2.An Experimental Study on Comedogenic effect of Various Korean Cosmetocs.

Varioua articles have been reported that extemal contactants were the cause Acne venenata. The daily use of cosmetics are tbe cause of Acne minor in adult wamen. To distinguish this from other type of Acne venenata due to extemal contactants Kligman had coined the term Acne cosmetica, which is a low grade persistant acneiform eruption with occasional papulopustulea in adult women. The authors have tested a wide range of available cosmetics(Total twenty including four kinds of cold cream species, two of cleansing cream species, three of nutrient cream species, six of lotion species, three of foundation make up species and two of foreign cosmetics) using the rabbit ear assay recently popularized by Kligman in order to evaluate comedogenic effects of various Korean ccsmetics. The results a.re as follows. 1. Among these twenty facial cosmetics, We observed no comedogenccicy in two facial cosmetics, mildly comcdogenecity in twelve facial cosmetics, moderately comedo- genecity in four facial cosmetics and severe comedogenecity in two facial cosmetics. 2. Histologic gradings were paralleled the findings of tbe naked eye in up to 65% 3. There were significant differences on comedogenecity between Korean cosmetics and foreign cosmetics which used by Kligman. 4. There were no significant diffrences between cold cream species, cleansing cream species. nutrient, cream species, lotion species and foundation make up species. 5, There were various differences among twenty kinds of every cosmetic specie.

3.A Low Frequency Oscillation in Pulmonary Circulation and Its Dynamic Relation to the Low Frequency Oscillation of Systemic Circulation : Power Spectrum and Phase Estimation by Autoregressive Algorithm and Cross Spectral Analysis.

BACKGROUND: Low frequency oscillation of systemic artery pressure was known as the marker of sympathetic modulation. Recently the low frequency oscillation of pulmonay artery pressure in pulmonary hypertensive patient was reported. But no further study about its quantitative relationshop and phasic coupling between the low frequency oscillation of pulmonary artery pressure and systemic artery pressure. Power spectral analysis with autoregressive algorithm and cross spectral analysis are powerful tool for investigation these relationship. METHOD: Analog signals of simultaneous measured left pulmonary and femoral artery pressure in thirty one patients with ventricular septal defect were digitized and stored. After modeling each time series with autoregressive algorithm, power spectral density function was obtained by calculation the frequency response function of each model, and then low frequency power was computed. Cross spectral density function provided squared coherence and phase spectrum. Phase between the low frquency oscillation of the two signal was measured from the phase spectrum when the squared coherence is above 0.5. RESULTS: The advantage of using autoregressive model was that the power spectral density function was continous and sharp spectral peak was usually found. In patients with Rp/Rs<0.25, the low frquency power of pulmonary artery pressure(12+/-12) was significantly smaller than that of the systemic artery pressure(144+/-242). In patients with Rp/Rs>or=0.25, there was no significant difference between the low frequency power of pulmonary artery pressure(384+/-461) and that of the systemic artery pressure(752+/-1241). In patients with Rp/Rs>or=0.25, it was more probable that low frequency oscillation of pulmonary and systemic artery pressure was timely coherent(sqaured coherence>0.5) than in patient with Rp/Rs<0.25. And their phase difference was 0~1.96 radian. CONCLUSION: Autoregressive algorithm is a more powerful tool for spectral analysis than the method of conventional spectrum estimation. When pulmonary vascular resistance remains low, the low frequency oscillation of pulmonary artery pressure was negligible compared to systemic artery pressure. But as pulmonary vascular resistance elevates, the low frequency power of pulmonary artery pressure is much the same as that of systemic artery pressure, and there is a explicit time realtionship that pulmonary artery pressure leads the systemic artery pressure about 0~3 seconds in the low frequency range.

4.Epidermal Glycogen in the Diseases Showing Epidermal Hyperplasia.

In order to assess the epidermal glycogen in the epidermal hyperplastic disease, PAS(Periodic Acid Schiff) stain with or without diastase is used. Epidermal PAS positive reaction is significantly increased in psoriasis (90. 0%), verruca vulgaris (100%) and lichen simplex chronicus (100%) than control group (12, 5%) Epidermal PAS positive reaction is relatively increased in prurigo nodularis(66. 7 %) and chronic contact dermatitis (75%) Strong PAS positive reaction is noticed in 3 of 5 cases (60%) with verruca vulgaris, but none of 22 cases with psoriasis. It is suggested that epidermal glycogen epidermal hyperplasia and that the more glycogen accumulated. than control group (12. 5%) in 3 of 5 cases (60%) with verruca vulaccumulation is accompanied with the hyperplastic the epidermis is, the more glycogen accumulated.

6.Change of Complex and Periodic Heart Rate Dynamics with Change of Pulmonary Artery Pressure in Infants with Ventricular Septal Defect.

BACKGROUND: We studied how periodic and complex heart rate dynamic changes as pulmonary artery pressure increases in 32 infants with ventricular septal defet. In addition, we tested the possibility that the dynamical changes can be used to noninvasively predict the pulmonary artery pressure. METHODS: During cardiac catherterization, mean pulmonary artery pressure was measured and, at the same time, 5minute segments of continous electrocardiographic recording was stored. High-(>0.15 hertz) and low-(0.03-0.15 hertz) frequency components of heart rate variability were computed using spectral analysis. Yhe overall complexity of heart rate time series was quantified by its approximate entropy. RESULT: Pulmonary hypertensive infants(mean pulmonary artery pressure>20mmHg, n=17) had significantly lower low-(p<0.05)and high-(p<0.05) frequency power and lower approximate entropy(p<0.0001) than pulmonary normotensive infants(mean pulmonary artery pressure20mmHg, n=15). The mean pulmonary artery pressure was significantly correlated not with the spectral powers but with approximate entropy(=-0.51, P=0.0001). Conclusion: It can be concluded that, in infants, pulmonary hypertension induced by left-to-right shunt lesions suppress both periodic and complex heart rate oscillation and that mean pulmonary artety pressure can be predicted by calculating approximate entropy of heart trate variability.

7.A Study on Needs of the Spinal anesthesia Patients.

The purpose of this study was to identify the needs which were perceived by patients who were received spinal anesthesia for surgery. The subjects consisted of 50 adult patients who were admitted to 2 university hospitals and 2 general hospitals in Pusan city and 1 general hospital in Koje City for surgery under spinal anesthesia. Thirty eight percent of subjects received information about anesthesia before the operation. The instrument for this study was developed by the researcher based on literature and a pretest. Data were collected from December 10, 1999 to February 10, 2000 and were analyzed by content analysis. The results were that there were 533 meaningful statements in the needs of spinal anesthesia patients. The needs of spinal anesthesia patients had 51 items (preoperation (6), induction of nesthesia(5), intraoperation (27), postoperation(13)) and 6 categories (information, emotional welfare, physical welfare, post anesthetic management, control of physical environment, humane treatment). From the results, it can be concluded that: 1. In the pre-operation period, we have to explain anesthesia procedures, adequate position of anesthesia, duration before anesthesia wears off and sensation of paralysis. We have to supply emotional support to relieve anxiety because of anesthesia. 2. In induction of anesthesia, we have to support patient's position for anesthesia, and relieve anxiety so that patients participate in induction of anesthesia well. 3. In intra-operative period, we have to check the level of anesthesia, and keep up a comfortable position for operation and care for physical discomfort such as thirst, nausea, vomiting, dyspnea and to maintain body temperature of the patient. Since the patient is conscious, we have to communicate with the patient to relieve anxiety, maintain privacy, inform the patient of the process of the operation and encourage the surgeon to explain the outcome of the operation. The operating team needs the careful about what they say and to place the instrument well. We have to ventilate the room air and reduce noise. 4. In the post-operative period, we have to explain the purpose and duration of bed rest, complications of anesthesia and care for physical discomfort such as pain, dysuria, headache, backache. Also we have to maintain body temperature of the patient and maintain privacy.

8.Microsurgical Nerve Graft in Traumatic Ulnar Defect

One of the most difficult problems confronting the orthopedic surgeon today is what to do when faced with a large defect in a peripheral nerve. Recent advances in engineering and neurophysiology have improved our technical ability to understand the consequences of severing a nerve. We have treated a large traumatic ulnar nerve defect with microsurgical nerve graft technique. In a follow-up study of twehre monthes post surgery, the results were satisfactory.

9.Dysplasia Epiphysealis Hemimelica: A Case Report

A case of dysplasia eplphyseaalis hemlmelica (Fairbank, 1956) in a 8 years old boy, showlng typical clinical and roentgenographical appearance, is reported together wlth a brief review of literature in this paper. This condition is a rare developmental disorder of childhood in which there is a asymmetrical cartilaginous overgrowth of one or more than one epiphysis, or of a tarsal or carpal bone. Thls disorder is usually Iimited to elther the medial or lateral half of a single extremity. The disease is characterised radiographically by delay in the appearance of the epiphyseal nucleus, which is mottied and of irregular density. The most common deformities were genu valgum or varum and valgus or equinus deformity of the ankle. These deformities tended to increased wlth growth but the size of the lesion increased in proportion to the growth of the normal bone. Concomitant involvement of the knee and ankle is the rule, and in those cases the affected limb Is usually longer than Its mate. The lesion Is often microscopically indistingulshable from an osteochondroma.

10.The Use of May Anatomical Bone Plate in The Treatment of Distal One

Many modalities of the treatment of fractures of distal 1/3 of tibia were introduced. But, May anatomical bone plate is considered to be a valuable method in treating distal 1/3 tibial fractures because of reduction potentiality by their contour, ability of firm fixation, decreased complications. Twenty two patients having distal 1/3 tibial fractures, were treated by May anatomical bone plate, at the Department of Orthopedic Surgery, Inchon Chirstian Hospital from January 1988 to December 1993. The results were follows; 1. Ninteen cases(86%) out of 22 were closed fractures and the remaining 3 cases(14%) were open fracrues, the comminuted fractures were 6 cases(22%) and the segmental fractures were 2 cases(9%). 2. Average interval from injuy to operation was 16.8 days and 21 cases had associated injuries. 3. The mean duration of bone union was 16.9 weeks. 4. Seven cases(31.8%) of delayed union and two cases(9.1%) of superficial infection were noted.
