1.A Giant Squamous Cell Carcinoma Arising in the Lesion of Discoid Lupus Erythematosus.

In the past, irritant or radiation therapy have been used to treat discoid lupus erythematosus, therefore malignant and proriferative change in the lesion of discoid lupus erythematosus were not uncommon. In recent year, however, because the preferred treatment of discoid lups erythematosus consisted of steroids and antimalarials, the association with squamous cell carcinoma has been extremely rare. We present a case of giant squamous cell carcinoma arising in the discoid lupus erythematosus in 39 year-old women. She had had a 20 year history of facial discoid lupus erythematoseus. In September of 1976, a thumb tip-sized tumor mass found on the right cheek and the maas grew into the adult palm size rapidly. The tumor show ulceration, serosanguinous discharge, easy bleeding, and foul oder. Histopathologically, the specimen obtained from plaques of the left cheek showed hyperkeratosis with plugging, atrophy of the stratum malpighii, a patchy, chiefly lymphoid cell infiltrate with a tendency to arrangement about the cutaneous appendages, hydropic degeneration of the basal cells, edema and vasodilatation in the dermis. The specimen obtained from tumor mass of the right cheek showed many nests consisted of well differentiated squamous cells, keratin pearls.

2.A Study on the Changes in the Lumbar Lordosis, Lumbosacral and Sacral Inclination Angle during Aging.

Three parameters (lumbar lordosis, lumbosacral and sacral inclination angle) were measured on 512 normal plain lateral radiographs of lumbosacral spine and the patterns of change during aging were studied. The following results were obtained. 1. The lumbar lordosis angle showed a tendency to increase with age. But a brief decrease was observed after the 6th decade in both sexes, between the 2(nd) and 3(rd) decades in males, and between the 3(rd) and 4(th) decades in females. The lumbar spines of females were generally more lordotic than those of males. 2. The lumbosacral angle appeared to increase with age. The angle in males showed a decline between the 2nd and 4th decades while the angle in females decreased after the 6th decade. Males usually had greater angles than females. 3. The sacral inclination angle continued to increase until the 6th decade, after which a decrease was noted. The angle in females was generally greater than that in males. These results showed that all three angles varied with age, and the pattern of change differed between male and female.

3.Two Cases of Inflammatory Linear Verrucose Epidermal Nevus.

Inflammatory linear verrucose epidermal nevus(ILVEN) is characterized by an early age of onset, predominance in females, pruritus, unresponsiveness to treatment, and a distinctive inflammatory and psoriasiform histologic appearance. We present two cases of typical ILVEN. One of them was a 16-year-old male, who had had a mild pruritic linear verrucose patch involving the right arm which grew slowly for about 8 years. The other case was a 14-year-old girl whose lesion appeared at the age of 13. This lesion was localized on the right posterior thigh and showed linear distribution and verrucose appearance. Histopathological findings of two cases showed hyperkeratosis with foci of parakeratosis, moderate to marked acanthosis, elongation of the rete ridges, and occasional slight spongiosis, and chronic inflammatory infiltration in the dermis.

4.A Case of Hellermann-Streiff Syndrome.

Hallermann-Streiff Syndrome is a rare congenital anomaly characterized by dyscephalia, dental anomalies, hypotrichosis, congenital cataract, bilateral microphthalmia and has characteristic feature of bird face due to marked mandibular hypoplasia and beaked nose. The authors have experienced a case of Hallermann-Streiff Syndrome combined with bilateral aphakia and glaucoma in a 34 years old woman and we report this case with the review of literatures.

5.Mucoepidermoid Carcinoma with Distant Metastases to the Kidney, Adrenal Gland, Skull and Gluteus Maximus Muscle: a Case Report.

INTRODUCTION: Distant metastases of mucoepidermoid carcinoma (MEC) are reported with the most common sites being the soft tissue of skin, lung, liver, and bone. We report here a very rare case of MEC with multiple metastases to the kidney, adrenal gland, skull and gluteus maximus muscle. CASE REPORT: A 63-year-old male patient presented with left-sided headache. Radiologic evaluations including CT and MRI showed ill-defined soft tissue lesion involving the left infratemporal fossa and left sphenoid sinus, and multiple enlarged lymph nodes in neck and mediastinum. PET-CT demonstrated multiple hypermetabolic lesions in and around the left kidney, left adrenal gland, right ischium, right gluteus maximus and skull base. These lesions were confirmed as MEC with multiple metastases through biopsy. DISCUSSION: Only one case of metastasis to the skull has been previously reported, and moreover, there has not been a case of metastatic MEC to the kidney, adrenal gland and gluteus maximus muscle so far in the medical literature. It is important to acknowledge the possibility of every unusual MEC metastases, since the presence of metastasis has statistically significant influence on the survival of MEC.

6.Relationship between Blood Pressure and Cognitive Function according to Age in Elderly Adults.

BACKGROUND: It has been reported that the association between blood pressure and cognitive impairment could be modified by factors such as gender, education, and Apo E genotypes. This study was aimed to investigate whether `age' could also be a modifier of the association. METHODS: The study group consisted of 489 participants aged 65 years or over who lived in Namwon, Korea. The presence of hypertension was ascertained by the measurement of their resting blood pressure. A cognitive impairment was considered to be present when a Korean Mini-Mental State Examination score was less than 19 (30th percentile). RESULTS: A higher systolic blood pressure was significantly associated with cognitive impairment, after an adjustment for age, gender, education, and occupation. However, the significance was lost in subjects aged 71 or over. CONCLUSIONS: High systolic blood pressure is a factor strongly associated with cognitive impairment in an aged population. However, in the subjects with far advanced age, the association is not significant. Further studies are required to elucidate whether blood pressure lowering strategies should be differentiated according to age in order to reduce cognitive impairment.

7.Chronic Disease and Health-Related Quality of Life in an Urban Elderlies.

BACKGROUND: In Korea, the proportion and absolute number of old people are increasing very rapidly. And the concern about the quality-of-life of elderly is to increase more and more among health care workers. The quality-of-life of elderly is affected by chronic medical conditions in some part. The purpose of this study is to evaluate the association between eight chronic medical conditions and health-related quality of life among the community-based elderly. METHODS: Target areas were 22 dongs in Dong-gu,Kwangju and 2 dongs in Buk-gu. Six hundreds subjects were randomly sampled from target ares. Interview survey was performed for chronic medical conditions by check list and health-related quality of life by COOP chart. Actual subjects were 568 elderlies. RESULTS: The results are followed ; 1) Compared with other domains of health-related quality of life, overall health was the most associated with chronic medical conditions, and social activity the least associated. 2) Stroke were significantly associated with every domain. And also back problems/joint problems were except the domain of social activity. Compared with other disease, stroke was the most associated with health-related quality of life except the domain of pain. 3) Migraine/chronic headache, gastroduodenal problems, and heart conditions were significantly associated with overall health, bodily pain, and one or two domains. 4) Hypertension and diabetes mellitus were signi ficantly associated with only one domain; overall health 5) Asthma and chronic bronchitis was not associated with any domains. CONCLUSION: Generally, health-related quality of life was highly associated with mobility limiting diseases such as back problems/joint problems stroke. And also moderately associated with pain relation disease such ans migraine/chronic headache, gastroduodenal problems, and heart condition.
