2.Management of Acetabular Fractures

The treatment of acetabular fractures is composed of operative and conservative methods, however, they are still controversial. Generally, because the acetabular fracture involves the joint surface and one sequalae such as post-traumatic arthritis can follow treatment, therefore an accurate reduction and postoperative early mobilization to obtain the prevenation of joint stiffness and arthritis are required. In order to achieve these purposes, we have studied and done a comparative survey between conservative and operative treatment on 55 patients with acetabular fractures, who were treated at our department from Jan. 1982 to Dec. 1988. Results were obtained as follows. 1. The prevalent age was the third decade and the cause of injury was traffic accidents in the majority. 2. The most common type of fracture was a posterior wall by Letournel classification. 3. Satisfactory results of operative treatment were 76% and conservative treatment 47%. 4. The complications were post-traumatic arthritis, ectopic ossification, peroneal nerve palsy and skin infection. 5. In the majority of displaced acetabular fractures, ORIF was recommended.

3.A Case of Portal and Splenic Vein Thrombosis Developed by Complication of Histoacryl Injection Therapy in Gastric Varix.

Liquid tissue adheisve, Histoacryl (n-butyl-2-cyanoacrylate) has been used for the treatment of gastric variceal bleeding. The techniques are as follows: 0.5cc Histoacryl mixed with Lipiodol per each injection are used. 3 to 4 injections are usually required for large variceal convolutes under the fluoroscopic visualization after the injection of Histoacryl. Complications of sclerotherapy with Histoacryl are bleeding, perforation, stenosis and embolism depending on the concentration and amount used, as well as the intensity of the treatment. Here we describe a case report developing portal and splenic vein thrombosis as a side effect after Histoacryl injection therapy for the treatment of gastric varix bleeding, A 59-year-old male patient with liver cirrhosis was admitted due to acute gastric varix bleeding. The control of gastrie variceal bleeding was achieved by several injections of 0.7c mixture of 0.5cc histoacryl and 0,8cc Lipiodol. However, simple X-ray and ultrasonography revealed the elements of Histoacryl-lipiodol mixture in the portal and splenic vein.

Endometriosis in the abdominal wall following Cesarean section is one of the very rare condition among the extrapelvic endometriosis. Although benign, endometriosis possesses the unique ability to invade tissue and to disseminate or metastasize by hematogeneous, lymphatic route, or direct implantation. We reported the case with brief review of the literature.

5.A Case of Polysplenia Associated with Complicated Cardiovascular Anomalies.

Splenic anomalies are commonly associated with anomalies of cardiovascular system and of other organ systems. "Polysplenia" refers to the state in which the bulk of splenic tissue is divided into two and others more equal-sized splenic masses. We experienced a case of Polysplenia associated with complicated cardiovascular anomalies(Situs ambiguus, D-loop, normally related great arteries, interrupted inferior vena cave, bilateral superior vena cava, single atrium, common A-V canal, infundibular stenosis). We have presented this rare case and reviewed brief literatures.

6.Psychiatric Diagnosis and Hypersexual Behavior Inventory, Sexual Addiction Screening Test, and Beck Depression Inventory/Beck Anxiety Inventory of the Sexual Offenders.

The current study evaluated DSM-IV psychiatric diagnoses and investigated the psychological characteristics of sexual offenders by using the Hypersexual Behavior Inventory(HBI), Sexual Addiction Screening Test (SAST), Beck Depression Inventory (BDI), and Beck Anxiety Inventory (BAI). Fifty male sexual offenders and 36 male healthy controls completed the psychiatric diagnosis and 30 of the 50 male sexual offenders completed the psychological tests listed above. Thirty-two of the 50 sexual offenders were patients with paraphilias, these sexual offenders had been admitted by the Ministry of Justice to the Institute for Forensic Psychiatry in Gongjui-si due to sexual violence. Participants displayed high lifetime rates of psychiatric disorders: 32 (64%) had paraphilias, 16 (32%) had depressive disorders, 10 (20%) had anxiety disorders, 9 (18%) had impulse control disorders, 6 (12%) had schizophrenia, 2 (4%) had bipolar disorders, and 16 (32%) had personality disorders or some other Axis II disorder. Scores for the sexual offenders were significantly higher both on the HBI and the SAST than the comparison group. Scores for the sexual offenders were also higher for the BDI and the BAI as compared to the control group, but this difference was not statistically significant. Results indicate that sexual offenders may have sexual addictions and be hypersexual. In addition to depressive and anxious psychopathologies, they also seem to have sexual psychopathologies. These results suggested that the psychopathology of sexual offenders may be different than that of a control group.

7.Radiological evaluation of G-I tract diverticulum in Korean

We reviewed 887 cases of esohagogram, 8863 cases of UGI series, 174 cases of small bowel series and 1926 casesof double contrast barium enema performed at the department of Radiology, Wonju College of Medicine from Jan. 1982 to Dec. 1984 to analyzed diverticula disease pattern of the GI tract in Korean. The results were as follows: 1.Esophageal diverticula The incidence was 3.27% and the sex ratio of male to female was 2.22:1 Age distribution wasrelatively even and most common in 5th decade. Most of them showed single in number, above 6mm sized and common inmiddle one third of both lateral side of esophagus. 2. Stomach diverticula The incidence was 0.07% and the sexratio of male to female was equal. Multiplicity was single in all cases. Most of them were above 11mm sized andcommon in gastric fundic area of greater curvature site of stomach. 3. Duodenal diverticula The incidence was1.51% and relatively even distribution in sex and age and common in after 5th decade. Most of them showed singlein number, 11-30mm sized and common in medial margin of 2nd portion of duodenum. 4. Colonic diverticula Theincidence was 2.34% and predominant in male and common in 5th. and 6th. decade. Most of them showed single innumber, below 5mm sized and common in right sided colon.

9.The effect of Korean mistletoe extract M11C (non-lectin components) on IL-1beta release and expression from macrophages.

BACKGROUND: Korean mistletoe (Viscum album) extract has been found to posses immunostimulatory activity. In this study, Korean mistletoe extract, M11C (non-lectin components), was used to know whether this extract might activate mouse peritoneal macrophages to produce interleukin 1 beta (IL-1 beta). METHODS: Hemagglutination assay was carried out to examine whether M11C contained a lectin or not . To know the effect of M11C on the production of IL-1 beta, the macrophages were treated by the M11C, and then collected the supernatant (M11C stimulated macrophages-conditioned media; MMCM). MMCM was analyzed for the IL-1 beta quantification and mRNA expression by means of ELISA and RT-PCR, respectively. RESULTS: Maximum effective dose and time of M11C on IL-1 beta production from macrophages were 20 micro gram/ml and 8 hours, respectively . This ELISA data was reconfirmed by immunoblotting assay . indicating that M11C is a good candidate for an immunomodulator. The dose and time dependent effect s of M11C on the expression of IL-1 beta mRNA from macrophages was also shown in expression of mRNA detected by RT-PCR. Treatment dose and time for the maximum expression of IL-1 beta mRNA were 20 micro gram/ml and 4 hours, respectively . Maximum gene expression of IL-1 beta was much earlier than maximum production of it. CONCLUSION: As results, Korean mistletoe extract, M11C, may be used for an immunomodulator. This will be able to make up for and solve the problems caused by existent immunoagent with many adverse effects through many other studies in future including one molecule extraction.

10.The Clinical Study of Lacrimal Sump Syndrome after Endonasal Dacryocystorhinostomy.

PURPOSE: To investigate the incidence and importance of diagnosis of lacrimal sump syndrome after endonasal dacryocystorhinostomy (DCR). METHODS: A retrospective analysis was performed on 837 eyes in 732 patients, (male 196, female 536) who had undergone endonasal DCR between January 2001 and June 2004. Dacryocystography (DCG) was performed in those patients who had recurrent epiphora or symptoms of dacryocystitis without saline regurgitation on syringing. RESULTS: Among the 90 eyes (10.8%) who had recurrent epiphora after endonasal DCR, 8 eyes were diagnosed with lacrimal sump syndrome and this comprised 8.9% of the causes of DCR failure. CONCLUSIONS: Lacrimal sump syndrome should be considered in the case of patients with recurrent epiphora. However, normal syringing finding after endonasal DCR, and revision may relieve the patients's symptom.
