In order to provide more effective and personalized healthcare services to patients and healthcare professionals, intelligent active knowledge management and reasoning systems with semantic interoperability are needed. Technological developments have changed ubiquitous healthcare making it more semantically interoperable and individual patient-based; however, there are also limitations to these methodologies. Based upon an extensive review of international literature, this paper describes two technological approaches to semantically interoperable electronic health records for ubiquitous healthcare data management: the ontology-based model and the information, or openEHR archetype model, and the link to standard terminologies such as SNOMED-CT.

3.Dosimetry and Medical Internal Radiation Dose of Re-188-DTPA for Endovascular Balloon Brachytherapy Against Restenosis after Coronary Angioplasty.

PURPOSE: Liquid beta emitter filled in angioplasty balloon could be used to perform endovascular balloon brachytherapy to prevent coronary artery restenosis. We investigated the dosimetry for Re-188-DTPA liquid-filled balloon and medical internal radiation dosimetry in case of balloon leakage. MATERIALS AND METHODS: We estimated radiation dose from an angioplasty balloon (20 mm lengfh, 3 mm diameter cylinder) to the adjacent vessel wall using Monte Carlo EGS4 code. We obtained time-activity curves of kidneys in normal dog and calculated Tmax, T1/2. Using MIRDOSE3 program, we estimated absorbed doses to the major organs (kidneys, bladder) and the whole body when we assumed that balloon leaked all the isotope contained. RESULTS: The radiation dose was 17.5 Gy at the balloon surface when we applied 3,700 MBq/ml of Re-188 for 100 seconds. Fifty percent of the energy deposited within 1 mm from the balloon surface. The estimated internal dose to the whole body was 0.005 mGy/MBq and 18.5 mGy for the spillage of 3,700 MBq of Re-188. CONCLUSION: We suggest that Re-188-DTPA can be used for endovascular balloon brachytherapy to inhibit coronary artery restenosis after angioplasty with tolerable whole body radiation dose in case of balloon rupture.

4.The Relationship between Physical Discomfort, Burnout, Depression, Social Supports and Emotional Labor of Clinical Nurses in Korea

PURPOSE: The purpose of this study was to investigate the reality, status of clinical nurses' emotional labor and the relationship with physical discomfort, burnout, depression and social support. METHODS: A thousand three hundred sixteen clinical nurses from 42 hospitals nationwide participated in this study. Questionnaires were developed for evaluating the reality and status of emotional labor of clinical nurses after interviewing focus groups and reviewing literatures. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient. RESULTS: Ninety eight percent of nurses had answered having emotional labor and they experienced 82 times per year, 9.6 times during last a month which means they experience it every other day. The one who provoke emotional labor were patients, guardians, physicians, supervisor, and colleagues in sequence. Eighty percent of nurses had intention to leave their jobs after experiencing emotional labor. They were doing more surface acting than deep acting of emotional labor. CONCLUSION: Clinical nurses in Korea experienced excessively high level of emotional labor and delayed responding to clients' requests due to lack of time for caring each patient was revealed as one of the main causes of emotional labor.

5.The Heterogeneity of Flow Distribution and Partition Coefficient in < 15O-H2O > Myocardium Positron Emission Tomography.

For estimation of regional myocardial blood flow with O-15 water PET, a few modifications considering partial volume effect based on single compartment model have been proposed. In this study, we attempted to quantify the degree of heterogeneity and to show the effect of tissue flow heterogeneity on partition coefficient(lamda) and to find the relation between perfusable tissue index(PTI) and (lamda) by computer simulation using two modified models. We simulated tissue curves for the regions with homogeous and heterogenous blood flow over a various flow range(0.2-4.0ml/g/min).Simulated heterogeneous tissue composed of 4 subregions of the same or different size of block which have different homogeneous flow and different degree of slope of distribution of blood flow. We measured the index representing heterogeneity of distribution of blood flow for each heterogeneous tissue by the constitution heterogeneity(CH). For model I, we assumed that tissue recovery coefficient (FMME) was the product of partial volume effect(FMMF) and PTI. Using model I, PTI, flow, and FMM were estimated. For model II, we assumed that partition coefficient was another variable which could represent tissue characteristics of heterogeneity of flow distribution. Using model II PTI, flow and (lamda) were estimated. For the simulated tissue with homogeneous flow, both models gave exactly the same estimates of three parameters. For the simulated tissue with heterogeneous flow distribution, in model I, flow and FMM were correctly estimated as CH was increased moderately. In model II, flow and (lamda) were decreased curvi-linearly as CH was increased. The degree of underestimation of (lamda) obtained using model II, was correlated with CH. The degree of underestimation of flow was dependent on the degree of underestimation of (lamda). PTI was somewhat overestimated and did not change according to CH. We conclude that estimated (lamda) reflect the degree of tissue heterogeneity of flow distribution. We could use the degree of underestimation of (lamda) to find the characteristic heterogeneity of tissue flow and use (lamda) to recover the underestimated flow.

6.Labeling and Biodistribution of Re-188-DTPA ( Diethylenetriaminepentaacetic acid ).

Re-188 is useful candidate for therapeutic radionuclide because it has a physical half life of 17 hours, contains beta ernissions suitable for therapy(maximum energy 2.12MeV) and emits a garnma ray that is suitable for quantitative diagnostic scanning(155keV). To use He-188 as a radionuclide compound of angioplasty balloon radiotherapy, we investigated the labelling method and biodistribution of Re-188- DTPA. We postulated that labeled Re-188-DTPA is preferable because it would be excreted via urinary system more easily than other compounds. To label Re-188 with DTI'A, 1ml of 222MBqI(6mCi) of Re-188 was added to DTPA solution(DTPA 20mg, SnC4 2HsO 10mg, pH 3.5) and boiled at 100C for 120min in water bath. pH was adjuted to 5 with 2.3Fo sodium acetate. I.abeling efficiency was measured using TLC-SG(acetone, saline). We evaluated biodistribution of Re-188-DTPA in sacrificed mice at 10 and 60 minutes after injection. We acquired images of kidneys, and drew tirne-activity r.urves in normal dogs and rats and calculated Trnax and Tl/2 in rats. The labelling efficiency was 95.7Yo on average. Labelling of Re-188-DTPA was stable(90% after 5hours) in vitro at room temperature. According to time-activity curves of dogs and rats, it took 15 to 20 minutes after injection for Re-188-DTPA to be washed out through kidneys. In conclusion, Re-188-DTPA was successfully labeled, Re-188-DTPA was stable in vitro and was excreted early via kidneys in animals. We could recornmend Re-188-DTPA as radionuclide of potential use in angioplasty balloon radiotherapy.

7.The Trends of Nursing Research in the Journal of the Korean Academy of Nursing.

This study examined all the research published in Journal of Korean Academy of Nursing (JKAN) for last 30 years, from its beginning year to year 1999, based on an objective frame of evaluation. The purpose of this study is to reflect the trend of nursing research in Korea to date, and to provide an optimal direction for future research. The total number of 959 studies were analyzed with the following results. 1. Since 1990s, the number of non-degree based research has been larger than that of degree-based research compared to 1970s and 1980s. Both collaborative and funded research has been particularly increased in a gradual rate. 2. Research on nurses or nursing students has decreased while research on more diverse populations such as patients, patient families, or community-dwelling healthy clients has increased. 3. While essential nursing concepts such as human, nursing, and health have been consistently dealt as main research topics, research related to the concept of environ- ment has been insufficiently conducted. 4. It was remarkable that the quality of nursing research has been improved due to the acknowledgement of the importance of the empirical research method within the nursing community. 5. Qualitative research has appeared in the journal since the late 1990s, but it is still regarded to be in its novice stage. 6. Although the utilization of research findings for expanding the nursing body of knowledge has been wide in diverse specialties, most studies are limited to focus on concept development or evolution yet. also, in some of these research finding are utilization already considerably as to develop nursing interventions. Based on these findings, conclusions are extended to the following discussions: For the last 30 years, nursing research in Korea has shown an acute methodological development in both quantity and quality. However, there has been a lack of research on theory development although it is one of the ultimate goals in nursing. Further research should be empirical enough to be utilized in an actual nursing care context, and thus to be basis of developing culturally competent nursing theories in Korea.

8.The Effect of the Cancer Genetic Education Program for Nurses.

PURPOSE: Advancing genetic knowledge for oncology nurses is especially important in Korea because physicians have launched to incorporate genetic risk assessment and genetic testing into their practice. The purpose of this paper was to identify the effect of the first academic cancer genetic risk assessment and counseling course for Korean nurses. METHODS: Thirty-five nurses were recruited and educated from June 8 to 14, 2006 in Seoul, Korea. Two measurement tools were used: 'knowledge about the hereditary breast and ovarian cancer (HBOC)' and 'knowledge about the cancer genetics'. RESULTS: Students' score of knowledge about HBOC at pre-education was 12.22+/-2.23, and after education, it increased to 13.62+/-1.76. This change was statistically significant (t=-3.253, p=.003). The score of knowledge about cancer genetics at pre-education was 11.31+/-3.44, and after education it has increased to 16.17+/-1.94. It also was statistically significant (t=-6.92, p=.000). CONCLUSION: This program was effective to be a starting point for establishing genetic educational planning for the oncology nurses in Korea. This academically-based course is recognized as valuable by oncology nurses. With this new knowledge, nurses can begin to expand their role in delivering comprehensive cancer care services in Korea.

10.Comparative Evaluation of Pre-Test Probability Models for Coronary Artery Disease with Assessment of a New Machine Learning-Based Model

Purpose:
This study aimed to validate pivotal pre-test probability (PTP)-coronary artery disease (CAD) models (CAD consortium model and IJC-CAD model).
Materials and Methods:
Traditional PTP models-CAD consortium models: two traditional PTP models were used under the CAD consortium framework, namely CAD1 and CAD2. Machine learning (ML)-based PTP models: two ML-based PTP models were derived from CAD1 and CAD2, and used to enhance predictive capabilities [ML-CAD2 and ML-IJC (IJC-CAD)]. The primary endpoint was obstructive CAD. The performance evaluation of these PTP models was conducted using receiver-operating characteristic analysis.
Results:
The study included 238 participants, among whom 157 individuals (65.9% of the total sample) had CAD. The IJC-CAD model demonstrated the highest performance with an area under the curve (AUC) of 0.860 [95% confidence interval (CI): 0.812– 0.909]. Following this, the ML-CAD2 model exhibited an AUC of 0.814 (95% CI: 0.758–0.870), CAD1 showed an AUC of 0.767 (95% CI: 0.705–0.830), and CAD2 had an AUC of 0.785 (95% CI: 0.726–0.845). Each of the PTP models was adjusted to have a CAD score cutoff that classified cases with a sensitivity of over 95%. The respective cutoff values were as follows: CAD1 and CAD2 >12, MLCAD2 >0.380, and IJC-CAD >0.367. All PTP models achieved a CAD sensitivity of over 95%. Similar to the AUC performance, the accuracy of the PTP models was highest for IJC-CAD, reaching 80.3%. The accuracy of ML-CAD2 was 77.7%, while that for CAD1 and CAD2 was 74.8% and 75.2%, respectively.
Conclusion
ML-CAD2 and IJC-CAD showed superior performance compared to traditional existing models (CAD1 and CAD2)
