1.Factors Influencing Health Promoting Behavior in the Elderly People: Perceived Conception of Health and Family Support.

PURPOSE: The purpose of this study was to describe perceived conception of health, family support and health promoting behavior; as well as to assess factors that influence health promoting behavior. METHOD: Study participants were 165 elderly people over the age of 65, living in C city. The instruments were Laffery's health concept scale, the family support scale by Kang, and the health promoting behavior scale by Walker et al. RESULTS: 1. The scores for level of health concept ranged from 28 to 112, and had a mean score of 75.16. The scores for level of family support ranged from 11 to 55, and had a mean score of 41.55. The scores for health promoting behavior ranged from 40 to 160 with mean score of 98.07. For health promoting behavior the participants revealed that the most frequent practices were in nutrition, and the least frequent, in exercise. 2. Higher levels of health conception and family support were correlated with an improving level of health promoting behavior . 3. The factor most influencing health promoting behavior in elderly people was family support. Family support accounted for 11% of the variance in health promoting behavior. A combination of health conception, education level and dwelling pattern accounted for 23% of the variance in health promoting behavior. CONCLUSION: Perceived health conception and family support were identified as important variables for health promoting behavior in elderly people.

3.Sexual Knowledge of the Male High School Students in a Rural Area of Gangwon Province.

PURPOSE: The purpose of this study was to identify the level of sexual knowledge of male high school students. METHOD: The participants were 503 male students from a high school in one county of Gangwon Province. The data were collected from May 1 to May 30, 2004. Collected data were analyzed using SPSS program to investigate the responses to the structured questionnaires used in the study. RESULTS: The level of sexual knowledge of high school students was 10.25(range 0 to 17). In terms of response distribution of sexual knowledge by items, the correct response rate was highest for "wet dream is discharge of semen during sleep", and the wrong response rate was lowest in "gonorrhea occurred most frequency in 15-25 year old adolescents". General characteristics showed significant differences in sexual knowledge depending on grade(grade 3 :10.45, grade 2: 10.40, grade1: 9.88). Depend on the sexual impulse and the solutions to sex problems, there were significant differences in sexual knowledge with respect to sexual characteristics. CONCLUSION: As a whole the sexual knowledge of the male high school students was low, sexual education programs for high school students should focus on sexually transmitted diseases and contraception.

4.Attitudes and Knowledge toward Koryo Hand Therapy(KHT) by the Experienced People.

PURPOSE: The purpose of this study was to identify the current attitudes and knowledge toward Koryo Hand Therapy(KHT) by the experienced people. METHOD: The Subjects for this study included 3,351 people living in Korea who experienced KHT. The data were collected from April 6 to November 25, 2004 using a structured questionnaire. The data were analyzed through the SAS 8.1 Windows Program using frequency, percentage, t-test, ANOVA and Pearson's correlation coefficient. RESULTS: The results of the study are as follows: 1) Families and relatives were a main source of information about Koryo Hand Therapy. The subjects were very satisfied with therapeutic and preventive effects of the therapy. Sujichim was the most preferable choice in the KHT. Most of the subjects thought that KHT could be applied to modern medicine. 2) Those who experienced KHT had a high level of knowledge and very positive attitudes toward KHT. 3) There were statistically significant differences in the attitude toward KHT according to age, gender, educational level, religion, income, place of residence, marital status and experience which studied KHT. 4) There were significant differences in the knowledge of KHT according to age, educational level, income, marital status and experience which studied KHT. 5) The data showed positive correlation between attitude and knowledge. CONCLUSION: According to the above findings, it can be concluded that KHT needs to be actively informed and more easily approached by general peoples. Nursing protocols of KHT needed to be developed.

5.H MR Spectroscopy on Parkinson's-like Syndrome Induced by Manganese Intoxication: A Case Report.

Manganese intoxificaton is a well-known cause of Parkinson's like syndrome. We describe a 46-year-old man who had been occupationally exposed to manganese and report the case with hydrogen magnetic resonance spectroscopy (1H MRS). Ratios of N-acetyl-aspartate (NAA) to creatine were significantly reduced in basal ganglia regions compared to normal subjects. The level of NAA was decreased in basal ganglia regions may indicate neuronal dysfunction. 1H MRS can provide detailed information of brain damage, therefore the 1H MRS is very useful in diagnosis of manganese intoxification.

6.The Relationship between Isoniazid Resistance and 463 CodonMutation of katG Gne in Mycobacterium Tuberculosis.

BACKGROUND: The 463 codon mutation of katG gene has been reported as an useful marker for the detection of isoniazid(INH) resistant strains of M. tuberculosis. This study aimed to elucidate relationship between 463 mutation in katG gene and INH resistance in M. tuberculosis. METHOD: DNA was extracted from 28 INH susceptible strains(MIC > or = 0.2microg/ml on the Lowenstein Jensen media) and used for amplification of 189bp fragment containing 463 codon by PCR. Amplified fragments were digested by restriction enzyme Msp I, analyzed by single strand conformation polymorphism(SSCP) in the MDE gel and sequenced to prove mutation. RESULT: Only 7(25%) out of 28 were digestible by restriction enzyme Msp I. The SSCP pattern of 21 strains were distinctly different from that of M. tuberculosis H37Rv. Msp I undigestible PCR fragment was substituted at 463 codon from Arg(CGG) to Leu(CTG). CONCLUSION: This finding clearly indicate no relationship between 463 codon mutation of the katG gene and INH resistance.

7.Effects of Epidural Clonidine on Lidocaine Epidural Anesthesia.

BACKGROUND: This study was undertaken in surgical patients to evaluate the comparative analgesic, hemodynamic, sedative effects of clonidine or epinephrine when added to lidocaine solution in patients receiving epidural anesthesia. METHODS: The effects of clonidine were assessed in 38 men scheduled for knee surgery. Patients received 20 ml of 2% lidocaine with clonidine 50 microgram (group C50, n=11), with clonidine 100 microgram (group C100, n=10), with epinephrine 100 microgram (group E, n=9), or plane (group L, n=8) via epidural catheter. Sensory anesthetic level was assessed by pinprick and the time to L1 analgesia and the maximum number of segments of analgesia were checked. Systolic blood pressure (BP) and heart rate (HR) were measured at 2, 5, 10, 15, 20, 30, 45, 60 min after epidural injection. Sedation was defined on a scale of 0 to 3. The sedation score was checked at 5-minute intervals after epidural injection. Samples of venous blood were obtained before and 5, 10, 20, 30, 45, 60, 90 min after epidural injection. RESULTS: No significant differences in the time of L1 analgesia and number of segments of analgesia found at any observation period among the four groups of patients. There was no decrease in BP and no differences among four groups. HR decreased significantly in patients given clonidine, but did not change in those given lidocaine plain or with epinephrine. Significant differences were also observed in sedation scores between clonidine group and group L or E. Maximal plasma lidocaine concentrations in group C100 were similar to the group C50 and L, but were significantly greater than those in group E (p<0.05). CONCLUSIONS: These results indicate that the addition of clonidine to lidocaine for epidural anesthesia provides a sedative effect and relatively stable hemodynamics, and that clonidine in a concentration 1:200,000 in contrast to 1:200,000 epinephrine, tends to increase rather than to suppress the plasma lidocaine concentration.

8.Changes of Intracoronary Electrocardiogram by Repeated Occlusion: Repefusion During Percutaneous Transluminal Coronary Angioplasty.

OBJECTIVE: Brief epidodes of ischemia have been shown to make the heart more resistant to subsequent ischemia in animal studies(known as ischemic preconditioning, IP). This phenomenon was tested in patients undergoing percutaneous transluminal coronary angioplasty(PTCA). METHODS: Thirteen patients who had significant epicardial coronary stenosis without myocardial infarction, ventricular hypertrophy or conduction defect, received two to four 2-min balloon inflations separated by 5 min of reperfusion. Surface electrocardiogram(S-ECG) and intracoronary electrocardiogram (IC-ECG) from an angioplasty guide wire were recorded before and after balloon inflation. RESULTS: The changes of ST segment were observed in 13 out of 15 lesions on IC-ECG and 7 on S-ECG. The maximal ST changes on IC-ECG and S-ECG were 20.2+/-13.7mm and 1.21.5mm respectively(p<0.01). The time to beginning of ST segment change after balloon inflation were 10.1+/-12.6 seconds and 63.3+/-14.2 seconds on IC-ECG and S-ECG, respectively(p<0.01). The maximal changes of ST segment on IC-ECG during the second inflation were significantly lower than that during the first(20.2+/-13.7 vs 16.312.3mm, p<0.05). However, changes of R wave, T wave and QT interval were not significantly different between two inflations. The recovery time to baseling ECG after initiation of reperfusion were 50.2+/-41.7 seconds and 38.5+/-29.6 seconds for the first inflation and the second, respectively(P<0.05). CONCLUSION: These results suggest that IC-ECG is more sensitive and reliable than S-ECG in detection of myocardial ischemia and that IP may occur during PTCA since ST segment shift is decreased and is normalized earlier at the second balloon inflation compared with the first.

A recent study showed that comparative sequence analysis of rpoB DNAs could reveal natural relationships in genus Mycobacterium [J Clin Microbiol. 37 (6). 1999]. rpoB DNAs showed interspecies variation and intraspecies conservation, Based on these data, we developed polymerase chain reaction-single strand conformation polymorphism (PCR-SSCP) protocols which enable species differentiation in genus Mycabacterium. When this assay was applied to 24 clinical isolates identified as M. avium complex (MAC) by biochemical test, these were successfully differentiated into M. avium and M. intracellulare. These results were concordant with those obtained by 16s rDNA analysis. It is the first report that PCR-SSCP analysis of rpoB DNA could be used for species differentiation of MAC strains.
