1.Retinal Pigment Epithelial Detachment in Posterior Scleritis.

Posterior scleritis must be one of the most underdiagnosed treatable conditions in ophthalmology, partly because its manifestations are so protean and partly because the diagnosis is rarely considered. Although ultrasonography and computer tomogram are ancilliary tests, a careful examination of the posterior segment of the eye including the area of the ora serrata, macula and disc is essential to discover the presence of a posterior scleritis. We experienced prolonged retinal pigment epithelial detachments which had appered at early phase of posterior scleritis.

3.Studies on the Systolic Time Intervals in the Patients with COPD.

In an attempt to clarify the relationshop between left ventricular function and chronic obstructive pulmonary disease(COPD), systolic time intervals were measured in 79 control subjects and 60 patients with COPD who had no evidence of organic heart disease. The patients with COPD were divided into three groups based on the percent predicted forced expired volume in one second. Significant differences in pre-ejection period index(PEPI) and left ventricular ejection time index(LVETI) existed between the control subjects and the patients with the most severe lung disease(Group III). A highly significant difference in PEP/LVET existed between these two groups. The abnormalities of systolic time intervals demonstrated in these patients are characteristic of left ventricular dysfunction and indicate that left ventricular dysfunction is frequently present in patients with moderate obstructive lung disease.

4.Ultrasonography in the Diagnosis of Appendicitis: Evaluation by Meta-analysis.

OBJECTIVE: We wanted to review the usefulness of ultrasonography (US) for the diagnosis of appendicitis and to evaluate the diagnostic accuracy of US according to patients' and researchers' characteristics. MATERIALS AND METHODS: The relevant Korean articles published between 1985 and 2003 were included in this study if the patients had clinical symptoms of acute appendicitis. The histopathologic findings were the reference standard and the data were presented for 2x2 tables. Articles were excluded if patients had no sonographic signs of appendicitis according to graded-compression US. Two reviewers independently extracted the data on study characteristics. The Hasselblad method was used to obtain the combined estimates of sensitivity and specificity for the performance of US. RESULTS: Twenty-two articles (2, 643 patients) fulfilled all inclusion criteria. The estimate of d calculated by combining the sensitivity and specificity was 2.0054 (95% confidence interval [CI]: 1.8553, 2.1554) by a random effects model. The overall sensitivity and specificity (95% CI) were 86.7% (85.4 to 88.0), and 90.0% (88.9 to 91.2), respectively. According to the subgroup meta-analysis by patients' characteristics, the d estimate (95% CI) of dominantly younger age, male, and highly clinical suggestive group for US was 2.2388 (1.8758 to 2.6019), 2.7131 (2.2493 to 3.1770), and 2.4582 (1.7387 to 3.1777), respectively. Also, according to subgroup meta-analysis by researchers' characteristics, the d value (95% CI) for US done by diagnostic radiologists and gray-scale was 2.0195 (1.7942 to 2.2447) and 2.2630 (1.8444 to 2.6815). CONCLUSION: This evidence suggests that US may be useful for the diagnosis of acute appendicitis, especially when patients are younger age, male, and highly clinical suggestive.

PURPOSE: The purpose of this preliminary study was to develop and evaluate the effects of East-West Self-help program for Rehabilitation of post-stroke clients. METHOD: This program is developed through literature review, survey and seminar. The program is consisted of six sessions twice a week for 6 weeks. This program is composed of health education on stroke, exercise, oriental nursing interventions, and therapeutic recreation. The outcomes have been evaluated on the basis of perceived health status, self-efficacy, U/E function, ADLs and subjective response. RESULTS: 1) After the 6 week program, the perceived health status had improved and the score of rehabilitation self-efficacy increased significantly. 2) After the 6 week program, the score of BADL increased significantly, but, the score of IADL was not statistically significant. 3) After the 6 week program, the amount of use and quality of movement of the affected U/E were increased significantly. 4) All of the participants showed satisfaction with this program. CONCLUSION: Considering these research results, the program is effective in improving functional abilities and self-management ability. Therefore this program could be continuously developed and implemented as a community based self-helf group program for post-stroke clients.

PURPOSE: The purpose of this study was to evaluate the effect of an East-West Self-help program for Rehabilitation of post-stroke patients. METHOD: A quasi-experimental design was used. The subjects were 75 post stroke clients(Exp. group : 38, Cont. group:37). The subjects of the experimental group participated in the Self-help group program of six sessions, twice a week, during 6 weeks. The program consisted of health education of stroke, exercise, oriental nursing interventions, and therapeutic recreation. The obtained data were analyzed by using the repeated measure ANOVA of SPSS. RESULT: 1) The score of rehabilitation self-efficacy increased significantly in the experimental group as compared to the control group. 2) The score of BADL, IADL, amount of use & quality of movement of the affected U/E, and grip power increased significantly in the experimental group as compared to the control group. 3)The level of blood cholesterol decreased significantly in the experimental group as compared to the control group. CONCLUSION: Considering these research results, the program is effective in improving functional abilities and self-management ability. Therefore this program could be implemented as a community based self-help group program for post stroke clients.

8.Prognostic Effect of Vascular Endothelial Growth Factor and Angiogenesis in Gastric Carcinoma.

PURPOSE: Many studies have shown that angiogenesis has an important role in the growth, progression, and metastasis of solid tumors. Recently, several angiogenic factors have been identified. Vascular endothelial growth factor (VEGF) is a well characterized inducer of angiogenesis. In this study, we investigated the prognostic significance of the expression of VEGF in patients with an advanced gastric carcinoma. MATERIALS AND METHODS: Specimens from 54 gastric adenocarcinoma patients were stained using a polyclonal antibody against VEGF. Correlations of the expression of VEGF, microvessel density, and various other clinicopathological factors were analysed. RESULTS: Seventeen (31.5%) and 37 cases (68.5%) were VEGF-negative and positive, respectively. There was significant correlation between the expression of VEGF and pathological differentiation. There were no significant correlations between the expression of VEGF, stage and recurrence of a gastric carcinoma. The microvessel density was significantly higher in the VEGF-positive than the VEGF-negative tumors. Survivals of the VEGF-negative patients were significantly prolonged compared to those of the VEGF-positive patients. CONCLUSION: The results of this study show that the expression of VEGF may be a useful prognostic factor for patients with a gastric adenocarcinoma.

9.Clinical analysis of 3rd Decade Male 47 Cases with Intracranial Tumors in Korea.

The authors represented a clinical analysis of 3 rd decade male 47 patients with intracranial tumors who had been histologically confirmed after operation and biopsy at the department of neurosurgery of Capital Armed Forces General Hospital From Feb. 1985 to Jan, 1988. We classified the intracranial tumors according to Russell and Rubinstein's classification. The results were as follows: 1) Among the intracranial tumors, gliomas were found most frequently(40.4%), and followed by pituitary adenomas(19.2%), pinealomas(10.6%), osteomas(6.4%), medulloblastomas(4.3%), craniopharyngiomas(4.3%), blood vessel tumors(4.3%). Pituitary adenomas occupied 19.2% of all intracranial tumors and as a single entity these were the highest incidence. 2) Tumors of the pineal region constitutes 10.6% of intracranial tumors. It was relatively high incidence compared with other reports. 3) Intracranial tumors occurred more frequently in supratentorial region(85.1%). The most frequent location was sellar and parasellare region(23.4%), and followed by frontal(14.9%), parietal(12.7%), pineal gland(10.6%), temporal(8.5%), cerebellar region(8.5%) in order. 4) The most common duration of symptoms were within 3 months(42.6%). The main clinical symptoms and signs were headache(80.9%), nausea or/and vomiting(55.3%), papilledema(44.7%), so called symptoms triad of the brain tumor, and other ophthalmic symptoms, gait disturbance, motor dysfunction,cerebellar sign in order.

10.Cytopathology of Hurthle Cell Adenoma: A Cese Report by Fine Needle Aspiration.

The increased use of thyroid fine needle aspiration (FNA) has refocused on Hurthle cell lesions. The cytologic diagnosis of Hurthle cell tumor is a challenge due to the presence of Hurthle cells in non-neoplastic lesions and the inability to differentiate between benign and malignant Hurthle cell tumor. We report a case of Hurthle cell adenoma(HCA) in a 68-year old woman, with reivew of the cytopathologic findings. FNA revealed losely cohesive or sheets of sheets of large oval to polygonal Hurthle cells containing bundant granular cytoplasm. The histopathologic examination confirmed the diagnosis of HCA with follicular growth pattern, Ultrastructurally, the cytoplasm was packed with variable sized mitochondria.
