1.Clinical Studies on 51 Cases of Varicocele.

A clinical observation was made on 51 cases of varicocele of the inpatient in the Department of Urology, National Police Hospital, during the period from January 1979 to April 1982. The results are as follows: 1. Left renal vein-internal spermatic confluence was joined right angle in all case and internal spermatic vein was demonstrated atonic, wide structure without constriction indicating valvular insufficiency in 8 cases. 2. The number of internal spermatic vein was between 1 and 4, showing the highest incidence in 2 vessels (8 cases). 3. The diameter of internal spermatic vein was between 2 mm and 6.5 ram, showing the highest incidence in 3 mm (8 cases). 4. This study showed lesser recurrence rate at the ligation of all Of the internal spermatic veins rather than ligation of the most markedly engorged one.

2.Morphological changes of the sensory retina following the occlusion of ciliary arteries in rabbits.

The whole retina, except for the medullary fiber zone in a rabbit eye, is supplied by choroidal circulation. Therefore, the histopathological changes of the sensory retina due to choroidal circulatory disturbance in rabbits may be comparable to that of the human sensory retina in the case of ophthalmic artery occlusion. This study was carried out to evaluate the histopathological changes of the ischemic retina secondary to the occlusion of choroidal circulation. The experimental occlusion of all posterior ciliary arteries and anterior ciliary arteries in the horizontal rectus muscle of rabbit eyes was performed and the subsequent histopathological changes of the sensory retina were observed by transmission electron microscopy. The morphological changes of the sensory retina following the occlusion of the ciliary arterial system are as follows: severe loss of the inner and outer segments of the photoreceptor, mild to moderate degeneration of the ganglion cells, and excellent preservation of the Muller's cell fibers and the extension of the cytoplasmic villous processes to the cytoplasmic vacuolar spaces of other degenerated cells. These findings indicate that the Muller's fibers in the ischemic condition of retina might contribute to the formation of gliosis or scarring of a damaged retina.

5.Expression of HSV-1 antigen in HSV-1 infected rabbit corneal cells in vitro.

This experiment was performed to determine whether cultured rabbit corneal cells were infected with HSV-1. The direct immunofluorescent method was used to detect the presence of the herpes simplex antigen in HSV-1 infected corneal epithelial cells, keratocytes, and endothelial cells in vitro with fluorescein-labeled antiserum to HSV-1. Immunofluorescent localization of the HSV-1 antigen was expressed in all three types of corneal cells.

6.Ultrastructure and blood-iris barrier in experimental rubeosis iridis in rabbit.

Iris neovascularization was produced in rabbits by hypotony following repeated aspiration of the vitreous. The hypotony was produced after 0.3 ml of vitreous fluid was aspirated using a 25-gauge needle through the pars plana of 10 rabbits. For the histochemical study, horseradish peroxidase(HRP) was injected through the ear lobe vein. After fixation of the iris tissue, the tissue was treated with diaminobenzidine and examined with both light microscopy and transmission electron microscopy. The newly-formed vessel was abundant, particularly on the upper stroma of the iris. The new vessel formation was evident due to the proliferation of endothelial cells, which may have been derived from preexisting iris vessels. The endothelial cells of the newly-formed vessels revealed prominent villous processes into the vascular lumen, formation of the marginal flap, numerous fenestrations in the endothelial junction, and reaction product onto extravascular space by the cytochemical electron microscopy. These results suggest that hypotony in the rabbit produces the disruption of the blood-iris barrier and the balance between angiogenesis-antiangiogenesis modulation.

7.Growth and Characterization of the Uveal Melanoma in Vitro.

This study was performed to observe the influence of the feeder layer on the culture of the human uveal melanoma cell. Two types of melanoma cells which were obtained from the patients with choroidal melanoma were implanted into the culture flask. The melanoma cell was seeded onto the culture dish covered with conjunctival fibroblast, Vero cell, and culture dish which did not contain the feeder layer respectively. The growth pattern of the melanoma cell was observed with phase contrast microscopy upto 30 days after seeding. There was no definite difference in growth pattern between each group. These results may indicate that the feeder layer is not an essential factor in the culture of uveal melanoma cell.

8.A Study of Individual Variation of Responsiveness to Vagal Stimulation in Children (Nonspectral Analysis).

BACKGROUND: It is well known that there is wide individual variation of responsiveness when parasympathetic nervous system is stimulated physiologically, pathologically, and therapeutically. But no appropriate explanation is present for that yet. So the purpose of this study is to verify individual diversity of vagal tone during resting state and after vagal stimulation and to find factor that may significantly contribute to it. METHOD: We serially measured variation of heart period(VHP : sec) which had been known to be an accurate index of cardiac parasympathetic tone during resting state(VHPb) and after vagal stimulation by ice water application to face(VHPv). And we analyzed correlation between VHPb, VHPv and ratio of VHPv to VHPb(ratio). RESULT: There was wide variation of vagal tone both in resting state (VHPb : 0.025-0.161) and after vagal stimulation (VHPv : 0.087-0.661). VHPb could be classified into 3 groups according to stem and leaf distribution(group A : VHPb could be classified into 3 groups according to stem and leaf distribution(group A : VHPb<0.07, B : 0.07-0.1, C : >0.1). The low VHPb group A showed dramatic increase in vagal tone after vagal stimulation (ratio 2.21-20.24 mean 7.7), on the other hand high VHPb group C did not (ratio 0.80-2.55 mean 1.39). There ws also a significant negative correlation between VHPb and ratio (r=0.69, p<0.001). CONCLUSION: These results suggest that there is wide individual diversity of responsiveness to vagal stimulation and magnitude of vagal tone in resting state is an important factor to determine the responsiveness to vagal stimulation.

9.The Onset of Coronary Artery Aneurysm and Changes of T Cell Subsets and the Effect of High Dose Gammaglobulin Therapy in Mucocutanenous Lymph Node Syndrome.

We studied the onset of coronary artery aneurysm formation in 42 patients of Mucocutaneous lymph node syndrome (MCLS), the serial changes of T cell subsets, and serum immunoglobulin levels including lgG, lgA, lgM, in 13 patients among them during acute (before high dose gamma-globulin therapy ) and subacute phase (after gamma-globulin therapy) to evaluate the role of the immunoregulatory abnormalities in coronary artery aneurysm formation. The following results are obtainded : 1) The coronary artery aneurysm was found in 16 patients (38%). They were detected in between 6th and 13th day of illness with the mean of 8.8+/-2.1 day. 2) The total T lymphocyte (T1), helper T lymphocyte (T4), and helper/supperessor ratio (T4/T8) decreased during acute phase. 3) Following gamma-globulin therapy it showed a increase in T4,T8, and all immunoglobulines. Above results showed that the most of aneurysms developed during acute phase and suppressed, rather than enhanced, T cell function significantly contributed to aneurysm formation, whereas high dose gamma-globulin retarded or reversed this suppression which would be one of mechanism of beneficial effort of gamma-globulin in MCLS.

10.Clinical Significance of Periampullary Choledochoduodenal Fistula.

Sine 1976, 2380 eases of ERCP were performed at the Kwangju Chrietian Hospital. Nighty eight patients with periampullary choledochaduodenal fistula diagnosed at ERCP were revewed, Following result were obtained 1) Cases were most commonly noted from fifth decade to seventh decade, and sex distribution was similar. 2) Right upper quadrant abdominal was most common clinical manifestation, and duration of illness prior to diagnosis were from 2 months to 20 years. Several weeks or days before ERCP, sudden disappearance of pain suggeeted spontaneous extrustion of a stone from the common bile duct to the duodenum with resultant PACD formation. 3) On past history, various types biliay operations had carried out in 26 patients. Subtotal gastrectomy with gastrojejunostomy in 1 patients, operation for CBD ascaris removal in 1 patient. 4) Most PACDF had finding formed recently on ERCP and fibrosis, inflamation, granulation, atrophic change, adhesive stenosis were noted additionally. 5) Most common cause of PACD by ERCP was biliary teact stone. 6) Endoscopic sphincteropapillotomy(EST) were performed in 39 patients with PACDF due to CBD stone and surgical intervention were done in 36 patients due to CBD stone also. It was interesting that EST were performed in 3 patients with old stenotic fistula.
