1.Reconstruction of Chronic Ligamentous Instability of the Knee

The methods of reconstruction for chronic Iigamentous instability of the knee have changed much since the concept of rotary instability was introduced by Slocum and Larson in 1968. The first step to successful management of these probrams is the accurate dlagnosis of the type or types of the instability that exist. Appropriate operative procedures should then be selected according to the type and degree of the instability as well as the age, general conditlon and the function demand of the patient. The stability can be restored passively by a near anatomical. repair of the injured ligaments, and can be reinforced actively by a proper muscle or tendon transplantation, such as the five-one procedure for anteromedial rotary instability. Nineteen knees of active, well motivated patients were operated on for chronic knee instabilities according to these new concepts. Although the follow-up period was relatively short, the results of the procedures was generally satisfactory.

2.Clinical Study on Hip Fractures

Fractures of the hip are usually classified first according to their anatomic location. Isolated fractures of the lesser or greater trochanter are not common and rarely requir surgery, Femoral neck fractures, intertrochanteric fractures, and subtrochanteric fractures are common categories that usually require open reduction and internal fixation. These fractures most often occur in elderly patients. The prognosis of these three major categories of hip fractures seems to be entirely different because of their anatomic characteristics. Seventy three cases of hip fractures were experienced at the Department of Orthopedic Surgery, Eul-Ji General Hospital over the period from Jan. 1975 to Aug. 1980 and the following results were obtained. 1. Of the 73 hip fractures, femoral neck fractures were 28 cases (38.5%), intertrochanteric fractures were 23 cases (31.5%), subtrochanteric fractures 11 cases (15%), and acetabular, fractures 11 cases (15%). 2. Traffic accident was the major cause of hip fractures in our series. 42 cases (57.2%) were caused by traffic accident, whereas 27 cases (37.0%) by a fall from the height. 3. Hip fractures occured most frequently in the age group from 31 to 50 years (53.5%). Male to female ratio was 2:1. 4. Cases caused by traffic accident visited hospital earlier than the others. 5. The most common type was Garden's type IV (53.6%) in the femoral neck fractures, Boyd-Griffins type Il (47.8%) in the intertrochanteric fractures, and Fielding's type II (54.5%) in the subtrochanteric fractures. 6. Metal devices used to fix hip fractures were Knowles pin, Jewett nail plate, compression hip screw, plate and screws, and Kuntscher nail. Internal fixation was done anatomical reduction has been achieved in stable fractures, and Dimon-Hughston reduction in unstable fractures. 7. Postoperative cast immobillzation was performed in 13 cases of 21 femoral neck fractures, 15 cases of 22 intertrochanteric fractures and 6 cases of 11 subtrochanteric fractures. The average duration of immobilization was 10 to 12 weeks. 8. Incidence of avascular necrosis in femoral neck fractures was 16.7%, nonunion in intertrochanteric fractures was 11.8%, and nonunion is subtrochanteric fractures was 12.5%.

3.Minimum Five-Year Results of Arthroscopic Management with Labral Preservation in Borderline Hip Dysplasia

Purpose:
This study examined the patient-reported outcome scores at mid-term follow-up after arthroscopic labral preservation in patients with borderline hip dysplasia.
Materials and Methods:
Data were collected retrospectively from patients who underwent arthroscopic labral preservation. Among the 61 patients classified with hip dysplasia, patients with arthritis beyond Tönnis grade II were excluded, Patients with subchondral bony exposure were excluded even if they had arthritis of Tönnis grade I. Forty-two patients with borderline hip dysplasia were enrolled among patients whose lateral center edge angle (LCEA) was between 18° and 25°. The PRO scores including the modified Harris hip score (mHHS), hip outcome score activities of daily living (HOS-ADL), hip outcome score sport-specific subscale (HOS-SSS) and visual analog scale (VAS), were collected and compared preoperatively, and then at one year and a minimum of five years after surgery.
Results:
The mean preoperative LCEA and Tönnis angle were 20.9° (range, 18° to 25°) and 6.3° (range, 0° to 18°), respectively. The mean follow-up was 64.8 (range, 60 to 84) months. The mean mHHS increased from 73.5 before surgery to 86.2 in the fifth year after surgery (p<0.01), the mean HOS-ADL from 70.5% before surgery to 84.9% in the fifth year after surgery (p<0.01), and the mean HOS-SSS from 51.4 before surgery to 69.4 in the fifth year after surgery (p<0.01). The mean VAS score improved from 6.3 before surgery to 1.5 in the fifth year after surgery (p<0.01).
Conclusion
Arthroscopic labral preservation performed in appropriately selected patients with borderline dysplasia showed good results at mid-term follow-up. In borderline hip dysplasia without the progress of osteoarthritis and subchondral bone exposure, arthroscopic labral preservation may be considered a good and less invasive option.
4.Lipid Peroxidation in Vivo Monitored as Ethane Exhalation in Hyperoxia.

In vivo ethane production in rats was used as an index of oxygen toxicity. The rats were allocated to four exposure conditions; hyperbaric oxygenation (HBO=5 ATA, 100% O2), normobaric oxygenation (NBO=1 ATA, 100% O2), hyperbaric aeration (HBA=5 ATA, 21% O2) and normobaric aeration (NBA=1 ATA, 21% O2). After 120 minutes of exposure, the rats exposed to high concentration and/or high pressure oxygen exhaled significantly larger amounts of ethane than those exposed to NBA, and the differences in ethane production between any two groups were statistically significant (p<0.01). This finding supports the hypothesis that hypothesis that hyperoxia increase oxygen free-radicals and the radicals produce ethane as a result of lipid peroxidation. It is notable that the ethane exhalation level of the HBA group was significantly higher than that of the NBO group. This difference could not be accounted for by the alveolar oxygen partial pressure difference between the two groups.

5.Treatment of hookworm infection with furfurol.

Furfurol(Furfudol), a new anthelmintics, was administered to 7 adults and 21 children (6-12 years old) in total amounts of 15.6 mg(base) for 1 to 3 days. It has proven to be effective against hookworms. The anthelmintic effect against hookworms with a single dose of 15.6 mg of furfurol was similar to that of a single dose of 2.5 gm of bephenium hydroxynaphthoate. However, when a total of 15.6 mg of the base was given to both children and adult, the side reactions were so mild that the drug can be administered for therapeutic regimens in mass treatment. Minor abdominal discomfort was the only common side effect. This is the primary advantage of furfurol as compared with bephenium hydroxynaphthoate. These results have indicated that furfurol is safe in therapeutic dosage and is an effective agent to treat patients infected with hookworms.

7.Effects of Hyperoxia on 8-Hydroxydeoxyguanosine Formation in Carbon Monoxide Exposed Rats.

Hyperbaric oxygen(HBO) therapy for carbon monoxide(CO) poisoning eventually inducing the hypoxia-reoxygenation condition, may produce oxygen free radicals, which forms 8-hydroxydeoxyguanosine(8-OH-dG) by attacking C-8 position of deoxy-guanosine (dG) in DNA. Effects of oxygen partial pressure or duration of HBO therapy with or without CO poisoning on the tissue 8-OH-dG formation were investigated. Male Sprague-Dawley rats were grouped and exposed to air(control group), 4000ppm of CO for 10 to 30 minutes(CO only group), air for 30 minutes after 30 minute exposure to 4000ppm of CO(CO-air exposure group), HBO after 30 minute exposure to 4000ppm of CO(CO-HBO group), or HBO therapy for 10-120 minutes (HBO only group). The 8-OH-dG concentrations in the brain and the lung tissues were measured with high performance liquid chromatography and electrochemical detector (ECD). Average concentrations of the 8-OH-dG of each group were statistically compared. In the brain tissues, 8-OH-dG concentrations of the CO only group, the CO-air exposure group, and the CO-HBO group did not significantly differ from those of the control group. Similar insignificance was also found between the CO-HBO group and the HBO only groups. No appreciable dose-response relationship was observed between the 8-OH-dG concentration and the oxygen partial pressure or the duration of HBO. However, the 8-OH-dG concentrations of the 30 minute CO only group were higher than those of the CO-air exposure group (p-value<0.05). In the lung tissues, there were no significant differences between the 8-OH-dG concentrations of the control group and those of the CO only group, the CO-air exposure group, and the CO-HBO group. However, mean 8-OH-dG concentration of the CO-air exposure group was significantly higher than that of the CO only group under the same CO exposure condition(p-value<0.05). With the duration of CO exposure, the 8-OH-dG concentrations of the lung tissues decreased significantly (p-value<0.05). The concentrations of 8-OH-dG in the lung tissues proportionally increased with the duration of HBO, but no such relation was observed with the oxygen partial pressure. These results suggest that the brain may be more resistant to oxygen free radicals as compared with the lungs, and that oxygen toxicity following HBO may be affected by factors other than oxygen free radicals.

8.Long-Term Efficacy and Rotational Stability of AcrySof Toric Intraocular Lens Implantation in Cataract Surgery.

PURPOSE: To evaluate the long-term efficacy and rotational stability of the AcrySof toric intraocular lens (IOL) in correcting preoperative astigmatism in cataract patients. METHODS: This prospective observational study included 30 eyes from 24 consecutive patients who underwent implantation of an AcrySof toric IOL with micro-coaxial cataract surgery between May 2008 and September 2008. Outcomes of visual acuity, refractive and keratometric astigmatism, and IOL rotation after 1 day, 1 month, 3 months, and long-term (mean, 13.3+/-5.0 months) follow-up were evaluated. RESULTS: At final follow-up, 73.3% of eyes showed an uncorrected visual acuity of 20/25 or better. The postoperative keratometric value was not different from the preoperative value; mean refractive astigmatism was reduced to -0.28+/-0.38 diopter (D) from -1.28+/-0.48 D. The mean rotation of the toric IOL was 3.45+/-3.39 degrees at final follow-up. One eye (3.3%) exhibited IOL rotation of 10.3 degrees, the remaining eyes (96.7%) had IOL rotation of less than 10 degrees. CONCLUSIONS: Early postoperative and long-term follow-up showed that implantation of the AcrySof toric IOL is an effective, safe, and predictable method for managing corneal astigmatism in cataract patients.

9.A Case of Mania Associated with Hypericum Perforatum in a Patient with Major Depression.

Recently Korean Food and Drug Administration approved Hypericum perforatum preparations (St. John's wort) as an over-the-counter drug. The authors reported a case of Hypericum perforatum induced mania in a patient with major depressive disorder and reviewed. A seventy-five years old female patient was diagnosed as major depression 5 years ago, and had been admitted in the psychiatric ward twice and had received regular outpatient treatment. Since October 1998, she took paroxetine 20 mg, and bromazepam 1.5 mg constantly. But all of a sudden, she stopped taking the drugs on June 20th 2000, and took Hypericum perforatum 300 mg three times daily. Ten days after, she became abruptly talkative, irritable, hyperactive, and euphoric. She was diagnosed as Hypericum perforatum-induced mania. Manic symptoms came down after several days by discontinuation of taking Hypericum perforatum. Since then she has been got a continuous treatment with paroxetine 20 mg, and she is in full remission state now. Hypericum perforatum is one of herb extracts, which is known to have the antidepressant effect like traditional and newer antidepressants. The side effects of Hypericum perforatum are gastrointestinal disturbance, allergy reaction, fatigue, photosensitivity. And more serious side effect is drug-drug interaction due to it's inducing effect of cytochrome P450 and P-glycoprotein. It is possible to switch from depression to manic episode as was in this case. Although it was approved as an over-the-counter drug in Korea, the use of Hypericum perforatum need to be thoroughly examined to prevent misused if not fully understood.

10.Incidence of Vaculating Toxin Producing Helicobacter pylori from Patients with Gastric Diseases.

OBJECTIVES: This study was carried out to survey the prevalence of Helicobacter pylori infection and the incidence of vacuolating toxin producing H. pylori. A further aim of this study was to evaluate the quantitative assay for cell vacuolation on the basis of the rapid uptake of neutral red dye by vaculoes of the cells. METHODS: We studied the gastric biopsy specimens of patients with 154 cases of gastritis, 74 cases of gastric ulcer, and 167 cases of gastric cancer and in 44 cases of healthy persons. One of the biopsy specimen was placed into a CLOtest plate for rapid urease test and the other one of the biopsy spcimen was inoculated on Brain Heart Infusion blood agar for culture. The culture supernatant of isolated H. pylori was serially diluted with BHI broth. After 24 hour incubation of cultured RK-13 cells treated with the culture supernatant of H. pylori, cytoplasmic vacuolation of the cells were observed microscopically. RESULTS: The positivity of urease test and the rate of isolation of H. pylori from urease positive gastric biopsy materials were 34.1% and 93.3% in healthy person, 55.8% and 70.9% in gastritis, 60.8% and 71.1% in gastric ulcer, and 56.3% and 96.8% in gastric cancer. The isolation rate of H. pylori from patients between 20 and 39 years old was 16.8%, for patients between 40 and 59 years old it was 51.9%, and for patients above 60 years old it was 31.2%. The isolation rate of the vacuolating toxin producing H. pylori from gastric biopsy specimens was 66.7% in a healthy person, 76.6% in gastritis, 79.4% in gastric ulcer, and 80% in gastric cancer. CONCLUSIONS: The isolation rate of H. pylori from the patients with gastric diseases is higher than the rate of H. pylori from healthy persons, but the isolation rate of the vacuolating toxin producing H. pylori is not different between the patients with gastric diseases and healthy persons. The titers of vacuolating toxin produced by some H. pylori isolated from the patients with gastric diseases are higher than those from healthy persons.
