2.The Immunohistochemical Study of Oncogene and Tumor Suppressor Gene Proteins on Bone Tumor.

The discovery of oncogenes and tumor suppressor genes have made it possible to partly understand the mechanism of cancer development. It is generally accepted that the cancer development is caused by specific gene alterations and now more than 100 oncogenes and suppressor genes are known to be involved in human carcinogenesis. However, there are only a few reports about oncogene expression on bone tumors. The author carried out an immunohistochemical study to reveal the oncogene and suppressor genes on carcinogenesis of bone tumors using antibodies against c-myc, c-H-ras, p53 and EGF. In 32 cases of osteochondrorma, EGF, p53 and c-myc antisera revealed positive reaction in 4 (12.5%), 2 (6.3%) and 7 (21.9%) cases, and, in 4 cases of chondrosarcoma, c-myc antisera revealed positive reaction in 2 (50%) cases. In 21 cases of osteosarcoma, the positive reaction of p53 was noted in 10 (47.6%) cases and that of c-myc in 3 (14.3%) cases. In 14 cases of fibrous bone tumors, there are only 2 (14.3%) cases of positive reaction with p53. These results suggest some roles of the p53 and c-myc genes in osteosarcoma development and c-myc gene in osteochondroma and chondrosarcoma development.

3.Intraarticular Osteochondromatosis of the Ankle Joint: A Case Report

Osteochondroma is the most common of the benign bone tumors. They are probably developmental malformations rather than true neoplasms. But rarely they develope in a joint, especially in the ankle joint. We reported a case of intraarticular osteochondromatosis of ankle joint.

4.Giant Intra-articular Osteochondroma of the Knee: A Case Report

The giant intra-articular osteochondroma of the knee is very rare and is probably result of synovial metaplasia of the fibrous capsule or of the adjacent connencive tissue of a joint, and of the fragment of articular cartilage. The gross findings and histology are consistent with the osteochondroma. The authors experienced a case of giant intra-articular osteochondroma of the right knee in a 19 years old man who had history of knee injury when he was 16 years old. The mass was excised and the symptome was relieved.

7.MR Cholangiography: Usefulness in Obstructive Jaundice.

PURPOSE: Three-dimensional(3D) magnetic resonance(MR) projection imaging was evaluated as a noninvasive alternative to direct cholangiography for the assessment of its reliability in patients with obsructive jaundice. METHODS AND MATERIALS: A heavily T2-weighted gradient-echo sequence(PSIF) was used for 3D MR projecion imaging of the biliary system in five healthy volunteers and 25 patients with obstructive jaundice. The 3D images of the bile ducts were formed by stacking consecutive coronal MR images obtained with a fast imaging method to a maximum-intensity projection algorithm. RESULTS: In the volunteers, MR cholangiography could demonstrate the anatomy of the biliary tract in only two subjects. The extrahepatic and intraheparic bile ducts were well visualized in 23 patients with obstructive jaundice. The leve of obstruction and the grade of dilatation were dipicted with MR cholangiography in all cases. And the cause of obstruction could be determined with MR cholangiography in 18 cases, MR cholangiography was successful in two patients in whom ERCP failed. The gallbladder or dilated pancreatic duct not demonstrated by means of direct cholangiography was demonstrated with MR cholangiography in six cases.

10.Clinical Study of Acetabular Erosion and Motion of Two Components in Bipolar Arthroplasty

The success of Moore and Thompson endoprosthesis in arthroplasty of the hip during the past three decades has prompted their continued use with but few alterations. Recently, to reduce the intolerance of acetabular cartilage and bone to frictional shear forces, the conventional endoprosthesis was modified, so many types of bipolar arthroplasty were introduced and implanted. But, the movements of the two components and acetabular erosion are rather controversial still. Among the 51 cases of bipolar endoprosthesis those have been operated at department of orthopaedic surgery, National Medical Center from 1981 to 1986, we analysed 40 cases for acetabular erosion and 22 cases for motion study of two components under the fluoroscopy. Following results were obtained, 1. Among 40 cases, there was no true protrusio acetabuli and increased CE angle in 12 cases(30%), which were limited to cartilage. 2. Under the fluoroscopic study, we found motion at inner and outer bearing joint in 14 cases, inner bearing joint only in 6 cases and outer bearing only in 2 cases. 3. The CE angle was increased more in younger age group than older group. 4. According to indication of bipolar arthroplasty, CE angle was increased more in disease group than trauma group. 5. The CE angle was increased more in leg length discrepancy and head piece size discrepancy group than the other group. 6. The CE angle can be substituted for joint space measurement. 7. The CE angle was increased more in low Harris hip rating score group. 8. Harris hip rating score had no correlation to motion of two components.
