2.Xepoderma Pigmentosum Complicated by Squamous Cell Epithelioma and its Treatment with Bleomycin.

Xeroderma pigmentosum, a rare, genetically determined dermatosis, is often cornplicated by malignant chagnes of various types-most commonly, basal-cell epithelioma, squamous cell epithelioma, less frequently malignant meIanoma, keratoacanthoma, fibrosarcoma, and endothelioma have been reported. In Korea, no case has ever been reported in literature except four cases at a symposium for dermatology, two of them complicated by basal-cell epithelioma, and the others by squamous cell epithelioma. This case reported was complicated by squamous cell epithelioma. As one of the anticancer treatments, especially for skin cancer, Bleomycin is, now, drawing the attention for its high therapeutic index with only few side effects. For this case, we injected Rleomycin 15mg at once, twice a week, for 8 weeks, So total injection was 16 times and 240mg of Bleomycin was given in total. While this amount given for this case was not enough for the complete treatment of squamous cell epithelioma, we got the following results: 1. Clinically, the ulcerated lesion of the cancer on the right ala nasi had shrunken in size with reepithelialization at the margin showing clear surface, and 2. microscopically, the atypical cancer cell masses and individual cell keratinizations had been decreased in size and in number, and less remarkable comparing to those in untreated stage at his first visit to the hospital.

3.Molecular Cloning of the Major Immunogen of Orientia tsutsugamushi Thai Strains and Development Passive Hemagglutination Test.

The 56-kilodalton protein genes of Orientia tsutsugamushi TA678, TA686, TA716, TA763 strains were amplified by PCR. The amplified products were sequenced and cloned into pIH821 vector. The recombinant proteins were expressed in Escherichia coli as fusion proteins with maltose binding protein. The recombinant proteins were purified and used for the sensitization of sheep RBCs and the reactivity of the recombinant 56-kDa proteins of Orientia tsutsugamushi TA 678, TA686, TA716 strains were analyzed with 40 sera from scrub typhus patients in Korea, 40 sera from scrub typhus in Thailand, Malaysia and Philippines. The 56-kDa protein coding DNA sequence of Orientia tsutsugamushi TA678, TA686, TA716 show 70 to 88% homology with other known strains and four variable regions are also observed. 39 of 40 sera from scrub typhus patients in Korea showed the strongest reactivity to the recombinant protein of Boryong strain and one sera showed the strongest reactivity to the recombinant protein of Gilliam strain. 14 of 40 sera from patients in Thailand, Malaysia and Philippines showed the strongest reactivity to the recombinant protein of TA686 strain and 12 sera showed the strongest reactivity to the recombinant protein of TA716 strain. No serum from patients in Thailand, Malaysia and Philippines showed the strongest reactivity to the recombinant protein of Boryong strain.

4.Antigenicity of the G2 Glycoprotein in Cell - Mediated Immune Response Against Hantanvirus Infection.

Hemorrhagic fever with renal syndrom is an acute febrile disease which is caused by Hantanvirus and several other viruses that belong to the genus Hantavirus. Gl and G2 glycoproteins of Hantanvirus have been thought to be involved in protective immunity against Hantanvirus infection. In this study, the antigenicity of G1 and G2 glycoproteins in cell mediated immune response was investigated. When peripheral blood mononuclear cell fraction from recovered hemorrhagic fever with renal syndrome patient was cultivated with a recombinant protein containing amino-terminal 78 amino acids of G2 glycoprotein, these cells were activated to proliferate and secreted significant amount of interleukin-2 and interferon-r. These results suggest that T cell epitope exists in the amino-terminal region of G2 glycoprotein.

5.Clinical Experience on 21 Myasthenia Gravis Patients.

The authors experienced 21 myasthenia gravis patients who were admitted to Kyung Hee University Hospital and Han Yang University Hospital from March 1979 to August 1983. Following results were obtained through treatment and follow up evaluation. 1. There was no sexual difference between male and female, of 21 patients male to female ratio was 10:11. The prevalent age group was 20s and 30s. (10 of 21 patients, 47.6%). 2. The most common initial symptom was ptosis (71.4%), followed by diplopia (38.0%), generalized muscle weakness (38.0%), swallowing difficulty(19.0%), Mastication difficulty (9.5%), dysarthria (4.8%), respiratory difficulty (4.8%), in order. 3. Among 21 patients, thymomas were found in 2 patients. (9.5%) 4. According to the modified Osserman's classification, the peak incidence group was IIA (7 cases, 33.3%) then IIB (5 cases, 23.8%). Group III and neonatal type were not seen. 5. In 14 patients (66.6%), diagnosis was made within 1 year after the onset of the symptoms, but the other 7 patients (33.3%) spent more than 1 year for the diagnosis. 6. All patients who received corticosteroid therapy or thymectormy were improved significantly after the treatment. 7. After the discharge, 3 patients (14.3%) make normal life, on the other hand 17 patients (80.9%) are in more or less restricted life.

9.A clinical study on prematurity who received blood transfusion.

A clinical study was made on 85 cases of prematurity under 37 weeks of gestational age who had history of blood transfusion during their admission at the National Medical Center from January 1989 to June 1992. The results were as follows: 1) Among 339 prematurity patients, 85 patients(25.1%) received blood transfusion once at least. 2) Male to female ratio was 1.13:1 and mean gestational age was 32.5 2.7weeks (range:25~36 weeks). 3) Most of blood transfusion(66/85 cases) were performed within first week of life. 4) 67 patients(78.8%) received blood transfusion 3 times or less. 5) Combined diseases were hyaline membrane disease(69.4%), jaundice(51.8%), sepsis(30.6%), PDA (14.1%), DIC (8.2%), pulmonary hemorrhage(7.1%), intraventricular hemorrhage (7.1%), pneumonia(7.1%) and hepatitis(4.7%). 6) Clinical manifestations at the time of blood transfusion were pallor (47.1%), decreased activity (44.7%), bradycardia (23.5%),apnea (21.2%), tachypnea (20.0%), lethargy (4.7%), poor weight gain (3.5%) and tachycardia (3.5%). 7) The values of hematocrit were significantly increased after transfusion(42.4 +/-7.2 vs 34.6 +/- 6.8vol%). 8) The episodes of bradycardia were significantly decreased after blood transfusion(0.09 +/- 0.39 vs 0.23+/- 0.78)while the episodes of apnea were not (0.15 +/-0.71 vs 0.27+/- 0.84).
