1.Giant Intra-articular Osteochondroma of the Knee: A Case Report

The giant intra-articular osteochondroma of the knee is very rare and is probably result of synovial metaplasia of the fibrous capsule or of the adjacent connencive tissue of a joint, and of the fragment of articular cartilage. The gross findings and histology are consistent with the osteochondroma. The authors experienced a case of giant intra-articular osteochondroma of the right knee in a 19 years old man who had history of knee injury when he was 16 years old. The mass was excised and the symptome was relieved.

2.A Clinical and Serologic Study of 21 Cases of Tsutsugamushi Disease Confirmed by Serologic test.

Tsutsugamushi disease is an acute febrile disease caused by Rickettsia tsutsugamushi, and which has been reported with increasing frequency thorough the nation since 1986. We experienced 21 cases of Tsutsugamushi Disease diagnosed with serologic test occurring in Taegu city and Kyungpook province during October-November, 1989. The results of survey are as follow. 1) Of 21 cases, 12 (57%) were males and 9 (43%) were females, and the peak incidence was the 4th decade. 2) The outbreak was in October to November and the peak incidence was in October. 3) The most frequent symptoms were fever and chill (100%), myalgia (95%), headache (90%), Eschar and rash were observed in 18 patients (86%) and the eschar was detected in all over the body, especially thorax (33%) and lower extremity (22%). 4) Laboratory features were SGOT elevation (83%), SGPT elevation (61%), LDH elevation (67%), leukocytosis (38%). 5) Indirect immunofluorescent antibody test was done in 18 patients and the antibody titer was above 1:320 in all patients. 6) The chloramphenicol, tetracycline or doxycycline regimens were very effective and mean duration of defervescence from initiation of therapy was 1.3 days. 7) The complication such as meningitis or shock, was not seen.

3.An Outbreak of Tsutsugamushi Disease in Vicinity of Taegu City and Kyungpook Province in 1988.

Since the first cases of tsutsugamushi disease reported in Korea in 1986, many cases has been reported and its occurrence has been increasing. Only 2 cases has been reported in Taegu city and Kyungpook area. We experienced an outbreak of 26 cases of tsutsugamushi disease in this area and their clinical manifestations are; 1. The outbreak occurred in September to November in 1988 and its peak occurrence was in October. 2. Mean age was 52 years old (18 to 69 years old) and peak incidence was in the 6th decade. 3. Major symptoms and abnormal signs are fever/chills (88%), myalgia (65%), headache (54%), nausea and vomiting (31%), and abdominal pain (27%). The eschar was detected on lower part of body in most of cases, and more frequently in male (M:F 100 vs 58%). 4. Patients were treated with tetracyclines (TC) and/or chlorampenicol (CM) and mean duration of defervescence from initiation of antibiotic therapy was 2.1days with TC and 2.5 days with CM. 5. Complications are 2 cases of meningitis and 1 case of shock, and all cases were recovered without any sequelae. As above, tsutsugamushi disease occurs in Taegu city and Kyung Pook area as other part of Korea and clinical manifestations are similar to other reports.

4.Affinity for 57Co-Vitamin B12 by a Wide Histologic Variety of Tumor Types in Mice.

The search for tumor-avid agents for use in nuclear medicine imaging is an ongoing field of importance. The purpose of this study was to determine the affinity for radiolabeled vitamin B12 by a wide histologic variety of tumor types in mice. Seventeen different types of tumor were grown subcutaneously in female Balb/C or Balb nu/nu(nude) mice. When the tumors reached about 1 cm in diameter, mice were injected intraperitoneally with 57Co-vitamin B12. Twenty-foul hours later, the mice were sacrificed. Organs and tissues were removed, weighed, and activity per mg determined by gamma counter. Values represented cpm/mg tissue that was normalized to 20 grams body weight for each mouse. A wide variety of tumor types showed significant uptake and concentration of 57Co-vitamin B12, as evidenced by tumor:tissue activity ratios. For many tissues of great importance in terms of background(bone, muscle, blood), the tumor:tissue activity ratios of uptake were high. These data strongly suggest that further efforts to evaluate the utility of radiolabeled adducts of vitamin B12 for clinical use in oncologic imaging are warranted.

5.Simple Auxiliary Liver Transplantation without Bile Duct Reconstruction in the Rat.

There has been numerous experimental liver transplantation models in the rodent : orthotopic versus hetrerotopic; whole liver versus partial liver; microvascular suture versus cuff method in technical aspects. Several simplified modifications were introduced for experimental liver transplantation for improving animal-survival rate. Liver transplantation without arterial reconstruction, the cuff tchnique or stent method were the examples of simplifing modification. In practice, complications related to biliary reconstruction are frequent causes of intraabdominal infection in our rat liver transplantation. Here we propose using simple liver transplantation model of partial heterotopic liver transplantation without artery and bile duct reconstruction in the studies which need only observation of immediate graft survival or very short-term follow-up. Here we examined the serial histologic changes of auxiliary liver grafts without biliary reconstruction during short period after transplantation. Until 7 days of transplantation, bile duct proliferation was confined in the portal area of grafts and did not distort the lobular structure. Early graft-survival and rapid immunologic response might be evaluated by this easy transplantation model.

8.Experience of Peer Support Work among People with Mental Illness in the Community:A Grounded Theory Approach

Purpose:
This study discovered a substantive theory of the experience and process of peer support work among people with mental illness.
Methods:
The participants were members of community-based mental health facilities and had been working as peer supporters for more than six months. The data were collected through in-depth interviews with twelve participants and analyzed using Corbin and Strauss’s grounded theory approach.
Results:
The core category was “becoming a healer going with patients in the journey of recovery,” and the core phenomenon was “identity confusion as a peer supporter.” The causal conditions were “starting peer support work without certainty” and “standing at the boundary between the therapist and patient.” The intervening conditions were “willingness to become a successful peer supporter,” “feeling a sense of homogeneity with the patient,” “accepting the mental illness,” and “support from people around.” The action and interaction strategies were “letting go of greed,” “being open about oneself,” “developing professional skills,” “maintaining wellness in the body and mind,” and “being with the patient.” The consequences were “becoming a useful person,” “changing attitude toward life,” “expansion of the sense of self-existence,” “recovering from mental illness,” and “discovering a role as peer supporter.” Finally, the substantive theory of “becoming a healer going with patients in the journey of recovery” was derived.
Conclusion
This study provides a holistic understanding of peer support work and the implications of interventions to help people with mental illness in a person-centered recovery process.
9.Job Retention Process among Working People with Mental Illness: A Grounded Theory Approach

The participants were members with mental illness at the S Community Mental Health Center in Gyeonggi Province and who had been working for more than six months. The data were collected through in-depth interviews with 11 participants between June 27 and August 20, 2018. The data were analyzed through Corbin and Strauss’s grounded theory method.
Results:
The core category was struggling to take root in the community as a productive member. The core phenomenon was the desire to be a productive person, and the causal condition was the willingness to change for a purposeful life. The action and interaction strategies included maintaining regular living patterns, maintaining medication, developing one’s tips for self-management, and self-approval. The intervening conditions were difficulties in forming social relationships, presence of symptoms, social resources, and acceptance of one’s mental illness. The consequences were restoration of family relationships, healthy pleasure through work, social inclusion, development of self-worth, and transition to an independent person.
Conclusion
Working people with mental illness are struggling to take root in the community as a productive member. This study suggests that a holistic understanding of the job retention experience among people with mental illness is required. The findings will provide the basis for developing interventions that can improve job retention among working people with mental illness.
