1.Clinical Trials of Demacre on Varioys Dermatoses.

Demacre', a new topical steroid cream containing 0.025% beclomethasone dipropionate, was used in several kinds of dermatoses. Total 35 patients including ll cases of contact dermatitis, 5 of atopic dermatitis, 3 of hand eczema, 3 of seborrheic dermatitis, 2 of neurodermatitis, 2 of nummular eczema, 2 of autosensitization dermatitis, 1 of chronic eczema, 3 of psoriasis, 1 of insect bite, 1 of miliaria rubra, and 1 of intertrigo were treated by Demacre'. Among these patients, we observed excellent effects in 4 patients (11%), good effects in 22 patients (63%), and fair effects in 3 patients (9%). Namely, among these 35 patients, 26 patients (74%) revealed excellent to good therapeutic effects on their skin conditions with Demacre'. No untoward side effects were noted in all subjects treated with Demacre.

4.Effect of Leptin on the Steroidogenesis of Cultured Human Granulosa Cells.

OBJECTIVE: To elucidate the location of leptin and receptors of ovary specimens obtained from patients undergoing hysterectomy by immunohistochemical staining and to determine the effect of leptin on the steroidogenesis of cultured granulosa cells. METHOD: In the culturing process of the granulosa cells, FSH (1 IU/ml)and leptin (50 ng/ml), IGF-I (50 ng/ml) was administered to each study group (Group I: FSH; Group II: FSH, leptin; Group III: FSH, IGF-I, leptin), and the levels of estradiol, progesterone, androstenedione in the culture media was measured by radioimmunoassay. Statistical analysis was conducted by one-way ANOVA with Scheffe test. RESULTS: The results showed that leptin and leptin receptors were both found to be strongly stained in granulosa and theca cells, and also in some interstitial cells. Leptin receptors were also observed in cultured granulosa cells. While there was no statistically significant difference in the androstnedione concentrations between the groups, estradiol concentrations was significantly decreased in Group IV (2202.0+/-151.14 pg/ml) compared to Group III (2859.0+/-122.6 pg/ml), and progesterone concentrations were also significantly decreased in Group II(4696.3+/-190.6 ng/ml) and Group IV (4517+/-206.78 ng/ml) compared to Group III(5546.0+/-179.5 ng/ml). CONCLUSTION: The study result of this study suggest that leptin is directly involved in the regulation of ovarian functions, in particular steroidogenesis.

6.A Case of Acute Febrile Neutrophilic Dermatosis (Sweet's Sundrom).

We presented a case of acute febrile neutrophilic dermatosis (Sweet's syndrome) in 57 years-old female. This case is the 31 st case in the literature and the first case in Korea. Erythematous plaque was presented on the face with tendency of recurrences. Histologically a dense inflammatory cell infiltration composed predominantly of polymorphonuclear neutrophiles, lmphocytes and histiocytes was seen in dermis. Positive laboratory findings were polymorphonuclear leukocytosis and elevated ESR. The symptoms were well centrolled with orally administered preclnisolone.

8.The Effects of Hope Intervention on the Hope and Quality of Life of Cancer Patients Staying at Home and Cared in Public Health Center.

PURPOSE: This study was to identify the effects of hope intervention on the hope and quality of life of cancer patients staying at home. METHOD: The study adopted the randomized control group design. The subjects consisted of randomly selected forty cancer patients who were registered at S-Gu Public Health Center. Hope intervention which was composed of hope assessment, positive self identity formation, hope objective setting, therapeutic relationship and spiritual & transcendental process improving, and hope evaluation was provided from October 22, 2007 to November 30, 2007. RESULT: Hypothesis 1-1 "The experimental group that received hope intervention will have a higher score of hope than the control group", was supported(t=-3.108, p=.004). Hypothesis 1-2 "The experimental group that received hope intervention will have a higher level of hope index than the control group", was supported (t=-4.219, p=.000). Therefore, Hypothesis 1 "The experimental group that received hope intervention will have a higher level of hope than the control group" was supported. Hypothesis 2 "The experimental group that received hope intervention will have a higher level of quality of life than the control group", was not supported (t=-1.726, p=.092). CONCLUSION: Hope intervention is an effective nursing intervention to enhance hope for patients with cancer staying at home.

9.The Clinical Consideration for Intracranial Epidural Abscess.

The intracranial epidural abscess is a not indenpendent disease which is practically always secondary to osteitis or osteomyelitis of overlying bone. Also it was developed from complication of frontal sinusitis, middle ear or mastoiditis, incomplete debridement of compound comminuted depressed fracture and post-operative complications. Occasionally it was derived from dural sinus thrombophlebitis especially cavernous sinus. It was frequently developed frontal and temporal region. We have experienced 5 cases of intracranial epidural abscesses for this one year. Followings are the results ; 1. The causes were complication of compound depressed fracture, chronic osteomyelitis and post-operative complications. 2. The usual symptom was headache, fever and increased lethargy. 3. The laboratory findings of CBC were the increased WBC count and ESR. The CSF findings revealed occasionally increased the cell count and sugar. 4. The pus culture revealed proteus, coliform bacilli and paracolon bacili. 5. Treatment was surgical removal of overlying diseased bone, drainage of the purulent materials and instillation of antibotics irrigation. 6. The prognosis of the 5 cases of the intracranial epidural abscess was good. There was not appeared any complication or sequelae.

10.A Case of "Flip-Flap" Pattern in Cerebral Neuclear Angiogram.

The characteristics of radionuclide brain scan imaging of intracranial lesion are determined, in part, by the rate and quantity of radioactive material in the region of involvement, which is associated with alterations in the blood brain barrier. Findings, noted on the blood flow studies, often serve to direct attention to forthcoming imaging of specific pathological change on the status studies, e.g, carotid artery occlusion and cerebral infarction. We have seen that the 62 year old woman with left side hemiparesis shows typical "Flip-Flap" pattern in brain scan imaging, thought to be due to carotid artery occlusion.
