1.The effects of insulin-like growth factors on the proliferation of endometrial stromal cells from patients with and without endometriosis.

OBJECTIVES: To test the hypothesis that the endometrial stromal cells from patients with endometriosis responds differently to the insulin-like growth factor (IGF) compared with those from patients without endometriosis. METHODS: IGFs in peritoneal fluid (PF) from patients with endometriosis(n=18) and without endometriosis(n=12;control patients) were measured by radioimmunoassay. Endometrial stromal cells from patients with endometriosis and control patients were cultured in serum free media(SFM) in the presence or absence of PF or IGF-I(0.25-25 ng/ml) or IGF-II(5-50 ng/ml) and the proliferation of endometrial stromal cells were evaluated by [3H] thymidine incorporation test. All statistics were performed by ANOVA test and student's t-test. RESULTS: When added to SFM, IGF-I(1-25 ng/ml) increased thymidine incorporation in both endometrial stromal cells from patients with endometriosis and control patients in dose dependent manner and IGF-II(5-25 ng/ml) gave similar response in latter cells but not in former cells. Within low IGF-I(less than 100 ng/ml) PF group or high IGF-I(more than 100ng/ml) PF group, the type of endometrial stromal cells did not result in any difference in thymidine incorporation. However, regardless of the source of stromal cells, high IGF-I PF group produced a greater extent of thymidine incorporation than low IGF-I PF group in patients with endometriosis but not in control patients. Also, thymidine incorporation was higher in high IGF-I PF group of former patients than in the same group of latter patients. PF induced higher thymidine incorporation in endometrial stromal cells than the same levels(0.25-2.5 ng/ml) of IGF-I directly added to SFM. CONCLUSIONS: The effects of IGF-I in PF on endometrial stromal cells are similar regardless of their source and IGF-I is one of several growth factors that may participate in the growth of endometrial stromal cells in pelvic endometriosis.

Endometriosis in the abdominal wall following Cesarean section is one of the very rare condition among the extrapelvic endometriosis. Although benign, endometriosis possesses the unique ability to invade tissue and to disseminate or metastasize by hematogeneous, lymphatic route, or direct implantation. We reported the case with brief review of the literature.

3.Appearance of Polymorphonuclear Leukocytes Following Corneal Scratching Wound by Impression Cytology.

The linear scratching wound was made gently on the corneal epithelium of rabbit with 21 gauge needle under an operating microscope. Impression cytology was performed at 30 minutes, 1, 2, 3 and 4 hour and 1, 2, 3 and 4 day after 0.5% tetracaine drops under an operating microscope. The filter paper was stained with hematoxylin and eosin. At 30 minute post-scratching, a few polymorphonuclear leukocytes appeared on the scratched cornea at 3 eyes (30%). At 3 and 4 hour, numerous polymorphonuclear leukocytes with corneal epithelial cells appeared on the scratched conea. By 3 day, no inflammatory cells were shown on the filter paper in all eyes. These findings suggest that the polymorphonuclear leukocytes could infiltrate on the corneal lesion at 30 minute post-scratching and the inflammatory cells might act even on the minute corneal lesion such as corneal erosion.

4.The Effect of Nutrition Education on Weight Control in Male Workers by the Community Health Center.

The principal objective of this study was to determine the importance of obesity management nutrition programs for obese male workers after performing a 10-week nutrition program and to compare changes in the obesity index, health behavior, meal intake, and blood constituents. The study subjects included 44 individuals, all without chronic disease. After the program, the weight, body mass index (BMI), body fat percentage, and waist-hip ratio (WHR) of the subjects dropped. Dietary life behavior scores increased. In particular, more than 30% increases were detected in self-control of surfeit, control of eating speed, temperance, and regular exercise items. Total caloric intake and average drinking intake dropped. Though insignificant, systolic pressure, total cholesterol, LDL-cholesterol, and sGPT all dropped as compared to the levels measured prior to the program. Fasting blood sucrose, triglyceride, g-GTP, and atherogenic index dropped significantly. HDL-cholesterol, though insignificant, increased. As the result of examining correlations between the subjects' health life behavior, caloric intake, and improvement in obesity, we noted a significant correlation between self-control of surfeit and temperance in the health life behavior item, along with significant correlations among total calorie intake, surfeit percentage, and daily average drinking intake. These results demonstrated that nutrition program for obese workers performed at the Community Health Center had the effect of reducing obesity, improving health behavior, and improving clinical levels. Thus, it is advisable that obesity management programs be achieved, linking the center of local societies' health promotion projects, community health centers, and workplaces, and various programs should be developed to maintain and continue changes in workers' behaviors, such that the positive effects of improvement in obesity could be maintained, even after the program.

5.Malignant Synovioma: Report of a case

A case of malignant synovioma of the right knee joint is presented. Microscopically it showed sarcomatous growth of spindle cells and irregular slit spaces which were lined by tumor cells. A–K amputation has carried out on 10th hospital day and discharged on 30th hospital day without any complications.

6.Conquering Process of the Group-bullies among Teenagers.

PURPOSE: This study was to develop a substantive theory on the conquering experience of group-bullies among teenagers, and thereafter suggest a model for solving the problems. METHOD: Data collection and analysis was proceeded by a method of Grounded Theory. Data was collected by interviewing the participants. Five teenagers participated in this study who had already overcome the situation. RESULT: In the analysis 31 concepts, 6 categories, and 14 subcategories are extracted from the raw data, and all are constituted on the paradigm model. The causal condition is a painstaking of participants. The context is affirmative reconsidering and the phenomenon is conquering difficulties. The intervening condition is a positive intervention of a supporting system and the strategy for action/interaction is building a relationship. The consequences of this analysis is harmonizing. CONCLUSION: The results of this study suggest adequate ways of conquering group-bullies. These are positive attitudes towards solving problems and the full use of support systems surrounding them.

7.Clinical Analysis of Syringomyelia.

Eight patients were diagnosed as syringomyelia by MRI at the Department of Neurosurgery of Maryknoll hosptial, Pusan from April, 1990 to April, 1991. The causes were chiari type 1 malformation(2 cases), neoplasm(1 case), trauma(1 case) and idiopathic(4 cases). Six procedures were performed in the five patients. The procedures were foramen magmum decompression(1 case), formen magmum decompression combined syringosubarachnoid shunt(1 case), syringosubarachnoid(1 case), syringoperitioneal(2 cases) and syringopleural shunts(1 case). We have reviewed the modes of clinical presentations, diagnostic evaluations and surgical considerations.

8.The Report of Two Cases of Primary Corneal Peripheral Ischemic Syndrome.

The authers had successfully treated two cases of primary corneal peripheral ischemic syndrome by subconjunctival heparin injection. The first 60 years male patient had a history of a progressive visual disturvance, photophobia and ocular pain in both eyes for 5 months. At the time of his first visit to our hospital, deep ring shaped ulceration at corneal periphery was observed. So he was treated by subconjunctival heparin injection and systemic steroid administration. The second 70 years female patient had a history of Progressive visual disturbance since 3 ears ago and photophobia tearing and ocular pain. At the time of her first to our hospital, deep ring shaped ulceration at corneal periphery was found. So she was treated by subconjunctival heparin injection and systemic steroid administration too. We found no evidence of further ulceration in two cases.

10.Effects of Carbogen Inhalation and Stellate Ganglion Block in the Treatment of Idiopathic Sudden Sensorineural Hearing Loss.

BACKGROUND AND OBJECTIVES: Impairment of inner ear circulation may be one of the pathophysiology of idiopathic sudden sensorineural hearing loss (ISSNHL). Carbogen inhalation therapy, in which patients breathe a gas mixture of 5% CO2 and 95% O2 by mask, and a stellate ganglion block (SGB) are potent vasodilatation and tissue oxygenation procedures widely used as treatment modalities in ISSNHL. The purpose of this study was to compare the effectiveness of each modality and to find out the differences in the effectiveness of carbogen inhalation therapy and SGB. MATERIALS AND METHOD: We reviewed the records of 313 patients who were admitted to the Masan Samsung Hospital from January 1995 to December 2000 for investigation of the efficacy of carbogen inhalation therapy and SGB. One hundred and thirty-one patients were treated with carbogen inhalation, SGB and drug medication, and 57 patients without SGB and 125 patients were treated with medication only. RESULTS: Therapeutic results of carbogen inhalation and SGB group was better than that of medication only group. But no statistical differences were found between the carbogen inhalation and SGB group. CONCLUSION: These results suggest that carbogen inhalation and SGB were effective treatment modalities of ISSNHL. We recommend that carbogen inhalation therapy and SGB could be utilized as the standard treatment of ISSNHL.
