3.Affinity for 57Co-Vitamin B12 by a Wide Histologic Variety of Tumor Types in Mice.

The search for tumor-avid agents for use in nuclear medicine imaging is an ongoing field of importance. The purpose of this study was to determine the affinity for radiolabeled vitamin B12 by a wide histologic variety of tumor types in mice. Seventeen different types of tumor were grown subcutaneously in female Balb/C or Balb nu/nu(nude) mice. When the tumors reached about 1 cm in diameter, mice were injected intraperitoneally with 57Co-vitamin B12. Twenty-foul hours later, the mice were sacrificed. Organs and tissues were removed, weighed, and activity per mg determined by gamma counter. Values represented cpm/mg tissue that was normalized to 20 grams body weight for each mouse. A wide variety of tumor types showed significant uptake and concentration of 57Co-vitamin B12, as evidenced by tumor:tissue activity ratios. For many tissues of great importance in terms of background(bone, muscle, blood), the tumor:tissue activity ratios of uptake were high. These data strongly suggest that further efforts to evaluate the utility of radiolabeled adducts of vitamin B12 for clinical use in oncologic imaging are warranted.

4.A Study on the Expressed Desire at Discharge of Patients to Use Home Nursing and Affecting Factors of the Desire.

The purpose of this study is to investigate factors related to the intent of using home nursing of chronic disease patients who got out of a university hospital. For the purpose. the study selected 153 patients who were hospitalized and left K university hospital with diagnoses of cancer. hypertension. diabetes and cerebral vascular accident and ordered to be discharged and performed interviews with them and surveys on their medical records to obtain the following results. For this study a direct-interview survey and medical record review was conducted from June 28 to Aug. 30. 1998. The frequency and mean values were computed to find the characteristics of the study subjects. and chi-test. t-test. factor analysis and multiple logistic regession analysis were applied for the analysis of the data. The following results were obtained. 1) When characteristics of the subjects were examined. men and women occupied for 58.8 % and 41.2 %. respectively. The subjects were 41.3 years old in average and had the monthly average earning of 0.99 million won or below. which was the most out of the total subjects at 34.6 %. Among the total. 87.6% resided in cities and 12.4 in counties. The most left the hospital with diagnosis of cancer at 51.6 %. followed by hypertension at 24.2%. diabetes at 13.7% and cerebral vascular accident at 7.2%. 2) 93.5% of the selected patients had the intent of using home nursing and 6.5%. didn't. Among those patients having the intent. 85.6% had the intent of paying for home nursing and 14.4%. didn't. The subjects expected that the nursing would be paid 9.143 won in average and 47.7 % of them preferred national authorities as the mam servers. 86.3% of the subjects thought that home nursing business had the main advantage of making it possible to learn nursing methods at home and thereby contributing to improving the ability of patients and their facilities to solve health problems. 3) Relations between the intent of use and characteristics of the subjects such as demography-related social. home environment. disease and physical function characteristics did not show statistically significant differences among one another. Compared to those who had no intent of using home nursing. the group having the intent had more cases of male patients. the age of 39 or below. residence in cities. 5 family members or more, no existence of home nursing servers, leaving the hospital from a non-hospitalized building, disease development for five months or below, hospitalization for ten days or more, non-hospitalization within the recent one month, two times or over of hospitalization, leaving the hospital with no demand of special treatment, operation underwent, poor results of treatment, leaving the hospital with demand of rehabilitation services, physical disablement and high evaluation point of daily life. 4) Among those patients having the intent of using home nursing, 47.6% demanded technical nursing and 55.9%, supportive nursing. As technical nursing, 'inject into a blood vessel' and 'treat pustule and teach basic prevention methods occupied for 57.4%, respectively, topping the list. Among demands of supportive nursing, 'observe patients' status and refer them to hospitals or community resources as available, if necessary' was the most with percentage point of 59.5. Regarding the intent of paying for home nursing, 39.2% of those patients wishing to use the nursing responded paying for technical services and 20.2, supportive services. In detail. 70.0% wanted to pay for a service stated as 'inject into a blood vessel' , highest among the former services and 30.7%, a service referred to as 'teaching exercises needed to make the body of patients move', highest among the latter. When this was analyzed in terms of a relation between the need(the need for home nursing) and the demand(the intent of paying for home nursing), The rate of the need to the demand was found two or three times higher in technical nursing(0.82) than in supportive nursing(0.35). In aspects of technical nursing, muscle injection(0.26, the 1st rank) was highest in the rate while among aspects of supportive nursing. a service referred to as 'teach exercises needed for making patients move their bodies normally' (0.58, the 1st rank). 5) factors I(satisfaction with hospital services), II(recognition of disease state), III(economy) and IV(period of disease) occupied for 34.4, 13.8, 11.9 and 9.2 percents, respectively among factors related to the intent by the subjects of using home nursing, totaled 59.3%. In conclusion, most of chronic disease patients have the intent of using hospital-based home nursing and satisfaction with hospital services is a factor affecting the intent most. Thus a post-management system is needed to continue providing health management to those patients after they leave the hospital. Further, supportive services should be provided in order that those who are satisfied with hospital services return to their community and live their independent lives. Based on these results, the researcher would make the following recommendation. 1) Because home nursing becomes more and more needed due to a sharp increase in chronic disease patients and elderly people, related rules and regulations should be made and implemented. 2) Hospital nurses specializing in home nursing should be cultivated.

5.Measurement of the Axial Rotational Axis of Distal Femur Using Different Landmarks.

PURPOSE: To know the discrepancy of rotational alignment using human femur. MATERIALS AND METHODS: We used 22 adult human femurs which were not able to distinguish age and sex and absent articular cartilage. In a horizontal plane, we made a cross-section perpendicularly at supra-condyle of femur to a mechanical axis and took a simple roentgenogram in coronal plane focusing on the center of transepicondylar axis. In terms of roentgenogram and gross specimen, the angle between per-pendicular to the anteroposterior axis and the posterior condylar axis, the angle between the transepi-condylar axis and the posterior condylar axis were measured by 5 observers. According to t-test in statisti-cal method, we figured out the difference between two angles and made a significance at p<0.05. RESULTS: About posterior condylar axis, the line perpendicular to anteroposterior axis was observed the average 4.8(+/-1.5)degree, 6.0(+/-1.9)degree external rotation in roentgenogram and gross specimen respectively. The transepicondylar axis was observed the average 3.5(+/-1.2)degree, 3.9(+/-1.4)degree external rotation in roentgenogram and gross specimen respectively. There was statistically significant difference between two angles. Moreover, angles measured by 5 observers were found that to have a line perpendicular to the anteroposterior axis has much more external rotation respectively and there was a statistically significant difference also. CONCLUSIONS: On determination of a rotational alignment using human femur, there is statistically sig-nificant difference between the anteroposterior axis and the transepicondylar axis in gross specimen as well as roentgenogram.

6.Peak Expiratory Flow(PEF) Measured by Peak Flow Meter and Correlation Between PEF and Other Ventilatory Parameters in Healthy Children.

BACKGROUND: In diagnosis or monitor of the airway obstruction in bronchial asthma, the measurement of FEV1 in the standard method because of its reproducibility and accuracy. But the measurement of peak expiratory flow(PEF) by peak flow meter is much simpler and easier than that of FEV1 especially in children. Yet there have been still no data of the predicted normal values of PEF measured by peak flow meter in Korean children. This study was conducted to provide equations to predict the normal value of PEF and correlation between PEF and FEV1 in healthy children. METHOD: PEF was measured by MiniWright peak flow meter, and the forced expiratory volume and the maximum expiratory flow volume curves were measured by Microspiro HI 501(Chest Co.) in 346 healthy children (age:5-16 years, 194 boys and 152 girls) without any respiratory symptoms during 2 weeks before the study. The regression equations for various ventilatory parameters according to age and/or height, and the regression equations of FEV1 by PEF were derived. RESULTS: 1. The regression equation for PEF(L/min) was:12.6×age(year)+3.4×height(cm)-263(R2=0.85) in boys, and 6×age(year)+3.9×height(cm)-293(R2=0.82) in girls. 2. The value of FEFmax(L/sec) derived from the maximum expiratory flow volume curves was multiplied by 60 to compare with PEF(L/min), and PEF was faster by 125 L/min in boys and 118 L/min in girls, respectively. 3. The regression equation for FEV1(ml) by PEF(L/min) was:7×PEF-550(R2=0.82) in boys, and 5.8×PEF-146(R2=0.81) in girls, respectively. CONCLUSION: This study provides regression equations predicting the normal values of PEF by age and/or height in children. And the equations for FEV1, a gold standard of ventilatory function, was predicted by PEF. So, in taking care of children with airway obstruction, PEF measured by the peak flow meter can provide useful information.

7.The role of leukotriene modifier in pediatric allergic disease.

Leukotriene (LT) modifiers are composed of leukotriene receptor antagonists and 5-lipoxygenase inhibitors. LTs, C4, D4, and E4 are collectively termed cysteinyl LTs and best are the characterized receptors for cysLTs are cysLT1 and cysLT2. cysLT1 ligation mediates sustained bronchial contraction, mucosal secretion, and edema, which are central to the pathogenesis of asthma. cysLT2 ligation is thought to contribute to edema, inflammation, and tissue fibrosis in asthma. LT modifiers attenuate bronchoconstriction responses and exert anti-inflammatory effects, reflected by reduced eosinophil counts in the peripheral blood, sputum, and bronchoalveolar lavage fluid of asthmatic patients. Inhaled corticosteroids are generally superior to LT modifiers as a first-line controller. However, LT modifiers are easy to administer, have good compliance, and have excellent safety. LT modifiers are recommended for asthmatic children aged < or =5 years as a first-line controller. The Japanese Guideline for Childhood Asthma recommends LT modifiers, as a first-line controller prior to inhaled corticosteroid for children aged <2 years. LT modifiers can improve asthma control as add-on therapy with ICS. They can also be effective for exercise-induced asthma. LT modifiers are recommended for the treatment of allergic rhinitis in combination with H1-antihistamines or as a first-line drug for patients who cannot or do not wish to use intranasal corticosteroids. LT modifiers can also be considered for add-on therapy in the treatment of chronic urticaria, atopic dermatitis, and other allergic diseases.

8.Prognosis of Congenital Extrahepatic Biliary Atresia After Hepatic Portoenterostomy.

PURPOSE: The surgical results for congenital extrahepatic biliary atresia have been improved since Kasai first described hepatic portoenterostomy(Kasai operation) in 1959. This study was conducted to evaluate the postoperative prognosis of congenital extrahepatic biliary atresia after hepatic portoenterostomy. METHODS: This clinical study was done by reviewing the medical records of 30 patients who have undergone hepatic portoenterostomy at the Severance Hospital, Yonsei University College of Medicine between January 1984 to June 1994 due to congenital extrahepatic biliary atresia. RESULTS: 1) The ages of the 30 patients ranged from 45 to 242 days and the mean age at operation was 90.8 days. Seventeen patients were male and 13 were female, and the male to female ratio was 1.3:1. 2) Of the total 30 patients, 9 cases(30%) had died, 12 cases(40%) were still being followed up, and 9 cases(30%) never returned for during follow-up. The calculated survival rate of more than 3 years was 53%. 3) Jaundice was improved in 8 patients(27%) after portoenterostomy, but the remaining 22 patients(73%) showed no improvement. The two year survival rate of the jaundice-free group was 100%, compared with 42% in the jaundice-persistent group, which showed a statistically significant difference (p<0.05). 4) Hepatic portoenterostomy was performed in 7 patients(23%) before the age of 60 days, and done in the remaining 23 patients(77%) after 60 days. The two year survival rate of the former group was 54%, compared with 70% in the latter group, which did not show a statistically significant difference. 5) Portal hypertension was complicated in 17 patients(57%), but not in the remaining 13 patients(43%). The two year survival rate of the former group was 54%, compared with 100% in the latter group, which showed a statistically significant difference between the two groups (p<0.05). 6) Cholangitis was complicated in 15 patients(50%), but not in the remaining 15 patients(50%). The two year survival rate of the former group was 53%, compared with 81% in the latter group, which did not show a statistically significant difference. CONCLUSIONS: The 3 year survival rate in patients who performed hepatic portoenterostomy due to congenital extrahepatic biliary atresia was 53%. The persistent jaundice and the development of portal hypertension after surgery were important determinants of long term survival of exteahepatic biliary atresia in this study.

9.Dyspnea Perception During Induced Bronchoconstriction Is Complicated by the Inhaled Methacholine in Children With Clinical Asthma.

PURPOSE: Dyspnea is not widely utilized as an indicator of asthma provocation despite its universal presentation. We hypothesized that dyspnea severity was proportionate with the lung function decline, methacholine dose-step, and the degree of bronchial hyperresponsiveness (BHR). METHODS: We retrospectively analyzed 73 children's bronchial provocation test data with an assessment of dyspnea at every dose-step. Dyspnea severity was scored using a modified Borg (mBorg) scale. A linear mixed effect analysis was performed to evaluate the relationship between the mBorg scale, the percentage fall in the forced expiratory volume in 1 second (FEV1) (ΔFEV1%), the methacholine dose-step, and the degree of BHR (BHR grade). RESULTS: Subjects were divided into 5 BHR groups based on their last methacholine dose-steps. The mBorg scores did not differ significantly among BHR groups (P=0.596, Kruskal-Wallis test). The linear mixed effect analysis showed that ΔFEV1% was affected by the methacholine dose-step (P < 0.001) and BHR grade (P < 0.001). The mBorg score was affected by the dose-step (P < 0.001) and BHR grade (P=0.019). We developed a model to predict the mBorg score and found that it was affected by the methacholine dose-step and ΔFEV1%, elevating it by a score of 0.039 (χ² [1]=21.06, P < 0.001) and 0.327 (χ² [1]=47.45, P < 0.001), respectively. A significant interaction was observed between the methacholine dose-step and ΔFEV1% (χ² [1]=16.20, P < 0.001). CONCLUSIONS: In asthmatic children, inhaled methacholine, as well as the degree of BHR and lung function decline, may affect dyspnea perception during the bronchial provocation test. If we wish to draw meaningful information from dyspnea perception, we have to consider various complicating factors underlying it.

10.Nodular opacities in pulmonary paragonimiasis:Radiologic-pathologic correlation.

The CT findings of pulmonary paragonimiasis are well known. However, the pathologic findings of nodular opacity have not been studied sufficiently. To clarify the nature of nodular opacity on CT in patient with paragominiasis, we reviewed CT scans of 9 patients retrospectively. Five of them were compared with pathologic findings of the resected specimen. Other four patients were confirmed by needle aspiration biopsy. CT scans showed nodular opacities of 5 to 30mm in diameter which were accompanied with perinodular air-space consolidations in all patients. The usual location of nodular opacities were peripheral zone of the lung. Correlation with pathologic findings demonstrated That the nodular opacity on CT scans corresponded to the parasitic granuloma with central ova and intracystic worm. The most frequent CT finding of pulmonary paragonimiasis was nodular opacities with perinodular airspace consolidations caused by parasitic granuloma or intracystic worm.
