3.A Histopathological Study of IgM Nephropathy.

461 cases of renal biopsy specimens were summerized correlated with their light microscopic and immunofluorescence findings, which obtained from Jan. 1981 to Jul. 1986 at Department of Pathology, Kyung Hee School of Medicine, The results were as follws: 1) The incidence of IgM nephropathy was about 5.8% of the primary glomerulopathy. 2) Sex distribution showed male preponderance with male: female ratio of 1.7:1. 3) Age distribution were 0-9 years 7.4%, 10-19 years 29.6%, 20-29 years 37.0%, 30-39 years 7.4%, 40-49 years 7.4% and 50-59 years 11.1%. 4) The clinical symptoms were gross hematuria 7.4%, microscopic hematuria 59.2%, proteinuria (nonnephrotic) 37.0%, nephrotic syndrome 55.5%, hypertension 7.8% and edema 59.2%. 5) 9 cases (33.3%) had past histories of upper respiratory infections. 6) The distributions of the light microscopic deagnosis were minimal histologic change 11 cases (40.7%), focal glomerulonephritis 7 cases (25.9%), measangial proliferative glomerulonephritis 5 cases (18.5%), focal and segmental glomerulosclerosis 1 case (3.7%) and poststreptococla glomerulonephritis 1 case (3.7%). 7) Immunofluorescence study showed significant diffuse mesangial granular deposits of IgM and C3 in all of the 27 cases. 8) It was demonstrated that in the patients with nephrotic syndrome, the prognosis for the patients with IgM deposition were less favorable than those without IgM deposition.

4.Epitope Mapping of HIV1 gp41 Protein for Korean Anti - HIV1 Antisera using Synthetic Peptides.

The N-terminal sequence of HIV1 gp41 (amino acid residues 584-623) was known to be the immundominant region of HIV1 gp41 protein. In order to determine epitope for gp41 protein of Korean anti-HIV1 positive sera, multiple antigenic peptides (MAPs) for the sequences corresponding to 584-604, 590-612, 604-623 and 584-618 of HIV1 gp41 were synthesized by solid phase method using Fmoc-Lys (Fmoc)-OH and used as coating antigens for ELISA. The reactivities of the synthetic peptides with Korean HIV1 positive (21 samples) and anti-HIV1 negative sera (22 samples) obtained from healthy blood doner were estimated by an indirect ELISA. MAPs for 584-604, 590-612 and 604-623 of gp41 reacted with 62 %, 100 % and 81 % of Korean anti-HIV1 positive sera tested, respectively. The results suggest that the epitope for HIV1 gp 41 for Korean anti-HIV1 positive sera is located in the region of amino acid 590-612 of gp41. MAP for gp41 (584-618) reacted with all (100 %) of anti-HIV1 positive sera tested, but did not react with anti-HlV1 negative sera. In addition, this MAP reacted stronger with seven samples of anti-HIV1 positive sera of anti-HIV1/2 combo performance panel than the mixture of 584-604, 590-612 and 604-623 of gp41, but did not react with anti-HIV negative serum. The high sensitivity and selectivity of MAP of gp41 (584-618) suggest that this peptide as a coating antigen in an ELISA system will be useful for antibody detection of HIV1.

5.Anatomy of Peroneal Buoy Flap

Authors analysed 36 cases(33 patients) of Peroneal Buoy Flap to investigate branching pattern, course, length of vascular pedicle, and perforating level of the perforating cutaneous branches from Oct. 1985 to July 1988. The results were as follows. 1. The perforating cutaneous branchea were classified into four types, the Straight Branch (18 cases), the Proximal Oblique Branch (8 cases), the Branch from Muscular Artery (8 cases), the Distal Oblique Branch(2 cases) respectively. The most common patten was Straight Branch. 2. There were 3 pathways of these branches, the most common one passed between the Soleus and Peroneus muscles(23 cases, 64%), and second one passed through the snterior part of Soleus muscle(8 cases, 22%), the third one passed through the posterior part of Peroneus muscle(5 cases, 14%). 3. The length of vascular pedicle in Buoy Flap was variable from 3cm to 15cm, but 27 cases(75%) were distributed between 4cm and 6cm. 4. The perforating level of branches were 6.3cm in average from Fibular Neck, 88% of them were distributed within 10cm. 5. Peroneal Buoy Flap in possible to reconstruct both seperated bone and skin defect in some distance by One-Stage Operation.

6.The Clinical Significance of Expression of Cyclooxygenase-2 in the Prognosis of the Papillary Thyroid Cancer.

PURPOSE: It is well known that cyclooxygenase-2 (COX-2) is associated with carcinogenesis in many human cancers. In thyroid cancers, COX-2 expression in aging patients is known as a powerful prognostic factor of differentiated thyroid cancer. Therefore, in this study, we investigated the expression of COX-2 in cases of papillary thyroid cancer (PTC), as well as its association with other prognostic factors of thyroid cancer. METHODS: A total of 213 cases of papillary thyroid cancer were enrolled in this study and their clinicopathological characteristics were investigated retrospectively by a review of medicalrecords. Immunohistochemical staining for COX-2 was performed in the malignant tissue of the cases and the resultsof were then evaluated to determine if they were associated with other prognostic factors. RESULTS: The mean age of patients was 46 years (Range: 17~77 years old) and the tumor sizesranged from 0.1 to 5 cm (mean 1.2±0.9 cm). The pathological findings were as follows; thyroid capsule invasion in 101 patients (47%), lymph node metastasis in 76 patients (36%), multiplicity in 47 patients (22%), and bilaterality in 36 patients (17%). Positive results of immunohistochemical staining for COX-2 were noted in 131 patients (62%), however, the statistical analyses showed no significant association between COX-2 expression and other prognostic factors of papillary thyroid cancer. CONCLUSION: No association of COX-2 expression and prognostic factors of papillary thyroid cancer were found in this study. However, a larger prospectivestudy with survival analyses would provide a better understanding of thesignificance of COX-2 expression in cases of papillary thyroid cancer.

7.Silver Diamine Fluoride Compound for Dental Caries and Its Characterisation Using Microscopic Computed Tomography and Nanoindentation

Background:
In our study, a silver diamine fluoride (SDF) compound for the treatment of dental caries was synthesized to characterize its remineralization activity upon direct application to deciduous teeth. This study aimed to use microscopic computed tomography (microCT) and nanoindentation to evaluate whether SDF composite application could effectively arrest dental caries in five exfoliated primary molars.
Methods:
Carious teeth were extracted and visually examined using quantitative photofluorescence devices (Qraycam and QraypenTM). After performing microCT, the SDF composite was applied to the teeth according to the manufacturer’s instructions. The researchers exchanged and precipitated the irritant saliva once daily for 1 week. The teeth were sectioned longitudinally through the centers of the mesial and distal surfaces, embedded, polished, and measured using nanoindentation. Thereafter, microCT was repeated. Statistical analyses were performed using GraphPad Prism software.
Results:
Following SDF composite application, a remineralized layer was observed on microCT images, and the hardness increased when measured using nanoindentation. We found that demineralized enamel presented with an increased number of irregular crystals in the deep carious lesion group compared with those in the shallow carious lesion group, resulting in a rougher surface.
Conclusion
The SDF composite may be used for remineralization of early caries and cessation of advanced caries in primary molars.
8.X-Ray Diffraction Analysis of Various Calcium Silicate-Based Materials

Background:
The purpose of this study was to evaluate the composition of the crystal phases of various calcium silicate-based materials (CSMs): ProRoot white MTAⓇ (mineral trioxide aggregate) (WMTA), Ortho MTAⓇ (OM), Endocem MTAⓇ (EM), Retro MTAⓇ (RM), Endocem ZrⓇ (EN-Z), BiodentineTM (BD), EZ-sealTM (EZ), and OrthoMTA III (OM3).
Methods:
In a sample holder, 5 g of the powder sample was placed and the top surface of the material was packed flat using a sterilized glass slide. The prepared slides were mounted on an X-ray diffraction (XRD) instrument (D8 Advance; Bruker AXS GmbH, Germany). The X-ray beam 2θ angle range was set at 10∼90° and scanned at 1.2° per minute. The Cu X-ray source set to operate at 40 kV and 40 mA in the continuous mode. The peaks in the diffraction pattern of each sample were analyzed using the software Diffrac (version 2.1). Then, the peaks were compared and matched with those of standard materials in the corresponding Powder Diffraction File (PDF-2, JCPDS International Center for Diffraction Data). A powder samples of the materials were analyzed using XRD and the peaks in diffraction pattern were compared to the Powder Diffraction File data.
Results:
Eight CSMs showed a similar diffraction pattern because their main component was calcium silicate. Eight CSMs showed similar diffraction peaks because calcium silicate was their main component. Two components were observed to have been added as radiopacifiers: bismuth oxide was detected in WMTA, OM, and EM while zirconium oxide was detected in RM, EN-Z, BD, EZ, and OM3. Unusual patterns were detected for the new material, OM3, which had strong peaks at low angles.
Conclusion
It was caused by the presence of Brushite, which is believed to have resulted in crystal growth in a particular direction for a specific purpose.
9.Aortic Thrombosis in Neonate Associated with Umbilical Artery Catheterization: Clinical Manifestations and Thalamus with Prognosis.

PURPOSE: To evaluate the clinical findings and ultrasonographic findings of aortic thrombosis which developed after umblical artery catheterization in neonates. METHODS: We retrospectively evaluated clnical records and ultrasonographic findings in five cases of aortic thrombosis, which developed after umblical artery catheterization. Four were premature newborns, and one was a term neonate but small for gestational age. Three were male infants. The mean age was 29.8 days (15-71 days) and the duration of umblical artery catheterization was 16.4 days (7-31 days). The gestational age of 5 cases was between 26 and 38 weeks (mean 33 weeks) and birth weight was between 930 and 2,600 g (mean 1,950 g). All cases were diagnosed by ultrasonography only, and in 2 cases color Doppler ultrasonography was performed additionally. RESULTS: There were several associated conditions including hypothermia (n=2), perinatal asphyxia (n=2), respiratory distress syndrome (n=5), and neonatal infection (n=5). The clinical manifestations of 5 cases were hypertension (n=4), thrombocytopenia (n=4), hematuria (n=2), renal failure (n=l), and congestive heart failure (n=l). In all five cases, ultrasonography showed hyperechoic thrombus with partial occulsion of aorta. The size of thrombi varied 1.2-4x 4-25 mm. All thrombi were located around the origin site of renal artery. CONCLUSION: Aortic thrombosis, one of the complications of umblical artery catheterization can easily be diagnosed by ultrasonography, so ultrasonography would be needed as a screening study in neonate with umblical artery catheterization.

10.Hypoxic - ischemic Encephalopathy in Term Infants: Correlation of Neurosonographic Findings in Basal Ganglia and Thalamus with Prognosis.

PURPOSE: To evaluate abnormal neurosonographic (NSG) findings of thalami and basal ganglia in full term babies with hypoxic-ischemic encephalopathy and to correlate the findings with follow-up studies and prognosis. METHODS: We evaluated 13 full term babies with abnormal NSG findings of thalarni and basal ganglia. NSG was performed within 7 days after clinical abnormalities. Follow-up NSG was done in 11 cases; CT scan in 4 and MRI in 7. We classified NSG findings as diffuse, unilateral, and focal types according to increased echogenicity and evaluated prognosis based on follow-up studies and neurological sequelae. RESULTS: Nine cases of diffuse type had diffuse echogenic changes of bilateral thalami and basal ganglia, slit-like lateral ventricles suggesting cerebral edema, and increased parenchymal echogenicity. In diffuse type, follow-up studies showed more prominent echogencities and ventricular dilatations and cerebromalacia. One case of unilateral type caused by thromboembolism had unilateral echogenicity of right thalamus and basal ganglia with increased echogenicity of the ipsilateral cerebral hemisphere and compression of the lateral ventricle, suggesting cerebral infarction. Follow-up study showed unilateral cystic cerebromalacia. Three cases of focal type had a localized echogenic area in thalamus with lacunar infarction, which decreased in size during follow-up. Among nine cases of diffuse type, one died within 2 days, two were discharged against medical advice, and six had severe neurologic sequelae. One case of unilateral type had a moderate degree of neurologic sequelae. All 3 cases of focal type had normal development. CONCLUSION: Pattems of abnormal echogenicity in thalami and basal ganglia in fullterm infants with hypoxic-ischemic encephalopathy are correlated with the outcome and may be helpful for treatment planning.
