Orthognathic Surgery

Endoprosthetic replacement was performed in 38 patients at the department of orthopedic surgery, Seoul National University Hospital in the past ten years from May, 1968, and following results were obtained. 1. Among 38 patients, fracture was in 33 patients, avascular necrosis in 3,and osteoarthritis in 2. 2. Austin-M oore prosthesis was used in 31 patients and Thompson prosthesis in 7. 3. The youngest age was 34 years old and the oldest 85, and the peak incidence was in 7th decade in male and 8th decade in female. 4. The most common type of femur neck fracture was transcervical (60.6%) followed with basal neck (21.1%) and subcapitel (18.2%) 5. Of 33 cases of femur neck fracture, 21 patients (63.6%) was admitted more than 3 weeks after injury. 6. Moore type Southern approach was used most frequently (55.2%). 7. In 36 patients (94.6%), early weight bearing was permitted within 3 weeks, and 25 patients (65.7%) were discharged within 2 weeks after surgery. 8. Minor complications were encountered in 12 patients (31.5%), and none were severe. 9. In follow up of more than 3 months, good to excellent results were obtained in 75%.

We report a caae of hepatic and intestinal infestation with paragonimus wewstermani complicating abscess formation. The pathway by which the paragonimus westermani reaehea liver and intestine is not well understood. However, there was possiblity that it may become lodged in other organs that the lung because of polonged larval migrations through the diaphragm or into various sites in the abdomen. The patient, 52-year-old female gave a history of having reyeatedly consumed raw crabs. An x-ray film of the chest showed clear lunga Skin test for paragonimus-westermani was positive. Abdominal ultrasonogram showed enlargement of the liver with multiple hypoechoic lesiona ERCP revealed multiple ie lesions in the right lobe of the liver. Abdaminal CT showed multifocal abscess cavities with slight rim enhancements. Above meetioned diagnostic procedures suggested liver abscess or hepatoma. Then, an exploratory laparotomy was done for a definite diagnosis, Frozen biopsy, from liver and surgically exe mass from ileocecal region revealed extrpulmonary paragnomiasis involving liver and cecum. Thus, the patient was treated with prasiquantel. Three months later, abdominal ultrasonogram demonstrated slight enlargement of the liver but no evidence of abnormal mass like lesions.

Hereditary motor and sensory neuropathy is an unusual disease which is characterized by deformity of phe foot, acral sensory loss, decreare of deep tendon reflexes, enlargement of peripheral nerve and diminished motor conduction velocity. We report a case of hereditary motor and sensory neuropathy type I.

Squamous cell carcinoma is a rare complication of chronic osteomyelitis. Squamous cell carcinoma of extremity campromises approximately 1 to 2% of all squamous cell carcinoma and of these 2.5 to 4.5% are secondary to chronic osteomyelitis. Biopsy of squamous cell carcinoma, which arises in the proliferating edge of the cutaneous ulcer and invades the bone, should include tissues from all portion of sites of ulcer including bone marrow spaces. But histological diagnosis may be difficult becasue of preexisiting and coexisisting metaplasia and pseudo-epitheliomatous hyperplasia. With prompt and aggressive surgical treatment, the prognosis for patient with squamous cell carcinoma secodary to chronic osteomyelitis is good. Routine regional lymphadenectomy at site of amputation seems unnecessary. This case report represets an unusual bone marrow involvement of squamous cell carcinoma which was complicated by femur fracture.

Acute stroke has been associated with a variety of cardiac abnormalities. Data derived from EGC monitoring have suggested that cardiac arrhythmia may be more common in stroke patient. Acute stroke such as cerebral infarction has been reported to increase serum cardiac enzyme and histologic changes, focal myocardial myocytolysis. Also, plasma norepinephrine was significantly elevated in a group of stroke patients compared with non-stroke control. These observations led to the hypothesis that acute stroke may increase sympathetic activity with resultant electrocardiographic abnormalities and myocardial cell necrosis. In order to test this hypothesis, we evaluated 24 acute stroke patients for several cardiac parameter including arrythmias defected by 24 hour Holter monitoring, serum cardiac enzymes such as CK, LDH, SGOT and plasma catecholamine values. Similar studies were also performed in 15 control subjects matched with stroke patients for age. The result were as follows; 1) Arrhythmias such as VPB, SVPB in the acute stroke group were more common than in the non-stroke group. VPB(209+/-61/24hr, P<0.005) SVPB(232+/-54/24hr, P<0.005). 2) Plasma norepinephrine, epinephrine and CK was significantly elevated in a group of stroke patients compared with non-stroke controls. norepinephrine(715.3+/-93.8 pg/ml, P<0.005) epinephrine(346.1+/-63.1 pg/ml, P<0.005). 3) Stroke patients with abnormal serum CK values(above 80 IU/L) had a higher(P<0.05) mean norepinephrine concentration(776.6+/-142.0 pg/ml) than remaining stroke patients(406.3+/-101.7 pg/ml). 4) Stroke patients with high plasma norepinephrine did not exhibit an increase in cardiac arrhythmias.
