1.The Measurement of the Contamination Period about the Storage Environment of Ethylene Oxide Gas Sterilized Packs Using in the Operating Room.

The contamination period of the sterilized pack is influenced by the storage environment such as air contamination, temperature, humidity and the quality of the wrapping material. This study was conducted to provide basic data, for the expiration date of the EO gas sterilized pack (which was wrapped with polypropylene / polyester one side and paper the other side) by investigating the storage environment and monthly contamination period. The duration of study was 12 months, from Dec. 1, 1997 to Nov. 30, 1998. A total of 144 EO gas sterilized packs were kept in six storage cabinets of storage area after selecting four Operating Room, the Central Supply Department and General Surgery Clinic of one University Hospital in Taegu city. The samples were cultured in fluid thioglycolate media 30m1 in 37degrees C incubator for 48 hours. Air microbial bioburden in the six storage area and storage cabinet was monitored by exposing blood agar plate for fit teen minutes and culture were observed at 37t incubator for 2 thours. Temperature and Humidity were investigated four times(8 am, 2, 5, & 10 pm) a day and the mean values calculated. The results of this study are as follows 1. There was no contaminations in EO gas sterilized packs for 12 months observation. 2. The mean air contamination rate of storage area and cabinet in Operating Room was 4.9 CFU and 2.4 CPU during observation period. The mean air contamination rate of storage area and cabinet was 4.2 CPU and 2.2 CPU each during observation period. The rate of air contamination in storage room was at least twice than that of storage in Operating Room. The period that showed highest air contamination was in Feb. 98 as 9.2 CPU and the lowest in Mar. 98 as 1.1 CPU. 3. The predominant microorganisms were gram post five staphylococci, gram negative staphylococci in all storage environment, gram positive bacilli, gram negative bacilli in C cabinet of Operating Room and storage area in General Surgery Clinic, Fungi in storage area of Operating Room. 4. The mean temperature was 24.9+/-0.6degrees C(mean standard deviation) in storage area and 24.8+/-0.5degrees C in storage cabinet of Operating Room. The temperature was not differentiated between storage area and cabinet of Operating Room. The mean humidity was 63.6 7.3% (mean standard deviation) in storage area, 663+/-7.9% in cabinet of Operating Room. The humidity was observed high rate in storage area than cabinet of the six area environment. There was no growth of microorganism in sterilized packs during the observation. So we concluded that the contamination period of EQ gas sterilized packs was over 12 months. The high rate of air contamination in storage area might increase the degree of contamination of sterilized packs and the infection rate of operative wound. So the methods which could reduce the air contamination should be developed.

2.Analysis of Review Contents of the Submitted Papers in Journal of Korean Academy of Nursing: Focus: The Submitted Papers in 2003.

PURPOSE: The objective of the study is to analyze the review contents of reviewers for the submitted papers in the Journal of Korean Academy of Nursing in 2003. METHOD: The review contents of the 165 papers were selected 217 papers that were submitted in 2003. Among those 165 papers, the 21 papers belonged to the 'Do not publish' list and 17 papers, 'Revise manuscript and resubmit', list and the 94 papers, 'Publish if revisions are made' list. There are more than two level differences among the four levels of decision in acceptance of publication in 33 papers. RESULTS: The analysis of the review contents for the papers were suggested according to review categories : introduction, method, results, discussion and conclusion. In addition, if papers had more than two levels of review they were rated poor accord or inconsistent. CONCLUSION: For the quality of the academic journal and development of the nursing science, it is important to disseminate and publish the research paper. Therefore, review of the submitted paper is also important. Implications for the profitable review were suggested in the study.

3.A Comparative Study between Rheumatoid Arthritis Patients and Healthy Persons on Activities of Daily Living, Fatigue and Depression.

This study was conducted to compare activities of daily living, fatigue and depression between rheumatoid arthritis patients and healthy persons. The subjects consisted of 53 rheumatoid arthritis patients and 53 healthy persons at a university hospital in Daegu City. Data were collected by means of structured interviews with questionnaires from July 20, 1999 to August 25, 1999. The instrument used in this study were the activities of daily living scale developed by Katz et al. (1970) and Barthel(1973), Multidimensional Assessment of Fatigue by Belza et al.(1995) and CES-D(Center for Epidemiologic Studies-Depression) scale. Analysis of data was done by use of descriptive statistics, Pearson Correlation, Chi-square test, t-test, ANOVA, MANCOVA and Duncan with the SPSS program. The major findings are summarized as follows: 1. The first hypothesis that the rheumatoid arthritis patients will have a lower degree of activities of daily living than the healthy persons was supported (F=4.584, p=.035). 2. The second hypothesis that the rheumatoid arthritis patient will have a higher degree of fatigue than the healthy persons was supported (F=7.799, p=.006). 3. The third hypothesis that the rheumatoid arthritis patients will have a higher degree of depression than the healthy persons was supported (F=4.768, p=.031). With the above results, it can be concluded that rheumatoid arthritis patients had a lower degree of activities of daily living and a higher degree of fatigue and depression than the healthy persons. Therefore, by providing appropriate nursing intervention, activities of daily living would be much better and fatigue and depression would be alleviated.

4.Studies of Air Pollution and Noise in Urban Korea.

Air pollution in the traffic areas in Seoul during the winter season was studied. The average level of sulfur dioxide concentration was 0.24 ppm through the day. Lowest was 0.14 ppm at 2~4 P.M. when the highest vertical temperature gradient was indicated, and 0.33~0.29 ppm during 10 P.M. to 8 A.M. at night. Average level of dust was 4.65 mppcf (2.1~12.8 mppcf) and higher levels were shown at 10 A.M., 6 P.M. and 10 P.M. as 5.44, 4.73 and 5.01 mppcf respectively. The noise level in the main traffic areas was 73.5 db in the average during this survey, 75.1 db in the daytime and 65.6 db in the night (11 P.M. ~7 A.M.) in the average. Various air pollutants in auto-exhaust gases and noise of automobiles were studied, since those are main sources of nuisance in the traffic areas. Carbon monoxide in the "Corona" taxi, formaldehyde in the jeep, and carbon monoxide, formaldehyde and sulfur dioxide in the "Senara" taxi were measured respectively in high concentration and there was no significant difference in other gases among the various type of vehides. As regards average Overall noise level at ordinary driving speeds, heavy vehicles emitted much more noise (30 KPH) (82.2~83.5 db) than light vehicles such as taxi (40~5O KPH) (71.5-80.2 db). The noise level when starting heavy vehicles was higher 88.5 db(94.2 phon) on the average and that of light vehicles was lower 69.4 db (76.2 phon). There were two types of automobile horn noise in the octave band; one was in 1,000 cps and the other 2,000~4,000cps. The average horn noise level of automobiles was 98 db (107.8 phon), with 106 db (11.05 phon) maximum and 88.8db (99.5 phon) minimum. In the epidemiological study of CO poisoning in urban areas, there were 260 deaths out of 532 patients in 201 incidents of poisoning occurring on a total of 128 days during January 1964 to Dec. 1966. The concentration of co in the main living rooms and kitchens was detected as 58 and 112 ppm respectively, and the level of carboxyl-hemoglobin in the blood of housewives was relatively high, 11.2% on the average. In outside air maximal concentration of CO was 80 ppm in Shinchon-Dong an urban area.

6.A Case of Cushing's Syndrome Associated with Ectopic ACTH Production in Patient with Small: cell Lung Cancer.

About 5% of patients with SCLC have the ectopic production of ACTH. Ectopic ACTH production of SCLC is suspected when patients of SCLC have unexplained metabolic alkalosis and hypokalemia. Most patients lack the classic feature of Cushing's syndrome. According to the recent report, they have poor prognosis, which median survival is less than 4 months and associated with a high rate of complication during chemotherapy. Also a case of paraneoplastic CRH production with SCLC can mimic the ectopic ACTH syndrome hut it can be distinguished by immunohistochemistry or direct measurement of serum CRH level. We report here a case of small cell lung cancer associated with Cushing's syndrome of ectopic ACTH production. That is immunohistochemically provened by staining with ACTH.

7.A Comparative Study on the Effects on Urinary Incontinence between Pelvic Floor Muscle Exercise and Magnetic Stimulation Therapy.

PURPOSE: To Perform a randomized comparative study investigating the effects of Pelvic Floor Muscle Exercise(PME) and Magnetic Stimulation Therapy(MST) and to identified the problems in each of PME & MST. METHOD: Forty-nine patients with mild stress incontinence were randomly assigned to either of two treatment groups (24 patients in the PME group and 25 in the MST group). The PME group had a video exercising program for 40 times every day during 6 weeks. The MST group was treated with BioCon-2000TM, 2 times/week for six weeks. Pre-test and post-test were performed by Prineometer, 1-hour pad test. and Jackson's BFLUTS questionnaire of Jackson. Collected data were analysed using SAS 9.1 by frequency, Kolmogorov-Smirnov Z. Chi-Square-test, t-test, Fisher Exact probability test, Paired t-test, and Wilcoxon's rank sum test. RESULTS: In comparison between before and after PME and MST, statistically significant difference was observed in maximal vaginal pressure, duration of vaginal contraction, amount of urine, and symptom of urination. In the comparison of the effectiveness between PME & MST, only the maximal of vaginal pressure (Z=2.58, p=.010) was significantly different. CONCLUSION: The factor more effective in the MST group than in the PME group was high compliance.

9.Silicone Intubation and Dacryocystographic Finding in Incomplete Nasolacrimal Duct Obstruction.

PURPOSE: To assess the relationship between the internal diameter of the nasolacrimal duct and the success rate of silicone tube intubation in incomplete nasolacrimal duct obstruction patients with epiphora. METHODS: Dacryocystography was performed 80 patients with symptoms of epiphora and the anteroposterior (AP) diameters and the lateral diameters of the nasolacrimal ducts were measured. These measurements were compared between the group of patients who demonstrated improvement after the operation, and the group without symptom improvement. RESULTS: The mean length of the nasolacrimal duct in the group with nasolacrimal stenosis was 11.7+/-0.1 mm, the mean lateral diameter was 1.0+/-0.2 mm, and the mean AP diameter was 1.1+/-0.1 mm. The AP diameter of the symptomatic nasolacrimal duct in the group with successful postoperative results was 1.1+/-0.1 mm, and 0.9+/-0.1 mm in the group with unsuccessful results. The group with successful postoperative results had an average of 18.1+/-7.0% stenosis in the side of the eye with nasolacrimal duct stenosis compared with the asymptomatic side, and the group with unsuccessful postoperative results had an average of 34%+/-3.6% stenosis. CONCLUSIONS: In cases with nasolacrimal duct stenosis, the success rate of silicone tube intubation was high when the AP diameter of the nasolacrimal duct was more than 1.1+/-0.1 mm and when the stenosis was less than 18.1+/-7.0% in the symptomatic side compared with the opposite side. These results are expected to help predict the success rates when preoperatively planning silicone tube intubation.

Endometriosis in the abdominal wall following Cesarean section is one of the very rare condition among the extrapelvic endometriosis. Although benign, endometriosis possesses the unique ability to invade tissue and to disseminate or metastasize by hematogeneous, lymphatic route, or direct implantation. We reported the case with brief review of the literature.
