1.Life Experiences of Uninfected Women Living with HIV-Infected Husbands: A Phenomenological Study.

PURPOSE: This study aimed to understand the meaning and essence of the life experiences of uninfected women living with HIV-infected husbands. METHODS: This qualitative study adopted van Manen's hermeneutic phenomenological method. Study participants were 8 females whose husband had been diagnosed with HIV for longer than 6 months, who had known about their husband's infection for more than 6 months, who were in a legal or common-law marriage and were living with their husbands at the time of interview for this study, and whose HIV antibody test results were negative. Data were collected from in-depth individual interviews with the participants from May to August 2016, and from related idiomatic expressions, literature, artwork, and phenomenological references. RESULTS: The following essential themes were identified regarding the life experiences of uninfected women living with HIV-infected husbands: ‘experiencing an abrupt change that came out of the blue and caused confusion’, ‘accepting one's fate and making desperate efforts to maintain one's family’, ‘dealing with a heavy burden alone’, ‘experiencing the harsh reality and fearful future’, and ‘finding consolation in the ordeal’. CONCLUSION: This study provided a holistic and in-depth understanding of the meaning and essence of the life experiences of uninfected women living with HIV-infected husbands. Thus, this study recognizes these unnoticed women as new nursing subjects. Further, the present findings can be used as important basic data for the development of nursing interventions and national policy guidelines for uninfected women living with HIV-infected husbands.

2.Comparision of Three Trichomonas Antigens for the Detection of IgG Antibody in Serum.

BACKGROUND: Direct wet mount examination of vaginal secretion, widely applied for the diagnosis of Trichomonas vaginalis infection in woman patients, is rapid and economical. However, the sensitivity of this technique is not so high. In this study enzyme-linked immunosorbent assay(ELISA) was employed for the detection of serum anti-T. vaginalis IgG antibodies from vaginal trichomoniasis patients. METHODS: Eighty sera from trichomonoasis patients who visited a Dr. Yoon Kyong's Obstetric & Gynecologic Clinic in Songnam and 30 non-infected healthy men were tested for detection of anti-T. vaginalis IgG antibody. Soluble lysate and excretory-secretory antigen prepared by mixing of six isolates of T. vaginalis, and lysate from one isolate(KT4) were used as antigen for ELISA. RESULTS: The sensitivity of ELISA using lysate of six isolates was 95.0%, and the sensitivity of the lysate from KT4 and mixed excretory-secretory antigen from 6 isolates were 86.4% and 76.3%, respectively. Specificities of ELISA by three 93.3%, 96.3% and 92.0%, respectively. CONCLUSION: It is suggested that ELISA using mixed lysate of T. vaginalis six isolates could be useful tools for the diagnosis of trichomoniasis.

3.A Study on the Rate of Breast-feeding Practice by Education and Continuous Telephone Follow-up.

Background & PURPOSE:Since the 1970's the rate of breast-feeding has decreased significantly. The Korean National Institute of Health reported that the rate of breastfeeding was 68.9% in 1982 and 14.1% in 1997. There are many influencing factors including: the lack of education and information on breast feeding, lack of faith in breast feeding, increment of the rate of working, lack of encouragement by supporters in difficult situations, and nurses' low level of knowledge about breast feeding. Such a lack of knowledge and support of breast-feeding at home by family members create another dilemma to the problem of breast-feeding. If problems arise and family members are unable to provide assistance due to the deficiency of knowledge, mothers show a tendency to abandon breast-feeding. The purpose of this research is to find out the rate of breast-feeding practice by time sequence of 1 week, 6 weeks and 12 weeks after birth and influencing factors on breast-feeding practice centered on the postpartal women who were 3discharged from one hospital, which is located in Seoul and provides simple breast-feeding education and continuous postnatal telephone consultation. Methodology: The subjects of this research were 54 women who gave birth in a hospital located in Seoul from 1 March 2000 to 31 April 2000. After birth the subjects were educated individually about breast-feeding and telephone consultations were conducted. On the 1st week, 6th week, and 12th week, the subjects were surveyed about their breast-feeding practice rates and methods by telephone. Results: 1) Complete breast-feeding rate: Within one week after birth, the subjects showed 64.2% complete breast-feeding rate. Within 6 weeks, 39.6%, and 12 weeks, 34.1%. 2)Partial breast-feeding rate: Within one week after birth, the subjects showed 32.1% partial breast-feeding rate. Within 6 weeks, 39.6%, and 12 weeks 15.1%. 3)Complete bottle-feeding rate Within one week after birth, the subjects showed 1.9% complete bottle-feeding rate. Within 6 weeks of giving birth 15.1%, and 12 weeks 17.0%. These results show that individual education about breast-feeding and continuous postnatal telephone consultation influenced on the practice of breast-feeding. On considering the reality of the hospital situation in which nurses could not operate education program due to the work-load, it is necessary to find out selectively those mothers who are unable to breast feed and provide education individually and continuous support by telephone follow up. Futhermore, the active role of lactation nurse specialist and their efficient management of breast-feeding for the successful practice is required.

4.A Study on Self-Achievements and Related Factors in Elders.

PURPOSE: This study was to identify the relationship among self-management, economic stability, self-esteem, self-achievement and predictor of the self-achievements in elders. METHODS: A total of 294 subjects aged between 60 and 92 were selected through convenient sampling. Data were collected by self-reported questionnaire from May to June, 2008. Collected data were analysed by the SPSS/WIN 15.0 program. RESULTS: Self-management, economic stability, self-esteem, self-achievement were positively correlated. The most significant predictor of self-achievement in elders were self-management and self-esteem. CONCLUSION: These findings suggest that self-achievement is influenced by self-management and self-esteem and needed for aged well.

5.A Literature Review of Research on Leadership of Korean Hospital Nurse

PURPOSE: This study was done to identify trends in leadership-related research by reviewing studies on hospital nurses in South Korea.METHODS: The research was conducted from April 2 to 14, 2018 and search databases were RISS, KISS, DBpia, KM base, NAL, and NDSL. Search terms were ‘leadership’ and ‘nursing’ in the Korean language, and total 141 papers were selected.RESULTS: Analysis of the research on leadership showed that 96.4% of research methods were quantitative research, and 60.3% were predictive researches. For study settings, 46.2% were general hospitals, 96.5% were staff nurses. In the study of leadership variables, 60.3% were independent variables and recent leadership was used as a mediating variable (4.3%) and a moderating variable (2.8%). Among the leadership types, self-leadership (31.2%) was the most common, and the main variable related to leadership was job satisfaction (25.8%).CONCLUSION: These results provide useful data for deriving new research ideas about nursing leadership. Furthermore, this study has significance for providing the evidence for developing new leadership programs and policies in nursing organizations.

6.Burnout Experience of HIV Counseling Nurses.

PURPOSE: This study was designed to describe the burnout experiences of nurses counseling people living with HIV/AIDS (PLWHA) in hospital-based counseling services in Korea. METHODS: A qualitative research technique with focus group interviews was used and the data were collected in 2012. Participants were 13 counseling nurses from 16 medical hospitals in South Korea who had worked full-time for more than six months as a counseling nurse for PLWHA. The 13 nurses made up 68.4% of all 19 official HIV counseling nurses in South Korea. Data were collected using focus group interviews and analyzed with the content analysis methodology of Downe-Wamboldt. RESULTS: Burnout experience of counseling nurses for PLWHA was analyzed for two domains-causes of, and effects of burnout. Each domain was classified into three categories; personal, job-related, and relationships. Further 17 sub-categories and 47 concepts were discovered. CONCLUSION: In this study various causes and effects of burnout experienced by nurses counseling PLWHA were identified. These findings can be used in developing effective strategies for nurses, administrators, and health policy makers to solve the burnout of counseling nurses. Further research is required to develop detailed and practical job description for nurses counseling PLWHA.

7.Analysis of Manuscripts Rejected by the Journal of Korean Academy of Nursing Administration 2012~2015 Jun.

PURPOSE: The purpose of this study was to analysis the manuscripts rejected for publication in the Journal of Korean Academy of Nursing Administration during the last three years six months (2012~2015 Jun). METHODS: Seventy eight rejected manuscripts were analyzed focusing on manuscripts characteristics, such as rejection rates, year of submission, occupation of first author and corresponding author, number of authors, funding, thesis or dissertation, article type, study participants, number of reviews prior to rejection, and 3rd reviewer. Also reviewers' quantitative evaluation scores and subjective comments were analyzed. Reviewers' subjective comments were analyzed using content analysis methodology. RESULTS: The mean rate for manuscript rejection was 28.9% and for quantitative research, qualitative research, and review papers the quantitative evaluation scores were 2.54+/-0.70, 2.39+/-0.69, and 2.39+/-0.69 out of 5 points, respectively. The most frequent subjective comment on rejected manuscripts was 'lack of rationale for research need'. CONCLUSION: In this study the characteristics and the reasons for rejecting manuscripts were identified. These findings can be used in developing effective strategies for researchers, reviewers and editors to improve the quality of research and research reviews of nursing administration research.

8.Clinical Significance of Predischarge Treadmill Exercise Test in Patients with Acute Myocardial Infarction.

To assess the diagnostic value of low level predischarge exercise test in the prediction of multivessel disease and left ventricular abnormality rate limited treadmill test, coronary arteriography and left ventriculography were admitted to the coronary care unit at Seoul National University Hospital from February 1985 to April 1986. The following results were obtained; 1) During and immediately after the treadmill test, ischemic ST segment depression were observed in 7 patients (20.6%). On subsequent coronary arteriography, 6 of them were found to have multivessel coronary artery disease. The sensitivity of ST segment depression in prediction of multivessel disease was 31.6%, the specificity was 93.3% and prediction value was 85.7%. 2) In 11 patients (32.3%), the exercise test were discontinued because of anginal pain, fatigue, or dyspnea. The sensitivity of above symptoms in prediction of multivessel disease was 47.4%, the specificity was 86.7% and prediction value was 81.8%. 3) The sensitivity of ST segment depression and/or symptom in prediction of multivessel disease was 68.4%, the specificity was 80%, and the prediction value was 81.25%. 4) The ST segment elevation were observed in 10 patients (29.4%). The sensitivity of ST segment elevation in prediction of complicated left ventricular aneurysm was 58.3%, the specificity was 87.5%, and the prediction value was 70%. 5) No serious complication developed by the treadmill test. It is concluded from above results that low level predischarge treadmill exercise test is useful and safe test in the prediction of multivessel disease and left ventricular aneurysm in patients with recent acute myocardial infarction.

9.Coronary Arteriographic Findings of Korean patients with Acute Myocardial Infarction.

The coronary arteriography and left ventriculography were performed on 63 patients with acute myocardial infarction who were admitted to coronary care unit of Seoul National University Hospital from September 1984 to October 1986 within 30 days after onset of acute myocardial infarction (median: 16 days) to delineate the extent of coronary artery disease and the left ventricular function. The results were as follows; 1) The ratio of male to female was 59:4 (14.75:1) and 34.9% of all patients were at their 6th decade. 2) The infarction were transmural in 58 patients (92.1%) and nontransmural in 5 patients (7.9%). Among 58 patients with transmural infarction, 33 (56.9%) had anterior wall infarction, 15 (25.9%) had inferior wall infarction and 10 (17.2%) had anteroinferior wall infarction. 3) Among 63 patients, 4 (6.3%) showed completely normal coronary artery on coronary arteriography and 3 (4.8%) had insignificant stenosis (lesser than 50%) reduction in luminal diameter). The 39.7% of all patients had one-vessel disease, 27% two-vessel disease and 22.2% three-vessel disease. 4) Among 58 patients with transmural infarction, 31 (53.4%) showed complete occlusion of infarct related artery. And 73.4% of the patients with inferior wall infarction showed complete occlusion of infarct related artery. None of the patients with non-transmural infarction had complete occlusion. 5) In 21 patients who had the coronary arteriography within 14 days after the onset, 12 (57.2%) showed complete occlusion of infarct related artery and among 42 patients who were studied 15-30 days after the onset, 19(45.2%) showed complete occlusion. 6) Nineteen patients (30.2%) were found to have left ventricular aneurysm. 7) The left ventricular ejection fraction were significantly higher in the patients with non-transmural infarction than in patients with transmural infarction. The difference in left ventricular ejection fraction between the patients with anterior infarction and with inferior infarction, between single vessel disease and multiple vessel disease were not significant. 8) The older age group showed a tendency to have higher prevalence of multivessel disease. 9) As complication of coronary arteriography and left ventriculography, one episode of ventricular fibrillation was observed without mortality. From the above results of this study, it is concluded that coronary arteriography and left ventriculography can be safely performed within 30 days after the onset of acute myocardial infarction: A significant number of patients had normal or minimally diseased coronary artery: more than half of the patients with transmural infarction had complete occlusion of infarct related artery: the patients with nontransmural infarction had better left ventricular function than with transmural infarction.

10.Clinical and Neuropsychological Characteristics of Patients with Stroke of the Basal Ganglia.

The experimental evidences suggest that the roles of basal ganglia are cognition and emotion through the corticostriatothalamocortical relationship. The patients with lesions in the caudate nucleus have high incidences of cognitive and behavioral abnormalities accompanied with the motor paralysis. In these patients, the accurate assessment of neuropsychologic dysfunctions and the prompt application of cognitive rehabilitation programs are important for the functional restoration. We evaluated the patients with stroke of basal ganglia for the clinical and neuropsychological characteristics in relation to the involved substructures of basal ganglia. Fourteen patients were evaluated for the clinical neurologic examinations, functional assessment by functional independence measure(FIM), speech assessment, and various neuropsychological tests for the assessment of attention and memory functions. The results were analysed a ccording to their lesion sites. Of the nine patients with stroke at the right basal ganglia, five patients had neglect of the contralateral hemispace and one had dysarthria. On the other hand, of the five patients with stroke at the left basal ganglia, none had hemispatial neglect and three had aphasia or dysarthria. Of the six patients with caudate lesions, three had aphasia or dysarthria, whereas of the eight patients without caudate lesion, one had dysarthria. The scores of Wechsler memory test were significantly lower in the patients with caudate lesions(P<0.05). Among the FIM subscales, the scores of communication and social cognition were significantly lower in the patients with caudate lesions(P<0.05). The Motor Score was significantly lower in the patients with putamen lesions(P<0.05). Among the FIM subscales, the scores of self care and locomotion were significantly lower in the patients with putamen lesions(P<0.05).
