The present study was performed to investigate the role catecholamine in the genesis of reperfusion injury of ischemic heart. The possible involvement of catecholamine in the xanthine oxidase-linked production of oxygen free radicals was studied. langendorff preparations of rat hearts were made ischemic for 60 min followed by reperfusion. Upon reperfusion norepinephrine(NE) was significantly released into the coronary effluent regardless of oxygenation of the perfusion solution. Both the increased releases of creatine phosphokinase(CPK) and malondialdehyde(MDA) and the production of superoxide anion in the ischemic-reperfused hearts were significantly reduced by the treatment with either reserpine, a catecholamine depletor, or propranolol, a beta-adrenergic receptor blocker. In the reserpinized hearts, infusion of exogenous NE reversed the releases of CPK and MDA and the superoxide anion production to the original higher levels. The releases of CPK and MDA as well as the production of superoxide anion induced by NE in the reserpinized hearts were significantly depressed either by allopurinol, a specific competitive inhibior of xanthine oxidase(XOD), or by the calcium removal from the perfusion solution. Compared with the XOD activity of control ischemic hearts, that of the hearts treated with reserpine or propranolol showed lower activity in the oxygen radical producing 0-form and higher activity in D/0-form. In the reserpinized ischemic hearts, infusion of exogenous NE increased 0-form, but decreased D/0-form of XOD. The changes in XOD activities induced by exogenous NE was prevented by phenylmethylsulfonyl fluoride(a serine protease inhibitor) and pimoxide(a calmodulin inhibitor) as well as by calcium removal from the perfusion solution. It is suggested from the results that in the inchemic-reperfused heart of rat catecholamine participates in D/0 to 0-form conversion of XOD by promoting the calcium-calmodulin-dependent proteolysis and plays a contributing role in the production of oxygen free radical.

In our country, patients with diabetes mellitus are searching for complementary treatments to recover from illness, while they received treatments from the doctor. However, have been evaluated or investigated systematically. This study was carried out to explore application of complementary treatments for patients with DM. For this survey, questionnaires were developed by researchers and the data was collected from July to October of 1999. Among the 223 subjects, there is one general hospital, one oriental hospital, 15 area C.H.P, ahd 2 area health centers. The results were as follows: 1. The total number of cases was 223 and the average age was 62.85 years old and average duration of DM was 8.1 years. The number of patients who had experience with alternative therapies was 145 (65%). The number of those who have not undergone treatments was 78 (35%). 2. The 43.5% of experienced CAM were advised family and relatives. Only 30.3% approved the effect of CAM and 52.5% said that If another a new CAM is introduced, they will try it. 4. Ninety three kinds of complementary treatments were used. Among the used items, 63.7% was various types of plants, 21.6% was animal material and 14.7% was the mixed group. As a single item, Bombyx Mori (Silkworm) was the most frequently used (10.5%) followed by the bean, mushroom, Morus bombycis (mulberry), Ginseng, Commelina Communis (Dalgaebi), Chinese medicine, root of Rosa rugosa (Haedangwha). 5. Among the used items, Trichosanthes kirilowii Max. Eucommia ulmoides Oliver, Commelina Communis, Aralia elata, pine needle, mulberry fruit, root of Rosa rugosa. Ginseng, Lycii Fructus, Dioscorea radix, Polygonatum odoratum, Cassia tora L, Bombyx Mori, loach, Crucian carp were based upon the pharmacological function of effect for control of diabetes mellitus symptom. 6. In the analysis of the relationships between the general characters of the patients with new complementary alternative medicine try and hospital treatment; 1) The shorter group suffered from DM (p=.038), poor Self-MBG (p=.037) and wanted to try new complementary alternative medicine. 2) The group of DM education experience were carried out hospital treatment well (p=.045). In conclusion, further study will be required for the patients experience using alternative therapies as the D-M in terms of holistic view of patients.

BACKGROUND: Intracellular calcium overload is a common final feature of the ischemic-reperfused heart and mediates the genesis of irreversible cell damage. Reactive oxygen medabolites have been known to play and important role as toxic mediators in myocardial injuries resulting from ischemia and reperfusion. In order to investigate the mechanism of intracellular calcium accumulation in the ischemic-reperfused myocardium, the present study observed the possible contribution of the reactive oxygen metabolite to the calcium transport of cardiac mitochondria. METHODS: Mitochondrial were isolated from rabbit hearts. The effects of a reactive oxygen metabolite, H2O2 on calcium uptake and release, redox states of endogenous pyridine nucleotides and glutathiones of mitochondria respiring with succinate were observed. Calcium uptake and release were monitored by dual-wave length spectrophotometer using a calcium indicator, arsenaze III. Contents and redox states of pyridine nucleotides and glutathiones were measured by enzymatic methods using spectrofluorometer and HPLC. RESULTS: Hydrogen peroxide(10-500microM) promoted calcium release dose-dependently from CA++-preloaded mitochondria, but did not affect the mitochondrial calcium uptake. The H2O2-induced calcium release was accompanied by simultaneous oxidation of the pyridine nucleotides and decrease in the content of the reduced form of glutathione(GSH). When mitochondria were treated with BCNU(N,N=bis(2-chloroethyl)-N-nitrosourea) to inhibit glutathione reductase and so as to reduce the GSH content, there were no increase in calcium release from the mitochondria. These results may indicate that H2O2 increases the permeability of cardiac mitochondrial membrane to calcium in association with the changes in redox state of endogenous pyridine nucleotides, but not with that of glutathiones. CONCLUSION: It is suggested that the reactive oxygen metabolites induce the release of calcium from mitochondria by altering the redox state of pyridine nucleotides, and it may partly be involved in the elevation of cytosolic calcium concentration in the ischemic-reperfused myocardial cells.

PURPOSE: This study was to identify factors that influence the life satisfaction of solitude elderly. METHOD: The subject were 100 homebound the elderly living alone(age=76.58) who were live in M city located in Jeollanamdo. Data were collected from June 1 to September 30, 2002. The instrument used for this study were a survey of general characteristics, residential environment, MUNSH, Health Self-rating scale, ADL, loneliness and POMS. The data was analyzed using descriptive statistics, t-test, and stepwise Multiple Regression. RESULT: In general the perceived health status was poor, but ADL was moderate., loneliness was high but mood and life satisfaction were low. In regression analysis, life satisfaction was significantly influenced by mood(51%), ADL(4%), loneliness(3%), and perceived health status(1%). These variables explained 59% of the variances in the life satisfaction.CONCLUSION: The result suggests that health care professionals should give more attention to helping the elderly raise their life-satisfaction. A further study is necessary to find out an effective nursing intervention for a better those in a comfortable residential environment, decreasing the loneliness and to promoting the mood for those elderly who live alone.

This study was to investigate the various parts of the mandible related to inferior alveolar nerve conductive anesthesia in 228 dry skulls of Korean adults. Five morphological variations were observed and four measurements were undertaken. The following results were obtained. 1. The Position of coronoid notch in the anterior border of the ramus of mandible is classified into three types and percentage of each type was as follows : Type I (Coronoid notch in the middle part of the anterior border of the ramus of mandible) : 83.3% Type II (Coronoid notch in the inferior part of the anterior border of the ramus of mandible) : 12.7% Type III (No coronoid notch in the anterior border of the ramps of mandible) : 4.0% 2. The position of the lingula of mandible to the imaginary line of the molar occlusal plane is classified into three types and percentage of each type was as follows : Type I (Above the imaginary line of the mandibular molar occlusal plane) : 87.3% Type II (Coincide with the imaginary line of the mandibular molar occlusal plane) : 8.6% Type III (Below the imaginary of mandibular molar occlusal plane) : 4.3% 3. The position of the lingula of the mandible in the internal oblique line-posterior border of the ramus of mandible dimension is classified into three types and percentage of each type was as follows : Type I (Anterior part in the internal oblique line-posterior border of the ramus of mandible dimension) : 82.9% Type II (Middle part in the internal oblique line-posterior border of the ramus of mandible dimension) : 11. 4% Type III (Posterior part in the internal oblique line-posterior border of the ramus of mandible dimension) : 5.7% 4. The position of the apex of the lingula of mandible to the deepest point of the coronoid notch in the anterior border of the ramus of mandible is classified into three types and percentage of each type was as follows : Type I (Coincide with the deepest point of the coronoid notch) : 82.0% Type II (Above the deepest point of the coronoid notch) : 13.2% Type III (Below the deepest point of the coronoid notch) : 4.8% 5. The position of the apex of the lingula of mandible to the imaginary line of the mandibular molar occlusal plane is classified into three types and percentage of each type was as follows : Type I (Above the 8mm) : 65.7% Type II (Above the 5mm) : 18.6% Type III (Above the 10mm) : 15.7% 6. Acute osteomyelitis is usually shown as a localized area of increased activity (hot uptake) in the infectious focus on bone scintigraphy. In our patient, absence of radioactivity (cold lesion) was noted in the distal metaphyseal and diaphyseal regions of his right femur. Initial x-ray was interpreted as normal except for mild soft tissue swelling in the right thigh. The lesion was confirmed as an acute osteomyelitis with subperiosteal abscess on surgery. Staphylococcus aureus was the etiologic organism. We describe a case of acute osteomyelitis in a one-year-old boy shown as a cold lesion on bone scan.

This study was the done to describe health behavior and determine affecting factors in middle aged women. The subjects of this study were 306 middle aged women, living in M city, during the period from June 28th to August. 31st 1999. The instruments for this study were Health Behavior Assesment tool developed by Kim (1998), Self esteem scale developed by Rosenberg(1965), the perceived health status scale by Lawston, et al.(1982), the Quality of life by Ro(1988), the attitude toward Climacteric symptom by Ji(1983). The data were analyzed using descriptive statistics, t-test, Pearson correlation coefficients and stepwise multiple regression. The result of this study are as follows; 1. The total mean score for Health behavior was 2.51(range 1-4). The mean scores on the subscale were 2.83 for nutrition, 2.81 for stress management, 2.71 for limitation of liking in the menstrual group, health behavior was 2.64. The mean scores on the subscale were 2.98 for nutrition, 2.85 for stress management, 2.82 for limitation of liking, 2.80 for energy conservation in the menopausal group, 2. The mean scores for cognitive-perceptual variables in the menstrual group were perception of health status: 2.61, quality of life: 3.17, self-esteem: 2.59, and attitude for climacteric symptom: 3.02. In the menopausal group the scores were perception of health status: 2.41, quality of life: 3.10, self-esteem: 2.62, attitude for climacteric symptom: 3.06. 3. Health behavior and self-esteem(r=.269, p=.000; r=.205, p=.042), attitude for climacteric symptom(r=.192, p=.005; r=.545, p=.000), quality of life(r=.385, p=.000; r= .195, p=.050) and health behavior were correlated positively, and the perception of health status and health behavior were correlated negatively(r=.-135, p=.050; r= -.207, p=.040 ) in the menstrual group and menopausal group. 4. Perception of health status, quality of life, age, self-esteem, job and marital status explained 33.7% of the variance for health behavior in the menstrual group; self-esteem and education explained 33.1% of the variance for health behavior in the menopausal group. In conclusion, health behavior should be considered when developing nursing strategies for middle-aged women, especially when dealing with perception of health status, quality of life, and self-esteem.

The use of antituberculous drug made it possible to develop the radical operative procedure in the treatment of the spine tuberculosis. The anterior fusion is the most frequently used surgical measure in the treatment of the spine tuberculosis but several problems were observed not infrequently. One hundred and twenty-nine cases of childhood spine tuberculosis were treated by the anterior spinal fusion for the period of ten years from 1968 to 1977 at Seoul National University Hospital, and following results were obtained. Problems of the anterior spinal fusion could be divided into two categories; one is the complications and the others are difficulties in the surgical procedure. Complications of the anterior spinal fusion were divided into early and late. Early complications include graft failure, pulmonary problems, cord compression and spinal fluid leakage. Late complications include increasing kyphosis, draining sinus and graft failure. Degrees of increasing kyphosis were the greatest in the age group between the and five years, and greater degrees of increasing kyphosis were observed in more joint involvement. Increasing kyphosis were observed more frequently in longer follow up. The causes of increasing kyphosis could be considered as the relative discrepancies between the growth of the anterior and posterior comparment, and graft failure.
