8.The change of exercise according to time and its predicting factors in the followed - up hypertensive patients of family physicians.

BACKGROUND: Hypertension is one of the common diseases which family physicians encounter to manage. Various research proved that appropriate aerobic exercise had effect on lowering blood pressure efficiently. This study was to find out the status of exercise, the change of exercise according to time and its predicting factors in hypertensive patients. METHODS: A total of 193 of the followed-up hypertensive patients of family physicians in Seoul and kyoungkido from May 11, 2001 to June 10, 2001 were interviewed by doctor according to the previously designed structured questionnaire. RESULTS: Eighty two patients (42.57) were male and mean and was 57.4 +/- 9.9 fears. Using Kaplan-Meier's method, 8% of non-exercise group (N= 115) in diagnosing hypertension started exercise after 1 fear and 40%, after 5 years. Otherwise, 5% of exercise group(N=78) stopped exercise after 1 year and 30%, after 5 years. The characteristics of 5 exercise groups according to sex, age, level of education and adequacy of exercise showed significant difference. In average blood pressure, lately started group showed the lowest (137/84 mmHg) and non-exercise group, the highest (146/91 mmHg) In the degree of recommendation. sporadic group showed the highest and non-exercise group, the lowest which showed significant difference (p =0.0024) . CONCLUSION: I conclude that lately started group among non-exercise group and continuous exercise group were mainly affected by recommendation to exercise of physician. As well as exercise, other nonpharmacologic treatment should be promoted by physician as an important strategy for treatment of hypertension.

9.A Case of Severe Pituitary Dwarfism due to Agenesis of Anterior Pituitary Gland with Pituitary Stalk Transection.

We experienced one case of severe pituitary dwarfism in a 10 years old female girl. Magnetic resonance image (MRI) revealed transection of the pituitary stalk stalk with the formation of high intensity ectopic posterior lobe located at the median eminence and agenesis of an anterior lobe of pituitary gland. The serum growth Hormone (GH) response to clonidine and L-dopa revealed severe GH deficiency. The patient had responses to TRH, normal TSH and partial prolactin response, respectively. There was not response LH and FSH to GnRH. The morning cortisol concentration and serum T4 concentration were decreased below the normal range. These findings and no hyperprolactinemia suggested the presence of a vascular connection between the pituitary gland and hypothalamus, which is not visible on MRI. Sofar, the primary cause of idiopathic pituitary dwarfism in many patients is injury to hypothalamus by perinatal insults. In this patient, there was no history of perinatal insults and postnatal head trauma but transection of the pituitary stalk. We report a case of severe pituitary dwarfism due to agenesis with brief review of related litereature.

10.The Comparison of the Effects of Propofol and Etomidate on Seizure Duration and Hemodynamic Responses during ECT.

BACKGROUND: The clinical efficacy of electroconvulsive therapy (ECT) primarily depends on the adequacy of the seizure duration, but the intravenous anesthetics which are commonly used for ECT may possess anticonvulsant properties and shorten the seizure duration. The aim of this study was to compare the effects of propofol and etomidate on seizure duration and hemodynamic responses during ECT. METHODS: 30 patients undergoing maintenance ECTs were evaluated and divided into two groups randomly. Hypnosis was induced with a bolus injection of either 1.5 mg/kg of propofol or 0.3 mg/kg of etomidate in each group. Time to unconsciousness, seizure duration, heart rate, mean arterial pressure and recovery time were measured after delivery of electrical stimulus. The dynamic energy (joules) delivered was recorded. Correlation between seizure duration and recovery time was calculated and the rates of seizure induction failure after first electrical stimulus were compared. RESULTS: The seizure duration was shorter in the propofol group (34.0 +/- 3.8 s) than in the etomidate group (50.0 +/- 4.0 s)(P < 0.01). The heart rate was significantly lower in the propofol group (132.1 +/- 3.8, 99.7 +/- 6.2 bpm) than in the etomidate group (146.0 +/- 4.2, 119.8 +/- 7.5 bpm) at the time of ECT and 1 min after ECT respectively (P < 0.05). The mean arterial pressure was significantly lower in the propofol group than in the etomidate group from the time of ECT to 10 min after ECT (P < 0.05). CONCLUSIONS: Propofol showed excellent hemodynamic stability and was a good hypnotic for ECT therapy, but etomidate might be a useful alternative to propofol in patients who have an inadequate seizure duration.
