1.Hyalinizing Trabecular Carcinoma of the Thyroid Gland: A report of two cases.

We report two cases of hyalinizing trabecular carcinoma (HTC) of the thyroid gland. These two patients were euthyroid women aged 36 and 65 years of age. The tumors were encapsulated and measured 0.8 and 4.0 cm in diameter, respectively. Histologically, the tumors were composed of a compact proliferation of cells in a lobular and trabecular pattern with an intervening hyalinized, fibrotic vascular stroma. Occasionally the cells were arrayed in microfollicles. Multiple and serial sections showed cords of tumor cells invading into the capsule in both cases and vascular invasion in one case. These findings suggested that HTCs are a malignant counterpart of hyalinizing trabecular adenoma, similar to conventional follicular tumor. Positive immunostaining of tumor cells for thyroglobulin and negative staining for high molecular weight cytokeratin, cytokeratin 19, neuron specific enolase, chromogranin, and synaptophysin allowed distinction from medullary carcinoma. Even though HTCs are an heterogeneous group of tumors, the present two cases are probably variants of follicular carcinoma rather than papillary carcinoma.

3.A Clinical Study on Antihypertensive Effects of Aldactazide (Spironolactone+Hydrochlorothiazide).

The antihypertensive effects fo aldactazide and serum electrolytes changes before and after treatment with aldactazide were observed. The followings were obtained: 1. Results of antihypertensive therapy with aldactazide in 23 hypertensive patients revealed good effects in 52.17%, fair in 26.09%, poor in 8.7%, and failure in 13.04% of cases. In 78.26% of cases, good or fair control of blood pressure was obtained. 2. Daily doses of aldactazide ranged from 25mg to 50mg depending upon the level of blood pressure. The antihypertensive effects were appeared after average 11 days of administration. 3. The side effects during treatment with aldactazide were dizziness, weakness, indigestion, headache, and restlessness in 26.28% of cases (6 of 23 cases). 4. No significant changes in serum Na and K values were observed before and after treatment with aldactazide for average 23 days.

4.Clinical Observation on Antihypertensive Effects of Diltiazem Hydrochloride(Herben(R)).

The antihypertensive effects of diltiazem was observed in 30 cases of essential hypertension, and following results were obtained. 1) Mean decrease in systolic and diastolic blood pressure by oral diltiazem was 42.0+/-2.5mmHg and 17.8+/-1.7mmHg. The results of antihypertensive therapy revealed good control in 50% fair control in 30% poor in 17% and failure in 3% of the cases. In 80% of the cases, good or fair control of Hypertension which means drop of diastolic pressure to the level of less than 100mmhg was observed. 2) Mean drop in heart rate was 21+/-2 beats/min. 3) Daily dose was 90-180mg. 4) The side effect of oral Diltiazem was mild headache and dizziness, respectively one case.

5.The Effect of Tumor Removal and Administration of OK432 on the Splenic Natural Killer Cell Activity in the Subcutaneous Tumor Bearing Rats.

To investigate the effect of tumor removal and administration of OK432 on the splenic natural killer (NK) cell activity in the subcutaneous tumor bearing rats, NK cell activity assay using a 4-hour 51Cr release assay and flow cytometric analysis for NK cell population were performed. The results were as follows: 1. Splenic NK cell activity and population in the subcutaneous tumor bearing rats decreased along with the growth of the tumor. 2. The rats with subcutaneous tumor removal showed decrease of splenic NK cell activity, but splenic NK cell population was not decreased. 3. In the rats with subcutaneous tumor removal and OK432 administration, splenic NK cell activity was significantly increased 1 week after administration of OK432 and then gradually returned to normal, whereas increase of NK cell population was not significant. In the present study, splenic NK cell activity was significantly decreased despite removal of subcutaneous tumor. But with the administration of OK432, splenic NK cell activity returned to normal. Considering the role of NK cells on the first line of defense against the metastatic implantation of circulating tumor emboli, we suggest that perioperative administration of immunopotentiator such as OK432 may improve the patient's outcome after surgery of human neoplasm.

6.Treatment of Tibial Nonunion with Subcortical Onlay Bone Graft and Plate Fixation

Tibial nonunion is often associated with the soft tissue atrophy and the local circulatory disturbance, so it is important to minimize the soft tissue injury and maintain the stability at fracture site for bone union. We reviewed 29 cases of subcortical onlay bone grafts and plate fixations for the nonunion and delayed union of tibial shafts, from Jan. 1988 through Jan. 1994. The results were as follows. 1. Twenty-nine patients who had nonunion of the tibia underwent subcortical cancellous onlay bone graft and plating, and 28 fractures(96.6%) went on to union. 2. Subcortical elevation or shingling of cortical bone fragments of the tibial cortex, is advocated to provide sound biological condition in atrophic soft tissue envelope. 3. Narrow DCP encouraged early motion and reliably promoted obtaining anatomical alignment. So, in treatment of protracted tibial nonunion, especially in an area with deficient soft tissue or unyielding scar tissue, subcortical onlay bone graft and plate fixation is very useful method, result in good union and no skin problem.

7.The Diagnostic Value of the Nitroblue Tetrazolium Dye Reduction Test in Orthopaedic Field

Nitroblue tetrazolium dye reduction test is now widely used for early detection of bacterial infection. Practically nitroblue tetrazolium dye reduction test (N.B.T. test) gives certain clue to rule out any possibility of aseptic inflammation. Authors performed N.B.T. test for 80 orthopaedic diseases and 40 controls, and the results were as followings: 1. Forty-one of 60 subjects with bacterial infection had more than 10% formazan positive neutrophils, and 18 of 20 subjects without bacterial infection showed less than 10%. formazan positive neutrophils, The groups of the elevated N.B.T. responses were mainly the subjects who had osteomyelitis, soft tissue inflammation, pyogenic arthritis, and bone and joint tuberculosis. 2. The mean proportion of N.B.T. positive neutrophils was 3.8±2.78% in 40 controls, 4.7±4.23% in 20 nonifectious group 12.7±10.3% in 12 chronic osteomyelitis, 12.5±10.7% in 15 treated bone and joint tuberculosis, 20.9±15.65% in 15 untreated bone and joint tuberculosis and 20±10.70% in 11 pyogenic arthritis and 7 other soft inflammatory group. 3. In noninfectious group, 7 of 8 subjects with elevated ESR and 3 of 4 subjects with elevated body temperature revealed N.B.T. responses less than 10%. 4. In 23 subjects with the elevated N.B.T. responses as well as leukecytosis, 22 subjects were the groups of bacterial infection and rest 1 was noninfectious in nature. 5. Therefore, the N.B.T. test is found to be useful for the purposes of the early diagnostic aid of the bacterial infection and differential diagnosis between bacterial infections and other diseases.

8.Effect of Superoxide Dismutase-Polyethyleneglycol(SOD-PEG) on the Infarct Size with Reperfusion in Rabbit Myocardial Infarction Model.

Many Investigations have evaluated the effect of oxygen free radical scavenger on the infarct size after reperfusion. but those were not consistent in results. Many hypotheses for the discrepancy were proposed but not satisfactory. The present study evaluated the ability of superoxide dismutase-polyethylene glycol(SOD-PEG) to reduce the infarct size,and the effect of cell swelling on the result, and parameters influencing the infarct size. Coronary artery was occluded for 45 minutes and reperfused for 4 hour.In 5 rabbits,SOD-PEG was infused for 15 minutes from 15 minutes before reperfused. In 7 rabbit, 5% dextrose solution was administrated instead. Infarct size was measured by triphenyl terazolium chloride and risk area by monastral blue. Water content was calculated from difference of weights before and after drying in 85degrees C for 24 hours. Blood pressure, heart rate, and double product were analysed. The results are as follows; 1) hemodynamic parameters of blood pressure, heart rate, and double product were not different between two groups. The risk volume (p>0.05) and infarct volume(p<0.05) were smaller in treated group than in control group, and the ratio of risk area to total myocardium was lower in treated group than in cotrol group without statistic significance. 2) The percentage of infarct size in risk area was smaller in treated group than control group(p<0.05) 3) There was no difference in water content of normal, risk, and infarct areas. 4) Blood pressure,heart rate,and double product were not related to the infarct size directly. 5) The ratio infarct size to risk area had good positive relation with the ratio of risk area to total myocardium in total(R=0.84, p<0.001) and control groups(R=0.89, p<0.01). In conclusion, SOD-PEG could not rule out the infarct size and cell swelling didn't infulence area in treated group although the ratio of risk volume to total was not different statisitically between two groups.

9.Effectiveness of Public Health Center Smoking Cessation Counseling Program using the Transtheoretical Model.

PURPOSE: The purpose of this study was to identify the effectiveness of public health center smoking cessation counseling program using the transtheoretical model on the process of change, smoking temptation, decisional balance and stage change transition. METHODS: A nonequivalent control group non-synchronized design was used for this study. The study population was 115 adult smokers. The counseling program was administered to the experimental group and a smoking cessation program from the Korean Ministry of Health & Welfare was administered to the control group. Descriptive analysis, chi-square-test, t-test, and ANCOVA were used with the SPSS 12.0 program to analyze the data. RESULTS: After treatment with this program, the experimental group showed significantly higher scores for process of change (t=4.148, p<.001), smoking temptation (t=-2.988, p=.003), and stage change transition (chi-squre=5.871, p=.031) compared to the control group. Experimental group members also showed significantly lower score for Pros of smoking (t=-3.151, p=.002). CONCLUSION: The findings indicate that this program could have positive effect on process of change, smoking temptation, decisional balance and stage transition for adult smokers. Based on these findings, the authors suggest additional counseling program focusing on smokers in specific stages.

10.Analysis of Service Occupation Types and Training Programs of Services for the Aged in Busan.

PURPOSE: The purpose of this study was to analyze service occupation types and to develop training programs for the aged. METHODS: This study used descriptive study design. The research process consisted of three stages: first, identified the demand of service manpower for the aged; second, investigated present jobs and education programs in Korea and Japan; and last, developed service jobs and education programs for the aged. RESULTS: Potential users considered "health management" to be the most important area. They thought "providing job" as second most important. According to the result of analyzing Korean policies, there were 9 service occupations in 5 domains. So, we derived 10 occupations such as 'daily living manager', 'care manager', 'care worker', 'health manager', 'education specialist', 'leisure manager', 'good manager', 'housing manager', 'financial specialist' and 'retirement consultant' in 5 domains as healthcare, leisure, goods, housing and finance. Finally, we developed their tailored training programs. CONCLUSION: According to this study, there should be various occupations qualified by the government, and training programs should be settled. And healthcare providers must included in developing standardized training programs.
