1.Drug Interaction in New Antipsychotics.

Recently atypical antipsychotics have been used as first line agent in the treatment of schizophrenia, and also played a significant role in the treatment of many kinds of psychiatric disorders. The pharmacokinetic and pharmacodynamic properties of these newer antipsychotics are well know through preclinical and early clinical trials. However, it is important to note the limitations of the results due to its relatively short experience. Clozapine is eliminated principally by the hepatic P450 1A2 and 3A4 cytochrome enzymes. 1A2 inducers such as carbamazepine and smoking can reduce its half-life, while 1A2 inhibitors such as SSRIs especially fluvoxamine can increase its duration of action. Carbamazepine should be avoided in a patient on clozapine because of carbamazepine's potential effects on bone marrow. Benzodiazepines tend to increase the chances of sedation delirium and respiratory depression. Risperidone is metabolized to 9-hydroxyriperidone by the hepatic P450 2D6 cytochrome enzymes. Fluoxetine and paroxetine, 2D6 inhibitors interfere with metabolism, but 9-hydroxyrisperidone has similar biological activity as parental drug, so it has little affect on the outcome. Olanzapine shows minimal capacity to inhibit cytochrome P450 isoenzymes and shows minimal chance of drug interaction. It is eliminated principally by the hepatic P450 1A2 and 2D6 cytochrome enzymes.

2.Circardian Variation of Premature Ventricular Complex in Dilated Cardiomyopathy.

BACKGROUND: Circardian variation in the onset of cardiovascular events includig sudden cardiac death, myocardial infarction and ventricular arrhythmias has been discribed. The frequency of ventricular premature complexes has also been reported to demonstrate a pattern consisting of a daytime peak and nightime nadir. We tried to see if the same circardian pattern is found in dilated cardiomyopathy patients. We have also studed how various modifying factors such as left ventricular ejection fration and ACE inhibitor use may affect the circardian pattern. METHOD: 24-hour ambulatory electrocaridiographic monitorings were performed in 50 dilated cardiomyopathy patients and 20 control subjects. Patients were prospectively divided in 2 groups based on LVEF and ACE inhibitor use. RESULTS: In dilated cardiomyopathy patients, the expected morning increase in VPC frequency is absent and show a peak in evening. This pattern is not correlated with heart rate. Evening peak is more prominent in low LVEF group and ACE inhibitor non-user group. CONCLUSION: In dilated cardiomyopathy patients, VPC frequency show a peak in the evening.

3.A Clinical Study of Nasolabial Cyst.

BACKGROUND AND OBJECTIVES: Being probably of embryonal in origin, the nasolabial cyst is relatively rare and is thought to arise either from the epithelial remnants entrapped along the lines of fusion during the development of face or from the remnants of the developing nasolacrimal duct. The purpose of this study was to examine the clinical and radiological features of nasolabial cyst in order to provide basis for its correct diagnosis and treatment. MATERIAL AND METHODS: The nine cases of nasolabial cyst which were treated in Masan Samsung Hospital from January in 1991 to April in 1997 were the used as subjects. These cases were examined according to age and sex distribution and their clinical features, radiologic and histologic findings, treatment and prognosis, were studied. RESULTS: In our study of nine cases of nasolabial cyst, more females than males were observed, and with respect to age, individuals in their Fortise were more frequently noted. Also, swelling of nasolabial fold were the most frequently observed among symptom and signs. Computerized tomography shows well-circumscribed cystic mass lateral to pyriform aperture. Seven cases underwent operation. Six cases were excised by intraoral sublabial approach, one case which forms fistula to skin was removed by transcutaneous approach. CONCLUSION: Nasolabial cyst is benign harmatomatous cyst of face. This lesion is often unrecognized or confused with other intranasal mass, other fissural and odontogenic cysts, midface infection, or swelling in the nasolabial area. Therefore careful clinical and radiologic evaluation should be considered in the differential diagnosis.

4.Cases Studies of Intussusception with Special References.

This paper present 105 cases which were admitted to the Department of Pediatrics of Dong San Presbyterian Hospital from Jan. 1978 to July, 1980 The results are summarized as follows; 1) Age incidence, 94% of patients were under age of two, 88.2% of patients were under one year, and adults were high incidence in Korea. 2) Male to female ratio was 2.1:1 3) In seasonal incidence, spring was relatively high. 4) In 76.4% of cases, their body weight were over 50 percentile of Korean normal infants and children. 5) The patients who visited hospital within 24 hrs. from onset of symptom was occupied in 57.7%. 6) The cardinal symptom and signs were vomiting (89.4%), abdominal pain. Irritability, crying, (72.9%), mucous bloody stool (70.6%) and abdominal mass (42.1%) 7) In analysis by intussusception type(40.0%), ileocolic type (23.5%), ileoileal type(17.6%), jejunojejunal type (17.6%), in adults. 8) In simple abdominal X-ray figures showed the dilatation due to gastrostomy 52.9% and phenomenon of fluid retention 30.6%. 9) Etiologic causes were confirmed only 7.1% in children and 29.4% in adults, the causes in 92% were idiopathic in children and 70.6% in adults respectively. 10) In previous illness, we find out its predisposing factors 42.4% in children. 11) Hydrostatic barium reduction was successed in 47.1% of all cases and operative cases were 47%

5.Haloperidol and clozapine differentially regulate signals pstream of glycogen synthase kinase 3 in the rat frontal cortex.

Glycogen synthase kinase 3 (GSK3) was recently suggested to be a potential target of psychotropics used in psychiatric illnesses such as schizophrenia and bipolar disorder. Relevant studies have found that antipsychotic drugs regulate GSK3 activity via an increase in either inhibitory serine phosphorylation or amount of GSK3 after acute or subchronic treatment. Recent evidence shows that GSK3 is regulated by dopaminergic or serotonergic systems implicated in the pathophysiology and treatment mechanisms of schizophrenia and bipolar disorder. Therefore, antipsychotics may regulate GSK3 via antagonizing dopaminergic or serotonergic activity. However, the signaling pathway that is involved in GSK3 regulation by dopaminergic or serotonergic systems has not been well established. Haloperidol is a typical antipsychotic with potent dopamine D(2) receptor antagonism. Clozapine is an atypical antipsychotic with potent serotonin 5HT(2) receptor antagonism. We injected rats with haloperidol or clozapine and examined the phosphorylation and amount of GSK3alpha/beta and its well-known upstream regulators Akt and Dvl in the rat frontal cortex by Western blotting. Both haloperidol and clozapine induced Ser21/9 phosphorylation of GSK3GSK3alpha/beta. Haloperidol increased the Ser473 phosphorylation of Akt transiently, whereas clozapine maintained the increase for 1 h. Haloperidol did not affect the phosphorylation and amount of Dvl, whereas clozapine increased both phosphorylation and the amount of Dvl. Our results suggest that GSK3 activity may be regulated by both typical and atypical antipsychotics and that Akt or Dvl, depending on the D(2)- or 5HT(2)- receptor antagonism properties of typical and atypical antipsychotics, mediate the regulation differently.

6.A Prospective Study of Prophylatic Antibiotics for Laparoscopic Cholecystectomy.

PURPOSE: We wanted to clinically evaluate the antibiotic therapy used from pre- to postlaparoscopic cholecystectomy along with the improved surgical techniques and instruments and the patient nutrition. METHODS: We prospective studied 105 patients who were treated with laparoscopic cholecystectomy for acute and chronic cholecystitis at Busan St. Mary Hospital from March 2007 to January 2008. The two groups were divided as follows: (A) the group was not injected with antibiotics, and the other group (B) was injected with antibiotics. We did not used antibiotics when the patient had no acute aggravating symptoms, and we exclude the patients with old age (<70 years), a past history of diabetes mellitus (DM) or hypertension (HTN), and a past operation history. RESULTS: Between the two group, there was no difference when they were compared for age, BMI (Body Mass Index), the operation time (minute) and the length of the hospital stay: (A) Group 49.3+/-13.0 years, 24.4+/-3.2 kg/m2, 37.9 +/- 10.0 minute and 4.6 +/- 1.3 days, respectively, and (B) Group 55.9+/-15.6 years, 23.5+/-3.5 kg/m2, 43.2 +/- 23.8 minute and 5.5 +/- 2.5 days, respectively. There was a significant different in the leukocyte count and the % of neutrophils: (A) Group 6396microliter(-1) and 53.4%, respectively and (B) Group 9354microliter(-1) and 66.6%, respectively. In (B) group, there were 2 cases of atrial fibrillation, 1 case of diarrhea and 1 case of wound seroma. There were no surgical related complications in group (A). CONCLUSION: For the well nourished chronic cholecystitis patients, the use of the prophylatic antibiotics may be less effective because of the many drug-induced side effects. More studies about the use of prophylactic antibiotics are needed.

7.Regulation of the Circadian Gene CLOCK Expression by KCl Depolarization in B35 Rat Neuroblastoma Cells.

OBJECTIVES: To investigate the effects of KCl on regulation of circadian gene CLOCK expression, we observed whether induction of CLOCK is influenced by KCl depolarization in B35 rat neuroblastoma cells. METHODS: B35 rat neuroblastoma cells were grown in Dulbecco's modified Eagle's medium supplemented with 10% FBS and 1% penicillin-streptomycin in a 37 degrees C humidified incubator with 5% CO2. Inhibitors including cycloheximide and actinomycin D were pretreated 1 hour before treatment with 50mM KCl. Immunoblotting with anti-CLOCK antibody was done. RESULTS: CLCOK is induced by 50 mM KCl in B35 Rat Neuroblastoma cells, and a maximal induction in CLOCK level reached peak at 8 to 20 hours. The pretreatment of cycloheximide and actinomycin D prevented the induction of CLOCK by 50 mM KCl. CONCLUSION: We suggest that KCl depolarization may play critical roles in several aspects of the circadian gene CLOCK expression.

8.Inefficiency in 6-Set Requests for Blood Culture Analysis of Cancer Patients with the Central Venous Catheter.

BACKGROUND: A central venous catheter (CVC) is commonly used for administering chemotherapy to cancer patients. The institutional guideline of the Gyeongsang National University Hospital (GNUH) for blood culture analysis of indwelling CVC patients recommended 6 sets (2 from the periphery and 4 from each lumen). We analyzed the usefulness of this guideline, because complying with this recommendation requires an abundant amount of the sample and it is both inconvenient and expensive. METHODS: Adult patients (age: > or =18 yr old) who were admitted to the cancer center of GNUH between January 2011 and April 2012 were requested to have their blood culture analysis done. The positive rate, contamination rate, and distribution of microorganisms were compared according to the number of requested sets. The positive results of the stipulated 6 sets were analyzed. RESULTS: A total of 5,263 blood cultures were analyzed during the study period; of them, 74.4% were requests of 2 sets and 20.0% were requests of 6 sets. The positive rates in 2 set requests and 6 set requests were 8.0% and 14.3%, respectively (P<0.001). The requests for 6 sets were repeated about 5 times. All 6 sets showed positive in 16 cases (9.1%), whereas a part of the 6 sets was positive in 18 cases (10.3%). CONCLUSIONS: Although the positive rate was relatively high in the 6 set-requested groups, they had to be repeatedly requested. Microbial growth in a part of the 6-set requests was observed in a very small proportion (10.3%) of the patients, indicating that the benefit of blood culture of 6 sets is very low.

9.Continuous Femoral Nerve Block Guided by Ultrasound Sonography in Metastatic Cancer Patient: A case report.

Terminal cancer pain is always a challenge to pain doctors.If the focus of the cancer pain is located in the restricted area, it is easy to control the pain. However, if the focus of cancer pain is widespread, it is very difficult to control the pain and it makes pain doctors embarrassed.Nowadays, the ultrasound-guided nerve block become a popular method. It is easy to find the nerve and helps to insert the catheter around the nerve for the continuous nerve block. We encountered a patient who was 59 years old with a stomach cancer and vertebral metastasis. He complained about the pain in his abdomen, back and left thigh. His abdominal and back pain vanished with the use of continuous lumbar epidural injection. However, the patient still complained about the pain and numbness on the left thigh, because of this, he couldn't get any sleep.We tried a continuous femoral nerve block under the ultrasound guidance, after that his pain was gone.The ultrasound-guided nerve block will be very helpful to control the pain.

10.A Case of Mucocele of the Submandibular Gland.

Cysts of the salivary gland are non-neoplastic, localized lesions of the duct system. They form about 6 % of lesions of the salivary glands. Mucoceles represent more than 70% of the salivary cysts and usually arise from minor salivary glands; however, major salivary glands, especially the sublingual glands, may also be involved. Characteristically, mucoceles consist of a soft fluctuant bluish or translucent swelling which contain mucus, varying in size. They are most commonly found on the lower lip, but may also occur on the buccal mucosa, floor of the mouth, palate, upper lip, and tongue. They are found in all age groups but are most frequent in children and young adults. There are two types of mucoceles-one in which mucus has extravasated into the tissues (extravasation cyst) and one which is lined with epithelium (retention cyst). Recently, we experienced an unusual case of huge extravasation mucocele arising from the submandibular gland. The mucocele and submandibular gland were completely excised surgically, which were confirmed by computed tomographic scanning and histopathologic examination.
