2.A Clinical Observation on Tumors of the Genitourinary Tract.

A clinical observation was made on 282 patients with tumors of the genitourinary tract during the period from Jan. 1978 to Mar. 1982. The results were as follows: 1. 282 of the 1457 inpatients (19.35%) were the patients with tumor of the genitourinary tract. Men with thetumor were predominated (male to female: 17.8:1). 2. Prostate was the most commonly involved organ (66.8%) and the bladder. kidney, ureter, penis were followed. 3. Age was ranged from 8 months to 84 years, showing the highest incidence in the 8th decade (39.3%) andover 50 years old were 89.3%. 4. Urinary retention was the most common symptom (34.7%) and dysuria, gross hematuria, palpable mass were followed. 5. On histologic examination, begin prostatic hyperplasia was the most common (66.9%) and transitional cell carcinoma of the bladder, adenocarcinoma of the prostate, renal cell carcinoma. squamous cell carcinoma ofthe penis were followed. 6. 223 of the 282 patients with tumor (79.1%) were performed various type of operation. Transurethral surgerywas the most commonly performed (46.2%).

8.Development of Biosignal Telemonitoring System Based on HL7 and MFER Standard.

OBJECTIVE: We developed a biosignal telemonitoring system which is based on HL7 and MFER standard. METHODS: For the communication of waveform data (ECG, EEG etc.) we adopted MFER(Medical waveform description Format Encoding Rules) standard and HL7(Health Level 7). MFER is a standard for encoding waveform biosignal such as ECG, EEG and so on. HL7 is a standard for electronic data communication between two different information systems. RESULTS: The telemornitoring system consists of an HL7 interface gateway and a central repository. The HL7 interface gateway has three modules of an MFER analyzer, an HL7 message generator, and a file sender. The central repository is a central database combined with an HL7 archiver. CONCLUSION: Through this study we might conclude that the proposed system can be a promising model for telemonitoring system in healthcare environment.

9.Cognitive Dysfunction in non-hypoxemic COPD Patients.

BACKGROUND: The cognitive function is impaired in patients with hypoxemic chronic obstructive pulmonary disease (COPD). However, there are conflicting results regarding the cognitive function in patients with non-hypoxemic COPD. COPD patients also have sleep disorders. This study examined the cognitive function in non-hypoxemic COPD patients, and nocturnal sleep was assessed in COPD patients with a cognitive dysfunction. METHODS: Twenty-eight COPD patients (mean age, 70.7 years) with an oxygen saturation > 90%, and 33 healthy control subjects (mean age, 69.5 years) who had visited for a routine check-up were selected. The neurocognitive tests were performed using the Korean version of the Consortium to Establish a Registry for Alzheimer's Disease (CERAD-K) Neuropsychological Battery. RESULTS: The scores of the word list recall test (p=0.03) and the word list recognition test (p=0.006) in the COPD group were significantly lower than those in the control group. Nine patients showed a significantly impaired cognitive function. Seven of these underwent polysomnography, which revealed apnea-hypopnea indices > or = five per hour in five patients. The median oxygen desaturation index and median limb movement index were 3.6/h and 38.6/h, respectively. CONCLUSION: These results suggest that the verbal memory function is impaired in non-hypoxemic COPD patients. Six out of seven COPD patients with an impaired cognitive function had sleep disorders of sleep apnea and/or periodic limb movements during sleep.

10.Pneumothorax during Retroperitoneal Laparoscopic Nephrectomy: A case report.

Recently, laparoscopic techniques are being increasingly used for retroperitoneal surgery. These procedures are considered relatively safe and non-invasive, however, there exists a small but important risk of developing complications related to insufflation with carbon dioxide (CO2). A 51-year-old female patient was admitted for retroperitoneal laparoscopic nephrectomy under general anesthesia. About 60 minutes after carbon dioxide pneumoperitoneum, the patient's oxygen saturation decreased. A chest x-ray was taken in the operating room, which showed pneumothorax in the right lung. We concluded that anesthesiologists should be aware of the occurrence of pneumothorax during the retroperitoneal laparoscopic procedure careful monitoring and appropriate management are needed.
