BACKGROUND: Relative changes of astroglial volume constitute the major part of brain edema, which is related to delayed neuronal damage. Several factors including glutamate may contribute to astroglial swelling. Intravenous anesthetic, ketamine was known to restore neuronal damage by inhibiting NMDA receptor activity. Therefore, we decided to investigate the effect of ketamine on the astrocyte swelling by glutamate in the present study. METHODS: To analyze cell swelling in vitro, glial cell line, U1242MG was used. The effects of glutamate (1, 2, 3 mM), and glutamate with ketamine (1 mM) on the regulation of astrocyte volume were achieved by flow cytometry system. To eliminate the dead cells from experimental cell suspension and to assess cell viability, fluorescent dye propidium iodide was used. RESULTS: Glutamate addition (1, 2, 3mM) caused astroglial swelling both in calcium present and calcium absent buffer. The difference of cellular swelling dependent on glutamate concentration was only seen in calcium free buffer (p<0.05). Ketamine per se did not affect astroglial volume. However, when it was added to glutamate perfusion, 1 mM ketamine diminished cellular swelling by glutamate during first 10 minutes (p<0.05), and cellular shrinkage by glutamate after 1 hour incubation (p<0.05). CONCLUSIONS: Ketamine (1 mM) is effective in the regulation of astroglial volume alterations induced by glutamate in both short time and long time perfusion.

The Waltons method-a method of dynamic reconstruction of old ligament injuries of the knee and a combination procedure of O'Donoghue, Bosworth, and Slocum-was introduced with one case experience. This method seemed to be better than other reconstructive procedures, especially in the aspect of rotatory and valgus instability of the knee.

In order to search for anti-Herpes simplex virus (HSV) type-1 agents from Korean medicinal herbs and Korean traditional prescriptions (herb complexes), we selected 80 medicinal herbs and 45 prescriptions, based on a review of the Korean traditional medicine books. Both methanol extracts and boiling-water extracts were tested by means of the MTT assay (tetrazolium based colorimetric assay). Ten of the 125 methanol extracts: CM-11, CM-18, CM-19, CM-21, CM-22, CM-39, MM-3, MM-18, MM-29, MM-73 (see explanation of nomenclature below), showed efficacy against HSV-1. Twelve of the water extracts: CW-2, CW-3-I, CW-3-II, CW-18, MW-3, MW-5 MW-6, MW-12, MW-47, MW-69, MW-73 and MW-75 were active. #3 (individual herb) and #73 (individual herb) were interesting because both water and methanol extracts were active. Especially, #3 is a part of composition of Hong-il-laksamd ngbang and Hojanghaedokt'ang which have anti-HSV-1 activitives. The SI value of MW-69 and CW-18 was relative high as 10.2+/-0.7 and 11.8+/-2.2. The cytotoxic effect on Vero cells of Panch' n-ch' ngbang, Taraxacum platycarpum H. Dahlst. and acycloguanosine was determined by MTT assay. Water extracts of Panch' ngch' ngbang (prescription) and Taraxacum platycarpum H. Dahlst. showed very weak cytotoxic effects on Vero cells at > 100 mug/ml but acycloguanosine showed strong cytotoxic effects on Vero cells at > 100 mu g/ml. As a result, #3, #73, MW-69 and CW-18 are considered as potentially useful for anti-HSV-1 agent and will be the focus of further research. Nurses experience role conflict between nursing theory that they learned in school and clinical nursing practice. This conflict lead to lower self-image, self-esteem, job dissatisfaction. Also, the professionalism of nurses is estimatable by the grade of autonomy. The professional autonomy requires individual and professional obligation about her decision and performance. A lack of professional autonomy results in the job dissatisfaction. Job dissatisfaction leads to absenteeism, increased proneness to mental and physical illness and higher turnover rates. The purpose of this study was o investigate professional autonomy and self-concept of clinical nurses. Data were collected from 262 clinical nurses in P city from June 1 to June 30, 1996. The instruments used for this study were made by Arthur(1990) and Schutzenhofer(1983). The data were analyzed for frequency and percentage, mean, SD, t-test, ANOVA, Pearson's correlation coefficient, stepwise multiple regression, using SAS PC+ Program. The findings were as it follows : 1. The mean of professional autonomy and self-concept were 152+/-18.48, 70.65+/-8.20(2.62), respectively. These showed mid level of professional autonomy and self-concept. 2. Professional autonomy of clinical nurses was found to vary significantly according to total years of clinical experience(F=4.49, p<0.01), position(F=3.49, p<0.05), and state under study for the degree(F=3.83, p<0.05). Professional self-concept was found to vary significantly according to age(F=3.52, p<0.05), marital status(F=7.39, p<0.001), total years of clinical experience(F=3.59, p<0.05), position(F=5.22, p<0.01), the expectant period being on the duty as clinical nurse(F=8.34, p<0.001), and motivation of choosing nursing(F=5.17, p<0.001). 3. The statistical relationship between clinical nurses' profession autonomy and self-concept was found as positive correlation9r=0.42246, p<0.001). 4. Professional autonomy was the highest factor predicting professional self-concept(17.85%). Professional autonomy and marital status accounted for 20.60% in professional self-concept of clinical nurses. In conclusion, Professional autonomy and self-concept of clinical nurses showed significantly positive relationship. Therefore, nursing education needs to develop programs and policies to increase professional autonomy and self-concept of clinical nurses.

PURPOSE: To evaluate the predictability of amount of hemothorax in the patients with blunt chest trauma, supine chest AP radiographs of 66 patients were reviewed and statistically analyzed. MATERIALS AND METHODS: In 66 patients, rib fractures were present in 53 patients, hemothorax in 46 patients, pneumothorax in 25 patients, and pulmonary contusions in 18 patients. Width and length of hemothorax were measured on supine chest AP radiograph, and were correlated with known drained amount of hemothorax. The presence and number of rib fracture, pulmonary contusion, subcutaneous emphysema, fracture of scapula and clavicle, and total opacification of hemithorax were also correlated with the drained amount of hemothorax. RESULTS: In multiple logistic regression analysis, width of hemothorax had the highest correlation with drained amount of hemothorax(regression coefficient 0.718, p value 0.00005). The presence and number of rib fracture, scapular fracture, subcutaneous emphysema were also correlated with drained amount of hemothorax. But length of hemothorax, pulmonary contusion, clavicular fracture, total opacification of hemithorax were not correlated with drained amount of hemothorax. CONCLUSION: Measured width of hemothorax in supine chest AP radiograph is the most reliable predictor for estimation of the amount of hemothorax, and may also be used as an indication for the application of closed thoracostomy in the treatment of hemothorax.

BACKGROUND: As of alpha2-agonist, clonidine reduces generalized sympathetic outflow in nervous system and also reduces acetylcholine release at cholinergic terminals presynaptically. So clonidine premedication is possibly able to decrease muscle contraction and prolong the duration of neuromuscular blockers. Therefore, the aim of our current study is to investigate the effect of oral clonidine on the duration of vecuronium. METHODS: Forty patients (ASA I or II) sheduled for elective low abdominal or extrimities operation were randomly divided into 2 groups. Clonidine group (n=20) received 5 microgram/kg oral clonidine at 90 min before operation. Control group (n=20) received nothing. Neuromuscular transmission was measured with relaxograph. After injection of vecuronium 0.1 mg/kg, we measured onset time (the time from injection of vecuronium to decrease to the 25% of baseline value, duration 1 (the time interval between injection and recovery of the first twitch to 25% of the baseline value), and duration 2 (the time interval between second injection of 0.02 mg/kg vecuronium and recovery of the first twitch to 25% of the baseline value). RESULTS: There were no statistical differences between control and clonidine group in onset time (2.6 +/- 0.6 min vs 2.7 +/- 0.5 min), duration 1 (37.5 +/- 8.9 min vs 40.3 +/- 8.6 min) and duration 2 (22.0 +/- 6.8 min vs 24.4 +/- 6.1 min). CONCLUSIONS: Five microgram/kg of oral clonidine premedication did not prolong the duration of vecuronium.

The purpose of this study was to determine the factors related to eating behavior of breakfast and dietary life score in elementary, middle, and high school students. This study was conducted by administering questionnaires, and data were analyzed by the SPSS program. The subjects were divided into three groups according to school group (elementary, middle, and high school). The subjects consisted of 1,879 students (male 973, female 906) from 11 schools in Kyungpook. The distribution of subjects was as follows: elementary school children 682, middle school students 702, and high school students 495. The results are summarized as follows. Dietary life score was 72.64 for elementary school students, 64.77 for middle school students, and 62.67 for high school students. Frequency of eating breakfast and reasons for skipping breakfast were significantly different according to school group (P<0.001, P<0.01, respectively). The main reason for skipping breakfast was 'habitual skip' for elementary and middle school students (54.8%, 46.7%) and 'getting up too late' in high school students (55.4%). There were no significant association between frequency of eating breakfast and BMI. However, there were significant differences in school performance and sleeping hours according to frequency of eating breakfast (P<0.001, P<0.01, respectively). Dietary life score was significantly different according to frequency of eating breakfast (P<0.001). These results stress the need for intervention programs aimed at decreasing the frequency of skipping breakfast in children and students at school. Further, those who prepares meals for children must increase their concern about preparing breakfast.
