4.A radiological study of recovery from hydronephrosis by ureteral ligation

The determination of the degree and ability of recovery of obstructive hydronephrosis is important in treatment and prediction of prognosis. Among the various methods in determination of the status of kidney, intravenous pyelography is the most important tool to evaluate the morphological and functional changes of kidney in vivo. The purpose of this experiment was the determination of process and recoverability of hydronephrosis depend on duration of unilateral uretreal obstruction. For the experiment, 32 rats were divided into 2 groups; the first group of 12 rats were used to determine the process of hydronephrosis during 1 to 4 weeks of unilateralureteral ligation and the second group of 20 rats were used to determine the recoverability of hydronephrotic kidney depend on same duration of ureteral ligation following relief of ligation. Intravenous pyelography as undertaken and renal angiography, gross and microscopic examination were added. The results obtained are summarized as follows; 1. Kidney enlargement and pelvic dilatation were progressively increased with a relation to duration of ureteral obstruction until 4 weeks. 2. Renal excretory function is not impaired until 3 weeks obstruction, but rapidly impaired to nonfunctioning in 4 weeks obstruction. 3. Renal recovery was possible following relief of ureteral ligation within 3 weeks of ureteral obstruction, but histopathological and functional recovery were impossible in 4 weeks of ureteral ligation. 4. There is no direct correlation between duration of obstruction and recoverability following relief within 3 weeks of uretral obstruction. 5. Intravenous pyelographyis a simple and accurate method to determine the degree of obstructive hydronephrosis and to evaluate morphological and functional recovery of kidney following relief of obstruction.

The majority of the cysts developed in pancreas are inflammatory pseudocyst but neoplastic cysts are rarely encountered Especially, mucinous cystadenoma which was begun and originated from epithelial cell of pancreatic duct is difficult to differentiate from pseudocysts by preoperative clinical, laboratory and radiologic findings. Mucinous cystadenoma has a malignant potentiality, so complete excision of cystadenoma is the treatment of choice. Recently, we experienced one case of mucionus cystadenoma in 37 year-old female, we report it with a review of the literature.

6.Infectivity of Orientia tsutsugamushi to Various Eukaryotic Cells and Their Cellular Invasion Mechanism.

Orientia tsutsugamushi is obligate intracellular bacterium that grows within the cytoplasm of the eukaryotic host cells. Therefore capability of the attachment, entry into the host cell and intracellular survival should be critical process for oriential infection. In this study we investigated the cellular invasion mechanism of Orientia tsutsugamushi and the role of transmembrane heparan sulfate proteoglycan, which binds diverse components at the cellular microenvironment and is implicated as host cell receptors for a variety of microbial pathogens. First of all Orientia tsutsugamushi can invade a wide range of nonprofessional phagocytic cells including fibroblast, epithelial cells a#nd endothelial cells of various host species, including B and T lymphocytes. Thus, it was postulated that the attachment of O. tsutsugamushi requires the recognition of ubiquitous surface structures of many kinds of host cells. Treatments with heparan sulfate and heparin inhibited the infection of Orientia tsutsugamushi in dose-dependent manner for L cell, mouse fibroblast, whereas other glycosaminoglycans such as chondroitin sulfate had no effect. Collectively, these findings provide strong evidence that initial interaction with heparan sulfate proteoglycan is required for the oriential invasion into host cells.

10.Prognostic Value of CEA and CA19 - 9 in Serum and Peritoneal Washing Fluid in Gastric Carcinoma.

PURPOSE: The clinical significance of preoperative serum levels of CEA and CA19-9, levels of CEA and CA19-9 in peritoneal washing fluid and free cancer cells in peritoneal washing fluid in gastric cancer patients were evaluated in this study. MATERIALS AND METHODS: Serum and peritoneal levels of CEA and CA19-9 and peritoneal washing cytology in 115 patients with gastric cancer were analyzed with respect to the prognostic factors using univariate and multivariate analysis. RESULTS: Positive rate of serum CEA and CA19-9 was 16.5%, 13.0%. And that of peritoneal washing CEA, CA19-9 and cytology was 15.7%, 7.8% and 9.6%. A univariate analysis showed that tumor markets in serum and peritoneal washing fluid and peritoneal washing cytology had significant correlations with the progression of the tumors, and patients with positive serum or peritoneal tumor markers had poorer survival after operation than did the patients with negative tumor markers. But in a multivariate analysis showed that only peritoneal CA19-9 was an independent risk factor. And combination of these five markers provided rnore predictable prognostic informations in a multivariate analysis. CONCLUSION: Combination of serum or peritoneal levels of CEA, CA19-9 and washing cytology appeared to be a useful marker for managing gastric cancer patients.
