4.A Case of Subungual Exostosis.

Subungual exostosis is a solitary fibrus nodule on the inner border of the terminal phalanx of the great toe. Rarely, the terminal phalanges of other toes, particularly the little toe, or even the fingers are involved. The first appearance is a small pinkish growth projecting slightly beyond the inner free edge of the nail, which pushes upward and backward. A case of Subungual exostosis affecting 20 year old man, who has been suffering from bean sized nolule on subungual area of terminal phalanx of left 5th toe. The skin iesion developed 1 year ago, and increased size insidiously and tendemess on pressure or tight shoes was noted. Diagnosis was confirmed by histopathologic and radiologic studies, and treated witb complete excision of tumor mass.

5.Position and Width of Normal Adult Optic Chiasm as Measured in Coronal MRI.

PURPOSE: Purpose :To evaluate the position and transverse dimension of the adult optic chiasm in normal Korean adult. MATERIALS AND METHODS: The authors analysed 3D coronal volume images (TR/TE= 30/13, flip angle= 30 degrees) of 136 normal adult subjects without known visual abnormality. All MRI examinations were performed using a 0.5T system. MRI was reviewed retrospectively to determine the position (horizontal and tilted) of the potic chiosm and the transverse dimension of the optic chiasm was measured. RESULTS: Seventy-five (55%) of 136 normal subjects had horizontal position, and sixty-one (45%) had tilted position. Thirty-eight (62%) of 61 with tilted position showed higher position on the right side, and twenty-three (38%) showed higher position on the side. The average transverse dimension(mean SD) was 15.2 +/- 0.7mm in men and 14.6 +/- 1.0mm in women. The difference of transverse dimension between men and women was statistically significant. CONCLUSION: Tilted position of the adult optic chiasm on coronal MRI was seen in approximately half of normal adults. The average of transverse dimension of normal optic chiasm was 15mm.

Recently, a benign variant of scleroderma, called the C.R.S. T, syndrome, has ben ibed by Winterbauer. Patients with this disorder have skin changes such as Cermal and subr,utaneous calcinosis, Raynauds phenomenon, sclerodactyly, and te1angiectasia. A case of C.R.S.T. syndrome in 25-year-old femala in reported with brief reviev of literatures. The patient showed miultiple pea sized nodular depoaits on dermal and subcuta.neous layers of both elbow and. knee joints, Rayaauds phenamenon, sclerrodactyly with acroselerosis and sclerodermatous change on both hands and forearm and telangiectasia on face and both palms On eeophagogram, slight disturhance of passage at the cardioesophageal junetion area was noted.

7.Changes of Ranges of Motion According to Ages and Manifestation Frequency of Impingement Sign in Shoulder Impingement Syndrome.

To analyze the changes of range of motion and manifestation frequency of impingement sign according to ages in shoulder impingement syndrome after excluding complete rotator tears by MRI, the authors reviewed 74 patients whose impingement injection tests to the subacromial space were positive in the Dankook university hospital. We used paired student t-test for comparison with the contralateral shoulder. l. Average forward elevation, abduction, external rotation and internal rotation of the shoulders were not significantly different in stage I and II, but significantly different statistically in stage II over 40 years old. 2. Average external rotation at 90degrees abductions of the shoulders was not significantly different statistically in stage I, II and stage II over 40 years old. 3. Frequencies of the painful arc were 83% in stage I, 71% in stage II and 58 % in stage II over 40 years old. Physical examinations of painful arc in 1 case of stage II, 7 cases of stage II over 40 years old were impossible because of limitation of motion. 4. Frequencies of lateral impingement sign were 100% in stage I, 86% in stage II, 100% in stage II over 40 years old. Frequencies of Neer's impingement sign were 75% in stage I, 67% in stage II, 88% in stage II over 40 years old. Frequencies of Hawkins impingement sign were 67% in stage I, 62% in stage II and 82% in stage over 40 years old. Except external rotation at 90degrees abduction, range of motion of the shoulder may be decreased in impingement syndrome over 40 years old. Because of difference of frequencies, more than two impingement signs should be tested for the diagnosis of shoulder impingement syndrome.
