2.A Clinical Study of the Patella Fracture

Vertical and marginal fractures of the patella are uncommon injuries compared with other types of patellar fracture. It's possible that they are more common than supposed, but often remain undiagnosed as acute injuries. Vertical and marginal fractures often results in less acute disability than stellate or transverse fracture and the routine radiographs are often unhelpful. Failure to diagnose acute vertical and marginal fractures of the patella, especially when the fragments are displaced, may result in prolonged disability of the knee and possible development of degenarative changes in the patello-femoral joint later. Twelve patients of the vertical and the marginal patellar fractures treated at St. Mary's Hospital from Feb. 1982 to Dec. 1987 were analyzed in clinical and radiological aspect. The results obtained from this study were as followings. 1. Vertical and marginal fractures had 15.4% of injury rate in all patellar fractures. 2. The fracture and its displacement were always present on the tangential view of the patella. 3. The mechanism of the fracture was s direct blow to the patella in which the affected knee was flexed. 4. The location of the fracture line was related to the degree of the flexed knee at the time of fracture. 5. Large separated fragments of the patella should be accurately reduced with rigid fixation in the rationale of the intraarticular fracture.

3.Diffuse Pigmented Villonodular Synovitis of the Hip Joint

In 1941, Jaffe and coworkers studied a lesion with histologic appearance of fibrous stroma, pigmented deposition and histiocytic infiltration as well as giant cell, for which they named pigmented villonodullar synovitis, bursitis and tenosynovitis. Thereafter many authors have disscussed etiology, clinical and radiological features, pathology and treatment regarding the disease. Recently we experienced a case of pigmented villonodular synovitis involving an hip joint which was treated by synovectomy and total hip replacement with good result.

5.An Analytic Study on the Effect of Carbon Disulfide on the Blood Pressure.

To investigate the effect of carbon disulfide on blood pressure, the BP measurements in the periodic health examination results and the medical records of factory clinic were reviewed The study subjects were composed of 1336 male and 544 female workers, who were categorized into three groups by the exposure status-highly exposed, moderately exposed and non-exposed group. The results of the study were as follows; 1. The age-adjusted mean systolic and diastolic BP of male workers were 122.35 mmHg/79.11mmHg in highly exposed, 121.57mmHg/79.05mmHg in moderately exposed and 122.67mmHg/82.27mmHg in non-exposed group. For female workers, BP were 115.13mmHg/74.49mmHg in moderately exposed and 113.48mmHg/74.30mmHg in non-exposed group. 2. In multiple regression analysis of maximum BP against Age and tenure, the slope coefficients of age and tenure on the systolic BP were 0.379, 0.667 respectively and those on the diastolic BP were 0.331, 0.405 respectively in highly exposed male workers. Tenure was a significant variable in this study. For female workers, however, the slope coefficients of tenure on BP were significant only for systolic BP of moderately- expected group. 3. In multiple regression analysis of Bp against age, cumulative exposure index(CEI), cholesterol, all the variables showed significant slope coefficients in male, but age and CEl on systolic BP were significant for female workers (P<0.05). 4. In the multiple analysis of the amount of Bp change and the velocity of Bp change among male workers, the slope coefficients of tenure tended to increase as exposure level increased. Among female workers, the slope coefficients of tenure were significant on the amount of Bp change and the velocity of Bp change in moderately exposed group.

6.One-Stage Resection and Anastomosis of Left Colon Cancer Obstruction.

Although the obstruction of the right colon is usually handled by primary anastomosis following resection, fear of the increased incidence of septic complication, especially anastomotic leakage with sepsis has turned surgeons away from doing anastomosis in the face of acute obstruction of the left colon. However, from recent reports, enough experiences have been accumulated to show that primary anastomosis is associated with minimum morbidity and mortality in the acute obstruction of the left colon. We experienced 54 cases of colon cancer obstruction at Holy Family Hospital from January 1988 to December 1997. Twenty six cases of them were right colon cancers, 24 cases were left colon cancers and 4 cases were rectal cancers. We reviewed these three groups for evaluation of the safety of one-stage resection and anastomosis of left colon cancer obstruction. The postoperative complication rate was 18% in right colon obstruction versus 38% in left colon obstruction. The most common complication was wound infection(43%). In using of primary resection and anastomosis, complication of right colon revealed 15% and left colon was 29%. But in a method of primary resection and anastomosis with decompression, complication of right colon was 17% and left colon was 13%. Especially on the left colon, primary resection and anastomosis with decompression revealed lower complication(13%) than that without decompression(67%). The mortality of colon cancer obstruction was 2% but this was a patient who had a poor general condition and took a primary resection and anastomosis without decompression. In cases of left colon cancer obstruction primary resection and anastomosis with decompression of left colon cancer obstruction can be a safe operation method with low morbidity and mortality.

8.Early Failure of Acetabular Component after Total Hip Arthroplasty in Irradiated Hip.

PURPOSE: The treatment of the osteonecrosis in the acetabulum and femoral head after irradiation has not been well estabilished. The early failure of acetabular component after total hip arthroplasty (THA) is the main concern in osteonecrosis after irradiation. MATERIALS AND METHODS: Our study included 18 patients with post-radiation osteonecrosis of 26 hips who had a THA, mean age 54.2 years. Average irradiation dosage was 4611 cGy and interval between irradiation and primary total hip replacement was 64.3 months. The cementless acetabular components were used in 16 hips (group one) and acetabular reinforcement rings were used in 10 hips (group two: Muller cup 6 cases, Ganz cup 4 cases). RESULTS: Acetabular loosenings were occurred at 8 of 16 hips (Group 1: 50%) during the mean follow-up 46 months. The cases of Muller cup (Group 2: 33%) was failed in 2 of 6 hips at postoperative 12 months and 22 months. CONCLUSION: Although follow up duration is short and cases are small, the use of cemented cup with acetabular reinforcement ring was better than non-cemented cup for THA in irradiated hip in terms of preventive of early failure of acetabular component.

9.The Analysis of 40Hz Event-Related Potentials in Schizpphrenia.

BACKGROUNDS: Gamma band oscillatory activity is considered to be related to congnitive functions and illustrates that the concept of event-related oscillations bridges the gap between single neurons and neural assemblies. An event-related gamma oscillation is the time - locked responses of specific frequency, and can be identified by computing the amplitude frequency characteristics of the averaged event-related potentials(ERPs) after stimulation. OBJECTIVES: We purposed to present experimental paradigm to investigate @-band oscillation activities from the recording of ERPs by using auditory oddball paradigm and investigate the difference of @-band activity between schizophrenia and normal controls. METHODS: The ERPs resulting from auditory stimuli with oddball paradigm in a group of schizophrenics(n=11) and also a group of age-, sex- and handedness matched normal controls, were recorded by 128 channel EEG. The @-band oscillatory activities were calculated by using time-frequency wavelet decomposition of the signal between 20 and 80Hz. The @-band oscillatory activities of both groups were compared by t-test. RESULTS: The @-band oscillatory of the leads Fz,Gz and Pz of both groups were represented well in the time - frequency maps. Significant increases of the @-band activity in normal controls compared with schizophrenics were observed around 160 msec. 350msec. and 800 msec after stimulation. CONCLUSIONS: Our results suggested that the increment in @-band oscillatory activity during cognitive operations and decreased @-band activity in schizophrenics may be associated with the cognitive dysfunctions and the pathophyiology of the schizophrenia.

10.The Geographic Patterns of The Adult Male Bladder Cancer Patients in Korea: Clue as to the Occupational Exposure.

We calculated the age specific Proportionate Morbidity Ratio(PMR)' for different areas as the outcome index by using the national cancer registry data(1988 1991) to assess the patterns of stomach cancer and bladder cancer. Also, we calculated 'Manufacturing workers Concentration Index(MCI)' for different areas as the exposure index by using the census data reported in 1980 to assess the relationship between cancer site specific PMRs and MCIs. As a result, we found the significant correlation(r=0.61, p=0.002) between the bladder cancer in the age of 40s and the industrial exposure indicator. Future analytic studies should be fruitful in identifying more occupational risk factors for bladder cancer. Investigation of cancer incidence including geographic variations and difference by age may identify patterns suggesting occupational exposures.
