 2, the Risk Management class (28.4 vs. 27.70 percent), followed by Tissue Perfusion Management; and in level 3, Vital Sign Monitoring (6.18 vs. 4.84 percent), followed by Health Screening. CONCLUSION: The study showed that there was a lack of specialized hospice nursing interventions such as emotional, family and spiritual support, and care for dying hospice patients.
