2.Xerderma Pigmentosum: Report of a Case.

A case of xeroderma pigmentosum complicated with squamous cell carcinoma is presented. A 15 year old male has had a history of a single walnut-sized tumorous growth of 6 months duration on tip of his nose and multiple light to dark brown pigmented macules on face, ears, neck, upper chest, extensor surface of both foreams and dorsal hands of about 14 years duration, recognizable photosensitivity of the cutaneous lesions and photophobia and lacrimation since early childhood. Xeroderma pigrnentosum was suspected under clinical background. Biopsy was performed on the facial pigmented lesions and on the tumorous mass on the nose, which revealed xeroderma pigmentosum and squamous cell carcinoma, respectively. The tumor was excised totally under local 2% procaine anesthesia, resulting in healing with scar formation and no recurrence was observed during 6 month's follow up. Dermabrasion of the entire face was done, with complete removal of the xerodermic skin lesions, but they all recurred after about 1 month.

4.A Study on Morphology and Size of the Maxillary Lateral Incisor in Korean Adult.

Authors examined the morphology and size in the maxillary lateral insicor from Korean adults and the following conclusion have been drawn. 1. In the shapes of labial surface in maxillary lateral incisors, the quite prominent middle lobe, moderate prominent middle lobe, less prominent midd1e lobe were 20.60%, 69.85%, 9.55%, respectively. 2. In the shapes of incisal margin in maxillary lateral incisors, the arc shaped to the distal, round, nearly straight were 24.96%, 60.20%, 15.11%, respectively. 3. In the shapes of incisal angle in maxillary lateral incisors, the equal sized mesioincisal angle and distoincisal angle, larger distoincisal angle, much larger distoincisal angle were 8.36%. 67.34%, 24.30%, respectively. 4. In the shapes of lingual surface in maxillary lateral incisors, the marked shovel (Ms), semi-shovel (Ss), trace shovel (Ts), no shovel (No) were 43.00%, 33.50%, 19.50%, 4.00%, respectively. 5. The size of the maxillary lateral incisors were as follows. 6. In comparison with Wheeler's data, crown length, crown width and crown thickness were larger in Korean adult, but the degree of cervical curvature were smallest.

5.Therapeutic Trial of Dinitrochlorobenzene ( DNCB ) Sensitization in Patients with Warts.

Warts are benign turnors caused by oncogenic human papovavirus which has been clearly identified on electronmicroscopy, although repeated atternpts at culture have not been successful. Various therapeutic modalities have been tried for the treatment of warts, however thete is no saisfactory method and the clinical course as well as the prognosis is always variable. Recently several authors have reported successful therapeutic results in the treatment of warts by DNCB sensitization method based on the findings that warts are often regressing spontaneously and that the spontaneous regression seems to depend on host's ability to mount an immune response against the wert virus. The present study was undertaken to evaluate the DNCB sensitization method for possible routine use in the treatment of warts. Fifteen cases including five verruca vulgaris and ten verruca plana juvenilis patients were sensitized with 1000ug of DNCB in acetone solution on either side of their inner forearms. Seven or ten days after sensitization, several warts, which were unintentionally selected, were challenged with 0.05 to 0.1ml of weaker DNCB solution(50ug-25ug/0.1ml)at weekly interval. Four patients with verruca vulgaris and three patients with verruca plana juvenilis did not return after 1 to 4 times of challenge, however six out of these 7 patients showed a slight decrease in size and number of the warts. One patient with verruca plana juvenilis developed marked irritative and delayed cutaneous hypersensitivity reaction to the 3 times of challenge, so that this patient was excluded from further study. The remaining 7 patients consisted of I verruca vulgaris and 6 verruca plana juvenilis patients showed complete disappearance of all warts including those which were never challenged, after 1 to 13 times of DNCB challenge application. The authors reviewed the possible therapeutic mechanism of DNCB sensitization in the wart patients, the possible advantage and disadvantage of this method and concluded that this method can be cautiously used in seleted patients with warts whose response to the conventional treatment methods are poor.

6.A Study on the Role of Catecholamine in Reperfusion Damage of Ischemic Heart in Rat : Effect on Xanthine Oxidase Conversion.

The present study was performed to investigate the role catecholamine in the genesis of reperfusion injury of ischemic heart. The possible involvement of catecholamine in the xanthine oxidase-linked production of oxygen free radicals was studied. langendorff preparations of rat hearts were made ischemic for 60 min followed by reperfusion. Upon reperfusion norepinephrine(NE) was significantly released into the coronary effluent regardless of oxygenation of the perfusion solution. Both the increased releases of creatine phosphokinase(CPK) and malondialdehyde(MDA) and the production of superoxide anion in the ischemic-reperfused hearts were significantly reduced by the treatment with either reserpine, a catecholamine depletor, or propranolol, a beta-adrenergic receptor blocker. In the reserpinized hearts, infusion of exogenous NE reversed the releases of CPK and MDA and the superoxide anion production to the original higher levels. The releases of CPK and MDA as well as the production of superoxide anion induced by NE in the reserpinized hearts were significantly depressed either by allopurinol, a specific competitive inhibior of xanthine oxidase(XOD), or by the calcium removal from the perfusion solution. Compared with the XOD activity of control ischemic hearts, that of the hearts treated with reserpine or propranolol showed lower activity in the oxygen radical producing 0-form and higher activity in D/0-form. In the reserpinized ischemic hearts, infusion of exogenous NE increased 0-form, but decreased D/0-form of XOD. The changes in XOD activities induced by exogenous NE was prevented by phenylmethylsulfonyl fluoride(a serine protease inhibitor) and pimoxide(a calmodulin inhibitor) as well as by calcium removal from the perfusion solution. It is suggested from the results that in the inchemic-reperfused heart of rat catecholamine participates in D/0 to 0-form conversion of XOD by promoting the calcium-calmodulin-dependent proteolysis and plays a contributing role in the production of oxygen free radical.

8.Therapeutic Trial of BCG in Patients with Psoriasis.

There have been reported some important findings in immunology of psoriasis, such as lower than normal level of IgM, defective humoral immune, responsiveness to newly administered antigens during cytotoxic drug therapy, lower than normal incidence of active sensitization with DNCB, prolonged skin heterograft survival and presence of anti-IgG antibody in peripheral lymphocytes. These findings are strongly suggestive of some humoral as well as cellular immunne dysfunction in patients with psoriasis. The present study was undertaken to evaluate the therapeutic effectiveness of BCG, a potent nonspecific immune stimulator, in patierts with psoriasis. Total 41 cases of more than 2 years' duration of illness and with widespread involvement and having had history of various previous treatment were slelcted at the Department of Dermatology, National Medical Center during October, 1973 and October, 1974, Liquid BCG(manufactured by NIH, Korea) was administered intradermally in a starting dose of 0.1 ml(0.01 mg in dry weight) weekly. The dosage of BCG was increased to 0.3 or 0.5ml after 3 or 5 injections and this dosage was maintained until some clinical changes were observed. The mean injection time to each patient was 20. 7.The results were as follows: 1. Eighteen cases showed no therapeutic response or aggravation of the lesions. 2. Six cases showed good therapeutic effect, resulting in only a few small lesions remained on elbows, knees or scalp areas. 3. Fourteen cases showed excellent therapeutic effect, that is complete disappearance of the lesions. During 2 to 6 months'follow up period, 7 cases showed relapse and the remainders are still under observation. The mechanism of influence of BCG on the clinical course of psoriatic patients is not determined, however, possible mechanism was discussed.

10.Effect of Ischemic Preconditioning on Catecholamine Release from the Isolated, Ischemic Reperfused Hearts of Rats.

BACKGROUND: Ischemic preconditioning reduces the infarct size and the severity of arrhythmia in a post-ischemic reperfused heart although the detailed mechanism is unknown. In the ischemic heart, a large amount of catecholamine is released from the adrenergic nerve terminal and this aggravates cell destruction and arrhythmia. In this study, the possibility for ischemic preconditioning to inhibit the release of endogenous catecholamine from the ischemic heart was tested to investigate the probable cardioprotective mechanism of ischemic preconditioning. METHODS: In the isolated, Langendorff perfused rat hearts, we observed the protective effect of ischemic preconditioning against post-ischemic reperfusion injury, and measured the amount of catecholamine released into coronary effuent. In addition, we observed the effect of catecholamine depletion on reperfusion injury in non-preconditioned and preconditioned hearts. RESULTS: During the reperfusion(20min) after ischemia(30min), the cardiac function was markedly depressed with the development of severe contracture. In the heart preconditioned by three sequential episodes of 5min ischemia and 5min reperfusion, the reperfusion contracture decreased significantly and the cardiac function was almost recovered to normal after 20min reperfusion. The release of lactate dehydrogenase was also decreased in the preconditioned heart. The release of endogenous catecholamine was abruptly increased immediately after the reperfusion and the release was exponentially decreased throughout the reperfusion period. THe pattern of catecholamine release was much different from that of lactate dehydrogenase release. In the preconditioned heart, the release was significantly decreased to about half of that in non-preconditioned t\heart. Endogenous catecholamine depletion by reserpine treatment did not affect the post-ischemic functional recovery in both non-preconditioned and preconditioned hearts. CONCLUSION: It is suggested from these results that ischemic preconditioning inhibis the release of endogenous catecholamine during ischemic period, which may be partly related to cardioporotective effect of preconditioning in ischemic and reperfused heart.
