1.Endoscopically assisted reduction and rigid fixation of the zygomatic arch fracture.

Bicoronal approach is adopted as an universal method for reduction and internal fixation of the fractured zygomatic arch in the treatment of complex zygomatic fracture. However bicoronal approach leaves many disadvantages, such as long incision, alopecia, prolonged edema, and paresthesia. Nowadays endoscopically assisted minimal invasive technique has been widespread not only for the aesthetic surgery, but also for the reconstructive surgical fields because of the benefits. We have been recently trying to apply endoscopic surgery to facial bone fracture surgery. Endoscopically assisted minimal invasive technique has been applied to 14 patients who admitted to Chosun university hospital from March 1996 to July 1997. We used 4 mm, 30' endoscope and two 1.5-2 cm temple incisions and a subciliary incision for approaching to the zygomatic arch. Protected drill bit and driver had access to fractured zygomatic arch though tiny(5 mm) stab incision over the zygomatic arch. We have been able to successfully reduce and make fixation of the fracture of zygomatic arch by using endoscopically assisted minimal invasive technique. Follow up for 3 months revealed good maintenance of the reduction. We think that application of the endoscopic technology to midfacial bone fracture will result in significant reduction in morbidity and improve postoperative results.

2.Clinical analysis of the Ankle Fracture

The ankle joint is a modified complex hinge joint which plays an important role in weight bearing, walking and standing. Hence, the ankle injuries include not only fractures of bone but often also the rupture of ligaments and soft tissues, so if the structures were not accurately repaired, many complications may be developed. The authors have reviewed a series of 70 cases of ankle fractures which were treated in department of orthopaedic surgery of Inha hospital from January 1988 to February 1992. Among the seventy cases, 41 patients (59.4%) were male, and 28 patients (40.6%) were female. Average age was 41.4 years and 3 cases (4.35%) were open fracture. The most common cause was slipping 34 cases (48.6%) and the other causes were traffic accident 23 cases (32.9%), fall from height 6 cases (8.6%), direct blow 5 cases (7.0%) and sports injury 2 cases (2.9%) in order. The rusults obtained form this study were as follows. l. According to the classification fo Lauge-Hansen, the most common fracture mechanism was supination-external rotation injury (24 cases: 34.4%). 2. Among them, 51 cases were treated by operative treatment and 19 cases by conservative treatment. 3. Ten cases (14.3%) in this series were complicated; traumatic arthritis in 7 cases, infection in 2 cases and nonunion 1 case. 4. Open reduction and internal fixation and early anatomical reduction were better than closed reduction and delayed reduction. 5. The incidence of traumatic arthritis occured higher in the cases treated with closed method.

7.Metabolism of C(14)-glucose by Eurytrema pancreaticum.

The glucose uptake rate by Eurytrema pancreaticum was a mean value of 16.44 +/- 2.42 micro-mole/hr/g, and total CO(2) production rate by the fluke averaged 5.82 +/- 0.97 micro-mole/hr/g. The relative specific activity of respiratory CO(2) showed a mean value of 5.75 +/- 0.84 per cent. The rate of CO(2) production derived from medium C(14)-glucose was a mean of 0.33 +/- 0.10 micor-mole/hr/g. Therefore, the average value of 0.32 +/- 0.04 per cent of glucose utilized by the flukes from the medium C(14)-glucose was oxidized to respiratory CO(2). The tissue concentration of glycogen in E. pancreaticum was a mean of 45.50 +/- 2.18 mg/g or 4.55 +/- 0.22 %/g. But the turnover rate of glycogen pool was a mean of 0.027 +/- 0.003 %/hr or 0.009 +/- 0.002 mg/hr/g. The average value of 0.64 +/- 0.23 percent of glucose utilized by the flukes from the medium C(14)-glucose was incorporated into the glycogen. These data account for that only 1 per cent of the utilized glucose by the flukes participated in furnishing the oxidation into respiratory CO(2) and the synthetic process into glycogen.

8.Surgical Experience of Basilar Invagination by Transoral Transpharyngeal Approach.

Basilar invagination or basilar impression involves the upward displacement of the margins of the foramen magnum into the base of the skull. That this entity may be asymptomatic or associated with symptoms referable to encroachment into the posterior fossa or embarrassment of the posterior circulation is well known. Currently the authors have experienced a young male patient with basilar invagination presented with progressive myelopathy and lower cranial neuropathies due to odontoid invagination. It was thought that a posterior decompression would be hazardous;therefore, the inferior clivus, odontoid process, and anterior arch of the atlas were removed transoral-transpharyngeally and bone fusion was performed with iliac bone. Furthermore the operative result was good without any serious complication. The operation technique is detailed.

9.Immune Cells (T Cell, T Subsets and B Cell) in the Peripheral Blood of Patients with Psoriasis and Effects of Photochemotherapy (PUVA) on These Cells.

Recent studies have demonstrated that the patients with psoriasis have various immune alternation in both humoral and cellular immunities. However the results of these are still controversial. These facts promoted us to analyze the immune cells (T cell, T subsets and B cell) by rosette method for these cells in peripheral blood of 15 patients with psoriasis. And we also analyzed the effects of photo- chemotherapy(PUVA) on these cells. The results were summerized as follows; 1. The mean values of T cell, T, T, cell and B cell in peripheral blood of the- patients with psoriasis before PUVA therapy were 56. 2+-5.7%, 35.1+-6.9%, 5. 5+l.4%, 11.6+4. 0% respectively, These results of immune cells showed signi- ficantly lower mean values when these were compaired with the mean values. of nomnal control group(T cell; 65. 2+-2.9%, TM cell; 45.8+-3.2%, T cell; 7.3+ 1.3% B cell; 16.2+2.4%), respectively (T cell, Tm Tg, p<0.01, B cell; p<0.05). 2. 10.An Extended Frontal Approach to Midline Lesions of the Anterior Skull Base.

Relatively huge lesions in the parasellar and midline anterior skull base region are difficult to approach for radical procedures. To minimize brain retraction and achieve excellent exposure for safe manipulation within these regions, the authors have performed an extended frontal approach, a fibrontal craniotomy and a bilateral orbitofrontal osteotomy in 11 patients, and an additional transfacial maxillotomy in 1 patient. An additional removal of the orbital rim offers excellent visualization and permits unhindered surgical manipulation including postoperative reconstruction at the anterior skull base. But disadvantages include prolonged operative time and frequent ilateral olfactory denervation. Six patients with benign tumors, two patients with malignant tumors, one patient with hematoma in the ehtmoid and sphenoid sinuses, and three patients with delayed traumatic CSF rhinorheas were operated on using this approaches, with good results. The operative technique and its results are detailed.
