1.Development of Keratitis by Soft Contact Lenses Contaminated with Candida albicans.

In order to study the development of fungal keratitis in wearing of the soft contact lenses contaminated with fungus in various conditions of the cornea, we contaminated the soft contact lenses with Candida albicans, and then placed them on the corneas in rabbits. The rabbits were divided into 4 groups(each 8 eyes). In group 1, dexamethasone was injected subconjunctivally once a day for 4 days and the corneal epithelium was removed 7mm in diameter. In group 2, dexamethasone was injected subconjunctivally once a day for 4 days without removal of the corneal epithelium. In group 3, the corneal epithelium was removed 7mm in diameter without subconjunctival injection of dexamethasone. In group 4, normal corneas were used as a control. The corneas were observed daily with slit-lamp. After 10 days, the infected corneas were examined with light microscope. Keratitis was developed in 5 eyes in group 1(5/8), 3 eyes in group 2(3/8), 2 eyes in group 3(2/8). But keratitis was not developed in group 4. Light microscopic examination showed active invasion of C, albicans forming pseudohyphae into the cornea associated with severe inflammation in group 1 and 2, but it showed mild inflammation and only a small number of C, albicans in the superficial cornea in group 3. In this study, it was found that immunosuppressed and de-epithelialized corneas are susceptible to C, albicans.

2.A Case of Penicillium Keratitis.

Although the incidence of the Penicillium keratitis increases, there is no available reports from the literature. Dense, gray-whitish infiltration combined with ulceration was observed at the central cornea of 62-y-o-man. Hypopyon was observed also. Colonies on Sabouraud's agar showed light violet pigmentation. On the light microscopic examination of the recipicent cornea after penetrating keratoplasty, proliferation of the fungal hyphae in the deep corneal stroma was observed, but collagen fibers of the superficial stroma were re-formed over the necrotic stromal tissue. Electron microscopy showed a hiypha with triple cell wall.

3.A study of the response of teachers and students on the traffic noise.

The purpose of this study is to reveal how the road traffic noise influences on the response of teachers and students, which composed of conversation, studying, relation, and physical disturbances. The research method used in this study was self- administrated questionnaire. Samples of the survey were composed of 420 persons(l14 teachers and 306 students) who are exposed to traffic noise less than 65 dB(A) from two junior high schools and 410 persons(140 teachers and 270 students) from two noisy junior high schools which the road traffic noise above 65 dB(A). In the response of both of the teachers and students in noisy(above 65 dB) schools complaints of disturbances of conversation, studying, relaxation, and physical disturbances are much higher than that of less noisy schools' teachers and students(p<0.01 ). On the occasion of time and season, the subjects answered the traffic noise cause high troublesome and stresses in the afternoon(12:00-17:00) and summer respectively. It is necessary to provide governmental comprehensive and fundamental measures to improve the noisy school environments.

4.Morphology and Ultrastructure of Microorganisms on Soft Contact Lens :Scanning Electron Microscopic Study.

In order to examine the morphology and ultrastructure of contaminated soft contact lens by scanning electron microscopy, soft contact lenses were contaminated with P.aeruginosa, S.aureus, S.epidermidis and C .albicans. After incubation of soft contact lens in P.aeruginosa suspension for 60 minutes, a number of P.aeruginosa, S.aureus and S.epidermidis were attached to soft contact lens and amorphous materials covering the some of the soft contact lens surface were observed. Also, C.albicans attached to soft contact lens, looking like healthy and the fine fibrillar material covered the surface of soft contact lens and interconnected between cell walls of C.albicans at 14 days after contamination. Soft contact lens may provide a suitable environment for microorganisms that they could attach and proliferate. And microoragisms themselves secreted amorphous materials to protect them from the outside and promote the proliferation. So, contaminated soft contact lens may play a role as pathogen to the cornea.

5.Squamous Cell Carcinoma Arising From Chronic Osteomyelitis: A Report of One Case

Squamous cell carcinoma is a rare complication of chronic osteomyelitis. Squamous cell carcinoma of extremity campromises approximately 1 to 2% of all squamous cell carcinoma and of these 2.5 to 4.5% are secondary to chronic osteomyelitis. Biopsy of squamous cell carcinoma, which arises in the proliferating edge of the cutaneous ulcer and invades the bone, should include tissues from all portion of sites of ulcer including bone marrow spaces. But histological diagnosis may be difficult becasue of preexisiting and coexisisting metaplasia and pseudo-epitheliomatous hyperplasia. With prompt and aggressive surgical treatment, the prognosis for patient with squamous cell carcinoma secodary to chronic osteomyelitis is good. Routine regional lymphadenectomy at site of amputation seems unnecessary. This case report represets an unusual bone marrow involvement of squamous cell carcinoma which was complicated by femur fracture.

6.Pervalence of sensiticity to aspirin (ASA) and food additives in subjects diagnosed as having intrinsic asthma.

Objective and METHOD: In order to identify the aggravating agents for intrinsic asthma, we performed ASA- and food additive-challenge tests on 182 subjects diagnosed as having intrinsic asthma. The following tests were performed: Lysine-aspirin bronchoprovocation test to confirm aspirin-sensitivity, sodium bi-sulfite (40-200mg) oral provocation test for sulfite sensitivity, tartrazine oral provocation test (50mg) for tartrazine sensitivity, and sodium benzoate (400mg) oral provocation test for sodium benzoate sensitivity. Positive reaction was defined as decrease in FEV, by more than 20% from the baseline value after the provocation. RESULT: Seventy-five (41.2%) of 182 subjects showed positive responses to more than one agent among the aspirin and three food additives challenged. The prevalence of aspirin-sensitivity was the highest (22.5%), followed by sulfite-sensitivity (8.8%), and then concurrent sensitivity to both aspirin and sulfite (6.0% ), to both aspirin and tartrazine (1.6% ), to aspirin, sulfite and tartrazine (1.1%) and to aspirin, sulfite and sodium benzoate (0.5%). Rhino-sinusitis was noted in 62.5% of aspirin-sensitive asthmatic subjects, 60% of sulfite-sensitive ones, and 80% of tartrazine-sensitive ones. Urticaria was noted in 21.4% of aspirin-sensitive asthmatic subjects, 16.6% of sulfite-sensitive ones and 6.3% of tartrazine-sensitive ones. Thirty-seven to 83% of positive responders had no adverse reaction history. CONCLUSION: These findings suggest that ASA and food additive challenge tests should be considered as a screening test to evaluate any aggravating factors in subjects with intrinsic asthma, even though they may not have experienced any adverse reactions.

7.Cystatin C as a Renal Function Marker for Neonates.

The prediction of acute kidney injury (AKI) is important in the management of neonates, and several renal biomarkers have been tested for clinical application. Cystatin C (CysC) is a promising marker of renal function, and its application in neonatal care showed several benefits. First, CysC levels are not affected by endogenous substances such as bilirubin, hemoglobin, and ketones in laboratory tests. Second, its level is not influenced by inflammation (e.g., sepsis), muscle mass, age, gender, or nutritional status. Third, CysC is not transmitted through the placenta. Fourth, standardized automated measurement methods are available. Fifth, CysC was shown to reflect renal maturation better than creatinine. Lastly, CysC was reported to change with a wider amplitude than creatinine, which allows a more sensitive detection of renal deterioration. Recently, several reference CysC levels for neonates, including premature infants, were reported. In the present review, we summarized information on serum CysC reference values from different studies. Few studies investigated the usefulness of CysC for predicting AKI in neonates with various diseases such as neonatal respiratory distress syndrome, sepsis, and neonatal asphyxia. Recent studies showed an association between serum CysC level and cardiovascular diseases or Alzheimer's disease, indicating that CysC may play a role as a marker for various diseases in the future. Although the use of CysC as a marker in neonatal care showed several benefits, reliable and detailed reference ranges need to be established, including those for neonates with diseases. In addition, other novel renal markers need to be tested in neonates.

10.A Case of Amylase Producing Small Cell Lung Cancer.

The majority of lung cancers associated with hyperamylasemia are adenocarcinomas. Here we report an unusual case of a 54-year-old male patient who complained of dyspnea, anterior chest wall discomfort and facial edema for one month, presenting with a huge mediastinal mass and hyperamylasemia complicated by pericardial effusion Histological evaluation of mediastinal mass revealed small cell carcinoma and pericardium showed nonspecific inflammation with fibrosis. The serum amylase had an electrophoretic mobility similar to that of salivary gland enzyme. There were no evidence of a salivary or pancreatic causes of hyperamylasemia. After chemotherapy, parenchymal lung lesions improved and hyperamylasemia disappeared. For the mannagement of peracardial effusion a pericardial window was forms(i. We concluded that the striking increase in serum amylase was due to the ectopic production of this enzyme by the tumor.
