1.Malignant Fibrous Histiocytoma: A Case Report

Malignant fibrous histiocytoma has recently become an established diagnosis for some tumors that arise in the musculo-skeletal tissue. But it contains areas that resemble with osteosarcoma, fibrosarcoma, the microscopic lesions are composed of interlacing bands of fibroblast, bizzare histiocytes and multinucleated giant cells, arranged in storiform pattern. At present, the treatment advocated for malignant fibrous histiocytoma has been total surgical removal and radiation therapy. A case of malignant fibrous histiocytoma affecting the proximal tibia is reported with brief review of literature.

2.The Diagnostic Value of the Synovial Biopsy by Franklin-Silverman Needle

Since last August, 29 cases of synovial punch biopsy have been carried out with Franklin-Silverman needle which was used in liver biopsy, instead of the specialized needle for the synovium. Authors report the applicable utility of Franklin-Silverman needle for the synovial biopsy, diagnostic value of this procedure and histopathologic findings of the results. The rate of the technical success to abtain synovial tissue was about 76%. Complications after procedure have been very rarely encountered, consisting of mild transient hemarthrosis. In 17 cases, the histopathologic changes was compatible with their clinical findings. In the remainders, we experienced the failures to get synovial tiasue at all in 7 cases and to give the significant synovial changes in 5 successful specimens. Fortunately, the important diagnostic aid was achieved in 3 cases which were conformed early diagnosis of tuberculous arthritis. This needle biopsy was also thought to contribute to rule out the gouty arthritis from other hyperuricemic conditions. But the histologic changes were non specific inflammatory findings in the synovial mambrane of rheumatoid arthritis, osteoarthritis, traumatic and infectious arthritis.

4.Radiologic findings of mesoblastic nephroma.

The mesoblastic nephroma is a rare benign renal tumor that frequently appears as a neonatal abdominal mass. This benign tumor is composed primarily of connective tissue that growth between intact nephrons and often replaces most of the renal parenchyme. Contrast media within the calyceal systems are seen within the tumor representing function by nephrons trapped within the mass. Prognosis after complete excision is excellent.

A case of multiple hamartomas of the unilateral chest wall in a four month old infant is presented. There have been a few reports on the CT findings of the chest wall hamartoma in infancy. We describe bone changes of the ribs and mineralization of this rare tumor on the CT scan, and the locations of two separate masses.

7.Surgical Treatment of Brachial Plexus Injury

The author has reviewed 62 patients with brachial plexus injuries who treated at Department of orthopaedic Surgery, Kyung Hee University Hospital during the period from Dec. 1978 to June 1985. Among these, conservative treatment was performed in 8 patients, 64 cases of 54 patients were treated by 4 types of operation, that is, exploration with neurolysis, neurorraphy, nerve graft and neurotization for restoration of elbow flexion, wrist, finger flexion and shoulder abduction. The patients were followed up more than one year to five years and eight months, average being two yesrs and six months. The whole arm type injury was the most common. The trunk level was the most common. The prognosis of whole arm type was the most severe. And the prognosis of supraclavicular lesion was worse than that of infraclavicular. There were two types of treatment, conservative treatment in 8 patients, operative treatment in 54 patients. By comparing results according to methods of treatment, neurotization with intercostal nerve were useful in avulsion cases of unsatisfactory results following neurolysis, neurorraphy and nerve graft. The operation time of the first three months to six months after injury gave the best chance of success. The result of conservative treatment in all 8 patients was poor.

8.Carpal Tunnel Syndrome among workers in a condom industry.

The objectives of this study are to investigate the prevalence of occupation related carpal tunnel syndrome(CTS) among workers in a condom industry; to analyse the sensitivity and specificity of clinical signs or symptoms such as hand diagram, Tinel's sign and Phalen's sign in carpal tunnel syndrome; and to test vibration threshold test using audiometry as a technically easy and noninvasive method in the diagnosis of carpal tunnel syndrome in stead of nerve conduction velocity(NCV). The study group was divided into exposed group(39 cases)and non-exposed group(48 cases) based on whether or not excessive use of wrist movements exist. 1. There are statistically significant differences in symptoms and signs of carpal tunnel syndrome such as hand diagram, Tinel's sign and Phalen's sign between exposed and non-exposed group(p<0.05). 2. Six cases(9 hands) were confirmed as carpal tunnel syndrome by NCV. Five cases(7 hands) belonged to exposed group, 1 case(2 hands) to nonexposed group. As there are significant differences in prevalence of carpal tunnel syndrome between two groups(p<0.05), excessive use of wrist in occupation is a risk factor of carpal tunnel syndrome. 3. When we use NCV as a gold standard in the diagnosis of carpal tunnel syndrome, sensitivity and specificity of hand diagram, Tinel's sign and Phalen's sign is as followed; hand diagram; sensitivity 88.9%, specificity 84.2%. Tinel's sign; sensitivity 55.6%, specificity 72.8%. Phalen's sign; sensitivity 14.3%, specificity 88.4%. Among above clinical signs and symptoms, hand diagram is the best clinical screening test. 4. The differences of vibration threshold between median and ulnar nerve at the same time are useful in the diagnosis of carpal tunnel syndrome but the time change of vibration threshold of median nerve over time are not sensitive enough. It is concluded that vibration threshold between median and ulnar nerve at the same time can be used as a supplementary or alternative criterion to indicate that the nerve dysfunction is located in the carpal tunnel.

9.Clinical Studies on 226 Cases of Stomach Cancer.

Among 7,500 patients whom we have performed gastrofiberoscopy from July 1979 to June 1,982, 226 cases of stomach cancer were diagnosed. The frequency was 3. 0%.
