Both idiopathic pulmonary fibrosis (IPF) and pulmonary involvement of collagen vascular disease(CVD) are well known cause of diffuse interstitial lung disease which lead to fibrosis and honeycombing. We analyzed HRCT findings of 33 patients with IPF and 14 patients with CVD in terms of predominant pattern, site of involvement, mediastinal lymph node enlargement, pleural change and pulmonary volume loss. Criteria of mediastinal lymph node enlargement and pleural thickening were 15mm in long diameter and 3mm, respectively. Volume loss of the lung was measured by using hilar height ratio (apex to hilum/hilum to diaphragmatic dome). Mean age was 61 years for IPF and 46 years for CVD and male: female ratio was 27:6, 4:10, respectively. Predominant HRCT pattern was honeycombing for IPF (63%), and ground-glass opacity for CVD (66%) (p=0.001). Predominantly, subpleural involvement was seen in 90% for IPF and 74% for CVD. Mediastinal lymph node enlargement was seen in 47% of the patient with IPF and 14% with CVD (p=0.004). pleural thickening was seen in 97% of the patients with IPF and 42% with CVD (P=0.002), Pleural effusion was seen in 10% of the patients with IPF and 36% with CVD (P=0.009). Hilar height ratio of more than 1.5 was seen in 84% of the patients with IPF and 29% with CVD. In conclusion, our study shows that patients with IPF are prone to have more progressed stage of pulmonary fibrosis than the patients with CVD on HRCT.

PURPOSE: We investigated reproducibility of the quantification of left ventricular volume and ejection fraction, and grading of myocardial wall motion and systolic thickening when we used gated myocardial SPECT and Cedars quantification software. MATERIALS AND METHODS: We performed gated myocardial SPECT in 33 consecutive patients twice in the same position after Tc-99m-MIBI SPECT. We used 16 frames per cycle for the gatingof sequential Tc-99m-MTBI SPECT. After reconstruction, we used Cedars quantitative gated SPECT and calculated ventricular volume and ejection fraction (EF), Wall motion was graded using 5 point score. Wall thickening was graded using 4 point score. Coefficient of variation for re-examination of volume and fraction were calculated. Kappa values (k-value) for assessing reproducibility of wall motion or wall thickening were calculated. RESULTS: Enddiastolic volumes (EDV) ranged from 58 mi to 248 ml (122 ml +/- 42 ml), endsystolic volumes (ESV) from 20 mi to 174 mi (65 ml +1- 39 ml), and EF from 20% to 68% (51% +/- 14%). Geometric mean of standard deviations of 33 patients was 5.0 ml for EDV, 3.9 ml for ESV and 1.9% for EF. Their average differences were not different from zero (p>0.05). k-value for wall motion using 2 consecutive images was 0.76 (confidence interval: 0.71-0.81). k-value was 0.87 (confidence interval:0.83-0.90) for assessment of wall thickening. CONCLUSION: We concluded that quantification of functional indices, assessment of wall motion and wall thickening using gated Tc-99m-MIBI SPECT was reproducible and we could use this method for the evaluation of short-acting drug effect.

OBJECTIVES: To determine the effectiveness and clinical utility of antenatal corticosteroids in the reduction of neonatal morbidity and mortality on preterm birth Material and method: Neonatal outcomes of 312 preterm babies were evaluated retrospectively. One hundred and two preterm babies(study group) were given dexamethasone more than 1 dose antenatally and 210 preterm babies(control group) were not given dexamethasone antenatally. Antenatal steroids were administered in the form of four 5mg intramuscular doses of dexamethasone 12 hours apart. Maternal and neonatal outcomes of study group were compared with those of control group. Student t- test, x2 test, Fisher's exact test, and logistic regression analysis were used where appropriate. p-value< 0.05 was considered significant. RESULTS: Antentenatal corticosteroid significantly decreased the incidence of RDS(OR:0.47, 95% CI:0.25-0.86), IVH/PVL(OR : 0.32, 95% CI : 0.12-0.86), necrotizing enterocolitis(OR : 0.49, 95% CI : 0.25-0.98), and neonatal death(OR: 0.30, 95% CI: 0.10 - 0.89) in preterm delivery. In the presence of PROM, antenatal corticosteoid seemed to have no protective effect on the neonatal complications such as RDS, IVH/PVL, NEC, PDA, and neonatal death. CONCLUSIONS: Antenatal administration of corticosteroids was effective to decrease the incidence of neonatal morbidity and neonatal mortality in the preterm neonates with no apparent maternal complications.

OBJECTIVES: This study was conducted to investigate the associations between obesity and the children's Nutrition Quotient (NQ) and to further examine the relationships between NQ and mini dietary assessment (MDA) of their parents. METHODS: The subjects were 355 children aged 3 to 5 years and their parents in Dongducheon. We collected parental-reported NQ questionnaires for children and MDA of parents. Anthropometric measurements, height, weight and BMI by BIA were measured in children. Overweight and obesity were determined according to age- and sex-specific BMI percentile from the 2007 Korean national growth chart. Statistical analyses consisted of the chi-squared test, ANOVA, partial correlations and logistic regression analysis adjustments for parents BMI. RESULTS: Approximately 20.8% of preschool children were classified as overweight or obese. Underweight children showed a significantly higher score for balance than overweight children. The NQ of the children was 61.9 ± 11.6, and NQ scores and their parents' MDA did not exhibit any significant differences according to degree of obesity. After adjusting for parent's BMI, children's BMI was significantly correlated with balance and moderation among NQ factors. Parent's MDA showed significant correlation with their children's NQ, balance, diversity, moderation, regularity, practice and NQ grade, except for diversity of father. Additionally, NQ grade had a significantly increased (150.1%) odds ratio (OR) of being overweight (95% CI 1.008–2.234). CONCLUSIONS: These results show that NQ for children is influenced by their parents' MDA and BMI. Furthermore, our findings support the association between overweight prevention and improvement of NQ grade among preschool children.

BACKGROUND: Legionella pneumophila has been recognized as an important cause of community-acquired pneumonia(CAP) requiring hospitalization. However, epidemiological data on the occurrence of legionella-related pneumonia is unavailable in Korea. The purpose of this study was to evaluated the etiological imprtance of legionella pneumophila serogroup 1 in patients hospitalized with CAP. METHOD: The CAP patient over 16 year-old were recruited from July 1999 to June 2000 at the Chunchon Sacred Heart Hospital. Fifty four patients (male 29, female 25, average age 63.8±15.3) were included in this study. A diagnosis of a legionella pneumophila infection was based on a urinary antigen test using the Binax Company enzyme immunoassay. The severity of pneumonia was assessed using the Fine's PORT scoring system. RESULT: The average Fine's PORT score was 99.7(±44.9). According to the risk classification proposed by the Infectious Disease Society of America, the number of patients in each class(from class I to class V) were 6(11.1%), 13(24.1%), 9(16.7%), 14(25.8%), and 12(22.2%), respectively. Thirty two patients(59.3%) were initially admitted to the intensive care unit. The mortality rate was 16.7%(9 in 54). In all patients, urinary antigens to Legionella pneumophila serogroup 1 were not detected. CONCLUSION: Legionella pneumophila may play little role in causing adult CAP in Korea. Therefore, the routine use of macrolide in the empirical treatment of the CAP patients based upon the ATS guidelines(1993) in Korea should be reevaluated.

BACKGROUND: It was known that conventional stress-redistribution imaging was not adequate for detection of severely ischemic but viable myocardium. Albeit the gold criteria of viable myocardium is the presence of metabolism which can be detected by PET, reinjection technique was reported to be able to identify most, if not all, of viable myocardium. Because reinjection imaging is performed immediately after redistribution imaging, an additional redistribution could be happened if we follow the patient longer. To prove the guess authors performed an additional delayed imaging 24 hours after reinjection of 201T1. METHODS: Subject patients were 20 ischemic heart disease patients who showed irreversible perfusion defect(s) on standard pharmacologic(dipyridamole) stress-redistribution images. Immediately after the redistribution images were obtained, 37 MBq thallium was injected at rest, and images were reacquired at 10 minutes and 24 hours after reinjection. Four sets of images(stress, redistribution, reinjection and delayed images) were then analyzed qualitatively and quantitatively. Left ventricle was arbitrarily divided into 9 segments(apex, proximal and distal portions of anterior, septal, inferior and lateral walls). RESULTS: These were 45 irreversible perfusion defects in 20 subject patients, of which 21(46.7%) showed improved thallium uptake after reinjection. Among these 21 segments 2 demonstrated further improvement of uptake on 24-hour delayed images, of the 24 regions determined to have persistent defects after reinjection. 10(41.7%) showed improved uptake on delayed images. CONCLUSIONS: In addition to reinjection imaging, 24-hour delayed imaging after reinjection was also helpful to identify severely ischemic but viable myocardium.

Intractable pain was controlled in 5 cases using dorsal root entry zone lesions. All patients had failed to conservative therapy. Dorsal root entry zone lesions were made to include the involved dermatomes added one half of the dermatomes above and below the painful areas. Four patient noticed good pain relief with follow up period ranging from 6 to 24 months. Patient whose lesions were performed using a CO2 laser mas suffered no complicated deficit. Dorsal root entry zone lesions appeared to be a satisfactory treatment for intractable pain in patients, who have failed to respond to more conservative mode of therapy.
