1.A Study on Knowledge and Attitudes Regarding Sexuality of Elderly People in Korea.

PURPOSE: The purpose of this study was to examine the level of knowledge and the attitudes of Korean adults toward sexuality of elderly people. METHODS: This was a descriptive study using a convenience sample of 241 adults. The questionnaire had 34 items for knowledge and 26 items for attitudes. RESULTS: The result showed that, related to sexuality of elderly people, the level of knowledge was average and attitudes were conservative. The level of knowledge was significantly higher in groups that were highly educated and of low economic status (p<.05). Attitudes toward sexuality of elderly people were significantly more conservative in female, older, less educated, and high economic status group. CONCLUSION: The findings suggest a need for more education for Korean adults regarding sexuality in elderly people.

3.Effects of a Discharge Education Program using Computerized Animation Video for Post-operative Colon Cancer Patients.

PURPOSE: This study was to identify the effects of a nurse-led education program using computerized animation video for post-operative colon cancer patients. METHODS: a total of 163 patients and 51 nurses were participated in this study. With a non-equivalent control group post-test design, patients were divided into three groups (77 got traditional education, 46 were applying brochure, 40 were watching video). Twelve-item animation video and brochure about the management after discharge for post-operative colon cancer patients were developed based on patient survey and the items of Korea Healthcare Accreditation. RESULTS: The computerized video watching group had better satisfaction than the others, but there was no significant difference about comprehension. When video was applied, satisfaction, usefulness, application, and perceived patients' comprehension of nurses were all increased. CONCLUSION: This video education program was developed by nurses and it had a special thing for patient to access the same program even after discharge using the authorization system. It would be helpful for nurses to be more concentrated on the direct care for hospitalized patients as well as for patients to provide self-care at home. This program would be adjusted into more various diseases and settings.

4.Basal cell adenocarcinoma of the minor salivary gland: a case report.

Basal cell adenocarcinoma is an epithelial neoplasm which is cytologically and histomorphologically similar to basal cell adenoma but is different because of the infilitrative growth. This tumor, a rare salivary gland tumor newly classified as basal cell adenocarcinoma by the WHO in 1991, is infiltrative, locally destructive and tends to recur but metastasis is less common. The differential diagnosis includes basal cell adenoma, adenoid cystic carcinoma, and basaloid squamous carcinoma. Nearly 90 percent of these tumors occurr in the parotid gland and can be classified into low grade carcinomas with a relative good prognosis. Basal cell adenocarcinoma of minor salivary gland is very rare and has a less favorable clinical course compared with that of the major salivary glands. This is a case of basal cell adenocarcinoma occurring at the minor salivary gland of the soft palate. We treated this patient with block excision and adjunctive radiation therapy.

5.Two imported cases of cutaneous larva migrans.

Cutaneous larva migrans (CLM) is a rare serpiginous cutaneous eruption caused by accidental penetration and migration in the skin with infective larvae of nematode that normally do not have the human as their host. Although CLM has a worldwide distribution, the infection is most frequent in warmer climates. More recently, they have been increasingly imported from the tropics or subtropics by travelers. We experienced two patients who had pruritic serpiginous linear eruption in their skin for a few weeks after traveling to the endemic areas (Brazil and Thailand, respectively). After the treatment with albendazole, the skin lesions resolved with post-inflammatory hyperpigmentation. We report herein two cases of cutaneous larva migrans successfully treated with albendazole.

6.Relationship Between Vascularity and Other Remodeling Parameters in Asthmatic Airway.

BACKGROUND: The pathological features in asthmatic airway remodeling are diverse. The aim of this study was to examine the degree of airway vascularity in relation to the other remodeling parameters in asthmatics. METHODS: Bronchial biopsies were done in 34 asthmatic patients, and 6 control subjects. The basement membrane thickness and the subepithelial thickness were measured in the hematoxylin-eosin stained tissue, and the degree of vascularity was measured using type IV collagen immunostaining. RESULTS: 1) Compared to the control subjects, the asthmatics showed a significant increase in the basement membrane thickness (6.92+/-2.01micrometer vs 9.67+/-2.84micrometer, p<0.05) and the subepithelial thickness (44.49 +/- 31.92micrometer vs 121.22+/-72.79micrometer, p<0.05). 2) Compared to the control subjects, the asthmatics showed a significant increase in the vascular area per unit submucosal area (4.51+/-2.13% vs 10.32+/-6.08%, p<0.05). In addition, the number of vessels per unit submucosal area showed an increased tendency without statistical significance. 3) In the asthmatics, the number of vessels and the vascular area per unit submucosal area showed no correlation with the basement membrane thickness, the subepithelial thickness, the severity, the forced expiratory volume in 1 second(FEV1), and the methacholine provocative concentration 20(PC20). CONCLUSION: This study showed that vascularity was an important parameter in asthmatic airway remodeling but it was not related to the other remodeling parameters such as the basement membrane thickness and the subepithelial thickness. Each of these asthmatic remodeling parameters may have a different clinical significance. Therefore, further studies will be needed.

A new surgical approach to the area of the infratemporal fossa and parapharyngeal space is described. This approach results in a wide-field exposure of the infratemporal fossa, pterygomaxillary space and parapharyngeal space. We used two osteotomies on the patient's mandible and temporary resection of zygomatic arch for superior margin of tumor. Lower lip splitting was not needed because the incision was started in the frontal scalp, curved in front of and below the external auditary canal, and extended anteriorly to the greater horn of hyoid bone on the neck along a skin crease. We had good results without sacrifice of the facial nerve, mandibular function and sensory supply of the face and oral cavity.

8.Providencia rettgeri Prostatic Abscess Cured by Medical Therapy.

A prostatic abscess is a rare, but potentially serious disease. The mainstay of treatment for the prostatic abscess is antibiotic administration and drainage. Here, we experienced a 66-year-old man with a prostatic abscess caused by Providencia rettgeri, which has not been reported as a pathogenic agent of a prostatic abscess. He was cured using antibiotics, without surgical drainage. This case suggests that the appropriate selection of patients for antibiotic therapy may provide an excellent prognosis.

9.A bleeding by colonic invasion of chest wall metastasis of hepatocellular carcinoma after RFA.

Radiofrequency ablation (RFA) has been widely used for treating localized hepatocellular carcinoma (HCC). Metastasis to the chest wall through the tract during performing RFA may occur uncommonly. However, it is extremely rare that massive hematochezia occurs by colonic invasion of metastatic focus on chest wall. A 48-year-old man was presented with approximately 500 cc of hematochezia. He was diagnosed as having HCC 6 years ago, and underwent RFA 3 times over a period of 5 years. Twelve months ago, right lobectomy was done on recurred lesion. 5 months ago, HCC recurred through invasion to the chest wall involving the right 5th rib, but there have been no recurrent tumor masses on the liver on CT until admission. Emergent colonoscopy demonstrated a 3 x 3 cm sized extrinsic mass on proximal transverse colon that had a blood clot. Pathology from colonic mass revealed pseudoglandular type of HCC.

10.A Case of Acute Suppurative Thyroiditis Complicated by Hypocalcemia.

We report a case of acute suppurative thyroiditis complicated by idiopathic hypoparathyroidism. The patient was a 49-year-old female who visited our clinic with a painful goiter on the left lobe of the thyroid gland. She was hypocalcemic, which was accompanied unusually by acute suppurative thyroiditis. She also suffered from tetany of both hands and legs. She had undergone a right subtotal thyroidectomy at another hospital 23 years previously due to a benign thyroid nodule. Intact parathyroid hormone (PTH) levels were increased, but other laboratory findings were normal. No special treatment was administered for the acute suppurative thyroiditis, except antibiotics, to observe its natural course. Her PTH levels decreased and serum calcium returned to normal. The complication of acute suppurative thyroiditis suggested the possible involvement of idiopathic hypoparathyroidism.
