3.A Case of Blue Rubber Bleb Nevus Syndrome.

Blue Rubber Bleb Nevus syndrome, or Bean's syndrome is a very rare disease characterized by an association of 1) hemangioma cutis in which the nevi feel like rubber blebs; 2) hemangiomas of the digestive organs; 3) iron-deficiency anemia due to hemorrhage from the digestive tract. Sometimes, the bleeding is so severe and massive that the patient needs blood transfusions and/or emergency operations. We present a 29-year-old man with Blue Rubber Bleb Nevus syndrome. This patient was admitted to out hospital due to recently aggrevated dizziness and intermittent rectal bleeding. Multiple bluish tumors were noted on the palm, sole and glans penis. Laboratory examination revealed severe iron-deficiency anemia. Barium studies revealed multiple polypoid masses in the stomach, small bowel and colorectum. On esophagogastroduodenoscopy and colonoscopy, three or four dozens of hemangiomas with variable size and shape were noted in the stomach, duodenum at or around the ampulla of Vater and colorectum. This patient was of particular interest because rectal bleeding occurred from rectal lesion which protruded out of anus and caused bleeding during defecation, and which showed as a 2.0 cm Yamada type III polypoid lesion. Endoscopic ultrasonography confirmed us that this lesion was confined to the mucosa and submucosa without extension into muscle layer. Endoscopic polypectomy of this lesion was performed because which considered as the main focus of current rectal bleeding. He was discharged without any complication and 4 months later, he was well without rectal bleeding and his hemoglobin level increased upto 13.5 g/dl. To our knowledge, this is the first case of Blue Rubber Bleb Nevus syndrome in Korea.

6.A Study on the Shape of the Proximal Femur for Designing Korean-type of the Femoral Stem

Avascular necrosis of the femoral head is the most common disease for which total hip arthroplasty is being performed in Korea. In order to develop a femoral stem having a proper fit to Korean femur, it is essential to study the change in the shape of the proximal endosteal canal of the femur resulting from disuse caused by pain avascular necrosis of the femoral head. The size and shape of the proximal femur were measured with 3-dimensional images reconstructed from the CT data of 116 femurs in 58 patients who had suffered from avascular necrosis of the femoral head. Among them, there were 30 patients who had suffered from unilateral involvement of avascular necrosis of the femoral head. The comparative study was made on the shape of the proximal endosteal canal of the femur between the normal and the diseased femurs. The shape of proximal endosteal canal of femur in avascular necrosis didn't show statistically significant difference from that of the normal femur. Therefore, the femoral stem designed from the data of normal femurs can be usually applied to the avascular necrosis without problem in obtaining the maximal fit between the stem and the endosteal canal. The canal flare index was greater and the proportion of champagne-flute type was higher in Korean than in Westerner. The shape of the endosteal canal of isthmus was nearly circular in Korean in contrast to anteroposteriorly ovoid in Westerner. The distribution of sagittal canal flare index which was newly defined in this study was entirely different from that of preexisting canal flare index in coronal plane. It will be necessary to study further in the future.

7.A Study of Lumbar Spinal Canal Using Computerized Axial Tomogram in Korean Adults

Appreciation of the normal variation of anterior and posterior diameter of the lumbar spinal canal under computerized axial tomogram may throw very important value on the diagnosis and treatment in lumbar spinal problems. The authers have measured the diameter of the lumbar spinal canal using EMI scanner CT 5005. The data was first based on the study of two cadaver lumbar spine and then the study of 268 normal Korean adults male 185, and female 83. The results were summerized as follows. 1. The diameter of the osseous segment was more narrow than articular segment in normal lumbar spinal canal, 2. The shape of lumbar spinal canal was oval in upper lumbar. but became triangular going down to the lower lumbar. There was no difference of diameter in sex. 4. The average diameter of anterior and posterior spinal canal was; L1 16.53± 0.18, L2 16.23± 0.215, L3 15.42± 0.26, L4 15.57± 0.43 and l% 17.7± 1.63mm respectively. 5. The average diameter of transverse spinal canal was; L1 23.31± 0.18, L2 23.76± 0.28, L3 24.78± 0.345, L4 26.07± 0.605 and L5 29.31± 1.19mm respectively. 6. It was more accurate to check out the pathology of the lumbar spinal canal using coinputerized axial tomogram than other method previously used.
