3.Immunohistochemical study of p21 and p53 expression in ameloblastoma.

The p53 protein was discovered in 1979 as cellular 53-kD nuclear phosphoprotein bound to the large transforming antigen of SV40 virus. P21WAF1/CIP1, which has been described as the critical downstream mediator of p53, is known to suppress DNA replication and arrest the G1 cell cycle by quaternary complex with cyclin D, cyclin-dependent kinase(CDK) and proliferating cell nuclear antigen(PCNA). In these days, some studies shows that the p21 can be induced by independent pathways. There are various reports about the expression of p21 (67%.82.4%) in oral squamous cell carcinoma. But these studies are mostly done in malignant tumor not in benign tumor. So we decided to study the expression of p21 in ameloblastoma and the relationship between p53 and p21 as a downstream mediator of p53 in ameloblastoma. We investigated the expression of p21 and p53 with the method of immunohistochemistry. We selected 30 cases of ameloblastoma tissue blocks (acanthomatous type: 5 cases, follicular type: 8 cases, plexiform type: 17 cases) imbedded in paraffin. We used 30 cases of normal gingival tissues and 30 cases of squamous cell carcinoma tissues (SCC) respectively and compared their results with those of ameloblastoma. We made slides with the streptavidin-biotin methods and used monoclonal antibody DO-7 (Novocastra, Newcastle, United Kingdom) as p53 antibody and monoclonal antibody M7202 (DAKO, California, U.S.A.) as p21 antibody. We used Pearson's correlation coefficient to analyse the relationship. The results were as follows: 1. p21 was expressed in ameloblastoma about 30% and this is lower than that of normal gingiva and SCC. 2. In normal gingiva and ameloblastoma, p21 expression was correlated with p53 expression. 3. In SCC, p21 were expressed about 83.3% and this is more than that of p53. But there was no correlation between p21 and p53 expression. We confirmed p21 expression and relation with p53 in ameloblastoma. But, to confirm the function of p21, more studies about p21 expression in malignant ameloblastoma and ameloblastic carcinoma are needed.

4.ANTI-TUMOR EFFECT OF TAXOL AND CISPLATIN IN ORAL SQUAMOUS CELL CARCINOMA AND OSTEOSARCOMA CELL LINES.

OBJECT: Docetaxel(Taxol), which has been used for breast cancer and ovarian cancer and cisplatin were applied to KB and HOS cell lines. Relative cytotoxicity of each drug to cell lines and its efficiency was evaluated and analyzed for clinical application. METHOD AND METERIAL: Groups were defined by drugs and cell lines combination and were evaluated. We cultured cell line under routine environment and determined optimal cell number and drug concentration. Drugs were applied to cell lines and we measured the optical density with ELISA after MTT application. Statistical analysis was made and clinical availability was evaluated. IC50 and assay AUC could be calculated. Clinical availability was evaluated and comparisons of anti-tumor effect of each drug on cell lines were performed. RESULTS: 1. Anti-tumor effects of taxol and cisplatin on each cell lines showed time and dose dependent relations with significance statistically. 2. Assay AUC in IC50 was higher than clinically achievable AUC in all groups except cisplatin-KB group. Anti-tumor effect of taxol to KB cell line was higher than that of cisplatin in concentration calculated from the clinically achievable AUC. So, we could suggest that taxol could be also used as a antitumor durg for oral cancer. Further study that is, in vivo study, evaluation of combination therapy with other drug, and method of increasing of AUC would be necessary.

5.The Study of the Signal Detection Test of the Traffic Accident Patients with and without Brain Lesions.

OBJECTIVES: The purpose of this study was to investigate the differences of signal detection between the traffic accident patients with and without brain lesions in neuroimaging techniques. METHODS: Subjects were 48 traffic accident patients. All subjects were asked to detect the signals in Signal Detection test of Vienna Test System. RESULTS: The number of correct response and the stimulus sensitivity were significantly higher in group without lesion than group with lesion, but the number of false alarm and the response criterion were not. CONCLUSIONS: These results suggest that a cause of lower signal detection of the patients with brain lesions was not motivational and emotional impairment but cognitive impairment.

6.The change of exercise according to time and its predicting factors in the followed - up hypertensive patients of family physicians.

BACKGROUND: Hypertension is one of the common diseases which family physicians encounter to manage. Various research proved that appropriate aerobic exercise had effect on lowering blood pressure efficiently. This study was to find out the status of exercise, the change of exercise according to time and its predicting factors in hypertensive patients. METHODS: A total of 193 of the followed-up hypertensive patients of family physicians in Seoul and kyoungkido from May 11, 2001 to June 10, 2001 were interviewed by doctor according to the previously designed structured questionnaire. RESULTS: Eighty two patients (42.57) were male and mean and was 57.4 +/- 9.9 fears. Using Kaplan-Meier's method, 8% of non-exercise group (N= 115) in diagnosing hypertension started exercise after 1 fear and 40%, after 5 years. Otherwise, 5% of exercise group(N=78) stopped exercise after 1 year and 30%, after 5 years. The characteristics of 5 exercise groups according to sex, age, level of education and adequacy of exercise showed significant difference. In average blood pressure, lately started group showed the lowest (137/84 mmHg) and non-exercise group, the highest (146/91 mmHg) In the degree of recommendation. sporadic group showed the highest and non-exercise group, the lowest which showed significant difference (p =0.0024) . CONCLUSION: I conclude that lately started group among non-exercise group and continuous exercise group were mainly affected by recommendation to exercise of physician. As well as exercise, other nonpharmacologic treatment should be promoted by physician as an important strategy for treatment of hypertension.

7.A Clinical Study of Nasolabial Cyst.

BACKGROUND AND OBJECTIVES: Being probably of embryonal in origin, the nasolabial cyst is relatively rare and is thought to arise either from the epithelial remnants entrapped along the lines of fusion during the development of face or from the remnants of the developing nasolacrimal duct. The purpose of this study was to examine the clinical and radiological features of nasolabial cyst in order to provide basis for its correct diagnosis and treatment. MATERIAL AND METHODS: The nine cases of nasolabial cyst which were treated in Masan Samsung Hospital from January in 1991 to April in 1997 were the used as subjects. These cases were examined according to age and sex distribution and their clinical features, radiologic and histologic findings, treatment and prognosis, were studied. RESULTS: In our study of nine cases of nasolabial cyst, more females than males were observed, and with respect to age, individuals in their Fortise were more frequently noted. Also, swelling of nasolabial fold were the most frequently observed among symptom and signs. Computerized tomography shows well-circumscribed cystic mass lateral to pyriform aperture. Seven cases underwent operation. Six cases were excised by intraoral sublabial approach, one case which forms fistula to skin was removed by transcutaneous approach. CONCLUSION: Nasolabial cyst is benign harmatomatous cyst of face. This lesion is often unrecognized or confused with other intranasal mass, other fissural and odontogenic cysts, midface infection, or swelling in the nasolabial area. Therefore careful clinical and radiologic evaluation should be considered in the differential diagnosis.

8.Fractures of the Femoral Neck in Children

Fractures of the femoral neck in children are rare and usually severe violence to occur, which accounts for the high frequency of associated injuries. Since the femoral neck in children has many anatomic and physiologic differences, not only are there many difficulties during the treatment period, but complications such as post-traumatic coxa vara, avascular necrosis of the femoral head, and premature epiphyseal closure may occur, sixty-two cases in sixty patients of fractures of the femoral neck in children which were treated at Severance Hospital, Yonsei University College of Medicine from Jan. 1971 to Dec. 1987 and at Young-Dong severance Hospital, Yonsei University college of Medicine from May. 1983 to Dec. 1987 were analyzed both clinically and radiologically. The following results were obtained: l. Of the 62 cases, the ratio of boys to girls was approximately 2:1 and the highest incidence was between 4 to 6 years old. 2. The main cause of fracture was traffic accidents, which occurred in 33 cases(53.2%). 3. According to the classification of Delbet and Colonna, the cervicotrochanteric fracture was the most common type(41.9% of the cases) and 29 cases(46.8%) were displaced fractures. 4. Thirty two cases(51.6%) were treated by conservative treatment and twenty eight cases (45.2%) by closed reduction and internal fixation. 5. Common associated injuries were genito-urinary tract injury, soft tissue injury, long bone fracture and pevic bone fractures. 6. All cases were followed for from 14 months to 15 years, average 8 years. 7. The results were analyzed according to Ratliff's assessment and 41 cases(66.1%) showed good results. 8. The total number of cases with complications was 33(53.2%). The most common complication being coxa vara and premature epiphyseal closure(32.2%). 9. Secondary treatment was done in 7 cases and correction osteotomy was done in 4 cases which had showed a good results. The authors found that for the treatment of type IV fractures, closed reduction followed by plaster of Paris immobilization was adeguate but for type I, II, III after early closed reduction, internal fixation was performed as soon as possible using threaded pins as the internal fixation device. The results were poor in type I and type II, fractures with severe displacement, and in cases in which the treatment was delayed due to associated injury.
