1.Relationships between Job Satisfaction and Burnout Experience among Nephrology Nurses.

The purpose of this study was to determine relationships between job satisfaction and burnout experience. The subjects were 225 nephrology nurses in Pusan, and Kyung Sang Namdo and Kyung Sang Bukdo. The data were collected from Nov. 20 to Dec. 3, 1996 using questionnaires method. Job satisfaction measured job satisfaction tool by Slavitt et al, and burnout experience measured burnout experience scale by Pines et al. The questionnaire consisted of question regarding job satisfaction scale(44 items 5 point scale) and burnout experience scale(21 items 7 point scale). The reliability of this instrument was that the hob satisfaction was Cronbach's alpha=0.8298 and the burnout experience was Cronbach's alpha=0.8960. The data were analyzed with the SPSS program using mean, standard deviation, frequency and percentage, t-test, ANOVA and Pearson's Correlation Coefficient. The results of this study were as follows : 1. In the demosociographic characteristics showed the highest level was as follows : 26-30 years old group(40.2%), married(56.4%), graduated junior college of nursing(87.6%), non the religious(35.6%), the effect of religion upon life is not effected(35.6%). In the characteristics related to nursing profession showed the highest level was as follows : Hospital style is secondary hospital(that have above 450 beds) (53.3%), staff nurse(72.9%), the length of clinical experience at hemodialysis room is less than 2 years(39.1%), number of patient was assigned a nephrology nurses is 5(40.4%), work in two shift(55.6%), the nurses professional motivation is family recommended(33.8%), the nurses intention to stay is until for needed(58.2%), the chance for professional growth is not enough(44.9%), degree of satisfaction with nursing is moderate(43.2%). 2. The mean score of the total hob satisfaction is 3.06 of 5 point Likert-type scale. Task requirements(3.51) among the component factors of the job satisfaction was the highest value and then the interaction among fellow nurses(3.34), job prestige/status(3.33), autonomy(3.27), organizational requirement(2.55), and pay(2.39) was the lowest 3. The mean score of the total burnout experience is 3.20 of 7 point Likert-type scale. Physical exhaustion(3.36) among the component factors of the burnout experience was the highest value and then emotional exhaustion(3.20), and mental exhaustion(2.95) was the lowest. 4. Job satisfaction according to demosociographic characteristics of the subjects showed significant differences in the effect of religion upon one's life(F=3.268, p=0.013). Job satisfaction according to characteristics related to nursing profession of the subjects showed significant differences in the hospital type(F=3.479, p=0.033), position(F=3.165, p=0.044), number of patient was assigned a nephrology nurses(F=2.552, p=0.040), nurses intention to stay(F=7.153, p=0.001), the chance for professional nursing growth(F=3.735, p=0.006), the degree of satisfaction with nursing(F=12.680,p=0.000). Burnout experience according to characteristics related to nursing profession of the subjects showed significant differences in the position(F=3.247, p=0.041), number of patient was assigned nephrology nurses(F=4.220, p=0.003), shift(F=3.148, p=0.045), nurses intention to stay(F=9.911, p=0.000), the degree of satisfaction with nursing(F=13.234, p=0.000). 5. Job satisfaction and burnout experience was signigicant negative correlation(r=-.5466, p<.001).

3.Relationship Between Body Image and Self-esteem in Patients with Chronic Renal Failure.

PURPOSE: The purpose of study was to identify the relationship between body image and self-esteem in patients with chronic renal failure. METHOD: The subjects of study were 97 patients with chronic renal failure who were outpatients at K medical center, B hospital, H hospital in Busan. Data were collected from Nov. 15th to Dec. 17th, 1999. The instruments used for study were the Body Cathexis Scale and Rogenberg's Self-esteem Scale. Collected data were analyzed by frequency, percentage, mean, standard deviation, t-test, ANOVA, Scheffe test, and Pearson's correlation coefficients. RESULT: 1. The mean score for body image was 137.46+/-22.21, the mean mark 2.92+/-0.47, and the mean score of self-esteem was 30.80+/-0.76, the mean mark 3.08+/-0.87. 2. There was a significant difference in the score for body image according to educational level (F=5.189, P=0.002) and type of residence (F=6.095, P=0.001). 3. There was a significant difference in the score for self-esteem according to age (F=3.615, P=0.009), educational level (F=4.772, P=0.004), marital status (F=3.498, P=0.019), and presence of children (F=2.511, P=0.014). 4. Body image in patients with chronic renal failure patients showed significant correlation with self-esteem (r=0.519, P=0.000). CONCLUSION: From this study, a relationship between body image and self-esteem in patients with chronic renal failure was identified. Therefore, nursing interventions are needed to promote body image and self esteem in patients with chronic renal patients.

5.Epitope Mapping of HIV1 gp41 Protein for Korean Anti - HIV1 Antisera using Synthetic Peptides.

The N-terminal sequence of HIV1 gp41 (amino acid residues 584-623) was known to be the immundominant region of HIV1 gp41 protein. In order to determine epitope for gp41 protein of Korean anti-HIV1 positive sera, multiple antigenic peptides (MAPs) for the sequences corresponding to 584-604, 590-612, 604-623 and 584-618 of HIV1 gp41 were synthesized by solid phase method using Fmoc-Lys (Fmoc)-OH and used as coating antigens for ELISA. The reactivities of the synthetic peptides with Korean HIV1 positive (21 samples) and anti-HIV1 negative sera (22 samples) obtained from healthy blood doner were estimated by an indirect ELISA. MAPs for 584-604, 590-612 and 604-623 of gp41 reacted with 62 %, 100 % and 81 % of Korean anti-HIV1 positive sera tested, respectively. The results suggest that the epitope for HIV1 gp 41 for Korean anti-HIV1 positive sera is located in the region of amino acid 590-612 of gp41. MAP for gp41 (584-618) reacted with all (100 %) of anti-HIV1 positive sera tested, but did not react with anti-HlV1 negative sera. In addition, this MAP reacted stronger with seven samples of anti-HIV1 positive sera of anti-HIV1/2 combo performance panel than the mixture of 584-604, 590-612 and 604-623 of gp41, but did not react with anti-HIV negative serum. The high sensitivity and selectivity of MAP of gp41 (584-618) suggest that this peptide as a coating antigen in an ELISA system will be useful for antibody detection of HIV1.

6.Evaluation of Differential Antigenic Properties of Selected B - cell Epitopes from the HIV - 1 p24 Protein using Synthetic Peptides.

The gag encoded p24 protein of human immunodeficiency virus-1 (HIV-1) is a major constitutent of the viral core, and is also known as one of the most immunodominant antigens in the host immune response against the HIV-1. Based on the neutralizing ability of anti-p24 antibodies as well as their rapid appearance in human serum after viral infection, the development of vaccines and diagnostic tools targeting the p24 protein and anti-p24 antibodies is of great interest. For the characterization of the immunological properties of the HIV-1 p24 protein, in a previous study, putative B-cell epitopes were identified by screening the reactivity of a goat anti-p24 antiserum to a large array of overlapping synthetic peptides covering the whole p24 sequence. Four peptides were identified for their abilities to elicit a strong B-cell response, which sequences comprises the regions p24 (164-182), (202-221), (217-236) and (232-256), respectively. In the present study, the immunogenicity and differential properties of each of these individual epitopes were further characterized. To evaluate the time course of the antibody response, BALB/c mice were immunized with the HIV-1 p24 protein and their serum titers against each of these peptides were determined. The earliest immune response was observed against the p24 (202-221) peptide, which also showed the highest antibody titer against the immunized antigen. Furthermore,. enzyme-linked immunosorbent assay with HIV-1 p24 protein coated microtiter plates revealed that anti-p24 (202-221) antiserum has the most pronounced reactivity against the native p24 protein. Since the p24 (202-221) epitope has also been reported to include a cytotoxic T-lymphocyte epitope, it is suggested that this region might represent a powerful antigenic site responsible for eliciting both T- and B-cell immune response. The possible application of this specific epitope in vaccine development or AIDS diagnosis is discussed.

7.A Case of Blue Rubber Bleb Nevus Syndrome.

Blue Rubber Bleb Nevus syndrome, or Bean's syndrome is a very rare disease characterized by an association of 1) hemangioma cutis in which the nevi feel like rubber blebs; 2) hemangiomas of the digestive organs; 3) iron-deficiency anemia due to hemorrhage from the digestive tract. Sometimes, the bleeding is so severe and massive that the patient needs blood transfusions and/or emergency operations. We present a 29-year-old man with Blue Rubber Bleb Nevus syndrome. This patient was admitted to out hospital due to recently aggrevated dizziness and intermittent rectal bleeding. Multiple bluish tumors were noted on the palm, sole and glans penis. Laboratory examination revealed severe iron-deficiency anemia. Barium studies revealed multiple polypoid masses in the stomach, small bowel and colorectum. On esophagogastroduodenoscopy and colonoscopy, three or four dozens of hemangiomas with variable size and shape were noted in the stomach, duodenum at or around the ampulla of Vater and colorectum. This patient was of particular interest because rectal bleeding occurred from rectal lesion which protruded out of anus and caused bleeding during defecation, and which showed as a 2.0 cm Yamada type III polypoid lesion. Endoscopic ultrasonography confirmed us that this lesion was confined to the mucosa and submucosa without extension into muscle layer. Endoscopic polypectomy of this lesion was performed because which considered as the main focus of current rectal bleeding. He was discharged without any complication and 4 months later, he was well without rectal bleeding and his hemoglobin level increased upto 13.5 g/dl. To our knowledge, this is the first case of Blue Rubber Bleb Nevus syndrome in Korea.

10.Total Knee Replacement in Gonarthrosis

We studied the results in forty-three knees in thirty patients-three male and twenty seven femalewho had rheumatoid arthritis, osteoarthritis, traumatic arthritis and osteonecrosis in the knee and were treated by total knee joint replacement. The average age of the patients was 56.7 years. The results were anaysed after follow-ups ranging from one year to three years and seven months, average being 23.6 months. The assessment was made upon the knee rating system of the Hospital for Special Surgery; the results were excellent in 29 knees, good in 12, fair in one and poor in one. The improvement of the flexion contractures was significant (postoperative average 1.7 degrees compared with preoperative average 20.1 degrees). The postoperative active flexion ranged from 70 degrees to 140 degrees (mean 110.2 degrees). One knee had a deep wound infection. The radiological loosening demonstrated in three cases, two of them had incomplete, nonprogressive radiolucent line, being less than one millimeter in width in the tibial component, one who had a deep infection showed three millimeters radiolucent line.
