The pelvis is a ring structure with strong ligamentous support. This support includes the symphysis pubis, the anterior and posterior sacroiliac ligaments, and the strong sacrospinous and sacrotuberous ligaments. Since the pelvis is a ring, disruption of any protion of the ring is always associated with disruption in another portion of the ring. Massive pelvic disruption is an important and often life threathening injury. It is usually due to a motor vehicle accident, a fall from a height, industrial injury, and so on. The authors reviewed 28 cases of unstable pelvic fracture treated at the orthopaedic department of Catholic Medical Center during the period from January 1975 to December 1980, and the injury was classified according to the Pennal and Tiles method, and analyzed the clinical data as to the cause of mechanical violence, type of fracture and treatment, post-traumatic pelvic instabilities, and the results. The persistant post-traumatic pelvic instability was assessed by physical examination, and Chamberlain and Trostlers stress Roentgen view of the pelvis. The clinical results obtained were as follows: 1. The incidence of the unstable pelvic fracture was 17.7% (28) of all pelvic fractures (158). The incidence of injury in male was approximately 5 times more frequent than female, and most of the patients were in the 3rd to 5th decade of their life. The causes of injury were road accident (54%), coal mine accident and fall from a height in orders. 2. The four-rami fracture with posterior disruption due to the lateral compression injury was the most common type of unstable pelvic fractures. 3. In conservatively treated patients, the pelvic instability persisted in 37.5% (6 in 16 cases) of the cases and all of them necessitated the joint stabilizing operation. Twelve cases were treated operatively. Among them, performed operations include wiring of symphysis pubis in 2 and sacroiliac joint fusion in 4 within 4 weeks after injury. But for the other 6 cases in which pelvic instability persisted in spite of the sufficient conservative treatment, sacroiliac joint fusion or sacroiliac joint fusion together with a symphyseal plate fixation were done. 4. Satisfactory results was obtained in 62.51% (10 in 16 cases) of nonoperated cases, and 91.7% (11 in 12 cases) of operated group. Authors consider that the most important step in the management of traumatic pelvic instability are; firstly the early recognition of the correlation of the fracture-fragments; the fracture mechanism, and the possible presence or the post-traumatic pelvic instability, and secondly finalizing the therapeutic plan. Blue Rubber Bleb Nevus syndrome, or Bean's syndrome is a very rare disease characterized by an association of 1) hemangioma cutis in which the nevi feel like rubber blebs; 2) hemangiomas of the digestive organs; 3) iron-deficiency anemia due to hemorrhage from the digestive tract. Sometimes, the bleeding is so severe and massive that the patient needs blood transfusions and/or emergency operations. We present a 29-year-old man with Blue Rubber Bleb Nevus syndrome. This patient was admitted to out hospital due to recently aggrevated dizziness and intermittent rectal bleeding. Multiple bluish tumors were noted on the palm, sole and glans penis. Laboratory examination revealed severe iron-deficiency anemia. Barium studies revealed multiple polypoid masses in the stomach, small bowel and colorectum. On esophagogastroduodenoscopy and colonoscopy, three or four dozens of hemangiomas with variable size and shape were noted in the stomach, duodenum at or around the ampulla of Vater and colorectum. This patient was of particular interest because rectal bleeding occurred from rectal lesion which protruded out of anus and caused bleeding during defecation, and which showed as a 2.0 cm Yamada type III polypoid lesion. Endoscopic ultrasonography confirmed us that this lesion was confined to the mucosa and submucosa without extension into muscle layer. Endoscopic polypectomy of this lesion was performed because which considered as the main focus of current rectal bleeding. He was discharged without any complication and 4 months later, he was well without rectal bleeding and his hemoglobin level increased upto 13.5 g/dl. To our knowledge, this is the first case of Blue Rubber Bleb Nevus syndrome in Korea.

PURPOSE: This prospective study was designed to evaluate the efficacy and safety of Nd: YAG laser for the treatment of benign prostatic hyperplasia(BPH). MATERIALS AND METHODS: From January 1994 to March 1995, 77 patients with BPH were underwent laser prostatectomy. Of the 77 patients 17 were performed under local anesthesia. Preoperative and postoperative(at 1, 3 and 6months after operation) evaluation consisted of uroflowmetry, American Urological Association(AUA) symptom score and posboid residual urine. There were no limits of the prostatic size for operation and 2 cases had 100gm of prostatic weight. Multiple dragging technique(from the bladder neck to the verumontanum) with 90 degrees fiber at 80 to 90watts power were used in near contact or contact. RESULTS: 69 patients of total 77 patients have completed 6months follow-up evaluation. Mean prostatic size was 39.7gm. Preoperative mean urine flow rate was 3.0ml/sec and postoperative urine flow rate at one, three and six months was 10.7, 13.8 and 13.6m1/sec, respectively. Preoperative AUA score was 26.6 and postoperative AUA score at one, three and six month was 9.6, 7.6, respectively. Preoperative residual urine was 251.4ml and postoperative residual urine at one, three and six months was 57.0, 15.3 and 19.4ml, respectively. In 17 cases laser prostatectomy was done under local anesthesia due to poor medical condition. The results of laser prostatectomy under local anesthesia was not different from the cases with regional anesthesia. And the degree of subjective Pain of the Patients in local anesthesia was tolerable. The total energy used was about 129,000 Joules and the mean operative time was 103 minutes. Foley catheter was removed on postoperative 5th day. A total of 7 patients failed in initial voiding trials and required catheterization longer than 2weeks postoperatively. In 3 cases of seven patients who complained discomfort on voiding after 3months, necrotic tissue was present on cystoscopy. There were no severe complications. CONCLUSIONS: It is concluded that the technique of transurethral laser-induced prostatectomy is effective and less invasive technique alterative than traditional transurethral resection of the prostate.

The antianginal effect of oral nicorandil was examined on 15 subjects with angina pectoris who had not been improved by previous treatment with beta blocker and/or calcium antagonists. Nicorandil, 5mg tid, was added to their previous regimen which were composed of beta blocker and/or calcium antagonists, and patients were followed up at least for 6 weeks. The following results were obtained; 1) Frequences of anginal attack were markedly reduced in 11(73%) of 15 patients after the use of nicorandil. 2) After the use of nicorandil, ST segment depression and T wave inversions were improved in 10(66.7%) of 15 patients. 3) In 10 patients who had been taken nicorandil over 12 weeks, 9 patients showed sustained improvement in clinical status, but no significant side effect was noted. 4) In two patients who had undergone exercise tolerance test, significant prolongation in exercise duration were noted. 5) The adverse effects of nicorandil were as follows; headache in 3, insomnia in 3, excessive perspiration, dizziness, thirst, constipation and facial fiushing in 1, respectively.

In order to assess the sensitively and specificity of the pulsed Doppler echocardiography in the aortic regurgitation, 44 patients with valvular heart diseases were examined by pulsed Doppler echocardiography. Every patient was examined in sequence of clinical examination including the physical, M-mode & 2D echocardiographic examination, Doppler technique and then angiocardiography. The obtained results were; 1) Pulsed Doppler echocardiography was very useful in the diagnosis of the aortic regugitation(sensitivity:100%, specificity:90%). 2) Pulsed Doppler echocardiography was especially useful in the detection of the aortic regurgitation in patients who were not detected by physical and conventional echocardiographic examination.

PURPOSE: The surgical removal of normal kidney elicits dramatic changes in the remaining kidney. Changes in urinary excretion of lithogenlc substances and urinary inhibitor may increase or decrease the stone-forming potential. The aim of this study was to determine if unilateral nephrectomy could enhance urinary calculi formation in animal model. MATERIALS AND METHODS: Adult male rats were divided randomly into 3 groups. Group 1(n=5) were fed with pellet and tap water for 4 weeks without nephrectomy Group 2(n=5) were fed with the same pellet and 0.8% ethylene glycol plus 1% ammonium chloride in drinking water for 4 weeks without nephrectomy Group 3(n=5) were fed with the same pellet and the same lithogenic diet from 2 weeks after uninephrectomy for 4 weeks. Each right kidney was harvested after 4 weeks feeding. One half of the each kidney were fixed for H & E stain to examine crystal deposits. The remaining half of the kidneys were treated with 12N KOH and 6N HCI for the measurement of calcium content with atomic absorption spectrophotometer. About 3-5cc of blood was collected from the inferior vela cava for determinations of serum creatinine, calcium, magnesium and phosphorus & chloride. Urine was collected over a 24 hour interval before and after lithogenic diet, and urine volume, 24-hour urinary excretions of oxalate, citrate, uric acid, calcium, magnesium, phosphate, sodium, potassium, chloride, and creatinine were measured. RESULTS: There was a significant difference between group 1(0.243 +/- 0.014mmo1/kg) and lithogenic diet groups(group 2 and group 3 in calcium content(p < 0.01)). But, there was no significant difference of calcium content between group 2(5.475 +/- 0.247mmo1/kg) and group 3(5.843 +/- 0.324mmol/kg)(p > 0.05). There were no calcium oxalate crystal deposit in the group 1, but there were about 2-5 crystal deposits with partially polarized light microscope( x 100) in group 2 and group 3, and there was no significant difference between group 2 and group 3. After taking lithogenic diet, the serum creatinine, serum magnesium, serum phosphors, urine pH & 24-hour urinary excretions of oxalate increased significantly in group 2 and group 3, whereas creatinine clearance & 24-hour urinary excretions of calcium, sodium, potassium, chloride, citrate decreased significantly in the same groups(p < 0.05). But, there were no significant difference of serum calcium, chloride, and 24-hour urinary excretions of magnesium, uric acid among three groups after 4 weeks feeding. CONCLUSIONS: Unilateral nephrectomy would not increase stone forming potential.
