Measurement of the normal pituitary gland height was performed on magnetic resonance (MR) images of 144 subjects with no known or suspected pituitary or hypothalamic diseases. Midsagittal T1 weighted images (T1WI) were used for measurement, and mean vertical height according to age and sex group was obtained. In all age groups, the pituitary height was greater in females than in meles, The group of 0-9 years in both genders showed the minimum mean pituitary height, The maximum mean height was observed in the 10-19 years age group in both genders. The height gradually decreased with increasing age after age 20 years. There was no subject with a height more than 9.0 mm in females or 8.0 mm in males. In conclusion the measurement of the normal pituitary pland height using mid sagittal MR imaging can be used for the evaluation of the pituitary gland lesions.

PURPOSE: To evaluate abnormal neurosonographic (NSG) findings of thalami and basal ganglia in full term babies with hypoxic-ischemic encephalopathy and to correlate the findings with follow-up studies and prognosis. METHODS: We evaluated 13 full term babies with abnormal NSG findings of thalarni and basal ganglia. NSG was performed within 7 days after clinical abnormalities. Follow-up NSG was done in 11 cases; CT scan in 4 and MRI in 7. We classified NSG findings as diffuse, unilateral, and focal types according to increased echogenicity and evaluated prognosis based on follow-up studies and neurological sequelae. RESULTS: Nine cases of diffuse type had diffuse echogenic changes of bilateral thalami and basal ganglia, slit-like lateral ventricles suggesting cerebral edema, and increased parenchymal echogenicity. In diffuse type, follow-up studies showed more prominent echogencities and ventricular dilatations and cerebromalacia. One case of unilateral type caused by thromboembolism had unilateral echogenicity of right thalamus and basal ganglia with increased echogenicity of the ipsilateral cerebral hemisphere and compression of the lateral ventricle, suggesting cerebral infarction. Follow-up study showed unilateral cystic cerebromalacia. Three cases of focal type had a localized echogenic area in thalamus with lacunar infarction, which decreased in size during follow-up. Among nine cases of diffuse type, one died within 2 days, two were discharged against medical advice, and six had severe neurologic sequelae. One case of unilateral type had a moderate degree of neurologic sequelae. All 3 cases of focal type had normal development. CONCLUSION: Pattems of abnormal echogenicity in thalami and basal ganglia in fullterm infants with hypoxic-ischemic encephalopathy are correlated with the outcome and may be helpful for treatment planning.

Iris neovascularization was produced in rabbits by hypotony following repeated aspiration of the vitreous. The hypotony was produced after 0.3 ml of vitreous fluid was aspirated using a 25-gauge needle through the pars plana of 10 rabbits. For the histochemical study, horseradish peroxidase(HRP) was injected through the ear lobe vein. After fixation of the iris tissue, the tissue was treated with diaminobenzidine and examined with both light microscopy and transmission electron microscopy. The newly-formed vessel was abundant, particularly on the upper stroma of the iris. The new vessel formation was evident due to the proliferation of endothelial cells, which may have been derived from preexisting iris vessels. The endothelial cells of the newly-formed vessels revealed prominent villous processes into the vascular lumen, formation of the marginal flap, numerous fenestrations in the endothelial junction, and reaction product onto extravascular space by the cytochemical electron microscopy. These results suggest that hypotony in the rabbit produces the disruption of the blood-iris barrier and the balance between angiogenesis-antiangiogenesis modulation.

To increase the success rate of intraocular pressure control in recalcitrant glaucoma, anterior chamber tube shunt to a surgical membrane (ACTSSM) surgery using silicone tube and expanded polytetrafluoroethylene (e-PTFE) can be performed. Applying mitomycin C (MMC) during ACTSSM surgery may increase the success rate by decreasing the fibroblast proliferation and collagen in the fibrous capsule. To evaluate the effects of MMC on the fibrous capsule formed after ACTSSM surgery, operations were performed on 18 white rabbits. Nine rabbits were treated with 0.04% MMC solution on the episclera for 5 minutes before ACTSSM (group A) and the others were not treated (group B). Among the complications of intracranial surgery, postoperative hemorrhage remote from the operative site is rare, especially when it occurs in the cerebellum after surgery involving the supratentorial region. We report three cases of cerebellar hemorrhage after supratentorial surgery : 1) A 66-year-old man with subdural hygroma in both frontotemporal areas ; 2) A 56-year-old man with chronic subdural hematoma in the left frontotemporal area ; 3) A 39-year-old woman with aneurysms at the right middle cerebral artery bifurcation and basilar tip. The possible etiologies of cerebellar hemorrhage in the three cases presented here were reviewed. We speculate that when the patient is in the supine position with head rotated and extended, the displacement of the cerebellum causes stretching of the superior vermian veins and their tributaries, resulting in tearing of these vessels, and that a sudden decrease in intracranial pressure may accelerate this process. To prevent this complication, meticulous perioperative management of the patient is essential.

PURPOSE: To determine the usefulness of MR urography technique for the evaluation of urinary systems in patients with obstructed urinary tract and normal volunteers with non-obstructed urinary tract after intravenous normal saline and diuretic injection. MATERIALS AND METHODS: Three normal volunteers and 12 patients with urinary tract obstruction [ureteral calculi(n=8), extraurinary mass(n=1), ureteral tumor invasion(n=3)] underwent MR urography using a 1.0T scanner and a 2D non-breath-hold heavily T2-weighted fast spin-echo sequence. These acquisition were postprocessed with a maximum intensity projection (MIP) algorithm. Two acquisitions were performed, the first prior to saline solution infusion following standard MR urography procedures, and the second, within 2-3 minutes of the infusion of 250 ml saline solution followed by 20 mg of Lasix administered intravenously. For this latter, drug-induced MR urography procedures were followed. RESULTS: In healthy volunteers (n=3) and those experiencing partial obstruction (n=4) by a urinary stone, drug-induced MR urography provided better images of the urinary tract than did standard MR urography. In those in whom a urinary stone or tumor had caused complete obstruction (n=8), standard MR urography provided good images, as did drug-induced MR urography. CONCLUSION: In patients with a partially or non-obstructed urinary tract, drug-induced MR urography provided better anatomic and functional details of the kidney and urinary tract than did standard MR urography. In those experiencing complete obstruction of the urinary tract, however, standard or drug-induced MR urography permitted very adequate evaluation of the tract, and drug-induced MR urography was unnecessary.

The changes of corneal curvatures were measured in 63 cataract cases before and 2 months after cataract extraction with the aid of ophthalmometer and photo keratoscope. The change of the cornea in the vertical curvature was shown to be greater than that of horizontal curvature, in cases of conventional large corneal incision(120-170 degrees) of cataract surgery. Vertical curvature of the cornea was decreased and horizontal curvature was increased. The corneal astigmatism before and 2 months after surgery were respectively 0.67D. and 1.56D. showing the difference of 0.89D.

The ulcerative colitis is a relatively common disease in the European and North American countries aince Dr. Wilks has first reported the cases in 1895, But in Korea, this disease is a rare entity and only a few case have been reported. However, rescently the diagnoetic methods for ulcerative colitis are much developed we have experienced 28 cases of ulcerative colitis which were diagnosed by endoscopy, barium enema exam and biopsy. Therefore a total of 28 patient with ulcerative colitis diagnosed and treated at the department of internal medicine of HYUH from June 1979 to June 1986 was reviewed. We obtained the results as follows; 1) The sex distribution assumed a ratio of 1 to 1.33 with 12 males and 16 females. 2) The age dietribution was relatively even but the majority of cases were between 5th and 6th decade (32.1%), 3) The most prominent clinical symptoms were rectal bleeding, abdominal pain, diarrhea, fever, wight loss and vomiting. 4) Significant laboratory findins were anemia, eleveited ESR, leukocytosis, positive stool OB, electrolyte imbalance and decreased serum albumin level. 5) As to the extent of disease determined by the barium enema examination and endoscopic exam., rectum or rectosigmoid colon is involved in majority of the cases (92.8%). 6) The endoscopic examination was performed in all 28 cases and showed significant findings such as ulceration, hyperemia, mucosal friability, bleeding etcs. 7) The barium enema examination was performed in 19 cases and showed positive findings such as ulceration, luminal narrowing, bowel shortening and rigidity in 17 cases. 8) 21 of 28 patients treated by medical therapy showed relatively improved but 4 of 28 patients was exacerbated or relapsed and one has expired.
