1.Factors Affecting the Thickness of Suction Ring in LASIK.

PURPOSE: Suction ring is needed to fix the cornea during LASIK procedure. But if we choose the suction ring inappropriately, we meet the various problems such as conjunctival edema, conjunctival hemorrhage, ocular pain, and prolongation of operation time. To investigates various factors affecting the thickness of suction ring(1-4) in LASIK procedure. METHODS: We performed LASIK procedure by using SCMD microkeratome and VISX STAR laser on 398 eyes from Dec. 1996 to Feb. 1999. We compared difference on pachymetry, keratometry, corneal diameter, spherical equivalent according to the thickness of suction ring and analyzed these factors with One-Way ANOVA test. RESULTS: The results are that keratometry affected size of suction ring significantly. The flap size and hinge size are affected by size of suction ring significantly, but others did not show significant results. CONCLUSIONS: These results suggest that we need to choose the size of suction ring according to keratometry during LASIK procedure for decreasing various complications.

2.A Case of Myoepithelial Carcinoma Arising in the Lacrimal Gland.

The vast majority of epithelial neoplasms of the lacrimal gland are pleomorphic adenomas, which comprise slightly more than 50%of epithelial tumors of this anatomic location.Other types of primary epithelial neoplasm of the lacrimal gland include adenoid cystic carcinoma, adenocarcinoma, mucoepidermoid carcinoma, and malignant mixoid tumors. Myoepitheliomas are rare tumors, most frequently reported in the salivary glands. A 61-year-old woman had painless proptosis of the right eye for 5 months. The orbit CT scan showed a lacrimal tumor involving adjacent frontoparietal bone.Histopathologically it was proved as myoepithelial carcinoma of the lacrimal gland.The patient underwent a right orbital exenteration and radiation therapy.We report this unusual case with literature review.

3.A Case of Progressive Recurrent Symblephron, Treated with Amniotic Membrane and Autograft Limbal-conjunctiva.

Although both medical and surgical approaches have been investigated, recurrent symblepharon due to chronic ocular surface inflammation is a difficult disease to manage.This condition can also result in many complications such as cosmetic and visual deteriorations.In this condition, we can use the amniotic membrane transplantation that has a biological contact inhibition effect for the prevention of recurrent symblepharon. We treated successfully a case of a progressive symblepharon involving central cornea from 2-year old. The 14 year-old male patient had suffered from serious cosmetic problem and undergone multiple dissection, argon laser photocoagulation, using cyclosporin eyedrops. He was treated using allograft amniotic membrane, autograft limbal-conjunctiva, and buccal mucosa transplantation. we think that amniotic membrane transplantation was very effective method for the prevention of recurrent symblepharon.

4.Effect of Visible Light Therapy in Stroke Patients.

OBJECTIVE: The aims of this study was to evaluate the effects of visible light therapy for the stroke patients. METHOD: Thirty stroke patients were enrolled and classified into two groups, experimental (n=19) and control group (n=11). Control group received conventional rehabilitation therapy. Experimental group received additional light therapy with red light (intensity 962 lux, wave length 620 nm) on wrist, elbow, and shoulder joint area (diameter 10 cm) at the same time, 3 times per day, 20 minutes per session, 5 days per week, for 2 weeks. All patients were assessed their paralytic upper extremity functions using manual muscle testing, two point discremination test (2-PD), Semmes-Weinstein monofilament test, and modified Ash-worth scale. To evaluate neurophysiological effects of light therapy, sympathetic skin response, thermography, and doppler ultrasound on radial artery were performed. RESULTS: Compared to the control group, the experimental group significantly improved in the 2-PD test, Semmes-Weistein monofilament test, and the hand grip power. Neurophysiologic parameters showed no significant difference between experimental and control groups. CONCLUSION: Visible light therapy maybe used as an effective therapeutic modality for improving hand function of stroke patients. Further studies are required to define the mechanism of effects on paralytic extremity of visible light.

5.Effect of Visible Light Therapy in Patients with Somatic Pain.

OBJECTIVE: The purpose of this study is to evaluate the effect of visible light therapy for the management of somatic pain. METHOD: Subjects consisted of 42 patients with pain and were divided into two groups; control (n=22) and experimental (n=20) groups. Control group received conventional physical therapy only, while experimental group received additional light therapy with blue light (light intensity 4080 lux, wave length 581 nm, distance from lamp 5 cm). Intensity of pain was assessed by visual analogue scale (VAS) and McGill pain questionnaire. Sympathetic skin response was measured to assess the status of autonomic nervous system. VAS and McGill pain questionnaire were administered before treatment and at 1 day, 2 days, 3 days, 1 week, and 2 weeks after treatment. Sympathetic skin response were performed before and 2 weeks aftertreatment. RESULTS: 1) In both experimental and control groups, VAS became significantly lower at two weeks after treatment compared to pretreatment scale (p<0.05). 2) McGill pain questionnaire showed significantly lower scores two weeks after treatment compared to pretreatment score, only in experimental group (p<0.05). 3) Experimental group showed significantly lower McGill pain questionnaire score than control group at two weeks after treatment (p<0.05). 4) Latency and amplitude of sympathetic skin response showed no significant difference between experimental and control groups. CONCLUSION: Visible light therapy can be used as an effective therapeutic modality for the management of symptomatic pain in combination with conventional physical therapy.

7.Geographic Distribution of Urologists in Korea, 2007 to 2012.

The adequacy of the urologist work force in Korea has never been investigated. This study investigated the geographic distribution of urologists in Korea. County level data from the National Health Insurance Service and National Statistical Office was analyzed in this ecological study. Urologist density was defined by the number of urologists per 100,000 individuals. National patterns of urologist density were mapped graphically at the county level using GIS software. To control the time sequence, regression analysis with fitted line plot was conducted. The difference of distribution of urologist density was analyzed by ANCOVA. Urologists density showed an uneven distribution according to county characteristics (metropolitan cities vs. nonmetropolitan cities vs. rural areas; mean square=102.329, P<0.001) and also according to year (mean square=9.747, P=0.048). Regression analysis between metropolitan and non-metropolitan cities showed significant difference in the change of urologists per year (P=0.019). Metropolitan cities vs. rural areas and non-metropolitan cities vs. rural areas showed no differences. Among the factors, the presence of training hospitals was the affecting factor for the uneven distribution of urologist density (P<0.001).Uneven distribution of urologists in Korea likely originated from the relatively low urologist density in rural areas. However, considering the time sequencing data from 2007 to 2012, there was a difference between the increase of urologist density in metropolitan and non-metropolitan cities.

8.A Case of Single Mass Forming Hepatic Tuberculoma.

Tuberculosis mainly develops in the lung, but may also rarely invade other parts of the abdominal region. Abdominal tuberculosis is associated with pulmonary tuberculosis in approximately 15% of cases, and abdominal tuberculosis primarily develops in the terminal ileum and lymphatic gland. Moreover, hepatic tuberculosis is uncommon and is usually accompanied with active pulmonary or miliary tuberculosis. Hence, the development of primary hepatic tuberculoma as a single liver mass is very unusual. In the present report, we describe the case of a 63-year-old man with a solitary hepatic tuberculoma; the 6.4 cm mass was incidentally detected during abdominal computed tomography in the asymptomatic patient, and the diagnosis was confirmed by liver biopsy through ultrasonography-guided fine-needle aspiration.

9.Encephalopathy Associated with Autoimmune Thyroid Disease Accompanied by Graves' Disease.

A patient with encephalopathy associated with autoimmune thyroid disease (EAATD), which is one of the most important differential diagnoses of treatable dementia, presents with various neurological symptoms, such as repetitive epileptic seizures, altered mental status, and cognitive dysfunction. Steroid treatment is effective for EAATD. The incidence of EAATD increases considerably with age, particularly in female patients. Most patients with EAATD have normal thyroid function test results or mild hypothyroidism. Patients with EAATD with Graves' disease are very rarely reported. Here, we report a case of a 63-year-old woman who complained of declining cognitive ability and ataxia. She was diagnosed with EAATD accompanied by Graves' disease. Her neurological symptoms improved after intravenous steroid administration.

10.Symptomatic epidural gas cyst treated with epidural block and percutaneous needle aspiration: A case report.

A 68-year-old woman suffered from lower back and radiating pain on her right buttock and posterior calf. Axial magnetic resonance imaging showed a 7 x 7 mm nodular lesion (T1 and, T2 low signal intensity) at the epidural space between the L5-S1 level and computed tomography revealed it was an epidural gas cyst. The authors performed an epidural block and percutaneous needle aspiration of the epidural gas cyst. The patient showed almost complete resolution of symptoms one year later. The authors suggest that an epidural nerve block with needle aspiration of a gas cyst could be an alternative treatment option for patients with a symptomatic epidural gas cyst before surgery.
