1.Studies of cocktail therapy with multiple cytokines for neoplasia or infectious disease of the dog I. cDNA cloning of canine IL-3 and IL-6.

This paper describes the cloning and sequence analysis of the cDNAs encoding the canine homologues of interleukin-3 (IL-3) and interleukin-6 (IL-6). The coding sequences for canine IL-3 and IL-6 were obtained by using the reverse transcription polymerase chain reaction (RT-PCR) with RNA harvested from canine peripheral blood mononuclear cells (PBMCs). Canine IL-3 cDNA includes a single open reading frame of 432 nucleotides, which encodes a 143 amino acid polypeptide and has 44.7, 42.4, 37 and 23.7% homology with the cow, sheep, human and rat IL-3 sequences, respectively. Canine IL-6 cDNA (GenBank accession number; AF275796) encodes a putative 20-amino acid signal peptide followed by a 187-amino acid mature protein. The predicted amino acid sequence of canine IL-6 shares 60.4, 77.2, 71.0, 55.8 and 42.0% sequence identity with those of human, feline, porcine, sheep and rat IL-6, respectively.

2.A Case of Asteroid Hyalitis.

Asteroid hyalitis is an uncommon condition in which small, solid, stellate, spherical or disk-shaped bodies are suspended in a substantially normal vitreous. The opacities occurs in strands or as discrete bodies without orderly arrangement. These move when the eyes move but always return to their original position. The authors experienced one case of asteroid hyalitis associated with arteriosclerosis and hypertension. so we report this with brief review of the literature.

3.Nagel's Anomaloscope Examination for Subdivision of 100 Cases of Congenital Color Defects.

100 cases (99 male, 1 female) with defective co1or vigion but good vigual acuity, and no retinal diseases that could be detected by screening with Iehihara's test were ge1ectgd from 2708 cases (maIe 2382, female 326) that had undergone a physicaI examination at National Semil Hogpital. The 100 cages wgre examined using a printed sample of plates including Ishihara-Ohuma's test and Hahn's double 15-hue test. After thig test, the 100 cages were examined using Nagel's anomaloscope. The anomaloscope using the wavelengths 670nm and 546nm to mathch 589nm is the most accurate means of diagnosing defects of red and green vision. Results of examination in classifing the types of color defects using the plates of both Ishihara-Oguma's test and the Hahn's doume 15-hue test as well as the Nagel's anomaloscope were as follows: 1. In the total of 2708 persons examined, there were 2382 males and 326 females from this 100 cases of congenital color defects were discovered 99(4.16%) males and female 1(0.31%). 2. The classification and rates of types of congenital color defects were as followg: 99 ma1eg were classified with 12(12.1 %) protanopia, 35(35.4%) deutanopia, 9(9.1 %) protanomalia and 43 (43.4 %) deuteranomalia, on the other hand one ferna1e was found out deuteranomalia.

4.The Incidence of Hypoxia Following the Use of Hypnotics and Analgesics during Spinal Anesthesia.

BACKGROUND: The purpose of this study was to find out the effect of hypnotics and analgesics on oxygen saturation by pulse oximetry in surgery patients under spinal anesthesia. METHODS: Sixty-five patients classified ASA physical status 1 or 2 scheduled for surgery under spinal anesthesia were studied. These patients were divided into three groups. The 22 subjects of the first group did not receive either midazolam or fentanyl. The 22 subjects of the second group received midazolam. The 21 subjects of the third group received midazolam (0.03 mg/kg) and fentanyl (1.0 microgram/ kg). Oxygen saturation was measured with a pulse oximetry. Measurements were made before spinal anesthesia, 5 minutes, and 10 minutes after starting of spinal anesthesia, 5 minutes, 10 minutes, 30 minutes, and 60 minutes after the start of the operation or intravenous injection of drugs. In addition, measurements were made on arrival in the recovery room, and 5 minutes, 10 minutes, 20 minutes, and 30 minutes after arrival in the recovery room. RESULTS: There were statistically significant differences in oxygen saturation at 5 minutes after the start of the operation or intravenous injection of drugs. The mean oxygen saturation for the first group was 98.2 +/- 1.8%, for the second group 97.9 +/- 2.6%, and for the third group 92.4 +/- 2.8%. Hypoxia cases at 5 minutes after the start of the operation or intravenous injection of a drug occurred in 4.5% of the first group, 9.1% of the second group, and 57.1% of the third group CONCLUSIONS: We concluded that oxygen saturation monitoring should be done routinely in patients receiving hypnotics and analgesics during spinal anesthesia, and oxygen should be administered to patients who develope hypoxia during spinal anesthesia.

6.Two Cases of Anterior Uveitis with Marie-Strumpell Ankylosing Spondylitis.

About 10% ~ 40% of patient with Marie-Strumpell ankylosing spondylitis have anterior uveitis. Recently, relationship between ankylosing spondylitis and anterior uveitis was discoverd by several studies of HLA-B27 antigen. Ankylosing spondylitis was found to be the commonest single etiological factor in analysis of 653 cases of uveitis seen in the Uveitis Clinic in United Kingdom. Ankylosing spondylitis is mostly found in male and most of their natures of uveitis are acute unilateral nongranulomatous anterior uveal involvement. Roentgenological findings of ankylosing spondylitis are characteristic demineralization and subchondral sclerosis of sacro-iliac joint and "Bamboo spine". Elevation of erythrocyte sedimentation rate in ankylosing spondylitis indicates active status of the disease. Authors have experienced 2 cases of anterior uveitis with Marie-Strumpell ankylosing spondylitis in our clinic. These cases have shown typical findings of "Bamboo spine", sacro iliac joint blurring and acute nongranulomatous unilateral uveal involvement.

8.Allergic Reaction to a Latex Face Mask for Manual Ventilatory Support : A case report.

The number of latex-induced allergic reactions, including anaphylaxis, has been increasing since latex induced contact urticaria was first reported in surgical patients by Nutter in 1979. Latex allergy is frequently seen in the healthcare industry, and is associated with systemic symptoms, hand eczema, and allergic contact dermatitis. Reactions range from contact urticaria, rhinitis and conjunctivitis, angioderma or bronchospasm to the recently recognized severe anaphylactic shock with cardiovascular collapse. We encountered a 55-year-old female patient who developed a severe anaphylactic reaction after manual vetilatory support using a facemask that contained latex. A subsequent allergy workup revealed a delayed-type hypersensitivity to latex. This case highlights the need for anesthesiologists to be able to diagnose the signs and symptoms of allergic reactions in patients during the peri-anesthetic period. Two types of allergic reactions to natural rubber latex (NRL) and rubber products are now known to exist: type I (immediate-type) and type IV (delayed-type hypersensitivity [DTH]). Patients with NRL allergy should be provided with information on non-latex devices and latex avoidance in medical, dental, and occupational settings. In addition, we should pay more attention to the management of allergic reactions to latex in high risk groups.

10.Secondary Cutaneous Amyloidosis in a Patient with Mycosis Fungoides.

Secondary cutaneous amyloidosis refers to clinically unapparent amyloid deposits within the skin in association with a pre-existing skin condition or skin tumors, such as basal cell carcinoma, porokeratosis, solar elastosis, Bowen's disease, and mycosis fungoides. A 70-year-old woman presented with a 6-month history of asymptomatic multiple yellowish plaques on both legs. She had been diagnosed with mycosis fungoides 7 years ago and was treated with psoralen and ultraviolet A radiation (PUVA) therapy, narrow-band ultraviolet B (UVB) therapy, and acitretin for 5 years. Finally, she reached complete remission of mycosis fungoides. However, new yellowish lesions started to appear 1 year after discontinuing the phototherapy. A physical examination revealed multiple yellowish plaques on both extremities. The plaques were well circumscribed and slightly elevated. All laboratory tests were normal. A biopsy specimen showed multiple nodular deposits of eosinophilic amorphous material in papillary dermis and upper reticular dermis. The deposits represented apple green birefringence on Congo red stain viewed under polarized light. Acellular small nodules in the upper dermis consisted of randomly oriented, non-branching, 6.67~12.7 nm thick amyloid fibrils on electron microscopy. We report an interesting and rare case of secondary cutaneous amyloidosis after narrow-band UVB therapy and PUVA therapy in a patient with mycosis fungoides.
